The histological examination of a meningioma-like tumour operatively removed from the left frontoparetial region of a 4-months-old baby, disclosed a high-cellular malignant neoplasia built up by streams of elongated spindle-shaped cells, and with a predominant storiform-pattern as usually seen in malignant fibrous histiocytoma (MFH). Immunohistochemical reactions for alfa-I-ACT and Lys were negative; tumor cells expressed vimentin. Electronmicroscopy disclosed fibroblast-like cells. This tumour was compared with a recurring malignant fibroblastic meningioma characterised, in the recurrence, by

A 34-year-old Chinese woman underwent successful gross total excision of a benign fibroblastic meningioma, pre-operatively diagnosed by computed tomography (CT), arising from the left lateral ventricle of the brain. After a quiescent period of 5 years, CT and magnetic resonance imaging (MRI) demonstrated tumour recurrence at the same site. Despite two further excisions of progressively malignant intraventricular meningiomas and treatment with radiotherapy, the patient rapidly developed bilateral cerebellopontine angle and spinal drop metastases. CT and MRI were useful in demonstrating the loca

Since clear cell meningioma has only recently been recognized as a morphologic entity, its pathobiology has not been studied. Fourteen examples occurring in seven females and six males, ages 9 to 82 years (mean 29 years), were examined; one was associated with type 2 neurofibromatosis. Of these cases, seven (50%) were spinal-intradural (six lumbar, one thoracic), three (21%) arose in the posterior fossa (cerebellopontine angle), three (21%) were supratentorial, and one (7%) was centered upon the foramen magnum. In one case (8%), two tumors were considered to be independent primaries. One tumor

In recent years there has been considerable progress in brain tumor neuropathology. Several new diagnostic entities have been recognized, subclassification schemes have been modified, and new concepts on the histogenesis and cell biology of brain tumors have emerged. In 1993, a revised WHO classification of brain tumors was published by an international committee. This article summarizes the pertinent new aspects. As novel tumor entities, the central neurocytoma, the dysembryoplastic neuroepithelial tumor (DNT), desmoplastic infantile ganglioglioma (DIG) and pleomorphic xanthoastrocytoma (PXA)

CAVERNOUS SINUS SURGERY has been performed increasingly in the last 2 decades because of new knowledge and technologies. With increasing international expertise in cavernous sinus surgery, the results must be analyzed critically to search for accurate prognosticators of outcome. We performed a retrospective review of 124 patients (40 male, 84 female; mean age, 45 years) who underwent cavernous sinus surgery for benign tumors from 1983 to 1992. Sixty-five percent had tumors encasing the internal carotid artery. Mean follow-up was 29 months (median, 26 mo). Gross total or near-total resection wa

PURPOSE: To evaluate efficacy and tolerance of external fractionated radiation therapy (RT) in the treatment of intracranial meningiomas.
METHODS AND MATERIALS: From January 1981 to September 1993, 91 patients with intracranial meningiomas were treated with fractionated RT. Indications were as follows: (a) incomplete surgical resection, 29 patients; (b) tumor recurrences without considering the amount of the second resection, if performed, 14 patients; (c) completely excised angioblastic, aggressive benign, and anaplastic tumors, 8 patients; (d) medically inoperable and basilar tumors where o

Anatomical and biological studies of cavernous sinus meningiomas help us understand the biological heterogeneity of these tumors. The question of whether cavernous sinus meningiomas infiltrate cranial nerves is clinically important because of the effect on treatment planning. In the authors' experience of treating 36 patients with cavernous sinus meningiomas, tumor invasion into a cranial nerve was documented in two patients in whom a cranial nerve was resected during the cavernous sinus dissection. In both patients, histological examination using hematoxylin and eosin and bodian stains showed

Little is known of the molecular genetic mechanisms contributing to meningioma tumor progression. We evaluated a total of 26 clinical cases of meningioma: twenty three patients with meningioma treated at our institution between 1978 and 1990 and three asymptomatic cases found initially at autopsy. In addition, histologically normal meninges obtained at post-mortem examination from 5 cases were evaluated. There were 13 men and 10 women in the patient group with a median age of 48.7 years, treated by surgery and/or irradiation. Median follow-up was 46 months (range 16-152 months). Archival cases

Meningiomas, pituitary adenomas, and vestibular schwannomas are the most common benign brain tumors in adults. Together they account for 30% to 40% of primary brain tumors. Surgery remains the treatment of choice for most of these tumors. Recent advances in surgical techniques and intraoperative monitoring have decreased significantly the morbidity associated with surgery. For patients with inoperable or recurrent tumors, there is increasing evidence that standard external beam radiotherapy, stereotactic radiosurgery, and stereotactic radiotherapy all have a therapeutic role. These advances ha

We present the case of a man who died 11 years after the onset of xanthoma disseminatum. His course was marked by numerous, large recurrent intracranial dural-based xanthomatous tumors requiring repeated neurosurgical intervention. Late in his course there was progressive quadriparesis, ataxia, ophthalmoplegia and bulbar palsy as a result of intraparenchymal brainstem involvement. At autopsy, there was extensive meningeal and intraparenchymal CNS disease and widespread systemic infiltrates. The gross and microscopic pathology are presented with immunohistochemical, ultrastructural and biochemi

Results of radiotherapy in 21 patients who had been treated for intracranial meningioma from April 1975 to March 1993 at Tokyo Metropolitan Komagome Hospital are presented. All patients were treated with 4 MV X-rays using conventional fractionation. The median tumor dose was 54.4 Gy (range 30 Gy to 70 Gy). Radiotherapy was delivered by conformation technique in 11, parallel opposed field in four and three ports technique in two. Of 21 meningiomas, 13 were histologically malignant and 8 were benign. The median follow-up period was 63 months. Overall survival rate at 5 years was 71% for benign a

Meningioma is one of the popular benign brain tumors. However, the recurrence of this tumor is not infrequently encountered. In an attempt to establish the useful adjuvant therapy for the recurrent meningioma, in vitro chemosensitivity study for meningioma was conducted. Among various chemotherapeutic agents tested here, cisplatin showed highest cytotoxicity on cultured meningioma cells. In conclusion, cisplatin may be useful in adjuvant chemotherapy for the recurrent meningioma.

A case of meningeal melanocytoma of the left sphenoid wing is reported and the other nine cases in the literature are reviewed. Meningeal melanocytoma is a benign melanotic tumor that derives from the melanocytes of the leptomeninges and may occur anywhere in the cranial and spinal meninges. Electron microscopy well demonstrates melanin and melanosomes within the tumor cells. The immunohistochemical pattern of this tumor includes strong positivity for S-100 protein, vimentin, and antimelanoma antibody and negativity for epithelial membrane antigen, neuron-specific enolase, cytokeratin, and gli

Schwannomas of the spinal accessory nerve are rare lesions. They can be characterized by their locations as either intrajugular or intracisternal schwannomas, most of them being intrajugular. Only five cases arising in the cisterna magna are reported in literature. Clinical symptoms generally consist of 11th cranial nerve palsy associated with cerebellar signs and myelopathy. An additional case, studied by MRI, is described. The patient showed a mild cerebellar syndrome and normal pressure hydrocephalus symptoms. He was operated on and the mass completely removed with good postoperative result

Treatment objectives for meningiomas of the cranial base include relief of neurologic disability and prevention of clinical progression or recurrence with the least morbidity. Recent advances in skull base surgical techniques, through an appreciation of skull base anatomy and institutional specialization, have contributed major improvements to the outlook for patients with these tumors, and previously inoperable cases may now often be removed completely with acceptable risk. Since significant morbidity may be incurred during surgical resection of these difficult lesions, especially in terms of

Intraosseous calvarial meningiomas, defined as meningiomas confined to the skull with no epidural or subcutaneous component, are rare and usually exhibit osteoblastic activity. We present a case of a primary intraosseous meningioma in the right frontal bone that presented radiologically as a solitary lytic skull lesion and was completely excised. The lesion was benign without recurrence for 1 1/2 years. According to our review of the literature, our case is the first with documentation on computed tomography. In cases of solitary osteolytic skull lesions, meningioma should be considered even t

Recurrence of intracranial meningiomas after surgery has long been recognized, but there is still no consensus about factors responsible for recurrence. To better understand such factors, we analysed data on 276 patients with meningiomas who were treated at our institution from 1976 to 1990 (mean follow-up = 5.1 years). Effects of sex, tumour histology, tumour site, and radiotherapy on recurrence were closely studied. Using World Health Organization criteria to define malignancy, 254 of the tumours were benign and 22 were atypical or malignant. For data analysis, distinction was made between "

Meningioma is a common, usually benign, sporadic and solitary tumor of the meninges covering the central nervous system. Meningiomas can become malignant, and such anaplastic tumors are associated with a high rate of recurrence and death from the disease. We analyzed 16 sporadic, anaplastic meningiomas for loss of alleles on the majority of chromosomal arms, in order to define regions in the genome which may be important for tumor progression. Loss of genetic material was observed on all but 2 chromosomes studied. While loss of heterozygozity (LOH) from chromosome 22 was the most frequent find

Most meningiomas are benign and therefore curable lesions. Currently, the best available treatment is complete surgical resection. Toward this end, interventional neuroradiologists should contribute to the efficient and total removal of tumor mass. Although some controversy exists as to the value of the embolization of meningiomas involving the convexity, preoperative embolization certainly is of value in more complex presentations, including giant meningiomas, meningiomas exhibiting malignant or angioblastic characteristics, as well as those involving the skull base, scalp, or critical vascul

BACKGROUND: Argyrophilic nucleolar organizer region (AgNOR) has been demonstrated in recent studies of human brain tumors, including meningiomas. The authors used this technique in meningiomas to analyze whether the mean numbers of AgNOR per nucleus (AgNOR score) are related to the recurrence rate or the proliferative potential of meningiomas.
METHODS: AgNOR staining technique was applied to paraffin-embedded sections of 60 meningiomas. Twenty of these specimens also were investigated immunohistochemically with monoclonal antibody (MoAb) against DNA polymerase alpha (Pol.alpha) and with MoAb
