OBJECTIVES: Peripheral neutrophil levels in amyotrophic lateral sclerosis (ALS) inversely correlate with survival, suggesting a role for neutrophils in disease progression. Here, we characterize markers of several neutrophil activation pathways and evaluate their associations with survival to identify potential mechanisms of disease.
METHODS: Blood samples were obtained from participants at the University of Michigan ALS Clinic or from healthy controls. Ex vivo neutrophil extracellular trap (NET) formation was quantified via image analysis of primary neutrophils. Neutrophil function markers o

Adult children of a parent with ALS may be highly burdened and in need of support, but studies of their experiences and needs are scarce. The aim of this study was to explore everyday experiences, challenges and needs of adult children living outside the home of a parent with amyotrophic lateral sclerosis (ALS). The design was qualitative using Interpretive Description methodology and Sense of Coherence as framework. Focus group interviews were conducted with 16 adult children. Participants experienced changes in relationship and roles with siblings and parents when ALS moved into the family.

Congenital myasthenic syndromes (CMS) are a group of rare disorders characterized by fatigable muscle weakness and caused by impaired neuromuscular junction (NMJ) function. CMS symptoms are highly variable, but it can be detrimental and lead to death. There are over 40 different genetic subtypes, including AGRN-CMS and COLQ-CMS. AGRN encodes for neuralagrin, which is released from the nerve terminal and triggers muscle-specific kinase phosphorylation (pMuSK). pMuSK is essential for NMJ development and maintenance, thus agrin deficiency causes NMJ impairment. COLQ encodes for collagenous subuni

Calcium-activated proteases, known as calpains, cleave proteins into smaller protein fragments, altering protein structure and function. Calpain overactivity is a pathological hallmark of many human diseases, including neurodegenerative diseases, which are characterized by excessive proteolytic cleavage and formation of protein aggregates. Current methods for determining calpain activity, such as examination of the presence or absence of calpain substrates in post-mortem tissue, are low-throughput, costly and retrospective, hampering the search for potentially therapeutically efficacious calpa

BACKGROUND: Spinal muscular atrophy (SMA) is characterized by the degeneration of anterior horn motor neurons. While nusinersen has altered the natural course of SMA, it may accumulate in the kidneys, leading to proteinuria. Therefore, routine pre-dose urine monitoring for proteinuria is recommended. To address this issue, we aimed to evaluate the urinary protein-to-osmolality ratio (UPr/Osm). We also aimed to evaluate the relation of urinary creatinine-to-osmolality ratio (UCr/Osm) to motor function scores.
METHODS: A retrospective study was performed on 42 SMA patients. UPr/Cr, UPr/Osm, and

An intronic hexanucleotide repeat expansion in C9orf72 is the leading genetic cause of both frontotemporal dementia and amyotrophic lateral sclerosis (C9-FTD/ALS). We have previously demonstrated that CRISPR-Cas9 excision of the repeat expansion in patient iPSCs reverts pathological hallmarks of C9-FTD/ALS. Here, we aim to identify efficient and safe gRNAs for CRISPR-spCas9 dual-gRNA excision of the C9-repeat expansion. Utilizing novel ddPCR and single-molecule sequencing assays, we screened 120 gRNA pairs, comparing 64 bi-allelic, intronic excisions of the repeat region to 56 allele-specific

Spinal muscular atrophy (SMA) imposes substantial physical and emotional burdens on families, particularly on primary caregivers. Although spiritual well-being has been linked to psychological resilience in chronic diseases, its relationship with caregiver burden, depressive symptoms, and quality of life in caregivers of children with SMA remains insufficiently studied. This study investigated these associations in parents of children with SMA. This cross-sectional online survey included 51 parents of children with genetically confirmed SMA (types I-III). Participants completed questionnaires

Acute idiopathic polyradiculoneuritis (AIP) is an immune-mediated peripheral neuropathy in dogs for which no specific treatment has been established in veterinary medicine. In human medicine, therapeutic plasma exchange (TPE) is a standard treatment for Guillain-Barré syndrome, a clinically analogous condition. In this case series, we describe 4 dogs diagnosed with AIP and treated with TPE. All dogs received 3 consecutive TPE sessions, processing between 4.2 and 4.8 total dog plasma volumes. Motor function improvement was observed after the first session in all cases, with near-complete neurol

Chronic low back pain (CLBP) is among the leading causes of global disability. Magnetic resonance imaging (MRI) of the lumbar spine commonly demonstrates degenerated intervertebral discs and/or disc bulging that is regarded as the key contributor to the development of pain. However, degenerative changes of the disc are prevalent in asymptomatic people, thereby calling into question the specificity of these findings. In addition, the assessment and management of CLBP are currently based predominantly on the concept of the disc as the main source of pain and associated disability. The current na

Neonates born with congenital vertical talus ('rocker-bottom foot') are challenging to diagnose and manage. The immediate management is generally based on assessing for Edwards syndrome (trisomy 18) or other similar severe life-limiting chromosomal disorders when there are dysmorphic features suggestive of a genetic syndrome, and no clear neurological focus such as a neural tube defect or spinal muscular atrophy disorder. Often the initial fluorescent in situ hybridisation genetic testing is reported as normal, appearing to exclude a trisomy diagnosis. We use two similar neonatal case scenario

BACKGROUND: Hirayama disease (HD) is a rare, self-limiting lower motor neuron disorder predominantly affecting young males in Asia. It is caused by dynamic compression of the lower cervical spinal cord during neck flexion, resulting in ischemic injury to the anterior horn cells.
CASE DESCRIPTION: A 15-year-old Indonesian male presented with a 6-month history of progressive right upper limb weakness and muscle wasting without sensory deficits or spasticity. Electromyography (EMG) showed motor neurogenic changes with ongoing denervation and fasciculations in the right upper limb, with possible

Spinal muscular atrophy (SMA) is an oft-fatal infantile-onset neuromuscular disease caused by homozygous loss of the Survival of Motor Neuron 1 (SMN1) gene and, consequently, low SMN protein. Administration of SMN-inducing agents to SMA newborns prevents early mortality, but therapeutic outcomes vary considerably, and disease mechanisms remain poorly understood. Genetic modifiers can provide clues to disease mechanisms and serve as targets for novel treatments. Here, we describe how one such modifier, an Hspa8G470R synaptic chaperone variant we identified, suppresses SMA in model mice. Our res

Spinal muscular atrophy (SMA) is a severe neuromuscular disorder in which presymptomatic treatment substantially improves survival and motor outcomes, yet newborn screening for SMA remains unevenly implemented across Europe, and evidence from lower-resource health systems is needed to guide scale-up. In this study, we assessed the feasibility, diagnostic performance, and public health implications of implementing neonatal SMA screening in the Republic of Moldova within an established national newborn screening framework. A pilot genetic screening program was conducted using dried blood spot (D

Analysis of risdiplam and its main metabolite is critical for pharmacokinetic studies and therapeutic drug monitoring in spinal muscular atrophy. However, current analytical methods face challenges due to low analyte stability, limited selectivity, and pronounced matrix effects, with only four studies published to date. In this work, a comprehensive analytical strategy was developed for the first time, combining optimized chromatographic separation with a novel sample preparation approach. The effects of methanol content, salt concentration, and pH on retention, peak symmetry, and resolution w

BACKGROUND: Chitotriosidase (Chit-1) and chitinase-3-like protein 1 (CHI3L1) protein levels are increased in the cerebrospinal fluid (CSF) of neurodegenerative diseases, including amyotrophic lateral sclerosis (ALS), frontotemporal dementia (FTD) and Alzheimer's disease (AD). Few studies have examined the spatial expression of chitinase-expressing cells with respect to neuropathologic hallmarks of disease.
METHODS: RNA sequencing was used to examine Chit-1 and CHI3L1 gene expression in the spinal cord and motor cortex. Immunohistochemistry was used to characterise the distribution of Chit-1 a

Background: 5q spinal muscular atrophy (SMA) is a hereditary neuromuscular disorder characterized by progressive muscle weakness. Nusinersen, the first disease-modifying therapy for SMA, has demonstrated efficacy in both clinical trials and real-world studies. However, the precise timing of therapeutic onset following Nusinersen administration remains unclear. Methods: This retrospective study analyzed clinical data from patients with genetically confirmed 5q SMA who received Nusinersen treatment for at least six months at Peking University First Hospital. Motor function was assessed using sta
