Silent dissemination of HTLV-1: evidence of intrafamilial transmission in a Brazilian reference centre.

BACKGROUND: The HTLV-1 affects 5 to 10 million people worldwide. It is estimated that 5 to 10% of the infected individuals develop severe diseases, such as HTLV-Associated Myelopathy/Tropical Spastic Paraparesis (HAM/TSP) or Adult T-Cell Leukaemia/Lymphoma (ATLL). HTLV-1 transmission can occur mainly through unprotected sexual contact and from mother to child during breastfeeding. No vaccines can contain this infection, and strategies to prevent transmission become a priority. Therefore, it is important to know the main dissemination routes of each region to design the best public health strat

HTLV-1 p13 Protein Hijacks Macrophage Polarization and Promotes T-Cell Recruitment.

The human T-cell leukemia type-1 (HTLV-1) retrovirus establishes chronic life-long infection in a fraction of infected individuals associated with severe pathological conditions. Although the mechanism driving disease development is not fully understood, current evidence indicates the essential functions of viral regulatory proteins. Among these, the p13 protein has previously been shown to localize to the inner mitochondrial membrane in T cells, altering mitochondrial biology and T-cell function. While CD4+ T cells are the primary cell target of HTLV-1 infection, genomic viral DNA has also be

Prevalence and first detection of HTLV-2 in a pregnant woman in rural Brazil.

Human T-lymphotropic virus (HTLV) is a retrovirus with two subtypes, HTLV-1 and HTLV-2. HTLV-1 is linked to severe diseases such as adult T-cell leukemia/lymphoma (ATLL) and HTLV-associated myelopathy, while HTLV-2 is considered less pathogenic but can cause fibromyalgia, hairy cell leukemia, and neurological disorders. Although HTLV-2 is less prevalent than HTLV-1, it is common in specific populations, such as indigenous groups and injection drug users. In Brazil, an estimated 200,000-250,000 people are infected with HTLV-2, mostly in the North, yet data on its prevalence in pregnant women re

Virus-induced RGMa expression drives neurodegeneration in HTLV-1-associated myelopathy.

Human T-lymphotropic virus type 1-associated (HTLV-1-associated) myelopathy (HAM, also known as tropical spastic paraparesis) is a rare neurodegenerative disease with largely elusive molecular mechanisms, impeding targeted therapeutic advancements. This study aimed to identify the critical molecule responsible for neuronal damage in HAM, its source, and the regulatory mechanisms controlling its expression. Utilizing patient-derived cells and established cell lines, we discovered that HTLV-1 Tax, in conjunction with specificity protein 1 (Sp1), enhanced the expression of repulsive guidance mole

Clinical and molecular evidences of HTLV-1 infection in inpatients diagnosed with diseases previously described as associated to this infection: A case series in Gabon, Central Africa.

The Human T-cell Leukemia Virus Type 1 (HTLV-1) infection is endemic in Gabon, and despite the high prevalence, very few cases of HTLV-1 associated diseases are sporadically described. A cross-sectional study was conducted in the main teaching hospital of Gabon. Using serological and molecular techniques, plasma samples were tested and nucleic materials of all positive samples extracted from the buffy coat, then a polymerase chain reaction (PCR) was performed to detect fragments of 220 and 522-base-pairs of HTLV-1 Tax/Rex and Env genes, respectively. From March to September 2022, a total of 35

Human T-Lymphotropic Virus (HTLV): Epidemiology, Genetic, Pathogenesis, and Future Challenges.

Human T-lymphotropic viruses (HTLVs) are deltaretroviruses infecting millions of individuals worldwide, with HTLV-1 and HTLV-2 being the most widespread and clinically relevant types. HTLV-1 is associated with severe diseases such as adult T-cell leukemia/lymphoma (ATL) and HTLV-1-associated myelopathy/tropical spastic paraparesis (HAM/TSP), while HTLV-2 shows a lower pathogenic potential, with occasional links to neurological disorders. HTLV-3 and HTLV-4, identified in Central Africa, remain poorly characterized but are genetically close to their simian counterparts, indicating recent zoonoti

Development and validation of a multienzyme isothermal rapid amplification and lateral flow dipstick combination assay for HTLV-1 proviral DNA detection.

BACKGROUND: Human T-cell lymphotropic virus-1(HTLV-1) infection may lead to adult T-cell leukemia, HTLV-associated myelopathy/tropical spastic paraplegia, and various neurological diseases. Therefore, developing a rapid and accurate detecting method is crucial for preventing and controlling HTLV-1 infection. This study aims to develop and validate a detection assay for HTLV-1 proviral DNA by combining multienzyme isothermal rapid amplification(MIRA) and lateral flow dipstick(LFD) techniques. METHODS: Primers and probes were designed based on the conserved sequence of the HTLV-1 Tax gene, and

An HTLV-1 carrier with sporadic late-onset nemaline myopathy accompanied by skin lesions indicating indolent adult T-cell leukemia/lymphoma and Sjögren's syndrome: a case report and literature review.

Sporadic late-onset nemaline myopathy (SLONM) is a rare acquired myopathy. Herein, we report a rare case of co-occurring SLONM, indolent adult T-cell leukemia/lymphoma (ATL), and Sjögren's syndrome (SS) in a 60-year-old woman who was a human T-cell leukemia virus type 1 (HTLV-1) carrier. She presented with sub-acute proximal muscle weakness and chest erythema. Muscle biopsy revealed perivascular inflammation and accumulation of nemaline bodies in the atrophic fibers. Skin biopsy indicated indolent ATL. Analysis of specific antibodies and salivary gland biopsy led to a SS diagnosis. CD4-positiv

Untangling the causes of geographical disparities in the clinical outcome of HTLV-1 infection: a critical perspective on the contribution of viral genetic diversity.

Human T-cell leukemia/lymphoma virus type 1 (HTLV-1) is the etiological agent of two major diseases, adult T-cell leukemia/lymphoma (ATLL) and HTLV-1-associated myelopathy/tropical spastic paraparesis (HAM/TSP), as well as various other inflammatory manifestations. The prevalence of HTLV-1 infection varies greatly from region to region, and available epidemiological data support the notion that even among regions of similarly high prevalence of HTLV-1 infection, such as Japan, South America, or Central Australia, the incidence of each associated disease may vary drastically. Here, we criticall

Machine learning based characterization of high risk carriers of HTLV-1-associated myelopathy (HAM).

HTLV-1-associated myelopathy (HAM) develops in a part of HTLV-1-infected individuals while most of the individuals remain asymptomatic. This complicates the identification of HTLV-1 carriers at elevated risk. In this study, we integrated HTLV-1 proviral load and antibody titers against Tax, Env, Gag p15, p19, and p24 proteins in a machine learning (ML) framework to identify and characterize high-risk individuals likely to develop HAM. We stratified asymptomatic carrier samples employing an anomaly detection model. We further developed and validated classifier models capable of distinguishing t

Magnetic resonance imaging in the diagnosis of HTLV-1-associated myelopathy.

The main neurologic manifestation of definitive human T-lymphotropic virus 1 (HTLV-1)-associated myelopathy (HAM) is spastic paraparesis, but it only occurs in 5% of the patients. In contrast, about 40% of HTLV-1-infected subjects present symptoms of urologic dysfunction, including nocturia, urgency, and incontinence, which may progress to an inability to void urine. As these patients do not present motor dysfunction, they are classified as probable HAM. Atrophy of the thoracic spinal cord (SC) is the main abnormality found on magnetic resonance image (MRI) scans of patients with definitive HA

High TCR Degeneracy Enhances Antiviral Efficacy of HTLV-1-Specific CTLs by Targeting Variant Viruses in HAM Patients.

T-cell receptors (TCRs) exhibit degeneracy, enabling individual TCRs to recognize multiple altered peptide ligands (APLs) derived from a single cognate antigen. This characteristic has been involved in the pathogenesis of autoimmune diseases through cross-reactivity between microbial and self-antigens. Cytotoxic T lymphocytes (CTLs), which recognize peptide-MHC class I complexes via TCRs, play a critical role in the immune response against viral infections. However, the extent to which TCR degeneracy within a population of virus-specific CTLs contributes to effective viral control remains poor

Human T-lymphotropic virus-1 proviral load among patients on maintenance hemodialysis.

INTRODUCTION: Human T-lymphotropic virus type 1(HTLV-1) is a complicated leukemogenic retrovirus, it is the first oncogenic virus discovered and identified as the leading cause of T-cell malignancy and HTLV related myelopathy with a long latency period. Transmission via infected blood products is one of the common routes of HTLV-1 infection, and hemodialysis patients are potentially more vulnerable to blood-borne viral infections such as HTLV. This study aimed to determine the factors associated with HTLV-1 proviral load among hemodialysis patients. MATERIALS AND METHODS: A cross-sectional st

HTLV-1 in Pregnancy and Neonatal Health: Evidence, Challenges, and Future Directions.

Human T-cell lymphotropic virus (HTLV), a retrovirus associated with severe conditions such as leukemia/lymphoma and myelopathy, exhibits variable global prevalence, with higher rates observed in regions such as northeastern Brazil and sub-Saharan Africa. While intrauterine transmission can occur via viral expression in placental tissue and contact with umbilical cord blood, the predominant route is vertical transmission through breastfeeding. Diagnostic testing, particularly serological screening with ELISA and confirmatory methods such as Western blot and PCR, is essential for early detectio

Human T-cell leukemia virus type 1: oncogenic potential and vaccine development strategies.

The human T-cell lymphotropic virus type 1 (HTLV-1) is a highly oncogenic retrovirus recognized as the causative agent of adult T-cell leukemia/lymphoma (ATLL) and HTLV-1-associated myelopathy/tropical spastic paraparesis (HAM/TSP). Among the key risk factors for ATLL development are high proviral load, reduced anti-Tax immune responses, and elevated levels of soluble interleukin-2 receptor. Unlike classical oncogenic viruses, HTLV-1 does not encode proto-oncogenes but instead drives cellular transformation through a combination of mechanisms, including viral gene dysregulation, chromatin remo

Human T-cell leukemia virus type I: modulation of viral gene expression and perturbation of host signaling pathways lead to persistent infection.

Human T-cell leukemia virus type I (HTLV-1) was the first human pathogenic retrovirus to be discovered. HTLV-1 induces a T-cell malignancy, adult T-cell leukemia-lymphoma (ATL), and inflammatory diseases, such as HTLV-1-associated myelopathy (HAM), HTLV-1 uveitis (HU), and HTLV-1-associated pulmonary disease (HAPD). Importantly, HTLV-1 maintains persistent infection by regulating viral gene expression and disrupting host signaling pathways - activities that are closely linked to its pathogenicity. By modulating the expression of viral genes, including tax and HTLV-1 bZIP factor (HBZ), HTLV-1 e

An HTLV-1-Infected Humanized Mouse Model Expressing HLA-A*02:01 Demonstrates Effective CTL-Mediated Suppression of HTLV-1.

Human T-cell leukemia virus type 1 (HTLV-1) establishes lifelong infection and is associated with severe diseases such as adult T-cell leukemia/lymphoma (ATL) and HTLV-1-associated myelopathy/tropical spastic paraparesis (HAM/TSP). Cytotoxic T lymphocytes (CTLs), especially those specific for the viral protein Tax, play a pivotal role in controlling HTLV-1 infection. However, conventional humanized mouse models fail to fully reconstitute human immune responses, limiting their utility for evaluating CTL-mediated immunity. This study aimed to establish a physiologically relevant in vivo model to
