Xuefu Zhuyu Capsule alleviates depression in post-stroke depression model rats via modulation of the gut microbiota-gut-brain axis.

Xuefu Zhuyu Capsule (XFZY) demonstrated potential in alleviating post-stroke depression (PSD), a condition whose underlying mechanisms may involve the gut-brain axis. This study aimed to explore the therapeutic effects of XFZY on PSD and its possible modulation of the gut microbiota-gut-brain axis in a rat model. Wistar rats were randomly assigned to sham, PSD, three XFZY dose (0.216, 0.432, 0.864 g/kg), and fluoxetine (1.80 mg/kg) groups (n = 12 per group). The PSD model was established using transient middle cerebral artery occlusion (t-MCAO) combined with chronic unpredictable mild stress (

Unknotting the nexus of asthma and neuroinflammation: from brain network alterations to therapeutic implications.

Asthma is increasingly recognized as a systemic inflammatory syndrome that extends beyond the respiratory tract, with emerging evidence highlighting its relevance to neuroinflammation. The lung-brain axis-via interconnected inflammatory, vascular, metabolic, neural, and microbial pathways-provides a framework for understanding how chronic pulmonary disease may sustain or exacerbate neuroinflammatory processes. Mechanistically, asthma promotes blood-brain barrier disruption, systemic inflammation that seeds central neuroinflammation, oxidative stress, mitochondrial dysfunction, gut-lung-brain m

Wrist Ganglion Cyst Localization in Relation to Cyst Size and Associated Injuries: An MRI-Based Retrospective Study.

BACKGROUND: Three siblings with goitre and latent to mild hypothyroidism were suspected of having thyroid peroxidase (TPO) abnormality. Direct sequencing of their genomic DNAs showed two novel mutations of the TPO gene, one of which was G1687T (Gly533Cys; exon 9) and the other 1808-13del (Asp574/Leu575del; exon 10). The two mutations were compound heterozygous, as the former was found in their father's DNA as heterozygous, and the latter was found in DNA from their mother, also as heterozygous. As Gly533 and Asp574/Leu575 were well-conserved amino acids in the peroxidase superfamily, Gly533Cys

Therapeutic Potential of Berberine in Obesity-Associated Neuroinflammation Through Shared Molecular Pathways: TLR4/NF-κB/MAPK, ROS/NRF2 and NLRP3.

Obesity is recognized as a pathological condition that induces chronic low-grade systemic inflammation capable of affecting multiple organs, including the central nervous system, thereby promoting neuroinflammation. Although adipose and neural tissues differ in their structural and functional characteristics, they share common inflammatory mechanisms involving the TLR4/NF-κB/MAPK signaling pathways, the ROS/NRF2 axis, and the NLRP3 inflammasome. This review summarizes current evidence demonstrating the ability of berberine (BBR) to modulate these shared molecular pathways across different expe

Severe ADEM-Like Neuroinflammatory Disease and Cerebrovascular Fragility With Recurrent Pseudoaneurysms and Moyamoya in a Familial Germline CBL Mutation: Expanding the Clinical Phenotype.

Heterozygous germline variants in CBL disrupt its function as a negative regulator of the Ras/MAPK pathway, classically predisposing to Juvenile myelomonocytic leukemia (JMML) and moyamoya. We describe two affected siblings carrying a paternally inherited CBL variant (c.1210 T> C, p. Cys404Arg) who exhibited markedly divergent phenotypes, uncovering novel neuroinflammatory and systemic vascular fragility manifestations. The older sibling, a previously healthy 4-year-old girl, presented with acute, rapidly progressive acute disseminated encephalomyelitis (ADEM)-like neuroinflammatory disease po

CRISPR interference in a Streptococcus agalactiae multi-locus sequence type 17 strain.

UNLABELLED: Group B Streptococcus (GBS), a common colonizer of the human genital and gastrointestinal tracts, is a leading cause of neonatal bacterial meningitis, which can lead to severe neurological complications. The hypervirulent serotype III, sequence type 17 (ST-17) strain COH1 is strongly associated with late-onset disease due to its unique set of virulence factors. However, genetic manipulation of ST-17 strains remains challenging, limiting the ability to study key pathogenic genes. In this study, we developed a CRISPR interference (CRISPRi) system utilizing an endogenous catalytically

Neurological and paraclinical features differentiating Systemic lupus erythematosus, Sjögren's disease, and neuromyelitis optica spectrum disorders.

PURPOSE OF REVIEW: To compare and elucidate the distinguishing clinical, laboratory, and imaging features of neuromyelitis optica spectrum disorder (NMOSD) when it occurs alone and when it coexists with systemic lupus erythematosus (SLE) or Sjögren's disease (SjD). RECENT FINDINGS: NMOSD is a chronic inflammatory autoimmune astrocitophaty that causes multifocal central nervous system involvement, primarily affecting the optic nerves and spinal cord. Serological testing for AQP4-IgG is the hallmark of NMOSD diagnosis. These antibodies are highly specific for NMOSD and are essential for differe

Molecular mechanisms of streptococcal disruption of the blood-brain barrier and their pathogenic role in bacterial meningitis.

Bacterial meningitis represents a devastating inflammatory disease of the central nervous system (CNS), characterized by the invasion of pathogens across the blood-brain barrier (BBB) and subsequent dysregulated immune responses. Key inflammatory mechanisms include pathogen recognition by microglial TLRs and NLRP3, neutrophil infiltration, and cytokine storms such as IL-1β and TNF-α, leading to BBB disruption, cerebral edema, and neuronal injury. Despite antimicrobial therapy, excessive inflammation often results in neurological sequelae. Emerging strategies target immunomodulation through inf

[Neuromyelitis optica spectrum disorder with positive aquaporin-4 antibodies and anti-myelin oligodendrocyte glycoprotein antibodies].

Neuromyelitis optica is a rare inflammatory demyelinating disorder of the central nervous system, most often associated with aquaporin-4 (AQP4) antibodies and, less frequently, with myelin oligodendrocyte glycoprotein (MOG) antibodies. AQP4-positive disease is considered an autoimmune astrocytopathy, whereas MOG-positive disease represents an oligodendrocytopathy. Although these entities have distinct pathophysiological mechanisms, rare cases of double AQP4/MOG positivity have been reported, raising specific diagnostic, prognostic, and therapeutic issues. We report the case of a 48-year-old wo

FKBP51 mediates mitochondrial function via NF-κB pathway during sleep fragmentation-induced cognitive impairment.

Sleep disruption triggers neuroinflammation and cognitive deficits. While FKBP51 (FK506-binding protein 51) is a potential therapeutic target for stress-related psychiatric disorders, its role in sleep regulation remains unclear. Here, we show that sleep fragmentation (SF) impairs cognitive function and elevates serum levels of IL-1β, IL-6, and TNF-α, alongside increased hippocampal mRNA expression of these cytokines. Integrated transcriptomic and proteomics profiling revealed upregulation of Fkbp5 gene in the hippocampus of SF-exposed mice. Moreover, genetic knockout of Fkbp5 (Fkbp5-/-) aboli

The role of systemic inflammation in hepatic encephalopathy: advances in inflammatory mechanisms, prevention and treatment research.

OBJECTIVES: This review synthesizes current evidence establishing systemic inflammation as a key pathogenic driver in hepatic encephalopathy (HE) beyond hyperammonemia. It does not replace the ammonia hypothesis but rather acts as a critical synergistic factor, modulating and amplifying ammonia neurotoxicity. It further evaluates the mechanisms linking inflammation to HE and the therapeutic advances in inflammation-targeted prevention and treatment strategies. METHODS: A comprehensive narrative literature review was conducted, analyzing relevant preclinical models and clinical studies. The se

Xiaoyaosan ameliorates stress-induced MASH via modulating IL-1R1/RXRα-mediated liver-brain crosstalk.

ETHNOPHARMACOLOGICAL RELEVANCE: Metabolic dysfunction-associated steatohepatitis (MASH) constitutes an advancing hepatic disorder endangering worldwide well-being with scarce therapeutic options. Social stress, an important trigger of MASH, exacerbates hepatic inflammation and lipid metabolism disorders. Xiaoyaosan (XYS), a classical herbal formula traditionally used to soothe the liver and relieve depression, to strengthen the spleen and regulate metabolism, is clinically used for emotional and metabolic imbalances, but its mechanism in stress-induced MASH remains unclear. AIM OF THE STUDY:

The vagus nerve activated by inflammation co-transmits acute visceral pain via the NTS-PVN pathway.

BACKGROUND: Acute pancreatitis (AP) induces visceral pain through inflammation and mechanical stress, yet current treatments remain inadequate. This therapeutic limitation reflects our limited knowledge of how nociceptive signals are transmitted from peripheral visceral afferents to central neural pathways. METHODS: A neural circuit connecting the pancreas and the brain was mapped using various viral tracer techniques. The role of this circuit in acute pancreatitis-induced visceral pain was verified using optogenetics, chemogenetics, fiber-optic in vivo calcium signal recording, and electroph

Celastrol ameliorates depressive-like behaviors in a rat model of post-stroke depression and is associated with reduced neuroinflammation and restoration of serotonergic- and synaptic-related proteins.

Post-stroke depression (PSD) is a common neuropsychiatric complication after stroke and is associated with impaired recovery and reduced quality of life. Increasing evidence suggests that neuroinflammation, serotonergic dysfunction, and synaptic impairment may all contribute to its pathophysiology. Celastrol, a bioactive compound with anti-inflammatory and neuroprotective properties, has shown potential therapeutic effects in several central nervous system disorders; however, its role in PSD remains insufficiently understood. In the present study, we investigated the effects of celastrol in a

Pharmacological inhibition of CDK9 counteracts neuroinflammatory impairments in sepsis-associated encephalopathy.

Sepsis-associated encephalopathy (SAE) is a diffuse dysfunction of the central nervous system and a major manifestation of sepsis, associated with acute clinical deterioration and poor prognosis. Despite advances in understanding its pathophysiology and sepsis-induced cerebral alterations, effective pharmacological treatments remain unavailable. Cyclin-dependent kinase 9 (CDK9) is a ubiquitous kinase implicated in several inflammatory processes, however, its role in sepsis remains poorly understood. This study investigated whether CDK9 activity contributes to SAE pathophysiology and whether it

Gut-derived IL-17A via STAT3/RORγt signaling underlies sleep disruption-induced depression: Targeting effects of Schisandrin B therapy.

BACKGROUND: Circadian rhythm disruption and chronic sleep deprivation are increasingly recognized as key contributors to depression, largely through gut-brain axis dysregulation and neuroinflammatory activation. IL-17A, a pro-inflammatory cytokine primarily derived from intestinal Th17 cells, has emerged as a pivotal mediator linking gut immune imbalance to central nervous system dysfunction. PURPOSE: This study aimed to elucidate the gut-derived IL-17A-STAT3/RORγt signaling mechanism underlying sleep-deprivation-induced depression and to determine whether Schisandrin B, a lignan from Schisan
