Mollaret's meningitis or the benign recurrent meningitis syndrome is a rare disorder not previously described in Africa. The syndrome has a characteristic clinical presentation with spinal fluid pleocytosis, often with unusual 'epithelial' cells. With contemporary techniques no causative organism has been incriminated. The aetiology remains speculative, but we report on a patient found to have a cerebrospinal fluid leak, which may represent a factor in the pathogenesis of this disorder.

A 2-year-old girl with extensive cutaneous xanthogranuloma and recurrent subdural effusions has been investigated. Differentiation from histiocytosis X depended upon light and electron microscopical studies of the cutaneous infiltrate and cells obtained from the cerebrospinal fluid. The recurrent subdural effusions responded well to cranial irradiation. Although juvenile xanthogranuloma is generally a benign cutaneous disorder, a spectrum of naevoid proliferative changes involving systemic organs may occur.

The records of 15 patients with a diagnosis of malignant meningioma were reviewed. In one of these patients, in whom invasion of the brain and pituitary gland was the only unusual feature, the tumor was reclassified as benign. A report is given on long-term results after the removal of spinal mengiomas and neurinomas. A total of 150 patients with meningiomas and 80 patients with spinal neurinomas were operated on within a period of 25 years at the Neurosurgical Clinic in the Division Medicine of the Karl-Marx-University Leipzig. A series of 126 patients with meningioma, operated upon 7-17 years ago, is reviewed as to the recurrence of the tumour. The overall rate of recurrence of the histologically benign lesions was 29 per cent. The most important factors influencing the prognosis were the site of the tumour and the degree of radicality of excision. A total of 516 charts of children who presented to the Oklahoma Children's Memorial Hospital Emergency Department or Walk-In Clinic with a temperature of 40 C(104 F) or more were reviewed. Hemangiopericytomas of the meninges are rare tumors. Three tumors of this type with a course over more than 10 years each are reported. All three tumors were primary diagnosed as meningiomas (one: vascular, two angioblastic). Three cases of benign, recurrent meningitis of Mollaret demonstrated unusual aspects of this disease. Postpartum pituitary necrosis was an antecedent event in one case. A second case occurred in an otherwise healthy 82-year-old woman, and a third patient was thought to have Histoplasma meningitis. The pathophysiology of Mollaret meningitis remains an enigma, and the therapy consists of supportive care.
