Traditionally, dermatological education emphasizes hair, skin, and nails in its curriculum. There is a practice gap with regard to knowledge of normal oral mucosa variants, performance of the oral examination, and competence in diagnosing and treating oral mucosal disorders. The oral mucosa falls within the purview of dermatology. It is the responsibility of dermatologists, along with oral medicine specialists, to help bridge this practice gap. The purpose of this Part II manuscript is to provide differential diagnoses for common oral mucosal conditions based on sites of involvement. We have p

Syphilis, an infection caused by Treponema pallidum, transmitted predominantly through sexual contact, affects several organs, causing skin, mucous membranes and systemic lesions. Despite being a secular disease, it still poses a major challenge for the public health system, since the number of cases continues to increase after years of warnings from the scientific community. Recognizing the clinical manifestations is essential for formulating the clinical hypothesis and diagnostic confirmation with complementary exams. However, recognizing skin lesions is not always simple, given the diversit

Secondary syphilis is commonly associated with well-known cutaneous and mucosal manifestations, including maculopapular rashes and condylomata lata. However, clinical presentation can vary significantly, often resembling other dermatological or infectious conditions, which can lead to diagnostic delays and challenges, particularly in immunocompetent individuals. We report the case of a 26-year-old heterosexual Asian man working as a baggage handler at a busy international airport, who presented with a rapidly progressive, painful rash. Initially treated with flucloxacillin, his condition worse

Syphilis, caused by the spirochete Treponema pallidum, is a sexually transmitted infection (STI) that has seen a resurgence worldwide, particularly among populations at a higher risk of co-infection with human immunodeficiency virus (HIV). The disease typically progresses through distinct stages: primary, secondary, latent, and tertiary, each with specific clinical manifestations. Secondary syphilis is characterized by systemic involvement and various mucocutaneous symptoms, including a maculopapular rash that frequently involves the palms and soles along with fever, lymphadenopathy, and mucou

Secondary syphilis can present with a diffuse, nonpruritic, contagious skin rash. The most common presentation of the rash consists of reddish-brown macules and papules seen on the palms and soles, often with papules and plaques on the torso and proximal extremities. However, the presentation may also include warty or raised mucous membrane lesions or patchy alopecia. In this case, we present a patient with rupioid syphilis, an atypical cutaneous manifestation of malignant secondary syphilis. Further, this case illustrates atypical biopsy findings in patients with rupioid syphilis. The variety

Syphilis is a sexually transmitted disease caused by the spirochetal bacterium Treponema pallidum that has been of public health concern for centuries. In the United States, it is currently a reportable disease and one which is recently generating increasing case numbers especially in at risk populations of immune deficiency and men who have sex with men. The present series examines biopsies from 13 patients collected over a 12-year period from a general hospital network in north suburban Cook County, Illinois. There were 13 patients (11 male: 2 female) with varied presentations, including pri

Despite advances in the control, diagnosis, and treatment of syphilis, its recognition is ill- understood or often not considered by dermatologists and other physicians who either have little specialized training in the minutiae of sexually transmitted infections (STIs) or whose dermatologic practice is only occasionally consulted by individuals from communities where STIs are prevalent. Our aim is to highlight contemporary ideas and findings on syphilis so that not only is an accurate diagnosis of syphilis made and recognized treatment given, but also necessary measures, such as counseling to

A rash is a disseminated eruption of cutaneous lesions with great variation in appearance, cause, and severity. When the physician is facing a rash, the history and physical examination of the patient are extremely important for the identification of the disease and its causal agent. There are various causes for a rash, which may be infectious, allergic, or rheumatologic, besides many others. Rashes associated with mucosal ulcers may have causes related to viral and bacterial infections or drug reactions. They may be associated with measles; erythema infectiosum; roseola infantum; rubella; han

The article presents a review of the literature with a description of typical manifestations in the maxillofacial area in secondary, tertiary period of syphilis and the description of photo-images of the authors' clinical observations. In the secondary period of syphilis rashes can be found both on the skin and mucous membranes. In secondary reccurent syphilis the oral cavity is affected in 56% of the cases. Typical manifestations of this period are: roseola, papular and pustular syphilides. In the mouth the most frequently observed manifestation is popular syphilide or erythematous sore throa

The incidence of syphilis is increasing, and it typically presents in patients with known risk factors, often to genitourinary physicians. Patients presenting to a dermatologist or ophthalmologist will more likely have secondary syphilis, with the potential for having the associated complications. Early recognition is therefore vital to limit both the disease and risk of further contact spread. In this review, we include two case histories demonstrating the value of recognizing oral signs. Additionally, we review the currently accepted diagnostic and therapeutic recommendations.

We have explored the rash that appears as target lesions, with the central and dominant diseases belonging to the Stevens-Johnson syndrome/toxic epidermal necrolysis group. After presenting the clinical patterns of an individual target lesion and classifying them into different types of lesions, the contribution has been organized with groups characterized by such specific findings according to the type of lesion: flat or raised, typical or atypical, presence or absence of fever, presence or absence of mucosal ulcerations, presence or absence of arthralgias, and/or internal organ involvement.

BACKGROUND: The clinical polymorphism of syphilis leads to diagnostic issues. We report a case of secondary syphilis revealed by skin and mucosal erosions, and responsible for sensorineural hearing loss and asymptomatic papillitis.
PATIENTS AND METHODS: A 55-year-old man presented oral and peri-anal erosions as the initial symptoms of secondary syphilis. He reported hypoacusis and a pure-tone audiogram revealed bilateral sensorineural hearing loss. Ophthalmological investigation revealed isolated right papillitis and superior temporal scotoma with blind-spot enlargement. TPHA-VDRL serology wa

BACKGROUND: Pustular syphilis is an extremely rare manifestation of secondary syphilis. Failure to recognize the disease can have devastating consequences.
METHODS: We present three patients with pustular syphilis as the primary manifestation of secondary syphilis.
RESULTS: Patient 1 was initially diagnosed by Venereal Disease Research Laboratory (VDRL) test (titers 1 : 32) and confirmed by enzyme immunoassay (EIA) for Treponema pallidum. Patient 2 was screened for syphilis by VDRL (titers 1 : 64), yielding a positive result, and tested negative for HIV. Secondary syphilis was confirmed by E

Sexually transmitted diseases (STDs) are posing a growing public health problem on a global and national scale. Syphilis is one of the common STD. The dentist is most likely to come across associated oral manifestation of the disease. This is a case report of 60-year-old male patient who presented with multiple erythmatous lesions with shallow ulcerations and crusted appearance on the lip, mimicking few mucocutaneous lesions of oral cavity. He gave history of sexual exposure;further work up revealed it to be a case of secondary syphilis.

Various sexual practices like fellatio, cunnilingus, or anilingus (rimming) can cause both symptomatic and asymptomatic oral infections in both sexes. Clinically apparent lesions are found in primary, secondary, and tertiary syphilis, in acute HIV infection and the subsequent stage of immunodeficiency (opportunistic infections), as well as in herpes and human papilloma virus infections. Genital candidiasis also can be transmitted to the oral cavity. Depending on the infective agent transmitted, ulcerative, inflammatory or papillomatous lesions of the lips, tongue, mucous membranes and pharynx

After years of declining incidence in many developed countries, syphilis infection has re-emerged as a major public health problem in the past decade. The secondary stage of syphilis epitomizes the capacity of the infection to present in myriad ways. The skin, lymph glands and mucosal membranes are the most commonly affected tissues.

To study the different patterns of Treponema pallidum distribution in primary and secondary syphilis, 34 biopsy specimens of 8 patients with primary and 26 with secondary syphilis were assessed. Histopathological features, silver stain, and immunohistochemical T pallidum polyclonal antibody expression were investigated. The number and distribution of spirochetes were evaluated, and ultrastructural studies were performed. Spirochetes were identified with Warthin-Starry stain in 17 specimens (4/8 primary and 13/26 secondary syphilis), whereas immunohistochemical analysis disclosed spirochetes in

Lichen planus is a chronic inflammatory disease that affects skin, mucous membranes, hairs, and nails. According to the site of involvement, lichen planus can be subdivided into palmoplantar, mucosal, nail, or scalp types. Palmoplantar lichen planus is a rare subtype of lichen planus which may create difficulty in diagnosis, as the lesion is limited to the palms and soles. We report a case of a 37-year-old male with lichen planus who presented with multiple erythematous, scaly, hyperkeratotic papules on his palms and soles mimicking secondary syphilis.

A 37 year old woman presented with a 4-month history of "plaques opalines" of the oral mucosa complaining of a rough feeling in her mouth and burning when swallowing. On the basis of clinical, histological and serological details and especially the presence of generalized indolent lymphadenopathy, the diagnosis of secondary syphilis was made. Although plaques opalines are a relatively rare manifestation of secondary syphilis, they can occasionally be the only sign and are important in the differential diagnostic considerations of gray-white mucosal lesions, especially with current continuing r
