Rare Fatal Event of Multidrug-resistant Shigella Septicemia and Cryptococcal Meningitis in an Immunocompromised Host.

Bacteriemia due to Shigella spp. is a rare event. However, an individual with impaired immunity remains at risk of such invasion where the infective dose is as low as 10 viable bacilli. This case report describes one such fatal event where a 40-year-old immunocompromised male presented to a tertiary-level healthcare facility with a history of diarrhea for 10 days and seizures. Prompt microbiological workup established multidrug-resistant Shigella flexneri septicemia and cryptococcal meningitis.

Leflunomide-associated fatal cryptococcal meningoencephalitis presenting as rapidly progressive dementia: a case report.

BACKGROUND: Cryptococcal meningoencephalitis is a serious opportunistic infection, classically associated with advanced immunosuppression. The increasing use of immunomodulatory drugs, such as leflunomide, has broadened the at-risk population. CASE REPORT: A 66-year-old woman with rheumatoid arthritis, on long-term leflunomide therapy, presented with progressive cognitive decline, behavioral changes, and myoclonus over a six-week period. The initial presentation mimicked rapidly progressive dementia. Cerebrospinal fluid analysis showed elevated opening pressure, hypoglycorrhachia, and hyperpr

Neuroprotective effects of DPP-4 inhibitors sitagliptin and vildagliptin in Parkinson's disease via autophagy modulation.

STUDY AIMS: Cardiac arrest is a condition with a high mortality rate, with most cases occurring outside hospital. First responders (defined as public responders in our study) play an increasingly important role in management through the early initiation of cardiopulmonary resuscitation and defibrillation. Several studies have shown that the involvement of public responders can result in faster initiation of resuscitation and increased survival. This study evaluated the management of out-of-hospital cardiac arrest by public responders and the effectiveness of the FRED-PA1 automated external def

Evaluating the Use of Lower Dose Flucytosine for the Treatment of Cryptococcal Meningitis: A Clinical Trial.

BACKGROUND: Cryptococcosis is a leading cause of death among people with human immunodeficeincy virus (HIV). Guidelines recommend combination therapy with amphotericin B and flucytosine, but standard flucytosine dosing of 100 mg/kg/day is associated with hematologic toxicity. The FLOOR trial investigated whether lower flucytosine dosing maintained therapeutic efficacy and reduced toxicity. METHODS: This phase 2, single-arm, open-label trial enrolled 48 adults with HIV and confirmed cryptococcal meningitis at 2 hospitals in Uganda. Participants received 10 days of flucytosine at 60 mg/kg/day i

Adrenal Corticosteroids and Increased Intracranial Pressure in Nonimmunocompromised Patients With Cryptococcal Meningitis.

Elevated opening pressures on lumbar puncture were followed in 7 nonimmunocompromised patients being treated for a postinfectious inflammatory response syndrome that had begun following antifungal treatment of cryptococcal meningitis. During the initial period of high-dose corticosteroid therapy, opening pressures on lumbar puncture fell an average of 14 cm. Despite this, 5 of 7 patients later required a ventriculoperitoneal shunt for pressure control.

Unexpected Variability in Laboratory and Clinical Assays Used to Quantify Cryptococcus neoformans Polysaccharide.

The detection and quantification of cryptococcal capsular polysaccharide (PS) antigen in body fluids is useful for the diagnosis of cryptococcosis. We compared the sensitivity of 4 assays for measuring the amount of cryptococcal PS and 2 clinical assays (lateral flow assay [IMMY] and the cryptococcal latex agglutination assay [Remel]) using the PS of 4 single-motif-expressing Cryptococcus strains. These showed relatively consistent laboratory assay interstrain sensitivity with significant interstrain variation, with the more sensitive clinical assays showing variation consistent with antigenic

Coccidioidomycosis in Europe: a systematic literature review of epidemiology, treatment and outcomes.

OBJECTIVES: Coccidioidomycosis (cocci) is a rare fungal disease with clinical manifestations ranging from asymptomatic to life-threatening severe pulmonary or disseminated infection. It is endemic to parts of the southwestern USA, Mexico and South America. Tourism and migration have led to cocci occurrences across Europe among those arriving from endemic areas. The pan-European epidemiology of cocci has not previously been systematically reviewed. METHODS: A systematic literature review was conducted with no limits for publication year, article type and language. We initially identified 797 r

The catalase gene CAT2 and its role in the virulence of one sub-cluster of Cryptococcus gattii VGI clinical isolates.

Cryptococcus gattii causes cryptococcosis and life-threatening cryptococcal meningitis. Currently, the pathogenic virulence mechanisms of C. gattii remain a significant area of ongoing research with considerable unexplored aspects. On the basis of our established research, a sub-cluster of strains with independent evolutionary relationships from WM276 in the phylogenetic analysis of VGI-type strains is identified. In vivo infection experiments on this sub-branch of strains reveal that there are hypervirulent strains and hypovirulent strains among these strains, and the virulence differences ar

Analytical validation: Comparison of two laboratory-based methods for CSF quantitative cryptococcal cultures.

Quantitative cryptococcal culture of the cerebrospinal fluid (CSF) is commonly used in cryptococcal meningitis research. We assessed the 'marble method' to evenly spread CSF onto two Sabouraud dextrose agar plates compared with the 'droplet method' using drops of CSF evenly placed around two halves of one agar plate. We found high concordance between the two methods. The Bland-Altman plot showed that the droplet method differed from the marble method by an average of -0.044 log10 CFU/ml CSF, with 95% limits of agreement from -0.324 to 0.235, indicating a small bias in favor of the marble metho

Multicenter Analysis of General presentations and Imaging features in Cerebral MUCORmycosis (MAGICMUCOR): toward different entities.

Central nervous system mucormycosis (CNS-M) is a severe disease with difficult and often delayed diagnosis, leading to high mortality. The aim of this new study was to assess clinical and radiological presentation according to underlying conditions and dissemination routes to optimize diagnostic strategies. We conducted a retrospective national study including 54 CNS-M cases diagnosed between 2005 and 2020, with brain imaging reviewed by two neuroradiologists. CNS-M resulted from presumed hematogenous dissemination in 29 patients (54%) and from direct extension in 25 (46%), known as rhino-orbi

A biomimetic nanovesicle derived from BCG-primed macrophages with mannose modification for targeted immunotherapy against cryptococcal meningitis.

Cryptococcal meningitis is a life-threatening fungal infection of the central nervous system (CNS), characterized by an immunosuppressive microenvironment driven by Cryptococcus neoformans-induced polarization of macrophages toward M2 phenotype, which undermines host immunity and limits the efficacy of conventional antifungal therapies. To address these challenges, we developed a biomimetic mannose-modified nanovesicle (MBNV) system derived from Bacillus Calmette-Guérin (BCG)-primed bone marrow-derived macrophages (BMDMs) for dual action CNS immunotherapy. In vitro and in vivo studies demonstr

Intracranial Candida Auris Infection: A Case Report and Scoping Review.

OBJECTIVE: Candida auris (C. auris) is a multidrug-resistant yeast capable of causing invasive infections in both immunocompromised and immunocompetent hosts. While bloodstream and wound infections are well described, central nervous system (CNS) involvement remains rare and poorly characterized. We present a case of C. auris CNS infection and a scoping review of adult cases to identify trends in diagnosis and treatment. METHODS: We searched PubMed, Embase, Scopus, Web of Science, and Europe PMC for reports from 2009 to 2025 using CNS-specific and C. auris-specific terms. After deduplication

Evaluating Desirability of Outcome Ranking (DOOR) and Win Ratio for Hierarchical Composite Endpoints in Cryptococcal Meningitis Trials.

BACKGROUND: Traditional antifungal trials have relied on all-cause mortality as the primary endpoint, which may overlook important treatment-related outcomes such as adverse events or tolerability. METHODS: We evaluated the use of hierarchical composite endpoints to capture clinically meaningful treatment differences in cryptococcal meningitis trials. A Delphi survey determined priorities in constructing the composite endpoint. We analyzed three randomized clinical trials on cryptococcal meningitis with win ratio and the Desirability of Outcome Ranking (DOOR). We estimated the Better DOOR Pro

Occult cryptococcal meningitis presenting as myelopathy from an arachnoid cyst.

An immunocompromised patient presented with acute-on-chronic back pain following minor trauma. The pain was accompanied by worsening right-sided weakness, paraesthesias, progressive gait impairment and hyperreflexia concerning for compressive myelopathy. The patient's spouse also reported months of increasing forgetfulness. Imaging revealed a thoracic arachnoid cyst, which was treated by fenestration and resection of the cyst via laminoplasty. At the time of surgery, the thickened cyst wall was sent off for pathological evaluation. Postoperatively, pathology identified yeast consistent with cr

Acetylation-mediated fluconazole inactivation: A novel antifungal resistance mechanism.

Antifungal resistance is considered a global health threat. However, enzymatic inactivation of antifungals, a common mechanism seen in antibacterial resistance, has not yet been described in fungi. From a One Health perspective, this study demonstrates that Cryptococcus deuterogattii and C. neoformans, the leading agents of cryptococcosis, enzymatically inactivate fluconazole. Agrochemicals induce the overexpression of genes that code for acetyltransferases, specifically GCN5 and NAT10. These enzymes catalyze the acetylation of fluconazole into O-acetyl-fluconazole. This metabolite is unable t
