PURPOSE: Granulomatous amoebic encephalitis (GAE) is the conventional label for central nervous system (CNS) infection caused principally by Acanthamoeba spp. and Balamuthia mandrillaris, but contemporary disease encompasses meningitic, focal, vascular, brainstem, and spinal phenotypes that extend beyond the classical description. This narrative review examines the changing epidemiology, clinical spectrum, diagnosis, and management of non-flagellated amoebic CNS infection in India.
METHODS: We narratively synthesised published Indian case reports, case series, molecular studies, surveillance

This report describes a case of Streptococcus intermedius meningitis with an unusually gradual clinical course in an elderly patient with cancer. Such a subacute-to-chronic presentation has not been reported previously. Although S. intermedius meningitis typically involves intracranial suppuration, systemic imaging has revealed no abscesses or other infectious foci. The identification of this pathogen can be challenging in some clinical settings. In this patient, S. intermedius bacteremia was confirmed, whereas cerebrospinal fluid cultures were negative. However, a 16S rRNA gene analysis succe

Candida dubliniensis is an opportunistic yeast closely related to Candida albicans and an uncommon cause of central nervous system (CNS) infection. While isolates are often susceptible to azoles, reduced susceptibility or acquired resistance may occur, making species identification and antifungal susceptibility testing clinically relevant. We report a 3-year-old boy with Philadelphia chromosome-positive B-cell precursor acute lymphoblastic leukemia (ALL) in hematologic remission who developed chronic meningitis during maintenance chemotherapy. The initial presentation was non-specific (marked

Brucellosis is a zoonotic infection caused by Brucella species, often leading to serious complications and presenting diagnostic challenges. A 45-year-old female patient presented with a headache that had persisted for two months and was recently accompanied by visual disturbances. Notably, the patient's headache began 6 weeks postpartum, and she had no prior history of illness, medication use, or headaches. Imaging studies were normal, and cerebrospinal fluid (CSF) analysis revealed meningitis, with a positive Wright test. After excluding other potential causes and considering the patient's c

Granulomatosis with polyangiitis (GPA) is a rare Antineutrophil Cytoplasmic Antibodies (ANCA)-associated vasculitis that typically affects the upper and lower respiratory tract and kidneys. In adolescents, atypical presentations with predominant neuro-otological features may delay diagnosis. We report a 15-year-old boy who presented with progressive bilateral sensorineural hearing loss, facial nerve palsy, multiple lower cranial nerve deficits, dysphagia, and hoarseness, initially suggestive of chronic meningitis. Magnetic resonance imaging showed pachymeningeal enhancement with skull-base inv

Primary diffuse leptomeningeal melanomatosis (PDLM) is an exceptionally rare and aggressive central nervous system melanocytic tumour, often masquerading as inflammatory or infective leptomeningeal disease. A woman in her early 60s presented with hydrocephalus and diffuse leptomeningeal enhancement, initially treated as neurosarcoidosis. This progressed, despite immunosuppression, and repeated lumbar punctures yielded no cerebrospinal fluid (CSF) ('dry taps'). Intradural biopsy identified a melanocytic neoplasm with a GNA11 mutation. Fewer than 25 cases of isolated PDLM have been reported, and

Neurocysticercosis is a frequent parasitic infection of the central nervous system. While parenchymal forms are well recognized, meningitis due to cysticercus remains underdiagnosed. This systematic review assessed clinical features, diagnostics, treatment, and outcomes of neurocysticercosis-associated meningitis. Following PRISMA guidelines and PROSPERO registration, case reports, series, and cohorts describing meningitis attributed to neurocysticercosis were analyzed. Extracted data included demographics, presentation, imaging, cerebrospinal fluid (CSF) findings, diagnostics, therapy, and ou

BACKGROUND: Cryptococcal meningitis is a life-threatening opportunistic fungal infection predominantly seen in immunocompromised hosts, yet it is increasingly recognized in apparently immunocompetent individuals. In tuberculosis-endemic regions, its diagnosis is frequently delayed or missed due to overlapping clinical and cerebrospinal fluid findings with tuberculous meningitis.
CASE PRESENTATION: A 44-year-old immunocompetent woman presented with a two-month history of progressive diffuse headache and subjective bilateral lower limb weakness. Neurological examination was unremarkable, and in

BACKGROUND: Retinal ischemia is a retinal disorder related to retinal vascular occlusion, glaucoma, diabetic retinopathy and age-related macular degeneration. The study aimed to evaluate the protective effects and underlying mechanisms of Chi-Ju-Di-Huang-Wan (CJDHW) against retinal ischemia in rats.
METHODS: High intraocular pressure (HIOP)-induced retinal ischemia was established in Wistar rats by raising their intraocular pressure to 120 mmHg for 60 min with in an eye whose anterior chamber was cannulated with a 30-guage needle adapted to a normal saline bottle through an intravenous line.

Central nervous system (CNS) tuberculosis (TB) is an uncommon but severe manifestation of Mycobacterium tuberculosis infection and is associated with significant morbidity and mortality. Its clinical and radiologic presentations are often nonspecific and may mimic malignancy, inflammatory disorders, or fungal infections, frequently resulting in diagnostic delay. Conventional cerebrospinal fluid (CSF) testing has limited sensitivity, and definitive diagnosis may require histopathologic confirmation when noninvasive studies are inconclusive. We report the case of a 76-year-old woman with a histo

BACKGROUND: The prevalence of hypertrophic pachymeningitis (HP) remains poorly defined worldwide. In Japan, the estimated prevalence is 0.949 cases per 100,000 inhabitants, with most cases related to ANCA-associated vasculitis or IgG4-related disease. In contrast, epidemiological data from other regions, including Latin America, are scarce.
METHODS: This study was conducted at a tertiary referral center in the city of São Paulo, Brazil. The prevalence of HP was estimated by comparison with the known prevalence of multiple sclerosis (MS), based on the number of patients with each condition fol

Diffuse leptomeningeal glioneuronal tumors (DLGNT) are rare central nervous system neoplasms characterized by the absence of a primary brain or spinal cord tumor. Patients exhibit common radiological and histopathological features; however, the clinical progression varies. We present the case of a 4-year-old girl initially diagnosed with constipation and failure to thrive, who subsequently developed neurological signs during hospitalization. Subsequently, she was referred with suspected tuberculosis meningitis, despite negative cerebrospinal fluid (CSF) culture results, and the patient presuma

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

BACKGROUND AND OBJECTIVE: Several studies have shown that idiopathic normal-pressure hydrocephalus (iNPH) can mimic other neurodegenerative disorders, particularly progressive supranuclear palsy (PSP). In this study, we investigated iNPH clinical and neuroimaging features in patients with autopsy-confirmed PSP or Lewy body disease (LBD) by assessing the normal pressure hydrocephalus (NPH) triad of symptoms and imaging features of disproportionately enlarged subarachnoid-space hydrocephalus (DESH) and Evans index (EI) on antemortem MRI scans.
METHODS: Among our study participants (N = 190), th

BACKGROUND: Subarachnoid neurocysticercosis (SANCC) is a condition manifested by chronic meningitis induced by infection with Taenia solium. We sought to determine if there is evidence of autoantibody production in SANCC and whether local production of autoantibodies could be driven by immunogenic homologues found in T. solium.
METHODS: Reactivity of pooled cerebral spinal fluid (CSF) from SANCC patients and uninfected controls was screened against a human proteome chip. Serum from 27 SANCC patients was then tested for antibodies to the 15 top screen hits using a Luciferase ImmunoPrecipitatio

A 25-year-old woman presented with mild fever and fatigue that began four months prior to admission. At 30 days after onset, she developed bilateral visual field impairment with hand tremors. Gait difficulty occurred at 60 days after onset. On admission, the patient was alert and well-oriented. Examination revealed central scotomas, papilledema in both eyes, postural tremors in both hands, hyperreflexia in the limbs, positive pathological reflexes in the lower extremities, and spastic gait. Cerebrospinal fluid analysis showed an elevated cell count and protein levels as well as positive glial

Nodular lymphocyte-predominant Hodgkin lymphoma (NLPHL) is an uncommon form of Hodgkin lymphoma (HL). It commonly presents as chronic, asymptomatic, slow-growing peripheral lymphadenopathy. Central nervous system involvement in NLPHL is rare. Here we report a case of a 28-year-old male patient who presented with a seven-week history of headache and neck pain. The evaluation revealed a diagnosis of subacute meningitis. He was newly diagnosed with NLPHL on lymph node biopsy. With this presentation of subacute aseptic meningitis with a negative workup for infectious, autoimmune etiology, and abse

A 47-year-old woman presented with a two-year history of progressive visual symptoms and headaches. Lumbar puncture revealed lymphocytic pleocytosis, elevated protein, low glucose, and a CSF CrAg titer of 1:256. She was treated empirically for cryptococcal meningitis with amphotericin B, flucytosine, and fluconazole for 15 months. Her symptoms persisted, and repeated CSF and serum CrAg, fungal cultures, and an extensive infectious workup were negative. CSF flow cytometry eventually demonstrated a monoclonal B-cell population suggestive of a lymphoproliferative process. Imaging, including MRI a

UNLABELLED: Sarcoidosis is complicated by neurosarcoidosis in 5-15% of cases. Neurosarcoidosis is characterized by the presence of non-necrotizing granulomas that can affect any part of the nervous system. Key clinical presentations include chronic meningitis, cranial nerve palsies, parenchymal cerebral and spinal lesions, peripheral neuropathy, or myopathy. Diagnosis requires histopathological confirmation of granulomatous disease and exclusion of other potential diagnoses. Immunosuppressive treatment strategies are based on cohort studies and expert consensus, as randomized controlled trials
