Subcutaneous panniculitis-like T-cell lymphoma with hemophagocytic syndrome successfully treated with cyclosporin A.

A 17-year-old girl previously in good health presented with a 2-month history of recurrent, high-grade fever; general fatigue; anorexia; a 10-kg weight loss; and multiple, painful, reddish skin lesions on the lower abdomen. Some lesions were ulcerated, with an oily yellowish brown discharge. A systemic review was unremarkable other than bleeding from the nose. Her medical and family histories were unremarkable. On examination, the patient was pale, jaundiced, and febrile (temperature of 39 degrees C). She had enlarged lymph nodes in the axillary and inguinal areas. There was moderate hepatospl

Gastric involvement in a patient with secondary syphilis.

A 30-year-old man was referred because of a short lasting history of severe epigastric pain and early satiety, with remarkable weight loss. Physical examination was fully consistent with secondary syphilis and upper GI tract endoscopy revealed multiple gastric mucopurulent erosions and several nodular lesions ranging from 2 to 3 cm in diameter, with central crater, abruptly ending at the pyloric level. Appropriate antibiotic treatment obtained a dramatic improvement and complete reversal of cutaneous and gastric lesions.

Aspects of oral syphilis.

The incidence of sexually transmitted diseases recently increased in the United States and Europe due to migration, increase in high-risk behavior, and abandonment of safer sex practices at the advent of anti-retroviral combination therapy for human immunodeficiency virus infection. This article presents four cases of primary oral anti perioral syphilis with differential diagnoses. It is important to bear this reappearing infection in mind to avoid latent infection. Resembling common oral infections, the primary affect disappears spontaneously, and the infection enters the second stage. The pa

[Syphilis. Clinical aspects of Treponema pallidum infection].

Syphilis is a sexually transmitted infection by Treponema pallidum. Without antibiotic treatment syphilis lasts for several decades and may develop up to 4 different clinical stages. Usually, the disease begins with a distinct painless and indurated ulcer at the contact site: the primary chancre. An indolent regional lymph node swelling is usually associated with the syphilitic chancre. After spontaneous healing of the primary lesion and several weeks of latency, the clinical symptoms of secondary syphilis occur. Treponema pallidum bacteremia leads to common symptoms like fever and malaise, bu

Secondary syphilis presenting as pseudolymphoma of the skin.

Secondary syphilis most commonly presents with a papulosquamous eruption that involves the palms, soles, and mucous membranes. The papulonodular variant has only been described 11 times in the literature. We describe a case of papulonodular secondary syphilis presenting as an atypical lymphoid hyperplasia suggestive of cutaneous lymphoma.

[Findings of the syphilis lesions in the skulls from the "Broumov Ossuary"].

Syphilis is a treponematosis clinically characterized by a primary lesion, a secondary rash affecting the skin and the mucous membranes and late lesions affecting cardiovascular and central nervous systems, viscera and bones. Off all the skeletal lesions, the most characteristic are those of the skull, most commonly affecting frontal and parietal bones and nasal and palatal region. The collection of 647 adult skulls of both sexes and 98 child and adolescent skulls from the "Broumov Ossuary" (13th - 18th century) was examined for the presence of the bone syphilis lesions. Seven cases of the gum

Nodular secondary syphilis.

Nodular secondary syphilis in a 23-year-old Indian man was characterized by numerous papular, nodular and plaque skin lesions, without involvement of the mucous membranes. The histopathology showed sarcoid-like granulomata with lymphocytes, histiocytes, eosinophils, plasma cells and multinucleated giant cells. The differential diagnosis included deep mycoses, leprosy, tuberculosis, sarcoidosis and lymphoma. The results of serological tests and the rapid response to penicillin indicated a correct diagnosis.

Primary and secondary syphilis--United States, 1997.

Syphilis is an acute and chronic sexually transmitted disease (STD) caused by infection with Treponema pallidum. The disease is characterized by skin and mucous membrane lesions in the acute phase (primary and secondary [P&S] syphilis) and lesions of the bone, viscera, and cardiovascular and neurologic systems in the chronic phase. Because syphilis enhances transmission of human immunodeficiency virus (HIV), prevention of syphilis is important for controlling HIV. During 1986-1990, an epidemic of syphilis occurred throughout the United States. Syphilis rates began to decline in 1991 and have d

A case of secondary syphilis with mucous patches on the hard palate.

A rare case of secondary syphilis showing mucous patches on the hard palate is reported. A 31-year-old male had two erosive patches which were slightly raised on his hard palate. A linear lesion was also present on the inside of his right alveolar process. Many red-brown macules on his abdomen and bilateral inguinal lymphadenopathy accompanied these symptoms. The serological tests for syphilis were positive: VDRL test 1:512, TPHA test 1:20,480, and IgM-FTA-ABS test 1:40. [Angina specifica and mucous plaques of the mouth cavity in lues II].

We report on a case of secondary syphilis that became clinically manifest as partially erosive, almost asymptomatic lesions of the oral mucosa. Dark-field microscopy, typical serological parameters and the presence of spirochetes in the biopsy material promptly led to the diagnosis. Depending on the clinical picture, the possibility of syphilis should not be overlooked in the differential diagnosis of disease of the oral mucosa.

Early oral presentation of lues maligna in a patient with HIV infection. A case report.

We present a case of a patient infected with the human immunodeficiency virus who developed syphilis manifested by atypical early oral and skin ulcerations. The profound immune defects associated with human immunodeficiency virus may lead to an altered clinical presentation and a more aggressive course in patients infected with Treponema pallidum. The unusual clinical manifestations observed in this case emphasize the importance of considering secondary syphilis in the differential diagnosis of any inflammatory mucosal and skin disorder in patients with the human immunodeficiency virus.

[A case of secondary syphilis simulating plasmocytoma].

A clinically unclear case is described of stomatitis with a long duration in a male of 28. Exfoliations in the region of the mouth angle were observed resembling pemphigus vegetans. Biopsy examination revealed a thick plasma cell infiltration which was considered to be an extramedullary plasmacytoma. However, a polyclonal composition of plasmacytes was found immunomorphologically producing immunoglobulins A, M and G with predominant production of IgG. This allowed rejection of neoplastic nature of the infiltrate, confirming its inflammatory origin and specific character. Serological tests (Was

[Acute primary phase as an indicator of HIV-1 infection. The general symptoms and polymorphic exanthema with mouth mucosal involvement 2 to 6 weeks before seroconversion].

The initial symptoms of an HIV-1 infection were observed in four patients. The following were characteristic for the acute primary phase: (a) initial maculopapular exanthema, especially of the trunk, with occasional transition into a papulovesical appearance; (b) involvement of the oral mucosa, often of aphthous character; and (c) general malaise with fever and lymphadenopathy. The observed cutaneous changes had, on one hand, features of a Coxsackie or mononucleosis exanthema, on the other of secondary syphilis. In three patients seroconversion occurred within 2-6 weeks, the fourth failed to r

Secondary syphilis: uncommon manifestations of a common disease.

Reports of cases of primary and secondary syphilis are increasing in the United States, particularly in urban areas and among homosexual men. While primary syphilis poses little diagnostic difficulty, many physicians are unfamiliar with the multisystem nature of secondary lues. Patients who have secondary syphilis commonly present with systemic signs, skin rash, mucous membrane lesions and generalized adenopathy. Less commonly, secondary syphilis may occur as acute meningitis, sensorineural hearing loss, iritis, anterior uveitis, optic neuritis, Bell's palsy, gastropathy, proctitis, hepatitis,

[Diagnosis and therapy of syphilitic changes of the skin and mucous membrane in the head and neck].

Since the mid seventies the German Federal Board of Statistics has recorded a decrease in the incidence of the classical venereal diseases gonorrhoea and syphilis. This decrease is based only on a decline in the morbidity of gonorrhoea. Statistics on syphilis show an increase, from about 6000 to 8900 new cases each year, that is an increase of 48% between 1974 and 1978. Some cities such as Cologne, with a total of 401 new cases each year, show an increase, while numbers in rural regions remain unchanged or show a slight decline. Between 1978 and 1980 at the ENT and Dermatologic OPC of Cologne
