Relationship Between Benign Intracranial Tumor Location and the Side of Spontaneous Cerebrospinal Fluid Leak.

AIM: To present cases of spontaneous cerebrospinal fluid (CSF) rhinorrhea and otorrhea, and to conduct a systematic literature review examining the relationship between benign tumor location and the leak side. MATERIAL and METHODS: A PRISMA-based systematic review was conducted to evaluate studies on spontaneous CSF leaks caused by benign intracranial tumors, alongside three newly reported cases. RESULTS: All three cases exhibited CSF leaks ipsilateral to the tumor: Case 1 involved a pituitary ACTH-secreting macroadenoma with rhinorrhea; Case 2, a pineocytoma with hydrocephalus and nasal encep

Ruptured intracranial dermoid cyst as an incidental finding: Case report.

Intracranial dermoid cysts are rare, benign congenital lesions originating from ectodermal remnants. Though typically asymptomatic, their rupture, although infrequent, can lead to significant neurological complications, such as chemical meningitis, vasospasm, and hydrocephalus. Imaging with computed tomography (CT) and magnetic resonance imaging (MRI) plays a crucial role in diagnosis, providing characteristic features related to the high lipid content of these lesions. An 86-year-old female presented with a 3-day history of headache, nausea, vomiting, and facial cellulitis after an arthropod

Telovelar Approach for Fourth-Ventricular Epidermoid Cyst: Anatomical Respect, Functional Recovery, and Long-Term Stability.

Background and Clinical Significance: Fourth-ventricular epidermoid cysts are rare intracranial lesions. They account for fewer than 1% of all primary brain tumors. Fourth-ventricular epidermoid cysts grow slowly because they are closely related to brainstem, cerebellum, and major blood vessels, so their treatment requires special caution. Because the cyst capsule attaches to functionally sensitive locations, complete removal is usually not possible without compromising some aspect of brain or spinal cord function. Surgical decision-making always involves weighing the need to remove the entire

Ecchordosis physaliphora presenting with cerebrospinal fluid leak and meningitis: illustrative case.

BACKGROUND: Ecchordosis physaliphora (EP) is a rare notochordal remnant and the benign counterpart to chordomas with the same cell of origin. They are most commonly located intracranially along the posterior clivus in the prepontine cistern. EPs are typically discovered incidentally, but a small subset can present with symptoms, including a CSF leak. There are very few reported cases of EP-related meningitis, which occur after a short course of CSF leakage. OBSERVATIONS: A 48-year-old female with a history of clear rhinorrhea for several years presented with meningitis and a neurological decl

Prognostic significance of ACADL and MCM2 in meningiomas: Current evidence and clinical implications.

This review summarizes the current evidence on the expression and prognostic significance of acyl-CoA dehydrogenase long-chain (ACADL) and minichromosome maintenance 2 (MCM2) in meningiomas. Meningiomas, which comprise approximately 41% of intracranial tumors, are classified by the World Health Organization into 15 histological subtypes and three grades of malignancy. However, owing to the biological heterogeneity within tumors of the same grade, alternative classifications based on molecular features have been proposed. One of these classifies meningiomas into four prognostically relevant mol

Endoscopically Treated Third Ventricle Colloid Cysts: A Systematic Review of Surgical and Clinical Outcomes.

BACKGROUND/OBJECTIVES: Third ventricle colloid cysts (TVCCs) are benign lesions that may cause acute hydrocephalus and, rarely, sudden death. Endoscopic resection has emerged as a minimally invasive alternative to microsurgical approaches. This systematic review aimed to evaluate the safety and efficacy of endoscopic resection of TVCCs. METHODS: Following PRISMA guidelines, a systematic search of major databases was performed to identify studies reporting clinical outcomes of endoscopic resection of TVCCs. Extracted data included the surgical technique, extent of resection, complications, rec

Risk Factors for Delayed Leptomeningeal Dissemination in Choroid Plexus Papillomas: A Systematic Review and Illustrative Case.

PURPOSE: Choroid plexus papilloma (CPP) is a rare, benign intraventricular tumor that typically has an excellent long-term survival rate after gross total resection. Nevertheless, some patients develop delayed leptomeningeal dissemination (LMD) years after the initial diagnosis, a phenomenon that remains poorly understood. The authors encountered a case of CPP with delayed LMD 14 years after resection, which prompted this systematic review to identify potential risk factors. METHODS: A systematic review was conducted according to PRISMA guidelines to identify CPP reports with subsequent delay

PD-L1 positivity predicts a unique hyperaggressive tumor group within MenG C meningiomas.

Molecular profiling has identified 3 groups of meningiomas, with MenG C tumors exhibiting the vast majority of recurrences. Efforts to find effective treatments for recurrent meningiomas have remained elusive. Higher WHO-grade meningiomas have exhibited greater Programmed Death Ligand 1 (PD-L1) expression through various methods, but the prognostic value of PD-L1 expression has not been described in the context of molecular profiling. Additionally, trials investigating PD-1/PD-L1-targeted immunotherapies have produced disappointing results. Here, we find that PD-L1 positivity, while prevalent

Severe cord compression due to unique extradural meningeal melanocytoma of intermediate grade.

BACKGROUND: Meningeal melanocytomas are rare primary melanocytic tumors of the leptomeninges estimated to occur in 1 out of 10 million patients. They typically arise intracranially or within the intradural extramedullary spinal canal, while extradural presentations are uncommon. Here, we present a 36-year-old male whose intermediate-grade lesion (i.e., containing features of both benign melanocytoma and malignant melanoma) caused extradural C6-T2 cord compression relieved with laminectomy/fusion. CASE DESCRIPTION: A 36-year-old male presented with progressive right-hand paresthesias, weakness

Transnasal Endoscopic Approach for the Revision of Rathke's Cyst and Successful Long-Term Follow-Up: A Case Report.

Rathke's cleft cysts are benign epithelial-lined sellar and suprasellar lesions that are often incidental findings but can cause visual deficits, hypopituitarism, and headaches when they enlarge and compress adjacent structures. Recurrence after surgical treatment remains challenging despite various surgical approaches. Marsupialization has emerged as an alternative technique for complex recurrent cases, establishing permanent communication between the cyst and the nasal cavity while minimizing surgical risk. We present a case of a 76-year-old man with type 2 diabetes mellitus, a history of me

Incidental meningiomas have favorable clinical outcomes across molecular classification systems.

BACKGROUND: Clinical and radiographic models predict incidental meningioma growth, but their molecular architecture is unknown. METHODS: We analyzed serial magnetic resonance imaging, IMPACT groups (Incidental Meningioma: Prognostic Analysis Using Patient Comorbidity and MRI Tests), targeted gene expression, DNA methylation, and copy number (CNA) profiling of 238 consecutive incidental meningiomas from a single neurosurgical center. RESULTS: The cohort was 81% female, with a median age of 59 years at detection and median tumor volume of 3.83 cm³. Symptoms developed in 15.5%; 93.7% were treat

Metastatic Meningioma with Systemic Involvement: Discussion of Molecular, Genomic, and Radiological Features.

OBJECTIVE: Meningiomas are typically benign, slow-growing tumors localized to the cranial or spinal regions, with less than 1% metastasizing systemically, posing a serious clinical challenge. This case series adds critically needed data to broaden existing knowledge and demonstrates that early molecular profiling may identify patients at risk for systemic metastasis, informing surveillance strategies and treatment decisions. METHODS: Between 2012 and 2024, we retrospectively reviewed patients with meningiomas from our institution's electronic medical record. Clinical data, including imaging,

Stagnation and Progression: How Glymphatic Failure Promotes Meningioma Malignancy in Aging.

Meningiomas in the elderly present a clinical paradox: despite typically benign histology, they frequently manifest disproportionate peritumoral brain edema (PTBE) and aggressive behavior. Traditional hormonal hypotheses fail to explain this age-specific malignancy. This review articulates a "Neuro-Glymphatic-Tumor Axis" as a working hypothesis, proposing that the senescent collapse of brain clearance infrastructure, rather than tumor genetics alone, may be a key driver of this progression. Specifically, the loss of astrocytic aquaporin-4 (AQP4) polarity and the atrophy of meningeal lymphatic

Intramedullary or invasive characteristics of spinal melanocytomas: a PRISMA compliant systematic review and pooled case analysis.

PURPOSE: Primary meningeal melanocytomas (MMs) of the spinal cord are rare lesions. Even rarer are intramedullary and invasive variants. These intramedullary lesions may exhibit more aggressive behavior than their "benign" classification appreciates. The role of adjuvant radiotherapy (aRT) in this context remains uncertain. METHODS: This study is a PRISMA-compliant systematic review with a pooled-case analysis, including two recent institutional cases included as illustrative examples. RESULTS: Sixty-three reports were identified of primary intramedullary or invasive MMs. Among 56 patients w

Concurrent de novo glioblastoma and meningiomatosis: a case report and systematic review of clinical, molecular, and topographical characteristics.

INTRODUCTION: The co-occurrence of glioblastoma (GBM) and meningioma in a single patient is an exceptionally rare clinical phenomenon, often associated with prior irradiation or genetic syndromes. This study presents a unique case of concurrent de novo GBM and meningiomatosis and provides a systematic review of the literature to characterize this rare association. CASE PRESENTATION AND METHODS: A 78-year-old female with a known history of untreated meningiomatosis presented with refractory seizures and aphasia. Imaging revealed multiple stable extra-axial lesions and a new, distinct intra-axi

The role of tumor-associated macrophages in the progression and recurrence of meningiomas.

PURPOSE: Meningiomas are generally benign; however, they have high recurrence rates. The tumor microenvironment, particularly tumor-associated macrophages (TAMs) may play a role in tumor progression and recurrence. This study aimed to investigate the role of TAMs in tumor recurrence and patient survival in meningiomas, as well as their associations with clinicopathological and prognostic parameters. METHODS: A total of 146 primary meningiomas were evaluated. The presence of TAMs was examined morphologically in hematoxylin-eosin stained tissue sections. The clinicopathological and prognostic d

"Chipped but not broken" - patients' symptoms and challenges beyond five years post radiotherapy for meningioma: a qualitative interview study.

BACKGROUND: Meningioma, the most prevalent brain tumor in adults, is predominantly benign and often treated with radiotherapy and/or surgery, generally with positive treatment outcomes. Radiotherapy can be administered using either conventional radiotherapy (CRT) or proton beam therapy (PBT). The treatment can cause chronic symptoms such as headache, fatigue, and neurocognitive impairments that can significantly affect daily life. However, there is a lack of research on the long-term consequences of treatment, particularly concerning patients' own experiences of related symptoms. Therefore, th

Giant intraosseous (ectopic) meningioma of the temporoparietal calvarium with rapid expansion over 5 years: A case report.

RATIONALE: Intraosseous (ectopic) meningiomas are rare extradural tumors arising within the calvarial bone and account for <1% of all meningiomas. Because their imaging appearance may range from osteolytic to sclerotic or mixed patterns, they can mimic primary bone tumors or metastatic lesions and lead to diagnostic uncertainty. PATIENT CONCERNS: A 50-year-old woman presented with a gradually enlarging right temporoparietal scalp mass associated with mild headache for 5 years. The lesion caused marked cosmetic asymmetry without neurological deficit. DIAGNOSES: Serial cranial computed tomogra

Persistent meningeal enhancement on MRI in an infant with culture-negative bacterial meningitis: a case report and systematic review of the literature (2014-2025).

BACKGROUND: Bacterial meningitis (BM) in infants may be complicated by persistent meningeal enhancement on magnetic resonance imaging (MRI) after clinical cure-a phenomenon frequently misdiagnosed as infection recurrence. However, long-term follow-up data on asymptomatic persistent meningeal enhancement in young infants remain scarce. Here, we report a rare case of 5-month persistent meningeal enhancement following culture-negative bacterial meningitis (CNBM) in a 70-day-old infant, and systematically reviewed 17 eligible studies on meningeal enhancement in infantile bacterial meningitis publi

Recurrent herpes simplex virus type 2 Mollaret meningitis in a man living with HIV: A case report and patient-initiated episodic valacyclovir strategy.

Recurrent benign lymphocytic meningitis (RBLM) or Mollaret meningitis is an uncommon manifestation of herpes simplex virus type 2 (HSV-2) infection, characterized by self-limiting aseptic meningitis with complete recovery between episodes. Despite high background prevalence of HSV-2 in people living with HIV (PLWH), RBLM remains extremely rare in this population. Only two cases, both involving female patients, have been reported. We report the case of a man with HIV who experienced recurrent HSV-2 meningitis despite sustained virological suppression for HIV, preserved CD4 cell count at the tim
