Host genetic determinants of HTLV-1 infection outcomes.

Human T-cell lymphotropic virus type 1 (HTLV-1) is a persistent human retrovirus associated with severe outcomes, most notably adult T-cell leukemia/lymphoma (ATLL) and HTLV-1-associated myelopathy/tropical spastic paraparesis (HAM/TSP). Despite lifelong infection, only a minority of carriers develop clinical disease, highlighting a decisive role for host genetic background in shaping viral control, immune activation, and pathogenic progression. This mini-review synthesizes evidence demonstrating how inherited host genetic variation and acquired somatic alterations influence HTLV infection out

Experiences from Two Decades of HTLV-1/2 Testing in a Low-Prevalence Area in Southern Germany, 2004 to 2024.

Human T-cell lymphotropic virus type 1 (HTLV-1) is a neglected pathogen with a heterogeneous global distribution. In low-prevalence areas, diagnostic testing is challenged by limited clinical awareness and the low positive predictive value of screening tests. We retrospectively analyzed 10,891 HTLV-1/2 test results from 7719 patient samples submitted to a specialized laboratory in Germany between 2004 and 2024. Antibody screening had a positive predictive value (PPV) of 31.7% for the Diasorin Murex HTLV I + II assay (39 of 123 reactive samples confirmed) and 34.5% for the Abbott Architect rHTL

CRISPR-mediated excision of HTLV-1 reduces proviral loads in PBMCs from HAM/TSP patients.

CRISPR technology is emerging as a promising therapeutic approach for eliminating chronic viral infections, such as herpesviruses and HIV. Here, for the first time, we demonstrate in vitro that CRISPR can be used to excise the HTLV-1 genome and reduce proviral loads in PBMCs from HAM/TSP (HTLV-1-associated myelopathy/tropical spastic paraparesis) patients. Single treatment with CRISPR-RNP (ribonucleoprotein) complexes composed of two gRNAs targeting the HTLV-1 env gene and 3'LTR sequences resulted in excision of a 2613 bp segment of the proviral genome, spanning tax and HBZ genes, without dete

Emerging Risk for Human T-Cell Leukemia Virus Type 1 Transmission with HIV-Positive Breastfeeding Support.

Human T-cell leukemia virus 1 (HTLV-1) is a neglected retrovirus affecting 5-10 million persons worldwide. Most infections are asymptomatic, but HTLV-1 can cause adult T-cell leukemia or lymphoma and HTLV-1-associated myelopathy. Although mother-to-child transmission through breastfeeding is preventable, few countries have policies that include antenatal screening. The World Health Organization recommends integrating HTLV-1 into HIV and sexually transmitted infection strategies. HIV guidelines in high-income countries increasingly support breastfeeding under controlled conditions, creating inc

Case of concurrent ALS and human T-cell leukaemia virus type 1-associated myositis.

A woman in her late 70s presented with progressive limb weakness, muscle atrophy and hyper-reflexia. Laboratory findings revealed elevated creatine kinase and positive serum human T-cell leukaemia virus type 1 (HTLV-1) antibody. Clinical and electrophysiological findings met revised El Escorial criteria for amyotrophic lateral sclerosis (ALS), but muscle MRI showed inflammatory changes. Muscle biopsy revealed both neurogenic and inflammatory features. While methylprednisolone showed no benefit, intravenous immunoglobulin therapy produced transient improvement in weakness with normalisation of

Dimethyl fumarate as a promising therapeutic candidate for virus-associated myelopathy.

Human T-cell lymphotropic virus type 1 (HTLV-1)-associated myelopathy/tropical spastic paraparesis (HAM/TSP) is a chronic, progressive neuroinflammatory disease with no effective treatment. In this study, we investigated whether dimethyl fumarate (DMF), an immunomodulatory agent approved for treating multiple sclerosis, exerts therapeutic effects relevant to HAM/TSP. Peripheral blood mononuclear cells (PBMCs) from 16 people living with HAM/TSP were used to evaluate the effects of DMF on cell viability, spontaneous proliferation, inflammatory cytokine production and HTLV-1 proviral load (PVL).

HTLV-1-associated myelopathy as a translational model of progressive neurodegeneration.

Human T-lymphotropic virus type 1 (HTLV-1)-associated myelopathy/tropical spastic paraparesis (HAM/TSP) and progressive forms of multiple sclerosis are chronic CNS diseases characterized by persistent inflammation and progressive neurodegeneration. Although triggered by distinct upstream events [persistent HTLV-1 infection in HAM/TSP and autoimmune mechanisms in multiple sclerosis, with increasing evidence implicating Epstein-Barr virus (EBV) in multiple sclerosis initiation], both conditions converge on shared inflammatory and neurodegenerative cascades. In HAM/TSP, a hierarchical Tax-MAP3K8-

Integrative analysis of ATAC-seq and RNA-seq for cells infected by human T-cell leukemia virus type 1.

Human T-cell leukemia virus type 1 (HTLV-1) causes adult T-cell leukemia (ATL) and HTLV-1-associated myelopathy (HAM) after a long latent period in a fraction of infected individuals. These HTLV-1-infected cells typically have phenotypes similar to that of CD4+T cells, but the cell status is not well understood. To extract the inherent information of HTLV-1-infected CD4+ cells, we integratively analyzed the ATAC-seq and RNA-seq data of the infected cells. Compared to CD4+T cells from healthy donors, we found anomalous chromatin accessibility in HTLV-1infected CD4+ cells derived from ATL cases

Exploring the impact of interferon-gamma single nucleotide polymorphisms on HTLV-1 infection: Unraveling genetic influences in viral pathogenesis.

Human T-lymphotropic virus-1 (HTLV-1) induces neoplastic adult T-cell leukemia/lymphoma (ATLL) and neurological HTLV-1 associated myelopathy (HAM) in approximately 3 %-5 % of infected individuals. The precise factors that facilitate disease manifestation are still unknown; interaction between the virus and the host's immune response is key. Cytokines regulates physiological activities and their dysregulation may initiate the pathogenesis of various malignant and infectious diseases. Genetic variations, particularly polymorphisms in gene regulatory regions, lead to varying cytokine production p

Incidence of HTLV-1-associated myelopathy in the UK from 1991 to 2024: a longitudinal observational cohort study.

INTRODUCTION: Human T-cell lymphotropic virus type 1 (HTLV-1) may cause spinal cord inflammation, leading to HTLV-1-associated myelopathy (HAM). HAM is a chronic and progressive neurological disorder that is associated with increased mortality and impaired quality of life. There are limited data on the incidence of HAM, with higher rates seen in Latin America and the Caribbean compared to Japan. We investigated the incidence of HAM in a cohort of initially asymptomatic people with HTLV-1 in the United Kingdom (UK). METHODS: This is a longitudinal retrospective observational study of people wi

Human T-Lymphotropic Virus Screening of Blood Donations in England Between 2002 and 2021-Comparison of Screening Strategies.

BACKGROUND: Human T-lymphotropic virus (HTLV) is associated with adult T-cell leukemia/lymphoma and myelopathy. Here we present virological and epidemiological data on HTLV screening of blood donations in England between 2002 and 2021, implemented to prevent its transmission via blood transfusion. METHODS: Data on HTLV testing of blood donations was reviewed; it was initially conducted in pools (2002-2012) and subsequently using individual samples (all donors, 2013-2016; first-time donors and non-leucodepleted component donors, 2017-2021). Data included annual number of donations screened, in

Human T-cell leukemia virus type 1 (HTLV-1) is a retrovirus responsible for adult T-cell leukemia/lymphoma (ATLL) and HTLV-1-associated myelopathy/tropical spastic paraparesis (HAM/TSP), a progressive neurodegenerative disease. Regulation of viral gene expression plays a key role in viral persistence and pathogenesis. However, the molecular mechanisms underlying this fine-tuned regulation remain poorly understood. Little is known regarding RNA chemical modifications of HTLV-1 RNA and how these affect viral biology and disease development. Post-transcriptional chemical modification of RNA is co

Trends of changes in human T-cell leukemia virus type 1 epidemiology in Japan and globally.

Human T-cell leukemia virus type 1 (HTLV-1) has been identified as a cause of adult T-cell leukemia-lymphoma (ATL) and HTLV-1-associated myelopathy/tropical spastic paraparesis (HAM/TSP). Despite several HTLV-1 endemic areas being identified, comprehensive investigations have yet to be conducted in all the regions of the world. This review aims to summarize the current reports on HTLV-1. As vertical transmission is known to be a risk factor for ATL development, prevention strategies have been initiated in Japan, and these efforts may be related to the decrease in the estimated number of HTLV-1

First Characterization of HTLV-1 Complete Genomes from Asymptomatic and Symptomatic Argentinean Patients.

HTLV-1 is the etiologic agent of adult T-cell leukemia/lymphoma (ATL/ATLL) and is related to HTLV-1-associated myelopathy/tropical spastic paraparesis (HAM/TSP). The factors that influence the development of the disease remain unknown. The majority of patients with HTLV-1 infection remain asymptomatic throughout their lives, while 2.5%-5% developed ATLL and 0.3%-2% HAM/TSP. About 10 to 20 million people worldwide are infected with HTLV-1. In Argentina, HTLV-1 infection has been documented particularly in the northeast provinces neighboring Bolivia and Paraguay. This study aims to analyze the n

Development of a new high-yield integration site assay reveals disease-specific patterns across HTLV-1-associated pathologies.

Human T lymphotropic virus type 1 (HTLV-1) chronic infection is maintained through mitotic proliferation of the infected CD4+ T cells, where the viral genome is integrated as a provirus in its host genome. HTLV-1 integration sites (ISs) have a part in HTLV-1-associated pathologies, with distinct IS patterns associated with malignant proliferation or inflammatory diseases. However, IS determination remains challenging because most assays rely on complex biological and biocomputing protocols. We present an IS assay that solely relies on PCR and Sanger sequencing, which allowed HTLV-1 IS determin

HTLV-1-infected cells drive the differentiation of monocytes into macrophages in vitro.

BACKGROUND: The human T-cell lymphotropic virus type 1 (HTLV-1) is a retrovirus that causes HTLV-1-associated myelopathy/tropical spastic paraparesis (HAM/TSP). HAM/TSP is a chronic inflammatory neurodegenerative disease characterized by leukocyte infiltration in the spinal cord. T-lymphocytes are the most important targets of HTLV-1 infection, but monocytes are also infected. Monocytes from HTLV-1-infected individuals exhibit important functional differences compared to cells from uninfected donors. Here, we investigated the effects of cell-cell physical contact and/or secreted factors of HTL

Dysregulation of the tumor suppressor Menin and its target Bach2 in HTLV-1 infection.

BACKGROUND: The tumor suppressor Menin, prone to mutations in both hereditary and sporadic endocrine tumors, along with its direct target Bach2, plays a crucial role in preventing autoimmunity by regulating CD4 + T cell senescence and maintaining cytokine homeostasis. Since human T-cell leukemia virus type 1 (HTLV-1) primarily infects CD4 + T cells, and its dysregulation contributes to both the hematological malignancy of adult T-cell leukemia/lymphoma (ATL) and HTLV-1-associated myelopathy/tropical spastic paraparesis (HAM/TSP), we examined the involvement of the Menin-Bach2 pathway in HTLV-1
