Hereditary spastic paraplegia (HSP) comprises a clinically and genetically diverse group of inherited neurodegenerative disorders unified by slowly progressive, usually symmetrical lower-limb spasticity secondary to corticospinal tract degeneration. For practising neurologists, the first step is recognising when a presentation is consistent with HSP and when it is not, and then carefully excluding treatable causes. We outline a pragmatic approach to assessing HSP, highlighting the broad clinical spectrum, 'red flags' that should trigger re-evaluation and the common acquired and genetic mimics.

SPG46 is a rare autosomal recessive form of hereditary spastic paraplegia (HSP) caused by biallelic pathogenic variants in GBA2, encoding the non-lysosomal glucosylceramidase. We generated an induced pluripotent stem cell (iPSC) line from peripheral blood mononuclear cells of a 40-year-old Korean patient with adult-onset complicated HSP carrying novel compound heterozygous GBA2 variants (NM_020944.3: c.2059_2072del, p.Tyr687ValfsTer23; c.2621C > T, p.Ser874Leu), using non-integrating Sendai virus vectors. The iPSC line showed a normal karyotype, expressed markers of undifferentiated hPSC, and

Pathogenic KIF1A variants are established cause neurodevelopmental and neurodegenerative disorders. However, the mechanisms by which they perturb early human neural development remains undefined. Here, we identified a heterozygous KIF1A c.773C > T (p.T258M) mutation in a Chinese family with hereditary spastic paraplegia and modeled its pathogenicity using patient-derived iPSCs and CRISPR-Cas9-corrected isogenic controls. We found that KIF1A-mutant neural stem cells (NSCs) exhibited hyperproliferation, impaired cell-cycle exit, and defective neuronal differentiation, accompanied by defective ne

The human brain is an energetically demanding organ. Neurons have traditionally been considered to rely predominantly on glucose to sustain neuronal activity under normal physiological conditions. However, recent advances challenge this long-standing view, demonstrating that neurons can oxidise endogenous and exogenous long-chain fatty acids through mitochondrial β-oxidation to create energy. In this mini-review, we discuss the emerging role of neuronal fatty acid β-oxidation and how fatty acid metabolism is integrated with glycolysis, lipid storages, and mitochondrial energy production. We fu

BACKGROUND: A single recurrent variant, NM_004974.4(KCNA2):c.881G>A:p.(Arg294His), causes hereditary spastic paraplegia (HSP) through a unique variant-specific mechanism. Data from paralogous variants suggest that this may be a druggable mechanism.
OBJECTIVE: The objective of this work is to give an overview of the clinical spectrum and course of KCNA2-p.(Arg294His)-associated HSP (HSP-KCNA2) and report treatment with eplerenone, a potassium-sparing diuretic.
METHODS: In this retrospective study, we recruited individuals with the variant of interest through international rare disease network

BACKGROUND: Hereditary spastic paraplegia (HSP) is a group of rare, genetically heterogeneous neurodegenerative conditions characterized by progressive lower limb spasticity. In children, rehabilitation management of HSP is commonly extrapolated from cerebral palsy practice, despite differences in etiology and disease progression. There is limited condition-specific evidence to guide rehabilitation clinical decision-making in pediatric HSP.
METHODS: A retrospective cohort study of children with HSP attending a tertiary hospital rehabilitation service (May 2018 - August 2024). Children were cl

Pathogenic variants in ATP1A3 are classically associated with alternating hemiplegia of childhood, CAPOS syndrome, and rapid-onset dystonia-parkinsonism. However, the phenotypic spectrum of ATP1A3-related disease has expanded considerably in recent years, including atypical presentations with spasticity and hereditary spastic paraplegia-like phenotypes. We describe a 43-year-old woman who developed a progressive upper motor neuron syndrome characterized by spastic paraparesis, generalized hyperreflexia, bilateral Babinski signs, inexhaustible ankle clonus, spastic dysarthria, dysphagia, and mi

Hereditary spastic paraplegia (HSP) is a heterogeneous group of genetically determined diseases, characterized by progressive spastic paraparesis of the lower limbs, associated with degeneration of the corticospinal tract and the posterior column of the spinal cord. Currently known to be about 70 genes are involved in HSP, though new potentially pathogenic variants are being reported regularly. Almost all patterns of inheritance like autosomal dominant, autosomal recessive, X-linked, and mitochondrial have been found in families of HSP patients. However, autosomal recessive form, hereditary sp

Defective feedback inhibition of serine palmitoyltransferase (SPT) caused by pathogenic SPTSSA variants underlies childhood-onset complex hereditary spastic paraplegia, yet the developmental timing and therapeutic reversibility of sphingolipid dysregulation remain unclear. We generated a knock-in mouse carrying the disease-associated SptssaT51I variant and show that heterozygous animals exhibit preserved intrinsic SPT activity but impaired ORMDL-mediated regulation, leading to sustained elevation of bioactive sphingolipid intermediates that peak during postnatal myelination. Although gross mye

To date, several genetic factors have been reported in cases presenting both congenital cataract (CC) and spastic paraplegia (SPG), but the overall genetic landscape of this condition remains insufficiently understood. Here, we describe the clinical, genetic, and functional findings in a single Japanese family presenting with hereditary CC and SPG. Affected individuals exhibited severe CC and mild SPG symptoms. Whole exome sequencing combined with stringent filtering revealed no pathogenic variants in the three known genes. Instead, we identified a heterozygous missense variant in PAX6 (NM_001

Hereditary spastic paraplegia (HSP) refers to a group of rare, progressive neurodegenerative disorders characterized by spasticity and weakness predominantly affecting the lower limbs, with significant clinical and genetic heterogeneity. Clinically, HSP is classified into "pure" and "complicated" forms. SPG15 (ZFYVE26) is a rare autosomal recessive subtype typically presenting in childhood with cognitive decline, followed by the development of gait disturbances and spasticity, often accompanied by thinning of the corpus callosum. Here, we report the case of a 19-year-old female patient, born t

Obesity significantly increases the risk of developing other chronic diseases and shortens the life expectancy by 5 to 8 years. Childhood obesity is particularly prominent as 80% of teenagers with obesity have shown it to persist into adulthood. With reference to the development of adult medication, the present review summarises the recent research progress in the development of pediatric anti-obesity drugs. The current management guidelines support the use of medication for adolescents aged 12 and above when lifestyle measures are ineffective. Early weight-loss drugs like the serotonergic ago

Oxidative stress plays a critical role in many diseases, making it essential to study its impact on disease progression. However, clinical trials have many limitations and, in some cases, may not be possible at all. In this case, the development of in vivo models is highly anticipated. This is especially relevant for neurodegenerative diseases. Drosophila melanogaster models have a number of advantages over many other animal models, including the availability and costeffectiveness of breeding, the accumulated knowledge of the Drosophila genome, and the ability to manipulate a large number of i

Adaptor protein complex 4-associated hereditary spastic paraplegia (AP-4-HSP), a childhood-onset neurogenetic disorder and frequent mimic of cerebral palsy, is caused by biallelic variants in the adaptor protein complex 4 (AP-4) subunit genes (AP4B1 [for SPG47], AP4M1 [for SPG50], AP4E1 [for SPG51], and AP4S1 [for SPG52]). Diagnosis is often confounded by variants of uncertain significance. We evaluated the ATG9A ratio, a measure of ATG9A mislocalization in patient-derived fibroblasts, as a functional assay of AP-4 deficiency. In six of eight individuals with suspected AP-4-HSP, the assay demo

BACKGROUND: Hereditary spastic paraplegia (HSP) genes are emerging as new causes of neurodevelopmental disorders (NDDs). While the association of intellectual disability and autism spectrum disorder (ASD) with many complex forms of HSP is well established, little is known about a possible association with childhood-onset SPG7.
AIM: To add new data on the phenotypic spectrum and associated NDDs in childhood-onset HSP.
RESULTS: We report three patients with biallelic variants in SPG7. Consistent with previous reports, childhood-onset SPG7 manifests as a complex HSP phenotype, with clinical fea

BACKGROUND The most frequently mutated gene in hereditary spastic paraplegia (HSP) is SPAST. Only symptomatic treatment is available for this disease. Fampridine has been successfully used to treat gait disturbances in some patients with HSP. A positive effect of fampridine has not been previously reported in HSP4 caused by the c.683-2A>C variant in the SPAST gene. CASE REPORT We report the case of a 63-year-old woman with hypogeusia and hyposmia for several years, pollakiuria, gait disturbances, reduced walking speed, occasional dysphagia, constipation and delayed defecation, occasional memor

We report the case of a 27-year-old man with a history of speech delay and chronic, progressive movement disorder. He first developed gait difficulty at the age of 12. Given clinical signs of bradykinesia and resting tremor, he received a clinical diagnosis of childhood-onset parkinsonism. Treatment with oral levodopa initially improved symptoms, but after 2 years, he developed motor fluctuations and dyskinesias. Additional signs of spasticity and brain MRI showing a thin corpus callosum prompted genetic testing that identified a heterozygous pathogenic variant in the PRKN gene. However, he ex

OBJECTIVE: Hereditary spastic paraplegias (HSPs) comprise a heterogeneous group of rare, neurodegenerative disorders. The most prominent HSP features: spastic paraparesis, mild somatosensory deficits and bladder dysfunction may be accompanied by additional symptoms i.e.: neuropathy, epilepsy, dementia. We aimed to determine subclinical involvement of nonmotor or sensory brain structures in hereditary spastic paraplegias type 3 A (SPG3A) and type 4 (SPG4). METHODS: Visual evoked potentials (VEPs), brainstem evoked potentials (BAEPs) and electroencephalography (EEG) were performed in 28 SPG4 and

BACKGROUND: Most current studies on Hereditary Spastic Paraplegia (HSP) focus on adolescents and adults. There is a lack of research on the effectiveness of structured, evidence-based developmental physiotherapy during early childhood. While early intervention strategies for children with motor development delays offer a useful framework, no specific guidelines have been developed for early physiotherapy in HSP. This report aimed to describe the effects of a home-based, evidence-based developmental physiotherapy approach in a child diagnosed with HSP Type 47 (SPG47).
