Persistent meningeal enhancement on MRI in an infant with culture-negative bacterial meningitis: a case report and systematic review of the literature (2014-2025).

BACKGROUND: Bacterial meningitis (BM) in infants may be complicated by persistent meningeal enhancement on magnetic resonance imaging (MRI) after clinical cure-a phenomenon frequently misdiagnosed as infection recurrence. However, long-term follow-up data on asymptomatic persistent meningeal enhancement in young infants remain scarce. Here, we report a rare case of 5-month persistent meningeal enhancement following culture-negative bacterial meningitis (CNBM) in a 70-day-old infant, and systematically reviewed 17 eligible studies on meningeal enhancement in infantile bacterial meningitis publi

Classification accuracy of a hierarchical molecular inference-based deep-learning system for CNS tumour diagnosis: a multi-institutional, retrospective study.

BACKGROUND: Recent advances in artificial intelligence (AI) and computer vision empower deep-learning models to infer molecular features from histopathological images to classify CNS tumours. The aim of this study was to test the classification accuracy of a molecular inference-based AI assistant for CNS tumour diagnosis. METHODS: In this multi-institutional, retrospective study, we used data from whole slide images of samples from patients aged 0-95 years, diagnosed with primary or recurrent CNS tumours. Reference diagnostic labels were determined by DNA methylation-based tumour classificati

A Rare Case of Slowly Progressive Meningitis Caused by Streptococcus intermedius.

This report describes a case of Streptococcus intermedius meningitis with an unusually gradual clinical course in an elderly patient with cancer. Such a subacute-to-chronic presentation has not been reported previously. Although S. intermedius meningitis typically involves intracranial suppuration, systemic imaging has revealed no abscesses or other infectious foci. The identification of this pathogen can be challenging in some clinical settings. In this patient, S. intermedius bacteremia was confirmed, whereas cerebrospinal fluid cultures were negative. However, a 16S rRNA gene analysis succe

Candida dubliniensis as a Cause of Chronic Meningitis in a 3-Year-Old Boy with Acute Lymphoblastic Leukemia.

Candida dubliniensis is an opportunistic yeast closely related to Candida albicans and an uncommon cause of central nervous system (CNS) infection. While isolates are often susceptible to azoles, reduced susceptibility or acquired resistance may occur, making species identification and antifungal susceptibility testing clinically relevant. We report a 3-year-old boy with Philadelphia chromosome-positive B-cell precursor acute lymphoblastic leukemia (ALL) in hematologic remission who developed chronic meningitis during maintenance chemotherapy. The initial presentation was non-specific (marked

Chronic Headache and Visual Field Loss: Manifestations in a Patient With Chronic Brucellosis Meningitis: Case Report With Brief Literature Review.

Brucellosis is a zoonotic infection caused by Brucella species, often leading to serious complications and presenting diagnostic challenges. A 45-year-old female patient presented with a headache that had persisted for two months and was recently accompanied by visual disturbances. Notably, the patient's headache began 6 weeks postpartum, and she had no prior history of illness, medication use, or headaches. Imaging studies were normal, and cerebrospinal fluid (CSF) analysis revealed meningitis, with a positive Wright test. After excluding other potential causes and considering the patient's c

Granulomatosis with polyangiitis presenting with predominant neuro-otological involvement in an adolescent male: A diagnostic challenge - A case report.

Granulomatosis with polyangiitis (GPA) is a rare Antineutrophil Cytoplasmic Antibodies (ANCA)-associated vasculitis that typically affects the upper and lower respiratory tract and kidneys. In adolescents, atypical presentations with predominant neuro-otological features may delay diagnosis. We report a 15-year-old boy who presented with progressive bilateral sensorineural hearing loss, facial nerve palsy, multiple lower cranial nerve deficits, dysphagia, and hoarseness, initially suggestive of chronic meningitis. Magnetic resonance imaging showed pachymeningeal enhancement with skull-base inv

Primary diffuse leptomeningeal melanomatosis presenting as chronic meningitis and dense spinal plaques.

Primary diffuse leptomeningeal melanomatosis (PDLM) is an exceptionally rare and aggressive central nervous system melanocytic tumour, often masquerading as inflammatory or infective leptomeningeal disease. A woman in her early 60s presented with hydrocephalus and diffuse leptomeningeal enhancement, initially treated as neurosarcoidosis. This progressed, despite immunosuppression, and repeated lumbar punctures yielded no cerebrospinal fluid (CSF) ('dry taps'). Intradural biopsy identified a melanocytic neoplasm with a GNA11 mutation. Fewer than 25 cases of isolated PDLM have been reported, and

Neurocysticercosis-Associated Meningitis: A Systematic Review.

Neurocysticercosis is a frequent parasitic infection of the central nervous system. While parenchymal forms are well recognized, meningitis due to cysticercus remains underdiagnosed. This systematic review assessed clinical features, diagnostics, treatment, and outcomes of neurocysticercosis-associated meningitis. Following PRISMA guidelines and PROSPERO registration, case reports, series, and cohorts describing meningitis attributed to neurocysticercosis were analyzed. Extracted data included demographics, presentation, imaging, cerebrospinal fluid (CSF) findings, diagnostics, therapy, and ou

Cryptococcal Meningitis Beyond Immunosuppression: A Case Report in an Immunocompetent Individual.

BACKGROUND: Cryptococcal meningitis is a life-threatening opportunistic fungal infection predominantly seen in immunocompromised hosts, yet it is increasingly recognized in apparently immunocompetent individuals. In tuberculosis-endemic regions, its diagnosis is frequently delayed or missed due to overlapping clinical and cerebrospinal fluid findings with tuberculous meningitis. CASE PRESENTATION: A 44-year-old immunocompetent woman presented with a two-month history of progressive diffuse headache and subjective bilateral lower limb weakness. Neurological examination was unremarkable, and in

Recurrent herpes simplex virus type 2 Mollaret meningitis in a man living with HIV: A case report and patient-initiated episodic valacyclovir strategy.

Recurrent benign lymphocytic meningitis (RBLM) or Mollaret meningitis is an uncommon manifestation of herpes simplex virus type 2 (HSV-2) infection, characterized by self-limiting aseptic meningitis with complete recovery between episodes. Despite high background prevalence of HSV-2 in people living with HIV (PLWH), RBLM remains extremely rare in this population. Only two cases, both involving female patients, have been reported. We report the case of a man with HIV who experienced recurrent HSV-2 meningitis despite sustained virological suppression for HIV, preserved CD4 cell count at the tim

Antimicrobial resistance and vaccine-preventable infections: concurrent health security challenges after the double earthquake in Venezuela.

SCIENTIFIC BACKGROUND: Meningiomas are the most common benign intracranial neoplasms with a slow growth presented as the intracranial lesion. These tumors are without any symptoms for a long time. At the time of diagnosis it is frequently an asymptomatic tumor. In that case the therapist may well suggest a wait-and-see strategy. The therapy of meningiomas focuses firstly on the microsurgical treatment. Volume reduction can be achieved immediately after treatment. Stereotactic radiosurgery is an important non-invasive treatment option for recurrent tumors or meningiomas with partial resection.

Towards credible digital twins for basic and preclinical research.

Atomoxetine (Strattera(R)) is a selective norepinephrine (noradrenaline) reuptake inhibitor that is not classified as a stimulant, and is indicated for use in patients with attention-deficit hyperactivity disorder (ADHD). Atomoxetine is effective and generally well tolerated. It is significantly more effective than placebo and standard current therapy and does not differ significantly from or is noninferior to immediate-release methylphenidate; however, it is significantly less effective than the extended-release methylphenidate formulation OROS(R) methylphenidate (hereafter referred to as osm

Isolated Central Nervous System Tuberculosis Presenting as Progressive Encephalopathy: A Report of a Biopsy-Proven Case.

Central nervous system (CNS) tuberculosis (TB) is an uncommon but severe manifestation of Mycobacterium tuberculosis infection and is associated with significant morbidity and mortality. Its clinical and radiologic presentations are often nonspecific and may mimic malignancy, inflammatory disorders, or fungal infections, frequently resulting in diagnostic delay. Conventional cerebrospinal fluid (CSF) testing has limited sensitivity, and definitive diagnosis may require histopathologic confirmation when noninvasive studies are inconclusive. We report the case of a 76-year-old woman with a histo

Associations between Age, Treatment Modality and Survival in Atypical Meningioma: Analysis of the SEER Database, 2000-2021.

PURPOSE: Atypical meningiomas have higher recurrence and mortality than benign variants, and age may influence treatment decisions and outcomes. We evaluated associations between age, treatment modality, and survival in patients with atypical meningioma. METHODS: Adults with histologically confirmed intracranial atypical meningioma were identified from the 2000-2021 Surveillance, Epidemiology, and End Results database. Multivariable Cox regression assessed overall survival (OS) and cancer-specific survival (CSS), and logistic regression evaluated predictors of gross total resection (GTR) vers

Surgical treatment of pulmonary hydatidosis: six years of experience and results at the National Hospital Guillermo Almenara Irigoyen, Lima, Peru.

Liposomal amphotericin B (AmBisome) is a lipid-associated formulation of the broad-spectrum polyene antifungal agent amphotericin B. It is active against clinically relevant yeasts and moulds, including Candida spp., Aspergillus spp. and filamentous moulds such as Zygomycetes, and is approved for the treatment of invasive fungal infections in many countries worldwide. It was developed to improve the tolerability profile of amphotericin B deoxycholate, which was for many decades considered the gold standard of antifungal treatment, despite being associated with infusion-related events and nephr

Molecular heterogeneity and prognostic biomarkers in primary meningeal melanocytic tumors.

Primary meningeal melanocytic tumors (PMMTs) are rare neoplasms currently stratified by the World Health Organization (WHO) into three grades of malignancy based exclusively on histopathological criteria, while the prognostic significance of molecular alterations remains poorly defined. This study aimed to investigate the prognostic relevance of histopathological and molecular features in PMMTs and to assess the diagnostic utility of immunohistochemical and molecular markers for distinguishing PMMTs from malignant melanotic nerve sheath tumors (MMNSTs). Thirty-six primary central nervous syste

A Rare Recurrent Intracranial Meningioma with Variable Histopathological Grades and Pulmonary Metastasis: A Case Report.

Meningiomas are predominantly benign intracranial neoplasms, with extracranial distant metastasis occurring in fewer than 1% of all patients; pulmonary metastasis represents an exceedingly rare complication of meningioma progression and recurrence. We describe a 44-year-old male patient with sequentially recurrent intracranial meningioma exhibiting dynamic histopathological grade evolution combined with synchronous bilateral pulmonary metastatic lesions, followed for an unprecedented 18 clinical years. Across follow-up, the patient received four open craniotomies, two sessions of stereotactic

Estimated prevalence and distribution of causes of immune-mediated hypertrophic pachymeningitis in São Paulo, Brazil.

BACKGROUND: The prevalence of hypertrophic pachymeningitis (HP) remains poorly defined worldwide. In Japan, the estimated prevalence is 0.949 cases per 100,000 inhabitants, with most cases related to ANCA-associated vasculitis or IgG4-related disease. In contrast, epidemiological data from other regions, including Latin America, are scarce. METHODS: This study was conducted at a tertiary referral center in the city of São Paulo, Brazil. The prevalence of HP was estimated by comparison with the known prevalence of multiple sclerosis (MS), based on the number of patients with each condition fol
