Expanding the phenotypic spectrum of SOD1‑related ALS: upper motor neuron predominance in a p.D91A case.

Mutations in superoxide dismutase 1 SOD1 are the second most common genetic cause of ALS, usually associated with prevalent lower motor neuron phenotypes. We describe a 66-year-old woman with slowly progressive spastic paraparesis, initially diagnosed as primary lateral sclerosis, who carried a heterozygous p.D91A mutation. Clinical and neurophysiological findings indicated predominant upper motor neuron involvement, an unusual presentation for this mutation. This case broadens the SOD1 phenotypic spectrum and highlights the importance of early genetic testing in atypical motor syndromes, give

Early-onset hereditary spastic paraplegia type 56 (SPG56): clinical-molecular correlations and functional validation of CYP2U1 variants.

BACKGROUND: Hereditary spastic paraplegia type 56 (SPG56) is a rare autosomal recessive neurodegenerative disorder caused by biallelic variants in the CYP2U1 gene, which encodes a cytochrome P450 enzyme involved in fatty acid metabolism and mitochondrial function. The clinical spectrum includes progressive spasticity of the lower limbs, developmental delay or regression, cognitive impairment, and variable ophthalmological findings. Although several cases have been reported in recent years, the functional characterization of individual variants remains limited. CASE PRESENTATION: Here we descr

Anesthetic Management of a Patient With Hereditary Spastic Paraplegia: A Case Report.

BACKGROUND: Hereditary spastic paraplegia is an extremely rare, heterogenous, inherited, progressive neurologic disorder characterized by lower-extremity weakness and spasticity in pure forms, with complex forms including symptoms such as seizures, dementia, and ataxia. Due to its rarity, evidence guiding anesthetic management is limited, consisting of only eight published case reports. CASE REPORT: We report the anesthetic management of a patient with pure hereditary spastic paraplegia undergoing renal mass cryoablation in the prone position. General endotracheal anesthesia was induced with

Genetic landscape of hereditary spastic paraplegias in the Korean population.

Hereditary spastic paraplegia (HSP) is a genetically neurodegenerative disorder with limited epidemiological data in Korea. This study aimed to characterize the genetic landscape of HSP in a large Korean cohort. We analyzed 657 patients with suspected HSP using Sanger sequencing and a targeted next-generation sequencing (NGS) panel covering 54 HSP-related genes. Variants were classified according to American College of Medical Genetics and Genomics/Association for Molecular Pathology guidelines, and copy number variations (CNVs) were detected using in-house methods and multiplex ligation-depen

Dilated Cardiomyopathy in a Woman With SPG4-Associated Hereditary Spastic Paraplegia.

BACKGROUND: Hereditary spastic paraplegia (HSP) is primarily an inherited neurodegenerative disorder, although reports suggest possible systemic involvement. Cardiac manifestations in SPG4-HSP remain poorly understood, and dilated cardiomyopathy (DCM) has not previously been documented. CASE SUMMARY: A 54-year-old woman with genetically confirmed SPG4-HSP presented with progressive dyspnea and a new left bundle branch block. Echocardiography and cardiac magnetic resonance revealed nonischemic DCM with reduced ejection fraction and dyssynchrony. Alternative causes were excluded, and extended c

LASER couples damage sensing to ESCRT assembly for lysosome repair.

Lysosomal membrane integrity is essential for cell survival, but how damage sensing is spatiotemporally coupled to repair remains poorly understood. Recruitment and assembly of endosomal sorting complex required for transport (ESCRT) I-III rapidly counteracts membrane damage, but it is unclear how ESCRT-I recognizes defective lysosomal membranes. Here, leveraging genome-wide CRISPRi screens in a damage-sensitized genetic background, we identified LC3/GABARAP-assisted stimulator for ESCRT recruitment (LASER), a multicomponent protein assembly that forms rapidly upon calcium release from damaged

An MRI-based radiomics model for precision subtyping of hereditary spastic paraplegia: discriminating SPG4 from SPG5.

BACKGROUND: Differentiating spastic paraplegia type 4 (SPG4) from type 5 (SPG5) is clinically relevant with differing therapeutic strategies, yet genetic testing remains limited necessitate alternative approaches. This study aimed to develop and validate an interpretable radiomics model based on spinal cord MRI for distinguishing SPG4 from SPG5. METHODS: In this prospective study, SPG4 (N = 40) and SPG5 (N = 30) were randomly assigned to training (n = 56) and test (n = 14) sets. Radiomic features were extracted from the whole spinal cord (WSC) and cervical spinal cord functional subregions (S

"Effects of robot-assisted gait training combined with traditional treatment in hereditary spastic paraplegia: An observational pilot cohort study".

Evidence-based guidelines for motor rehabilitation in Hereditary Spastic Paraplegia (HSP) are still lacking but robotic gait training may offer benefits for motor function and walking. The aim of this study is to evaluate the effects of a robot-assisted gait training (RAGT) combined with traditional treatment (TT) on gait pattern, motor skills and quality of life in subjects affected by HSP. 40 patients (33 pure; 20 pediatric) underwent an intensive program with RAGT and TT daily (five-times a week for three consecutive weeks). Patients were evaluated pre and post intervention with gait analys

A novel homozygous variant in the POLR1A gene: a complicated hereditary spastic paraplegia (c-HSP) or a hypomyelinating leukodystrophy type-27 (HLD27) phenotype?

BACKGROUND: RNA polymerase I enzyme plays a pivotal role in the biosynthesis of 28S, 18S, and 5.8S ribosomal RNAs, which are crucial components of the protein synthesis machinery. The largest subunit of this enzyme, POLR1A, is encoded by the POLR1A gene. Homozygous variants in POLR1A cause an ultra-rare disorder known as hypomyelinating leukodystrophy type-27 (HLD27) with only four families reported worldwide to date. Here, we describe a fifth family harboring a novel homozygous variant in the POLR1A gene, presenting with atypical features and initially suspected of having complicated heredita

Coenzyme Q4 gene compound heterozygous mutations cause hereditary spastic paraplegias: a case report.

BACKGROUND: Complicated hereditary spastic paraplegias (complicated HSPs) are a subtype of hereditary spastic paraplegias (HSPs), which refer to a group of hereditary neurodegenerative diseases of the nervous system characterized by stiffness of the lower limbs and hyperreflexia, and are accompanied by other symptoms. HSPs exhibit high clinical and genetic heterogeneity, making diagnosis challenging, especially in early-onset cases. Mutations in the COQ4 gene have been rarely reported to be associated with complicated HSPs, and most previously reported COQ4-related HSP cases lack detailed clin

Beyond Binding Affinity: How a Conformation-Specific Salt Bridge Tunes KIF1A Mechanochemistry.

Mutations at KIF1A residue R350 are linked to hereditary spastic paraplegia type 30 (SPG30), part of the spectrum of KIF1A-associated neurological disorders (KAND). Recent high-resolution cryo-EM structures of the KIF1A R350G and R350W mutants bound to microtubules in both nucleotide-free (apo) and AMP-PNP-bound states revealed a salt bridge between KIF1A R350 and α-tubulin E415 that forms specifically in the open motor domain conformation and is abolished by both substitutions. Single-molecule motility assays further showed that disruption of this conformation-dependent electrostatic interact

The Long Haul: Microtubule Motors as the Essential Supply Line for Neuronal Longevity.

The extreme morphology and polarised architecture of neurons require the highly sophisticated microtubule transport system for both construction and lifelong survival. Genomic evidence from an expanding landscape of human mutations supports the essential role of the microtubule transport machinery. During neurodevelopment, mutations disrupt the proliferation and migration of neuronal precursors, as well as the initial establishment of polarity. In the mature nervous system, the reliance on microtubule transport shifts to the long-term maintenance of axon integrity and synaptic proteostasis. Ac

Digital Motor Outcomes Capture Upper Limb and Mild Stage Impairment in Hereditary Spastic Paraplegia.

INTRODUCTION: Digital motor outcomes may surpass the sensitivity of clinician-reported outcomes in hereditary spastic paraplegia (HSP), particularly in cases of mild upper limb impairment and early disease stages. We thus validated a trial-ready quantitative motor (Q-Motor) assessment of rapid alternating limb movement tasks against clinician-reported, patient-focused, and performance outcomes in HSP. METHODS: Exploratory single-center cross-sectional assessment in 41 patients with different HSP genotypes (age: 48 ± 14 years), with speeded foot tapping, diadochokinesia, and finger tapping mea

Proteomic analysis reveals early pathological defects in corticospinal motor neurons of a spastin model of hereditary spastic paraplegia, which are improved by NU-9 treatment.

Upper motor neuron (UMN) degeneration is a characteristic feature of hereditary spastic paraplegia (HSP), a genetically heterogeneous heritable neurodegenerative disorder resulting from mutations in over ninety genes. The mutations in the SPAST gene, which encodes the microtubule-severing protein spastin, are responsible for about 40% of all HSP cases. To date, the cellular and molecular mechanisms linking mutant spastin protein to UMN vulnerability in HSP patients remain unknown and there are no disease modifying therapies. To address this knowledge gap, we isolated pure populations of cortic

Rescue of mitochondrial and neurite pathology in SPG7 hereditary spastic paraplegia patient-derived cortical neurons.

Biallelic pathogenic variants in SPG7 are a frequent cause of hereditary spastic paraplegia leading to progressive disability due to a length-dependent degeneration of cerebellar and cortical projection neurons. While underlying mechanisms have been linked to impaired mitochondrial function, no disease-modifying therapy is available. We generated induced pluripotent stem cell-derived cortical neurons from SPG7 patients with non-sense or truncating variants and from matched controls. We performed detailed phenotyping of neuronal differentiation, as well as mitochondrial and neuritic morphology

Hereditary spastic paraplegia (HSP) gene 11 (Spg11) attenuates lipid accumulation in myeloid cells and neuroinflammation in the midbrain without affecting α-synuclein pathology.

Hereditary spastic paraplegia type 11 (SPG11-HSP) is a neurodegenerative disorder caused by mutations in SPG11, which encodes the large scaffolding protein spatacsin, involved in lysosomal and autophagosomal trafficking. A portion of patients with SPG11 mutations present with parkinsonism features. While spatacsin dysfunction is linked to neurodegeneration, the underlying cellular mechanisms, especially in the midbrain, remain largely unclear. Here, we demonstrate that loss of Spg11 in mice results in neuroinflammation and lipid accumulation in myeloid cells. Bulk RNA sequencing revealed a str
