Evaluation of enzyme linked immunosorbent--assay for the detection of anticysticercus antibodies in cerebrospinal fluid from patients with neurocysticercosis.

Enzyme linked Immunosorbent Assay (ELISA) was done for the detection of antibodies to Cysticercus cellulosae in 135 cerebrospinal fluid (CSF) and 152 serum samples from patients suspected clinically of neurocysticercosis (NC), neurological disorders other than NC and controls by the use of crude cyst extract antigen. This assay was compared with the standard technique of indirect haemagglutination test (IHA). The results of the two techniques were matched with retrospective analysis of proven diagnosis of these patients. ELISA and IHA was found to be positive respectively in 88 and 84 percent

Chronic Mycobacterium avium complex infection of the central nervous system in a nonimmunosuppressed woman.

We report the case of a woman who developed chronic meningitis and a brain abscess due to Mycobacterium avium, without immunosuppressed state. She was treated with antituberculous drugs but the infection progressed and she died 4 years after first becoming ill. The occurrence of CNS infection with M. avium in the absence of acquired immunosuppression is rare. Its diagnosis is difficult, delaying clinical recognition and appropriate therapy.

[Intraventricular antibiotic therapy].

While shunt infections are regularly treated with intraventricular antibiotics, the validity of such an application in bacterial meningitis of other origin is controversial. We report two cases of partly successful treatment with intraventricular Ceftazidime (10-20 mg twice per week). One patient with pseudomonas aeruginosa meningitis who was treated as an out-patient for nearly two years died after an attempted withdrawal of the intraventricular treatment. In our experience, intraventricular application of antibiotics can be a part of the therapeutic regimen in all cases of chronic meningitis

Infective agents in the central nervous system.

Almost all agents can cause infection within the central nervous system. The extent of infection ranges from diffuse involvement of the meninges, brain, or spinal cord to localized involvement presenting as a space-occupying lesion. Epidemiologic considerations, appreciation of the presenting clinical syndrome (acute bacterial meningitis, acute aseptic meningitis, chronic meningitis, or space-occupying lesion), and cerebrospinal fluid analysis facilitate arrival at a diagnosis.

Chronic or recurrent meningitis. Neurosurgical perspectives.

Chronic meningitis is not a commonly encountered clinical problem in the day-to-day patient care responsibilities of neurosurgeons. It is, however, important for the neurosurgeon to be familiar with the clinical problems, diagnostic challenges, and therapeutic options required to establish a definitive diagnosis. This article concentrates on specific entities causing chronic or recurrent meningitis that may require neurosurgical participation.

Neurocysticercosis. An introduction with special emphasis on new developments in pharmacotherapy.

Neurocysticercosis is the most important parasitic infection of the nervous system in countries of the third world. The disease is caused by the cystic larval stage of Taenia solium, the pork tape worm. The clinical picture of neurocysticercosis is due to the parasite itself and/or an inflammatory reaction around degenerating cysts in the central nervous system. It may cause epilepsy, chronic meningitis and hydrocephalus. The diagnosis is greatly facilitated by the use of computer tomography, magnetic resonance imaging and serological tests. Praziquantel and albendazole now offer a reasonable

Long-term undiagnosed syphilis with clinical presentation of meningitis.

A 43-year-old woman, with an 11-year history of progressive cutaneous lesions, was considered to have granuloma annulare and was treated as such for 4 years. She then developed chronic meningitis. Serological investigation revealed active syphilis. Appropriate antibiotic treatment led to a rapid resolution of the clinical symptoms. The recent reports of similar cases indicate the importance of an adequate knowledge of clinical manifestations and pathological patterns of syphilis.

Cryptococcal meningitis in Auckland 1969-89.

Twenty-six patients with cryptococcal meningitis were seen in Auckland between 1969 and 1989. The incidence of cryptococcal meningitis in Auckland residents was 0.12 cases/100,000/year. Ten (38%) of the patients were Maori or Pacific Island Polynesians. Nineteen (73%) had a predisposing cause, including immunosuppressive therapy in nine and the acquired immunodeficiency syndrome (AIDS) in seven. The most common presenting syndrome was a subacute or chronic meningitis. Other clinical syndromes included a slowly progressive ataxia, polyradiculopathy, and headache with vomiting. In two patients,

[HIV and nervous system manifestations: a review].

Since 1986 the notation Human Immunodeficiency Virus (HIV) is used for the retroviral agent of the Acquired Immunodeficiency Syndrome (AIDS). At the beginning of the therapeutical interest in the immunodeficiency syndrome have been primarily focussed in the internal complications. 1982 one reported for the first time about nervous system manifestations (NS-M) in HIV-patients; according to the latest reports NS-M are diagnosed in 39-63% of these patients. In this review all important aspects of the pathogenesis, clinic and therapy for the HIV-associated peripheric- and central-neurological (lik

The pseudotumor syndrome. Disorders of cerebrospinal fluid circulation causing intracranial hypertension without ventriculomegaly.

We report a series of eight cases that show a close resemblance to, but are not identical with, pseudotumor cerebri (PTC) as normally defined. The majority of these cases are characterized by raised intracranial pressure without ventriculomegaly. They include two cases of cranial venous outflow obstruction in which clinical or radiologic abnormalities precluded the diagnosis of PTC proper (cases 1 and 2); one case of chronic meningitis in which an abnormal cerebrospinal fluid (CSF) composition precluded the diagnosis of PTC (case 3); two cases without either papilledema or a measured increase

Extrapulmonary cryptococcosis in children with acquired immunodeficiency syndrome.

There is a paucity of published information available on extrapulmonary cryptococcosis (EC) in children infected with human immunodeficiency virus, the etiologic agent of the acquired immunodeficiency syndrome. We surveyed investigators in pediatric acquired immunodeficiency syndrome around the country regarding their experience with EC. Investigators from 33 (87%) of 38 institutions responded and information on 13 patients from 11 institutions was analyzed. EC was the acquired immunodeficiency syndrome indicator disease in 9 (69%) of 13 patients. Median age was 8 years with a range of 2 to 17

Nocardial meningitis: case reports and review.

Twenty-eight episodes of nocardial central nervous system infection fulfilling criteria for meningitis were reviewed. In 21 cases Nocardia was cultured from cerebrospinal fluid (CSF). Associated predisposing conditions were present in 75% of cases. The typical presentation was subacute to chronic meningitis characterized by fever (68%), stiff neck (64%), and headache (55%). CSF studies revealed neutrophilic pleocytosis (83% of cases, greater than 500 white blood cells/mm3), hypoglycorrhachia (64%, less than 40 mg of glucose/dL), and elevated protein level (61%, greater than 100 mg/dL). Gd-DTPA enhanced MR imaging in intracranial tuberculosis.

Twenty-six patients with intracranial tuberculosis (Tb) (10 with acute meningitis, 5 with chronic meningitis, 5 with meningitic sequelae and 6 with localized tuberculoma(s)) were examined with MR before and after Gd-DTPA enhancement (0.1 mmol/kg), using 2.0T superconducting unit, and the images were retrospectively analyzed and compared with CT scans. Without Gd-DTPA enhancement, the MR images were generally insensitive to detection of active meningeal inflammation and granulomas. The signal intensity of granulomas was usually isointense to gray matter on both T1- and T2-weighted images, wheth

First report of chronic meningitis caused by Trichosporon beigelii.

Trichosporon beigelii (Trichosporon cutaneum) was identified as the causative agent of chronic meningitis in a 15-year-old boy with acute lymphocytic leukaemia. After a neutropenic episode following cytostatic treatment and itraconazole therapy as prophylaxis, cerebrospinal fluid (CSF) samples yielded growth of Trichosporon beigelii. Treatment with amphotericin B, flucytosine and high doses of fluconazole was followed by clinical improvement, although CSF pleocytosis remained. The cross-reactivity between Cryptococcus neoformans and Trichosporon beigelii in a cryptococcal antigen latex test wa

[Sleep disorders from the neurological viewpoint].

Sleep can be measured by EEG or a monitor which records motor activity. In this way objective data on sleep behavior can be obtained from patients. Total sleep time over 24 hours remains remarkably constant above age 20. Therefore, any changes in total sleep time can be considered to be pathologic. A true shortening of sleep time is rare and is usually associated with a psychosis. More often "common sleep disorders" are encountered, which show a normal total sleep time per 24 hours. Prolonged sleep, usually associated with increased day-time somnolence, is often caused by chronic meningitis or

Cysticercal meningitis--a pernicious form of neurocysticercosis which responds poorly to praziquantel.

Chronic cysticercal meningitis was diagnosed in 6 of 260 cases of neurocysticercosis. Clinical features usually associated with meningitis, such as fever, cranial nerve palsies and nuchal rigidity, were absent and the diagnosis was reached because of persistently abnormal cerebrospinal fluid (CSF) associated with active neurocysticercosis on computed tomography. Other causes of chronic meningitis were excluded. Hydrocephalus, either obstructive or communicating, was present in all 6 patients. The results of medical treatment with praziquantel were poor and there was no improvement either clini

Histoplasma capsulatum infections of the central nervous system. A clinical review.

Central nervous system manifestations occur in 10 to 20% of patients with disseminated histoplasmosis. Additionally, histoplasmosis may be the cause of cases of chronic meningitis in patients with no other evidence for dissemination. Histoplasmosis may also cause cerebral or spinal cord mass lesions resembling neoplasms or abscesses, and encephalitis. Diagnosis of chronic meningitis or mass lesions caused by H. capsulatum may be difficult and involves careful analysis of serologic tests for antibodies, cultures and tests for HPA in body fluids. Amphotericin B remains the treatment of choice, b

Detection of mycobacterial antigen in cerebrospinal fluid in patients with chronic meningitis by inhibition enzyme linked immunosorbent assay.

In a retrospective study, cerebrospinal fluid (CSF) specimens from 50 patients of chronic meningitis were investigated for tuberculous etiology by inhibition enzyme-linked immunosorbent assay (ELISA). CSF specimens from 10 patients of tuberculous meningitis in whom Mycobacterium tuberculosis was isolated, formed the positive control group of this study. The assay yielded positive results in CSF specimens for mycobacterial antigens in 8 of the 50 patients of chronic meningitis. No false positive results were recorded in 50 patients of non-meningitic (negative control group) neurological disease
