Proteomic alterations in cerebrospinal fluid of spinal muscular atrophy patients undergoing nusinersen therapy: a systematic review and meta-analysis of potential biomarkers of treatment response.

This systematic review and meta-analysis synthesizes evidence on cerebrospinal fluid (CSF) proteomic alterations in patients with spinal muscular atrophy (SMA) treated with nusinersen. Across 21 studies, consistent post-treatment decreases were observed in neurofilament light chain (NfL), phosphorylated neurofilament heavy chain (pNF-H), and tau protein, which correlated with motor improvement, particularly in younger patients. Synaptic stress markers (e.g., α-synuclein, DJ-1) and metabolic proteins (e.g., apolipoprotein A1, transthyretin) demonstrated dynamic responses, suggesting systemic ef

A qualitative study of the discrepancy between patient expectations and assessment practices in 5q-adult spinal muscular atrophy in France.

BACKGROUND: Despite advancements in spinal muscular atrophy (SMA) management, evaluating outcomes in adults remains difficult. Standard motor scales often show floor/ceiling effects and fail to capture patient-relevant constructs. This creates a disconnect between clinical measurements and the lived experience of adults with SMA, particularly in regulatory contexts requiring demonstrable benefit. This qualitative study explored the evaluation priorities, expectations, and preferences of adults with SMA and the corresponding assessment challenges faced by healthcare practitioners (HCPs). A standardized framework resolves ambiguity in motor neuron loss across neurodegenerative diseases.

Motor neuron (MN) loss is a hallmark of neurodegenerative disorders, yet its assessment remains variable, confounding mechanistic and therapeutic interpretation. To address this, we conducted a systematic review and meta-analysis of spinal muscular atrophy (SMA) mouse studies, revealing 60% variability in reported MN loss, largely attributable to nonspecific spinal cord sampling. Using a whole-segment approach with tissue clearing, MN tracing, and multimodal imaging, we confirmed segment-dependent differences in MN counts. Common MN markers (SMI-32, Nissl) lacked specificity, whereas choline a

Impact of Early Intervention on Motor Milestone Achievement in Spinal Muscular Atrophy: Insights from Cure SMA Survey Data.

INTRODUCTION: There is a paucity of real-world evidence on the impact of early treatment as well as polytherapy use on advanced motor milestone achievement in children with spinal muscular atrophy (SMA) with ≤ 2 survival motor neuron 2 gene (SMN2) copies. The objective of this study is to evaluate predictors of motor milestone change in children with ≤ 2 SMN2 copies, focusing on the timing of treatment initiation and the impact of treatment decisions. METHODS: This study employed a mixed longitudinal and repeated cross-sectional design. Data were drawn from the Cure SMA Community Update Surve

Levers and constraints on general practitioners offering and managing reproductive genetic carrier screening in Australian primary care: a rich graphic of the complex context.

Reproductive genetic carrier screening (RGCS) identifies people who have an increased chance of having a child with a serious genetic condition. In Australia, since November 2022, RGCS for three conditions, cystic fibrosis, spinal muscular atrophy, and fragile X syndrome, has been publicly funded via Medicare (Australia's universal health insurance scheme). Therefore, tests are offered at no cost to eligible people and are accessible through general practitioners (GPs), making primary care a key point of access. This study aimed to develop a rich graphic illustrating the ecosystem of offering

Modulating microtubule stability via α-tubulin acetylation partially restores Golgi fragmentation in spinal muscular atrophy.

BACKGROUND/AIM: Spinal muscular atrophy (SMA) is a neurodegenerative disease caused by the loss of survival of motor neuron (SMN) protein. SMN deficiency leads to perturbations of the cytoskeleton, including microtubules, which are mainly involved in motility-related cellular processes. However, the molecular mechanisms of microtubule dysregulation in SMA remain elusive. Alpha (α)-tubulin is a structural component of microtubules, and its posttranslational modifications affect microtubule dynamics. Here, we aimed to investigate α-tubulin acetylation and related molecular mechanisms in SMA. MA

Reduced skeletal muscle perfusion accompanies skeletal muscle atrophy after severe spinal cord injury.

The goal of this study was to gain insight into an understudied physiological mechanism that may influence skeletal muscle atrophy following spinal cord injury (SCI). Specifically, we quantified skeletal muscle blood flow (BF) rates in paralyzed hindlimbs throughout the acute to subacute recovery period in a severe contusion SCI model. Secondary objectives were to characterize temporal changes in circulating and tissue-level markers of capillary density and vascular dysfunction, and vascular-related gene expression within hindlimb skeletal muscle. We hypothesized that SCI would reduce skeletal

Ethics and equity in access to disease-modifying therapies and newborn screening for spinal muscular atrophy: A scoping review.

AIM: To review barriers to ethical and equitable access to disease-modifying therapies (DMTs) and newborn screening (NBS) for spinal muscular atrophy (SMA). METHOD: We searched PubMed, Scopus, Web of Science, EBSCOhost, the Cochrane Library, Google Scholar, and Primo for content on the ethics or equity of access to SMA DMTs or NBS from January 2014 to May 2025. Only evidence sources that had a primary focus on equity or ethics regarding access to either DMTs or NBS for children aged less than 18 years were eligible. RESULTS: Forty publications were included: all but one were published in Eng

MyomiR Networks in Spinal Muscular Atrophy: Associations With Clinical Severity and Treatment Response.

Disease-modifying therapies have significantly influenced the clinical course of spinal muscular atrophy (SMA), yet objective biomarkers for monitoring disease progression and treatment remain limited. We profiled four muscle-specific miRNAs (myomiRs), ten bioinformatically predicted mRNA targets, two functionally associated lncRNAs, and SMN transcripts in whole blood from 50 adults with SMA types II-IV. Using RT-qPCR, we assessed associations between baseline RNA expression and demographic and clinical parameters, including SMA type, ambulatory status, motor and respiratory function, and expl

Combining SMN2 splicing modifiers with HDAC6 inhibition improves spinal muscular atrophy outcomes.

Spinal muscular atrophy (SMA) is a severe neuromuscular disorder caused by SMN gene defects. It leads to motor neuron death and muscle weakness. Without treatment, most affected children don't survive past age two. Recently, new gene therapies help SMA children survive, but treated patients now face ongoing muscle atrophy and functional deficits, creating a novel clinical presentation. Over the last years, treatments of various animal models of neuromuscular disorders have shown the ability of inhibitors of the non-conventional histone deacetylase 6 (HDAC6) to reduce muscle atrophy. This study

Progressive Proximal Muscle Weakness Due to a 51 CAG Repeat Expansion in Exon 1 of the Androgen Receptor Gene: A Case Report of Kennedy Disease.

BACKGROUND Kennedy disease, also known as spinal and bulbar muscular atrophy (SBMA), is a rare and incurable X-linked neuromuscular disorder mainly affecting men aged 30 to 60 years. Polymyositis can present similarly, but can be excluded by measuring muscle enzymes, performing muscle imaging, and electromyography. This report describes the case of a 52-year-old man with a 10-year history of progressive limb weakness due to Kennedy disease, established by genetic testing. CASE REPORT A 52-year-old man presented with a 10-year history of gradually progressive proximal limb weakness and persiste

Payer and Manufacturer Agreements for High-Cost Therapy: A Narrative Review.

Between 2022 and 2025, several ultra-high-cost, one-time therapies were approved in the United States, offering potentially curative options for conditions such as spinal muscular atrophy, hemophilia, and sickle cell disease. Despite their clinical value, upfront costs often exceeding $2-4 million create major challenges for payers, especially Medicaid programs with fixed budgets. Traditional reimbursement models, designed for chronic therapies, may not align with the long-term benefits and financial risks of these treatments. This has led to growing interest in alternative payment models, inc

[Spinal muscular atrophy: Clinical and genetic aspects, and therapeutic alternatives].

Spinal muscular atrophy (SMA) is a neuromuscular disorder caused by a mutation in the SMN1 gene, located on chromosome 5q13. It is characterized primarily by neuronal degeneration due to a deficiency in producing full-length survival motor neuron protein (FL-SMN), which results in progressive muscle weakness with complications such as scoliosis, paralysis, and even death. This article reviews the clinical and genetic aspects of the disease, its diagnosis and classification, as well as therapeutic alternatives. In this context, it highlights the role of the molecular determination of the causal

Lung Function and Respiratory Muscle Strength in Symptomatic Children and Adults With Spinal Muscular Atrophy Treated With Nusinersen.

BACKGROUND AND OBJECTIVES: Spinal muscular atrophy (SMA) causes progressive muscle weakness. Respiratory muscle weakness results in progressive lung function decline, scoliosis, and chest wall deformities that predispose to respiratory failure. The efficacy of survival motor neuron protein-augmenting therapies on motor function in patients with SMA has been reported extensively, but how respiratory function changes under treatment remains underexplored. The aim of this study was to evaluate longitudinal changes in lung function and respiratory muscle strength during nusinersen treatment. METH
