Selective vulnerability of the centrifugal and centripetal axons of primary sensory neurons.

The primary sensory neurons are unique in possessing one axon that lies within the peripheral nervous system and another that extends into the central nervous system. The two, therefore, have different environments and are associated with different supporting cells. Most of the distal ("dying-back") axonopathies that have so far been studied display simultaneous distal degeneration of both the peripherally and centrally directed axons of the dorsal root ganglion cells ("central-peripheral distal axonopathy"); others may display selective distal degeneration of the peripheral axons. CFS transferrin in various neurological diseases.

The introduction of isoelectrofocusing on polyacrylamide gel followed by direct immunofixation, in the analysis of CSF proteins, emphasized the interest in transferrin examination, mainly in order to find eventual abnormalities in patients with neurological diseases. Stibler (1979), using these techniques, demonstrated the presence in CSF of two subtypes of transferrin C, called C1 and C2, transmitted by autosomal codominant inheritance, according to the C1, C2 and C2-1 phenotypes. The rather frequently occurring variant of transferrin in CSF is the C2-1 subtype: a double banded pattern, which

Pattern visual evoked responses in hereditary spastic paraplegia.

Pattern visual evoked responses were studied in 13 patients from nine families with dominant herditary spastic paraplegia and in seven sporadic cases. The responses were normal in all the dominantly inherited cases but abnormal in three of the seven sporadic cases.

Spinal somatosensory evoked potentials in hereditary spastic paraplegia.

Cervical somatosensory evoked potentials elicited by median nerve stimulation were recorded from 18 cases of hereditary spastic paraplegia. Motor and sensory nerve conduction in the median nerve was normal in each. In one third of the patients no spinal evoked potential was detectable. In the remainder the amplitude of the evoked potential was reduced in comparison with a control series; the latency was not significantly different. These changes suggest that a selective degeneration of the centrally directed axons derived from the dorsal root ganglion cells occurs in this disorder. The finding

Hereditary "pure" spastic paraplegia: a clinical and genetic study of 22 families.

In 22 families with the "pure" form of hereditary spastic paraplegia inheritance was autosomal dominant in 19 and autosomal recessive in three. Examination of intrafamilial correlation of age of onset in the dominant cases suggested that the disorder is genetically heterogeneous. Two forms of dominant hereditary spastic paraplegia were identified: one with an age of onset mostly below 35 years (type I), and the other onset usually over 35 years (type II). In the type I cases, delay in walking was not infrequent and spasticity of the lower limbs was more marked than weakness. The disorder was v

Heel deformity in hereditary spastic paraplegia.

The association of pes cavus deformity and neurological disorder is well known. Varus deformity of the heel is often associated with, and may even precede the development of pes cavus. Clinical and radiographic examinations of the feet of members of three kindreds of hereditary spastic paraplegia, a rare neurologic disorder, suggested that the autosomal dominant form manifests a significantly higher incidence and degree of heel varus deformity than the autosomal recessive form of the disease. Therefore, heel varus deformity may be a sign suggestive of autosomal dominant inheritance of heredita

Hereditary neurologic disorders, characterized by ataxia. Immunological in vitro parameters and HLA.

Nineteen untreated patients with hereditary ataxia, hereditary spastic paraplegia and Charcot-Marie-Tooth's syndrome were investigated for immunological in vitro parameters (lymphocyte transformation with PHA, con A, PWM, antigens, determination of T- and B-lymphocytes and HLA-typing). When compared with normal controls, no significant deviation was found.

Visual evoked potentials in two forms of hereditary spastic paraplegia.

Visual evoked potentials (VEPs) were used to study two forms of hereditary spastic paraplegia. We recorded the VEP in 6 homozygotes and 5 obligate heterozygotes in one family with recessively transmitted disease and in 5 persons with dominantly transmitted hereditary spastic paraplegia. The VEPs in the affected persons from both families were similar; both had delayed conduction times and changes in the wave form. The VEPs of the obligate heterozygotes were indistinguishable from those of the unaffected control subjects. Comparison of our findings with those of other disease processes that aff

Hereditary spastic paraplegia with neurogenic bladder disturbances and syndactylia.

The present paper reports a family settled in the northern part of Norway, with a hereditary neurological disorder consisting clinically of spastic paraplegia associated with neurogenic bladder disturbances and syndactylia. Nine out of 22 members in three generations exhibit these clinical features. The bladder disturbances, being incomplete supranuclear bladder paresis (uninhibited neurogenic bladder), were the main complaint and occurred at an early stage of the disease. The family probably represents an unusual form of hereditary spastic paraplegia (HSP). The mode of inheritance is consider

Computed tomography in hereditary ataxias.

Cranial CT in 39 patients (23 belonged to 8 families) with four different groups of hereditary ataxia (HA) showed mainly three combinations of atrophic findings: (1) cerebellar ataxia (CA, n = 17) had marked atrophy of the cerebellum and/or the brain stem combined with moderate cerebral atrophy; (2) an intermediate group consisting of hereditary spastic paraplegia (HSP, n = 10) and Friedreich's ataxia (FA, n = 7), both with moderate infra- and supratentorial atrophy; (3) atrophy was hardly demonstrated in the group of Charcot-Marie-Tooth disease (CMT, n = 5). HA cases with atrophy could be dis

Computerized tomography in hereditary ataxias.

Thirty-nine patients with hereditary ataxia (HA), hereditary spastic paraplegia or Charcot-Marie-Tooth disease were investigated with computerized cranial tomography (CT). Infratentorial as well as supratentorial atrophies were registrated and scored. These were compared with the patient's neurological symptoms, which were related to the lesion in the central nervous system, and scored. There was correlation between the distribution of brainstem/cerebellar symptoms and the distribution and degree of infratentorial atrophy. HA cases with cerebral cortical atrophy had significantly higher dement

Peroneal muscular atrophy (PMA) and related disorders. II. Histological findings in sural nerves.

Biopsy of the sural nerves distinguished two groups of patients with peroneal muscular atrophy (Charcot-Marie-Tooth): a hypertrophic type and a neuronal type. In patients with the hypertrophic type (10 nerves), 30-100 per cent of teased fibres of the sural nerve had demyelinated segments, numerous onion-bulb formations and often an increase in endoneurial space. Large and small fibres, with a diameter of more than 7 micron and less than 5 micron were diminished in number. Regeneration was scarce. There were more fibres with 60-120 myelin lamellae than in normal nerve, suggesting an atrophy of

Familial spastic paraplegia with distal muscle wasting in the Old Order Amish; atypical Troyer syndrome or "new" syndrome.

The Troyer syndrome was found by Cross & McKusick (1967) in 20 members of 12 Old Order Amish families in Holmes County, Ohio; it is a form of hereditary spastic paraplegia combined with distal muscle wasting, i.e. signs of involvement of lower motor neurons. The condition usually begins at 1 to 2 years and progresses at variable rates. Further manifestations include growth retardation, delayed speech development with dysarthria and drooling, and cerebellar signs; mental functions are usually not affected but severe emotional lability is a common finding. [Involvement of the peripheral motor neuron in hereditary spastic paraplegia (author's transl)].

Two patients from families with hereditary spastic paraplegia (recessive and dominant type) were examined electromyographically. It was found that the peripheral motor neuron is involved. It is assumed that in cases of infantile recessive familial spastic paraplegia the peripheral motor neuron is rather more involved than in those of the dominant type of spastic paraplegia.

Protein patterns of cerebrospinal fluid in hereditary ataxias and hereditary spastic paraplegia.

The CSF findings in hereditary ataxias and allief disorders have hitherto mostly been reported as normal if one excludes Refsum's syndrome. The CSF-protein patterns found on isoelectric focusing and quantitative paper electrophoresis were studied in 12 patients with hereditary ataxias and hereditary spastic paraplegia. Using a recently-developed technique of isoelectric focusing of CSF-proteins in flat beds of polyacrylamide gel, the authors could show abnormal CSF-protein patterns in all but 1 of the present cases. The aberrant CSF-protein patterns found showed differences between the syndrom
