Meningioma.

Meningiomas more often behave as benign tumors although their very rate of recurrence is unknown. These neoplasms have been exhaustively studied but neither their subtypes nor their histologic features were ever considered as reliable data of their biologic behavior. Different types of assays have been achieved in order to assess the aggressiveness of these neoplasms. The first one concerned the mitosis index and different markers of proliferation. Other ways have been explored as the dual expression of the differentiation and the steroid receptors of the neoplasms, with no evidence of a clear

Mollaret's meningitis associated with herpes simplex type 2 infection.

We describe three patients with benign recurrent aseptic meningitis (Mollaret's meningitis). For one of these cases, the episodes of meningitis were associated with herpetic outbreaks. Mollaret cells, which are a hallmark of Mollaret's meningitis, were present in the CSF from two of the three patients. In all cases, herpes simplex virus type 2 DNA was present in the CSF during the acute illness as detected by polymerase chain reaction amplification, although viral cultures from CSF were all negative. Herpesviruses, notorious for frequent and sporadic recurrence, are ideal candidates for the ca

[Multiple intracranial and spinal meningiomas].

Meningeomas are regarded as benign tumors that as a rule can be totally removed. The great prevalence of the benign types of meningeomas does not exclude their recurrence. In some cases (6-8% in the recent literature) a solitary globular tumor represents only the most visible actual growth in context of a systemic tumoral disease spreading over a wide area of arachnoid with simultaneous or delayed growth. In a series of 188 patients operated in St. Gall because of meningeomas, 13 (8%) have multiple simultaneous or delayed tumors, in one case in the spine, without neurofibromatosis. Several ope

Melanotic neuroectodermal tumor of the skull and meninges in infancy.

Three cases of melanotic neuroectodermal tumors of infancy are presented. Two were localized on the midline, involving the skull and extending subdurally. One was located on the inner aspect of the dura and developed intracranially. Two had a benign course following gross total removal. One had a malignant course, recurring locally and spreading within the brain. The difficulties of removing these tumors when they are implanted on the midline are stressed. Histological features of prognostic value are pointed out. Further support for neural crest origin of these tumors is given.

An in vitro study of the effect of photodynamic therapy on human meningiomas.

Despite being benign CNS tumours, meningiomas are not always curable and the likelihood of recurrence depends upon the completeness of initial removal. Adjuvant therapy for incompletely resected meningiomas is generally unsatisfactory and such lesions continue to pose difficult management problems. Photodynamic therapy (PDT) has been employed in the management of recurrent cerebral gliomas but its activity against meningiomas has not been specifically studied. An in vitro study of the effects of PDT against a variety of meningiomas was therefore conducted. It was found that PDT using haematopo

Osseous metastases from a benign intraventricular meningioma. Case report.

The authors report a case of a 6 month-old male presenting with increasing head circumference and a large benign intraventricular meningioma which was grossly removed at the initial surgery. Twenty-four months later, the patient returned with subcutaneous and osseous metastatic lesions at the site of the previous craniotomy, revealing the same histology as the original tumour. The presentation, pathology and management of this unique case are discussed.

[Contribution of nucleolar organizers staining to the prognostic evaluation of meningioma].

Meningioma is often regarded as a benign tumour. It is easy to diagnose, but its prognosis is difficult to determine. Recurrences are numerous and occur at different periods of time. Various predictability factors have been studied, including Nucleolar Organizers Regions (NORs) staining. This technique is easy to perform and enables retrospective studies to be carried out. Comparisons of staining indices with the histoprognostic grades established by Zülch, and correlations between these indices, as well as with the time elapsed before a recurrence occurred, showed that NORs staining can be us

Activity of ornithine decarboxylase (ODC) and polyamine levels as biochemical markers of malignancy in human brain tumors.

The content of the polyamines putrescine, spermidine and spermine, and the activity of their metabolic key enzyme ornithine decarboxylase (ODC) were measured in tissue samples obtained during operation of 45 patients with primary or recurrent gliomas, meningiomas and pituitary adenomas. Biochemical analysis and histopathological classification were carried out in the same tumor samples. In benign tumors ODC activity was less than 10 nmol/g/h, whereas in malignant gliomas values up to 34 nmol/g/h were observed. In rapidly growing tumors pronounced heterogeneity was observed with high values in

Association of meningiomas with dural "tails"; surgical significance.

Intracranial meningiomas are characteristically benign tumours with a tendency to recur following surgical resection. Our group is investigating the pathogenesis of meningioma recurrence. In our initial studies we identified two cases of dural "tails" associated with intracranial meningiomas. Gadolinium-enhanced magnetic resonance images were utilized to identify the dural "tails" preoperatively. These images aided us in performing a more complete surgical resection of the meningiomas. Histopathological confirmation of meningotheliomatous cell infiltration into the dural "tails" demonstrates t

[Cranial vault meningioma. Apropos of 3 cases].

The authors report three different types of cranial bone lesions due to a meningioma of the convexity. This rare tumour is most often benign, with variable radiological appearances. The diagnosis is often suggested by tangential views of the cranial vault and CT scan but meningioma remains a histological diagnosis, notably in its primary intra-osseous form without actual meningeal involvement. The prognosis is good and the risk purely local, by vascular or cerebral compression. Treatment is surgical, sometimes complicated by recurrence.

The role of radiotherapy in the treatment of subtotally resected benign meningiomas.

Thirty-six patients with benign meningioma were treated for primary or recurrent disease by subtotal resection and external beam irradiation from 1968-1986 at Massachusetts General Hospital. Comparison is made with 79 patients treated by subtotal surgery alone from 1962-1980. Progression-free survival for 17 patients irradiated after initial incomplete surgery was 88% at 8 years compared with 48% for similar patients treated by surgery alone (p = 0.057). 16 patients incompletely resected at time of first recurrence were irradiated and 78% were progression-free at 8 years while 11% of a similar

[Orbital, osseous, meningeal and cerebral findings in oculodermal melanocytosis (nevus of Ota). Clinico-histopathologic correlation in 2 patients].

In two patients suffering from congenital oculodermal melanocytosis (nevus of Ota) orbital, osseous, meningeal, and cerebral manifestations are documented. In a 38-year-old European with a nevus of Ota on the right side an ipsilateral orbital tumor was diagnosed and surgically removed. The histological examination revealed a primary spindle shaped malignant melanoma of the orbit. In addition, many benign melanocytes were found in the optic nerve, in the extraocular muscles, in the orbital fat tissue, and in the dura of the sphenoid. In a 17-year-old Ethiopian with a nevus of Ota of the right s

Mifepristone (RU 486) treatment of meningiomas.

Meningiomas are common brain tumours which are generally benign, well circumscribed and slow growing. In a minority of patients complete surgical removal is not possible and re-growth of tumour tissue is a major clinical problem. Most meningiomas contain progesterone receptors. The anti-progestational drug mifepristone (RU 486) binds to these receptors. Ten patients were treated with 12 recurrent or primary "inoperable" meningiomas, all of whom had shown recent neuroradiological and/or ophthalmological evidence of tumour growth. They received 200 mg mifepristone daily for 12 months. Most patie

[Intrathecal dissemination of meningiomas; a case report].

A case of the cytologically verified intrathecal dissemination of benign meningiomas was reported. A 36-year-old man was admitted to our hospital because trans-sphenoidal surgery was planned for the repair of CSF rhinorrhea caused by a recurrent pituitary adenoma. The CT scan demonstrated incidental multiple tumors spread throughout in the pontomedullary and ambient cisterns. The head and spinal MRI showed more than fifteen small tumors in the posterior fossa and the thoracic spinal canal. CSF cytology revealed benign fibroblastic or meningotheliomatous meningioma with whorl formation and psam

Evaluation and emergency treatment of headache.

Headache is a common complaint in patients presenting to the emergency department. Most such headaches are benign, but some have a more severe organic cause. Occasionally, patients present with a chronic headache disorder with which they can no longer cope. The new International Headache Society Classification of Headache is reviewed along with the differential diagnosis of benign headache disorders. Headache diagnosis by history is examined in detail followed by a discussion of the emergency presentation of headache patients. Causes for concern are presented, along with a detailed discussion

[Argyrophilic nucleolar organizer region proteins (Ag-NORs) in human central nervous tumors].

A silver colloid staining technique for identifying nucleolar organizer region-associated proteins (Ag-NORs) was applied to 17 primary gliomas and 16 meningiomas. These comprised 8 glioblastomas, one pleomorphic xanthoastrocytoma, 8 benign astrocytoma, 10 nonrecurrent meningiomas and 6 recurrent meningiomas, in which the mean number of Ag-NORs' per nuclei, were 3.37, 2.34, 2.57, 2.16 and 2.80 respectively, identifying significant differences among these groups. Ag-NORs of four giant cell glioblastomas showed low numbers of Ag-NORs, suggesting better prognosis. In spite of the time consuming an

Histologically benign recurrent meningioma metastasizing to the parotid gland: case report and review of the literature.

This report presents a very rare case of metastatic meningioma of the parotid gland from a recurring intracranial lesion. The primary tumor, intracranial residues, and parotid metastasis were histologically benign. Meningiomas rarely metastasize; even rarer are cases in which both the primary and the secondary tumors have benign histological characteristics. The 11 cases reported in the literature have been critically reviewed. The case we present is noteworthy also for the exceptional localization of the metastasis in the parotid gland.

Rapid detection of proliferating potential in human brain tumors by nucleolar organizer region staining on squash preparations.

Rapid detection of the proliferating potential of 37 human brain tumors was attempted using squash preparations stained by a silver colloid technique for argyrophilic protein associated with nucleolar organizer regions (AgNORs). Less than 1 h was required for staining. The mean number of AgNORs in cell nuclei of malignant or recurrent brain tumors (16 cases) including meningeal sarcoma, recurrent meningioma, recurrent craniopharyngioma, anaplastic astrocytoma, glioblastoma multiforme and metastatic brain tumor was 3.18, and the number for benign brain tumors (21 cases) including meningioma, ne
