UNLABELLED: While South African guidelines recommend a lumbar puncture (LP) to exclude cryptococcal meningitis (CM) among all people with a newly positive cryptococcal antigen (CrAg) test, irrespective of CM symptoms, this is not always feasible. High blood CrAg lateral flow assay (LFA) titers are associated with concurrent CM and increased mortality. Single-strip CrAg semi-quantitative (SQ) tests could risk-stratify people with antigenemia. Consecutive fresh LFA-positive remnant plasma samples from a CD4 laboratory network collected between April and July 2021 were retested. We described LFA

Neutralizing anti-cytokine autoantibodies (ACAAs) have been associated with cryptococcal meningitis (CM), but the influence of HIV co-infection remains undefined. We investigated plasma ACAA profiles and function in a cross-sectional Vietnamese cohort stratified by HIV and CM status (n = 20 per group). We quantified plasma ACAAs against interferons (IFNs), interleukins (ILs), and granulocyte-macrophage colony-stimulating factor (GM-CSF) using a particle-based assay, and assessed their neutralizing activity in high-titer samples. Associations between ACAA levels and CM were analyzed using Firth

BACKGROUND: Cryptococcal meningitis (CM) is associated with high mortality and neurodevelopmental sequelae and predominantly affects people living with HIV. Screening and pre-emptive treatment for cryptococcal antigen (CrAg) in blood in adults with CD4 <200 cells/µL reduces mortality. The World Health Organization does not recommend screening children <10 years due to a presumed low prevalence of CM. Nevertheless, CrAg testing is performed for all ages in South Africa in those with CD4 <100 cells/µL.
METHODS: Children (<18 years) with CD4 <100 cells/µL and a CrAg screening result were identif

We report a case of recurrent cryptococcal meningitis in a pregnant woman living with vertically acquired HIV. Despite effective treatment of primary infection prior to pregnancy, she later experienced a microbiological relapse of cryptococcal meningitis during pregnancy, and an immune-reconstitution inflammatory syndrome in the post-partum period. This case emphasises the impact of pregnancy related immunological changes on the natural history of complex infections. This case illustrates the challenges of managing opportunistic infections throughout pregnancy and the influence of perinatal im

BACKGROUND: The impact of cryptococcosis on cerebral structure and MRI characteristics in people living with HIV (PWH) who have cryptococcal meningoencephalitis remains underexplored. Existing evidence mainly comes from small-scale studies. This study aimed to clarify the effects of cryptococcal meningoencephalitis on brain structural changes and MRI features in PWH. METHODS: A total of 190 patients with intracranial cryptococcosis were enrolled and categorized, based on HIV serostatus, into the PWH group (n = 127) and the people linving without HIV(PWOH) group (n = 63). Laboratory parameters,

Elevated intracranial pressure contributes to high mortality in cryptococcal meningitis. We evaluated the association between therapeutic lumbar punctures (LPs) and survival among patients hospitalized with cryptococcal meningitis in the United States. Repeat LPs were associated with lower mortality (hazard ratio [HR] 0.38; 95% confidence interval (CI), 0.29-0.49) overall, in people with HIV (HR 0.33; 95% CI, 0.21-0.50), in transplant recipients (HR 0.21; 95% CI, 0.08-0.58), and in persons without HIV or transplant (HR 0.44; 95% CI, 0.30-0.64) compared with receipt of only diagnostic LP. OBJECTIVES: In this era of widely available antiretroviral therapy (ART), information regarding new onset seizure, seizure recurrence and mortality is needed to guide clinical decision-making.
METHODS: A rural, prospective, cohort study was conducted April 2016 to June 2019. Inclusion criteria were HIV positivity, age ≥ 18 years, and new onset seizure. Two-year seizure recurrence was the outcome. Risk factors for seizure recurrence were evaluated using a Fine-Gray model to account for the competing outcome of death.
RESULTS: 258 people were screened, 95 were eligible and 89 were enrolled. 47

BACKGROUND: Despite the expansion of antiretroviral treatment programmes, the incidence and mortality of HIV-associated cryptococcal meningitis remain high in Africa. Cryptococcal antigen (CrAg) in the blood precedes meningitis. CrAg screening of individuals with advanced HIV disease, combined with pre-emptive fluconazole treatment, reduces the risk of cryptococcal meningitis and associated mortality. However, mortality among individuals with antigenaemia treated with fluconazole is higher than that of CD4-matched individuals with a CrAg-negative test. Autopsy studies have found that cryptococ

BACKGROUND: HIV-1 subtype C (HIV-1C) may use multiple coreceptors to enter cells. However, the differences in coreceptor usage between HIV-1C Env variants from peripheral blood and those from the central nervous system (CNS) are still not completely understood. This study characterised the coreceptor usage of viral populations that circulate in the central nervous system (CNS) and plasma compartments to enhance understanding of HIV-1C pathogenesis and inform the appropriate use of entry inhibitors.
METHODS: We characterised the in vitro coreceptor usage of HIV-1C envelope protein (Env) varian

BACKGROUND: Cryptococcal infection is a major cause of morbidity and mortality among people with HIV (PWH), particularly those with advanced immunosuppression. Cryptococcal infection, detected by a laboratory-confirmed positive serum cryptococcal antigen (CrAg) test (cryptococcal antigenemia), typically precedes cryptococcal meningitis and provides a critical window for prevention. However, recent data on its risk factors in Uganda are limited. We assessed the risk factors for cryptococcal infection among PWH with non-suppressed viral load and severe immunosuppression receiving care at a refer

INTRODUCTION: Cryptococcal meningitis causes an estimated 19% of AIDS-related deaths globally and is the leading cause of meningitis in adults with HIV. Subclinical infection with cryptococcal antigen (CrAg) is detectable in plasma, and CrAg titer of>=1:160 is predictive of meningitis or death. We evaluated if plasma CrAg titer changed over time in Uganda with the expansion of national CrAg screening programs and antiretroviral (ART) access.
METHODS: We prospectively screened adults with advanced HIV disease (CD4 ≤ 200 cells/μl) for CrAg from 2017 through 2022 using the lateral flow assay and

Cryptococcus neoformans is an opportunistic fungal infection and a leading cause of HIV-associated meningitis, contributing significantly to AIDS-related mortality, especially in Africa. Despite advancements in treatment, case fatality rates remain high, emphasizing the need for a better understanding of host immune responses. This study investigated the association between variation in the human leukocyte antigen (HLA) genes and the risk of cryptococcal disease in Ugandan individuals with HIV. We first described the allele prevalence in a prospectively enrolled Ugandan population using next-g

Cryptococcal meningitis is a life-threatening opportunistic infection in advanced HIV disease. Cryptococcal antigen (CrAg) detection by lateral flow assay (LFA) is the diagnostic cornerstone, offering high sensitivity and specificity. However, false-negative results may arise from the prozone effect - antigen excess disrupting antibody-antigen lattice formation. We report a 37-year-old man with newly diagnosed advanced HIV infection (CD4+ count 26 cells/µL) presenting with altered mental status. Initial CrAg LFA on serum and cerebrospinal fluid (CSF) was negative. India ink staining revealed e

Introduction: Cryptococcal meningitis (CM) is a leading cause of mortality among people living with human immunodeficiency virus (HIV) in Sub-Saharan Africa, requiring rapid diagnosis. Although the Food and Drug Administration (FDA)-approved Dynamiker cryptococcal antigen lateral flow assay (CrAg LFA) offers a potential point-of-care solution, its use remains limited in high-burden settings.Hypothesis/Gap statement: Despite FDA approval of Dynamiker CrAg LFA for the diagnosis of CM, the underutilization of the test in settings with a high burden of CM is of particular concern. Therefore, there
