Progenitor transcriptional alterations link fetal and adult pathology in Huntington's disease.

Huntington's disease (HD) is an autosomal dominant neurodegenerative disorder caused by a CAG repeat expansion in the HTT gene. Although the mutant huntingtin protein is expressed from conception, clinical symptoms typically emerge in mid-adulthood. Imaging and molecular studies have raised the possibility that HD may involve neurodevelopmental abnormalities, but direct evidence at the single-cell level in the human fetal brain has been lacking. Here we investigated whether early, cell type-specific transcriptional vulnerabilities arise during neurodevelopment, persist into adult pathology, an

Liquid-liquid phase separation in neurodegenerative diseases: Mechanisms, pathological transitions, and therapeutic opportunities.

Liquid-liquid phase separation (LLPS) provides a biophysical framework for understanding biomolecular organization, yet its pathological relevance in neurodegeneration remains incompletely resolved. In this narrative review, we critically synthesize current evidence on LLPS in Huntington's disease, Parkinson's disease, Alzheimer's disease, and amyotrophic lateral sclerosis, focusing on mutant huntingtin, α-synuclein, amyloid-β, tau, TDP-43, and FUS. We first outline the molecular principles-including intrinsic disorder, multivalent interactions, and environmental sensitivity-that govern their

Metal Ions and Protein Aggregation: Unraveling Molecular Interactions and Theranostic Prospects in Proteinopathies.

Protein aggregation is a pathological process associated with several human diseases termed proteinopathies, including neurodegenerative and metabolic disorders such as Alzheimer's disease, Parkinson's disease, type II diabetes, amyotrophic lateral sclerosis, and Huntington's disease. In these conditions, specific proteins misfold and accumulate into insoluble aggregates, disrupting normal cellular function and contributing to disease progression. Many studies have investigated the molecular mechanisms that underlie the aggregation pathways, the structure of aggregates, such as oligomeric inte

Neurodegeneration in Sub-Saharan Africa: Intersections of Biological Ageing, Genomic Diversity and the Lifetime Exposome.

Neurodegenerative diseases are emerging as an important consequence of population ageing in sub-Saharan Africa (SSA), but their scientific relevance extends beyond a projected rise in case numbers. SSA combines exceptional human genomic diversity with marked heterogeneity in vascular and metabolic risk, infection, nutrition, occupational exposures, air pollution, and access to health care. This combination creates an underused setting in which to study why biological ageing produces neurodegeneration in some individuals but relative resilience in others. This review critically synthesises evid

Drosophila Models of Human Neurodegenerative Diseases.

In his book Insects and the Life of Man: Collected Essays on Pure Science and Applied Biology, Sir V. B. Wigglesworth examined how a fundamental understanding of insect physiology and biology can be applied to some of society's greatest challenges, including human health. Here we reflect on how, in the 50 years since his book's publication, Drosophila melanogaster have played a critical role in advancing our understanding of human neurodegenerative diseases. Focusing on Parkinson's disease, Huntington's disease, and the frontotemporal dementia / amyotrophic lateral sclerosis spectrum, we exami

Emotion regulation difficulties are selectively associated with executive and behavioural apathy in Huntington's disease.

OBJECTIVE: Apathy occurs frequently in psychiatric and neurological disorders and is associated with functional impairment, reduced quality of life, and caregiver burden. Its underlying mechanisms are incompletely understood, but recent studies indicate that apathy is multidimensional and has heterogeneous manifestations across disorders. Self-regulatory processes, such as emotion regulation, may contribute to these manifestations, but this relationship has received little attention. Huntington's disease (HD) provides a clinical model for examining whether emotion regulation difficulties are a

Antidopaminergic Medications and Clinical Outcomes in Huntington Disease: Evidence From the PROOF-HD Placebo-Controlled Trial.

BACKGROUND AND OBJECTIVES: Antidopaminergic medications (ADMs), including vesicular monoamine transporter-2 (VMAT2) inhibitors and antipsychotics, are widely used to manage motor and psychiatric symptoms in Huntington disease (HD). Observational evidence suggests that ADM use may be associated with accelerated worsening of clinical outcome measures used in HD trials, but this relationship has not been examined in a controlled trial setting. We assessed the association between ADM exposure and longitudinal clinical outcomes in the Pridopidine's Outcome On Function in Huntington Disease (PROOF-H

Cas9 nickase-mediated contraction of CAG/CTG repeats is associated with phenotypic improvements in a Huntington's disease mouse model.

Expanded CAG/CTG repeats cause more than 15 different diseases that all remain without a disease-modifying treatment. Because repeat length accounts for most of the variation in disease severity, contracting CAG/CTG repeats presents an attractive therapeutic avenue. Here, we show that the CRISPR-Cas9 nickase targeted to CAG/CTG repeats leads to efficient repeat contractions in Huntington's disease (HD) patient-derived neurons and astrocytes and in myotonic dystrophy type 1 patient-derived neurons. The approach was allele selective and free of detectable off-target mutations. Injection of the C

Beyond inherited repeat length: somatic expansions in Huntington disease.

Huntington disease (HD) is a rare, genetic, autosomal dominant neurodegenerative disease, caused by an unstable CAG repeat expansion in the HTT gene. Over the past decade, somatic instability has become a major focus of investigation. Large genome-wide association studies together with work on post-mortem tissue, animal and cellular models, have brought this mechanism to the forefront, linking it to age at onset, disease severity and progression, reshaping our understanding of HD as a dynamic DNA-driven process over the lifetime, and highlighting it as a potentially actionable target. However,

Cold activates a BH2 metabolic network promoting longevity and proteostasis.

Moderately cold temperatures extend lifespan and promote health across species1-3, yet the molecular mechanisms underlying these benefits remain unclear. Here we define a metabolite-driven, multi-tissue signalling system that links cold temperature to enhanced longevity and proteostasis in Caenorhabditis elegans. Cold temperature upregulates the biopterin-synthesis enzyme CAT-4 (GCH1 in humans), increasing production of dihydrobiopterin (BH2), a transportable reservoir for the bioactive cofactor tetrahydrobiopterin (BH4). BH2 is produced in muscle and transported to distal tissues, including t

Neuroprotective Role of Withaferin A in Huntington's Disease: Current Evidence and Future Perspectives.

Huntington's disease (HD) is an inherited neurodegenerative disorder caused by an expanded CAG repeat in the huntingtin (HTT) gene. Its pathogenesis involves mutant huntingtin aggregation, impaired proteostasis and autophagy, transcriptional dysregulation, mitochondrial dysfunction, oxidative stress, neuroinflammation and progressive neuronal loss. Withaferin A (WA), a steroidal lactone derived from Withania somnifera, has demonstrated antioxidant, anti-inflammatory, proteostasis-modulating and stress-response effects in experimental studies. In a transgenic HD mouse model, WA-induced heat-sho

Mechanistic Evaluation of Psilocybin in Neurodegenerative Disorders: Relevance of 5-HT2A Receptors and Downstream Pathways.

More than fifty million individuals worldwide are experiencing neurodegenerative disorders, which include Parkinson's disease, Alzheimer's disease, amyotrophic lateral sclerosis, Huntington's disease, multiple sclerosis, and frontotemporal dementia. These disorders have a significant role in the gradual degradation of cognition and motor skills. Most current treatment approaches have little effect on changing the course of the disease and are largely symptomatic. Psilocybin, a naturally occurring tryptamine alkaloid from psilocybin mushrooms, has become a promising medicinal agent because of i

Huntington's disease clinical trials update: August 2026.

In this edition, we highlight several important developments across the Huntington's disease therapeutic pipeline. We discuss the launch and subsequent discontinuation of POINT-HD, the first-in-human phase I study of RG6496 (Hoffmann-La Roche), an allele-selective antisense oligonucleotide designed to selectively lower mutant huntingtin while largely preserving wild-type huntingtin expression. We also discuss the launch of two phase III efficacy trials: INVEST-HD, evaluating votoplam (Novartis Pharmaceuticals), and PRECISE-HD, evaluating pridopidine (Prilenia). In addition, we summarise recent

The potential of AI inspired approaches for patient monitoring in drug development for Huntington's disease: capturing the digital exhaust for activities of daily living.

Diagnosis, assessment and monitoring of Huntington's disease (HD) requires measurement of a complex array of motor, cognitive and psychiatric symptoms over sustained periods. However, the prodromal stages often span over a decade with symptoms presenting in a non-linear manner with short term fluctuations. This presents challenges for investigational drug development as treatment effects may become masked. To help address this issue, a number of digital health technology tools are being investigated for use in HD studies with increasing degrees of utility and sophistication. Herein we outline
