Three patients who presented with intracranial meningiomas 12, 15, and 20 years, respectively, after therapeutic high-dose irradiation of a primary brain tumor are described. Analysis of these cases and similar documented cases suggests that meningiomas after high-dose irradiation constitute a recognizable entity. A patient presented with a histologically benign intracranial meningioma which, after multiple recurrences, underwent malignant transformation. The patient survived 18 years following initial presentation. Pulmonary metastases were present over the final 8 years. Eleven subtotal resections of the meningioma were performed, including exenteration of the right eye and a thoracotomy. Radiation therapy and chemotherapy were relatively ineffective. The problem and treatment of recurrent meningiomas is briefly reviewed.

The incidence of intracranial meningioma in Manitoba, Canada, was reviewed from 1980 through 1987. During that time, 193 tumors were diagnosed, with a male:female ratio of 1:2. Pre-operative embolization of juvenile nasopharyngeal angiofibroma was performed in 15 patients. The lesion was supplied by the internal carotid arteries (8 cases), by the internal maxillary artery (15 cases), the accessory meningeal artery (10 cases) and the ascending pharyngeal artery (10 cases). Since its first description by Kepes in 1979, pleomorphic xanthoastrocytoma (PXA) has been considered a tumor with a benign course. Two cases are presented here that support the concept that PXA may be more accurately considered part of a spectrum of astrocytomas that occasionally may act aggressively. Total surgical excision is the main goal of therapy for intracranial meningiomas. The controversy today involves the efficacy of postoperative radiation therapy. To evaluate this question, 132 patients with benign intracranial meningiomas, treated between October 1964 and April 1985, were evaluated. All patients had a minimum 2-year follow-up. A case of benign meningioma of the superior sagittal sinus is reported, which recurred three times during a period of six years. At the time of the first recurrence the overlying bone was eroded and the tumor grew subcutaneously; the second recurrence was associated with a pulmonary metastatic deposit of extraordinary size. A series of 21 patients with atypical and malignant meningiomas is presented. Histological criteria such as high cellularity, typical and atypical mitosis, necrosis, infiltration of the underlying brain, poor differentiation, and distant metastasis define nonbenign meningiomas. 536 meningioma cases were examined in order to assess incidence in terms of sex age, site and recurrences and to discover whether recurrence was related to the site, dimensions or histological features of the tumour. The data obtained indicate that in order to assess the biological behaviour of meningiomas, their histological features, accessibility and dimensions must be borne in mind. In fact benign tumours quickly recur if they are large or inaccessible. In contrast long-term recurrences are probably due to the presence of included cells as reported in the literature.

Proliferative activity of meningiomas was examined in 12 consecutive cases by administering bromodeoxyuridine (BrdU) before surgical removal and by immunohistochemical staining of the removed tumours using anti-BrdU monoclonal antibody (anti-BrdU MAb) to detect BrdU-labelled tumour cells. Our experience with benign and malignant intracranial meningiomas between 1970 and 1983 is reported. Fourteen cases were treated after surgery, 10 benign and four malignant, following complete or incomplete resection or recurrence after resection. Two of 10 benign meningiomas have recurred and one of the two has been controlled by reoperation.
