Enhanced splicing modulation by NMA-modified antisense oligonucleotides.

Aberrant RNA splicing contributes to many human diseases, and splice-switching antisense oligonucleotides are ideally suited as a therapeutic strategy to modulate splicing and restore normal gene expression. Nusinersen (Spinraza™) has revolutionized the treatment of spinal muscular atrophy. It is a splice-switching oligonucleotide (SSO) modified with 2'-O-methoxyethyl (MOE). Here, we evaluate a next-generation ribose modification, 2'-O-[2-(methylamino)-2-oxoethyl] (NMA), which enhances the pharmacological properties of SSOs. We identified a long-lasting NMA-modified human candidate SSO, salane

Perioperative Management for Patients With Spinal Muscular Atrophy Undergoing Scoliosis Surgery: A Single-Center Retrospective Study.

OBJECTIVE: Spinal muscular atrophy (SMA) is a rare neuromuscular disorder frequently associated with progressive scoliosis, often requiring complex surgical intervention. Perioperative management for SMA patients remains challenging due to severe respiratory compromise, difficult airway anatomy, and a high risk of perioperative complications. This study aimed to analyze perioperative strategies and outcomes in SMA patients undergoing posterior spinal fusion for scoliosis correction. METHODS: We retrospectively reviewed SMA patients who underwent posterior spinal fusion at our institution from

An approach for the systematic profiling of drug-induced remodeling of RNA-RBP (RNA-binding protein) interactions.

Nucleic acid-based therapeutics targeting RNA-related regulatory networks have received substantial attention in drug development, among which the interventions of RNA-RBP (RNA-binding protein) interactions or RNA-protein complex assemblies represent a promising strategy for treating various diseases. RBPs play essential roles in post-transcriptional regulation, and their interactions with RNA are closely associated with physiological homeostasis and pathological progression. Although RNA-RBP interactions are increasingly recognized as therapeutic targets, the lack of systematic methods for sc

Long-term comparative analysis of AAV9-mediated gene replacement therapies for spinal muscular atrophy in mice.

Spinal muscular atrophy (SMA) results from a deficiency of the survival motor neuron (SMN) protein. Zolgensma, an adeno-associated virus (AAV)-based SMN1 gene-replacement therapy, is approved for SMA, though its long-term efficacy and safety remain uncertain. This study compares a Zolgensma-like benchmark vector with a 2nd-generation vector featuring a codon-optimized SMN1 transgene under the control of an endogenous SMN1 promoter. In SMA mice, intracerebroventricular delivery of the 2nd-generation vector improved survival and phenotypic outcomes compared with the benchmark. However, motor imp

Nanotube-Assisted Motor Neuron and Neuromuscular Junction Stabilization in Spinal Muscular Atrophy: A Hypothesis for Adjunctive Therapy.

Spinal muscular atrophy (SMA) therapies that restore SMN expression improve survival and motor function but often fail to fully stabilize distal motor units or sustain endurance. We propose a hypothesis-driven adjunctive approach, intended to complement SMN-restoring therapies, in which localized nanotube-enabled interfaces acting at or near the distal motor unit and neuromuscular junction enhance neuromuscular transmission reliability in surviving, remodeled motor units. The model predicts a temporal cascade: improved junctional reliability and reduced activity-dependent failure, followed by

Disease-Modifying Therapies in Spinal Muscular Atrophy: Neurodevelopmental and Behavioral Outcomes in the Treatment Era.

Spinal muscular atrophy (SMA) is a monogenic neuromuscular disorder caused by deficiency of the survival motor neuron (SMN) protein. Historically, SMA was regarded primarily as a disorder of lower motor neuron degeneration in which cognitive function was considered preserved. However, the advent of disease-modifying therapies (DMTs) has altered survival and motor outcomes, bringing cognitive, neurodevelopmental, and behavioral aspects of SMA into sharper clinical focus. This review re-examines cognitive and neurodevelopmental outcomes in SMA across historical and contemporary contexts, integra

Rare SMA Patients: A Comprehensive Look at Clinical Features, Genetic Profiles and Therapeutic Approaches.

Spinal muscular atrophy 5q (5q SMA) is one of the most prevalent autosomal recessive disorders globally. The underlying cause of 5q SMA is attributed to variants in SMN1. To date, there are no reported cases of gene-based therapy in rare patients with 5q SMA caused by subtle SMN1 variants of unknown clinical significance. We included 10 patients with the clinical manifestations of 5q SMA associated with intragenic variants in combination with a heterozygous SMN1 deletion in this retrospective study. Previously reported pathogenic or likely pathogenic variants were identified (e.g., c.*3+1del,

Advancements in Prenatal Diagnosis and Potential Fetal Therapies for Spinal Muscular Atrophy.

PURPOSE: Spinal Muscular Atrophy (SMA) is a rare autosomal recessive disorder caused by SMN1 gene mutations, resulting in muscle weakness and atrophy, respiratory failure, and death. SMA disease modifying therapies (DMTs) include the antisense oligonucleotide (ASO) nusinersen administered intrathecally, onasemnogene abeparvovec, single-dose intravenous gene replacement therapy that introduces functional SMN1 via an adeno-associated viral vector, and oral risdiplam, which modifies SMN2 splicing to increase SMN protein production. With DMTs, infants can achieve previously unattainable developmen

External controls for rare disease drug development: Lessons for emerging and advanced therapeutic modalities.

External controls, particularly natural history studies, play an increasingly important role in rare disease therapeutic development where traditional randomized trials are often infeasible. This review examines regulatory acceptance patterns for gene and cell therapies approved between 2019 and 2025, analyzing successful cases like onasemnogene abeparvovec (Zolgensma) for spinal muscular atrophy and elivaldogene autotemcel (Skysona) for cerebral adrenoleukodystrophy alongside unsuccessful applications. Key success factors include systematic data collection, clinically meaningful endpoints, ap

Behavioral barriers in the management of spinal muscular atrophy: The role of procrastination, regret, and burnout.

BACKGROUND: Decision-making in complex medical conditions is a cognitively demanding task influenced by clinician-specific behavioral factors. Procrastination, the voluntary delay of intended actions despite foreseeable adverse outcomes, is a self-regulation failure often exacerbated by high-stress clinical environments. This study evaluated the prevalence of procrastination among healthcare professionals (HCPs) managing spinal muscular atrophy (SMA) and analyzed its associations with burnout and healthcare-related regret. METHODS: We conducted a non-interventional, cross-sectional, web-based

Extracellular vesicles as biomarkers of disease progression and therapeutic response in patients with spinal muscular atrophy.

Spinal muscular atrophy (SMA) is a devastating genetic disorder characterized by loss of motor neurons and muscle atrophy. In the most severe form, affected infants experience progressive weakness and, if untreated, typically do not survive beyond 2 years of age. Although several disease-modifying therapies are currently available, treatment response varies and there are no clinically available molecular biomarkers to accurately assess therapeutic efficacy. Extracellular vesicles (EVs) are small, membrane-bound nanoparticles released from all cell types, and contain a diverse cargo reflective

Cost-Effectiveness of Universal Newborn Screening and Disease-Modifying Therapies for Spinal Muscular Atrophy in Taiwan.

OBJECTIVES: Spinal muscular atrophy (SMA) is a rare neuromuscular disorder and an important genetic cause of infant mortality. Universal newborn screening (NBS) enables presymptomatic disease-modifying therapy (DMT) but increases upfront costs. This study evaluated the cost-effectiveness of universal SMA NBS in Taiwan. METHODS: This study built a state-transition simulation model for a hypothetical SMA birth cohort using local epidemiology and costs and published clinical inputs. Strategies were as follows: (1) NBS with presymptomatic DMTs and (2) clinical identification (CI) with symptomatic

Determinants of hepatic enzyme elevations following onasemnogene abeparvovec: Results from a unified immunomodulation protocol.

IntroductionOnasemnogene abeparvovec is an adeno-associated virus serotype 9 (AAV9) gene therapy for spinal muscular atrophy (SMA) that is frequently associated with immune-mediated hepatotoxicity. Previous studies have suggested that older age and higher body weight may increase the risk or severity of hepatic enzyme elevation; however, interpretation is limited by heterogeneous immunosuppression strategies across centers.MethodWe conducted a retrospective cohort study of children with SMA who received onasemnogene abeparvovec between 2020 and 2025 and were managed using a unified, protocol-d
