Potential Roles of Neuroimaging in Motor Neuron Disease.

PURPOSE OF REVIEW: The review is intended to summarize and discuss the increasing roles and perspectives of neuroimaging in the clinical and scientific work-up of amyotrophic lateral sclerosis (ALS) and other motor neuron diseases (MND). RECENT FINDINGS: The applications of magnetic resonance imaging (MRI) and positron emission tomography (PET) identify characteristic disease-specific patterns especially for ALS including its clinical subtypes. Guided by neuroanatomical pattern stratification, these MND-associated alterations could be detected by dedicated analysis parameters from MRI, includ

Consensus for the Diagnosis and Treatment of Patients With Spinal Muscular Atrophy (SMA) in Latin America.

BACKGROUND: 5q spinal muscular atrophy (5q SMA) is an autosomal recessive neuromuscular disorder caused by mutations in the survival motor neuron 1 gene (SMN1), leading to deficiency of SMN protein and a variable clinical spectrum influenced by survival motor neuron 2 gene (SMN2) copy number. The advent of disease-modifying therapies (DMTs) has transformed its clinical course, requiring timely diagnosis, multidisciplinary care, and longitudinal response assessment. In Latin America (LATAM), significant gaps and heterogeneity persist in access to genetic diagnosis, newborn screening, DMTs, and

Biphasic SMN condensates safeguard stress-induced cytoplasmic mislocalization of splicing RNPs.

Biogenesis of UsnRNPs occurs in distinct steps in the nucleus and the cytoplasm. Sequential cytoplasmic actions of CLNS1A in the PRMT5 complex and of the SMN complex assemble the Sm core structure consisting of RNA and proteins. Nuclear SMN, condensed in Cajal Bodies, promotes late maturation steps. Whether cytoplasmic SMN undergoes condensation, and how this contributes to UsnRNP biogenesis or homeostasis, is poorly defined. Here, we show that molecular crowding stress induces rapid, reversible condensation of cytoplasmic SMN into droplets and filamentous assemblies, S-bodies, that sequester

Fibro-Adipogenic Progenitor Ablation Triggers Muscle Atrophy Through Cell Death-Induced Inflammation.

BACKGROUND: Skeletal muscle mass maintenance involves coordination of myofibers with mononuclear cell populations, including satellite cells, resident macrophages and fibro-adipogenic progenitors (FAPs). FAPs, identified by surface expression of PDGFRα, are important contributors to muscle homeostasis, as genetic ablation of FAPs induces rapid muscle atrophy. However, mechanisms underlying this response remain poorly understood. METHODS: We utilized tamoxifen-inducible FAP specific diptheria toxin (DTA) mice (PdgfraCre-ERT2/+; Rosa26^DTA/+) to evaluate consequences of FAP deletion, measuring

Broader autism phenotype, the default mode network, and brain entropy: A network-level hypothesis.

Despite well-documented disparities by sexual and gender minority (SGM) status in suicide attempt and mortality rates, few studies have investigated the lived experiences that contribute to SGM people's disproportionate risk of suicide. Having a history of at least one near-fatal (or medically serious) suicide attempt serves as a proxy for suicide mortality, but no known study has involved SGM people who have made such an attempt. Ideation-to-action theories of suicide posit that individuals acquire the capability for suicide through repeated exposure to painful and provocative events - namely

Serum circulating cell-free messenger RNA profile response to risdiplam treatment in adult patients with late-onset spinal muscular atrophy.

SMN-enhancing therapies have improved outcomes in adult late-onset spinal muscular atrophy (loSMA), but minimally invasive biomarkers that reflect systemic disease and treatment effects are lacking. In this retrospective cohort study, we profiled circulating cell-free messenger RNA (ccfmRNA) in serum samples from 17 adults with loSMA type 2 (n = 14) and type 3 (n = 3) and healthy controls (n = 12) using the NanoString neuroimmunology panel before (T0) and after 8-10 (T1) and 12-14 (T2) months of risdiplam treatment. A separate exploratory subgroup of patients switching from nusinersen to risdi

Unbiased bioinformatic and clinical analysis identifies glial fibrillary acidic protein as a potential biomarker in spinal muscular atrophy.

Spinal muscular atrophy (SMA) is an autosomal recessive disorder caused by bi-allelic pathogenic variants in SMN1, leading to motor neuron degeneration, muscle atrophy and progressive weakness. Although disease-modifying therapies have recently become available, reliable biomarkers for disease monitoring remain limited. This study aimed to identify potential biomarkers of disease severity and monitoring in SMA using an unbiased approach based on differential gene expression datasets. A meta-analysis of datasets from 15 studies in SMA mouse models identified genes differentially expressed in at

Real-World Treatment Patterns in Patients with Spinal Muscular Atrophy Receiving Multiple Disease-Modifying Therapies.

INTRODUCTION: Nusinersen (approved in 2016), onasemnogene abeparvovec (intravenous infusion [OAV], 2019; intrathecal injection, 2025), and risdiplam (2020) are disease-modifying therapies (DMTs) available in the US for spinal muscular atrophy (SMA). Real-world evidence on treatment patterns among patients receiving multiple DMT types remains limited. METHODS: This retrospective study used the Komodo Health Research Database (2016-2024) to identify patients with SMA treated with DMTs. First DMT distribution and the proportion of patients receiving a second DMT were described for patients who i

Neuroprotective Effects of Eugenol in Alzheimer's Disease: Mitigating Oxidative Stress, Inflammation and Amyloid Plaques.

Eugenol, a phenolic phytochemical found in many medicinal plants, exhibits various pharmacological properties, including analgesic, antipyretic, antioxidant, anti-inflammatory, antimicrobial, anticancer, neuroprotective, and anaesthetic effects. It has shown potential in addressing neurodegenerative diseases like Alzheimer's disease (AD), Parkinson's disease, and motor neuron disease, which are primarily caused by mechanisms such as apoptosis, protein accumulation, aging, and oxidative stress within the central nervous system (CNS). This review explores the mechanisms through which eugenol may

Cerebellar pathology contributes to neurodevelopmental deficits in spinal muscular atrophy.

Spinal muscular atrophy (SMA) is a neuromuscular disease characterized by ubiquitous survival motor neuron (SMN) deficiency and loss of motor neurons. The persistence of motor and communication impairments, together with emerging cognitive and social deficits in severe type I SMA patients treated early with SMN-restoring therapies, suggests a broader dysfunction involving neural circuits of the brain. To explore the potential supraspinal contributions to these emerging phenotypes, we investigated the cerebellum, a brain region crucial for both motor and cognitive behaviours. Here, we identify

DG9-Conjugated Morpholino Rescues Phenotype in Spinal Muscular Atrophy Mice.

Spinal muscular atrophy (SMA) is a progressive neurodegenerative disease characterized by decreased expression of the survival of motor neuron 1 (SMN1) gene. Antisense oligonucleotide (ASO)-based therapies have been developed recently as a means of increasing the production of functional SMN protein by targeting the SMN2 gene. Nusinersen, an FDA-approved ASO, has shown success in SMA treatment. However, it faces several challenges that limit its therapeutic potential. One of the main challenges is the administration route, as nusinersen requires intrathecal injection, which involves direct del

Genotype-specific interferon signatures in amyotrophic lateral sclerosis relate to disease severity.

Innate immune signalling pathways are hyperactivated in the CNS of patients with amyotrophic lateral sclerosis (ALS), as well as in preclinical models with diverse causative backgrounds including TDP-43, SOD1 and C9orf72 mutations. This raises an important question of whether these pathways are key pathogenic features of the disease, and whether therapeutic amelioration could be beneficial. Here, we systematically profile type-I interferon (IFN)-stimulated gene (ISG) expression signatures using a non-biased approach in CNS tissue from a cohort of 36 individuals with ALS, including sporadic ALS

The "Azzopardi phenomenon" in high-grade astrocytoma with a PNET-like component.

Spinal muscular atrophy (SMA) is a neuromuscular disease caused by low levels of survival motor neuron (SMN) protein. Several therapeutic approaches boosting SMN are approved for human patients, delivering remarkable improvements in lifespan and symptoms. However, emerging phenotypes, including neurodevelopmental comorbidities, are being reported in some treated patients with SMA, indicative of alterations in brain development. Here, using a mouse model of severe SMA, we revealed an underlying neurodevelopmental phenotype in SMA where prenatal SMN-dependent defects in translation drove disrupt

Decoding inflammatory pathways in spinal muscular atrophy: implications for next-generation therapies.

Spinal muscular atrophy (SMA) is a devastating neurodegenerative disorder caused by SMN1 gene mutations that lead to compromised production of survival motor neuron (SMN) protein. The inflammatory footprint of SMA extends beyond neural tissues, leading to significant inflammatory manifestations across multiple body systems. In motor neurons, several molecular pathways contribute to local degeneration: cytokine activation, purinergic signalling, NF-κB pathway stimulation, complement cascade activation and glutamate excitotoxicity. These mechanisms, which involve reactive glial and immune cells,

Unraveling the Potential of Stem Cell Therapy in Motor Neuron Disease: A Narrative Review.

Motor neuron disorders (MNDs), including ALS, are deadly neurodegenerative conditions that cause progressive motor neuron degeneration. With neuroprotection and the potential for neuron regeneration employing MSCs, ESCs, iPSCs, and NSCs, stem cell treatment presents a viable alternative to current medicines, which only control a limited number of symptoms. Following PRISMA criteria, this narrative review methodically screened 1248 records from the Cochrane, Web of Science, PubMed, and Scopus databases. Following a thorough screening process, 22 studies, including preclinical models and 19 clin

Alterations along the neuroendocrine axis of leptin homeostasis: white adipose tissue and hypothalamus in a severe SMA mouse model.

Spinal Muscular Atrophy (SMA) is characterized by a reduction of survival of motoneuron (SMN) protein, resulting in proximal muscle atrophy. SMA is a multi-system disease involving patients with alterations in multiple organs and metabolic pathways. Approved therapies focus on increasing SMN protein level either in the central nervous system or systemically. However, none of these therapies result in a cure. Patients show perturbations in several organs, including altered lipid metabolism such as leptin proteohormone levels, dicarboxylic aciduria and altered β-oxidation. In this study, we desc

BM-derived mesenchymal stem cell microvesicles protect enteric neural precursor cells and alleviate diabetes-associated enteric neuropathy.

Enteric nervous system (ENS) injury, characterized by progressive degeneration of enteric neurons and glial cells, is a common diabetic complication with no effective cure beyond symptomatic management. Enteric neural precursor cells (ENPCs) play a key role in maintaining neurogenesis and gliogenesis within the adult ENS. Here, we demonstrate that bone marrow mesenchymal stem cell-derived microvesicles (BMSC-MVs) alleviate diabetic ENS injury. In both diabetic patients and mouse models, gastrointestinal transit was delayed, ENS structure was impaired, and neurogenesis and gliogenesis from ENPC
