Necroptosis is a regulated and inflammatory form of cell death that has emerged as a key contributor to neuromuscular junction (NMJ) dysfunction. This narrative review aims to synthesize current evidence on the role of necroptosis in NMJ pathology and its potential therapeutic implications. First, we present the conceptual framework linking necroptosis to NMJ degradation, focusing on core mediators such as receptor-interacting protein kinase 1 (RIPK1), RIPK3, and mixed lineage kinase domain-like pseudokinase (MLKL). Second, we summarize the available evidence across NMJ compartments, including

BACKGROUND: Most spinal muscular atrophy (SMA) carrier screening assays quantify SMN1 exon 7 copy number, providing limited information about the prevalence or structure of SMN1-SMN2 hybrid alleles generated through gene conversion. As long-read sequencing (LRS) studies begin to resolve these alleles at high resolution, typically in modest cohorts, large population-scale data remain essential for defining the broader allele-frequency context in which such structural signatures occur.
METHODS: We evaluated SMN1 exon 7 and exon 8 copy number in 18,015 individuals undergoing expanded carrier scr

Spinal muscular atrophy type 4 (SMA‑4) represents a rare, late‑onset motor neuronopathy with insidious proximal limb weakness and preserved walking ability. Its slow progression and vague initial symptoms often cause years of diagnostic uncertainty. We describe a 38‑year‑old man who experienced slowly worsening, fatigue‑related proximal leg weakness over three years. Initial suspicion fell on a metabolic myopathy, and later, lumbar spine magnetic resonance imaging (MRI) showed multilevel degenerative changes with canal stenosis, findings that were initially attributed to the cause but did not

BACKGROUND AND OBJECTIVE: Nusinersen, onasemnogene abeparvovec-xioi, and risdiplam are disease-modifying therapies that have demonstrated clinical benefits for patients with spinal muscular atrophy. However, their high costs pose significant challenges for healthcare payers. This study evaluated the potential budget impact of these treatments compared with best supportive care from a US healthcare payer perspective.
METHODS: A budget impact analysis was conducted over a 5-year time horizon using a state-transition model to estimate direct healthcare costs associated with each disease-modifyin

L-type amino acid transporter 1 (LAT1, encoded by Slc7a5) contributes to amino acid homeostasis and signaling in numerous cell types. Several lines of evidence implicate LAT1 in mammalian central nervous system development, but its functional significance in specific neuronal subtypes is largely unknown. Here, we demonstrate that LAT1/Slc7a5 expression in synapsin 1 (Syn1)-expressing neurons is essential for motor circuit development and motor coordination at the perinatal stage. Mice lacking Slc7a5 in Syn1-expressing neurons exhibited progressive motor coordination deficits and early postnata

Amyotrophic lateral sclerosis (ALS) is the most common motor neuron disease, marked by progressive degeneration of upper and lower motor neurons. Clinically, genetically, and pathologically heterogeneous, ALS poses a major challenge for disease modeling and therapeutic translation. Over the past two decades, induced pluripotent stem cells (iPSCs) have reshaped our understanding of ALS pathogenesis and emerged as a promising translational platform for therapy development. ALS modeling has further expanded with the advent of three-dimensional systems, including ALS-on-chip platforms and organoid

BACKGROUND AND OBJECTIVES: Amyotrophic lateral sclerosis (ALS) is a progressive and fatal neurodegenerative motor neuron disease with a pathophysiology that features dysregulated natural killer (NK) cells that are capable of damaging neurons. Although NK cells are associated with ALS progression and survival, their specific characteristics and how these characteristics change over the course of disease is unknown. The current study examines NK cell gene expression during ALS with the goal of identifying dysregulated genes and pathways in NK cells over the course of disease in order to identify

Extracellular vesicles (EVs) have emerged as pivotal modulators of neuromuscular junction (NMJ) biology, reshaping our understanding of synaptic communication, maintenance, and degeneration. This review consolidates current insights into the roles of EVs derived from motor neurons, muscle fibers, and Schwann cells in regulating NMJ integrity. In healthy states, EVs deliver trophic factors, structural proteins, and regulatory RNAs that promote the clustering of acetylcholine receptors, presynaptic stability, and axonal growth. Motor neuron EVs carry Wnt7a, synaptophysin, and PGC-1α, while muscl

OBJECTIVE: Amyotrophic lateral sclerosis (ALS) is a clinically heterogeneous disease influenced by multiple biological processes. Altered energy metabolism, particularly hypermetabolism, is observed in some people living with ALS (plwALS) and is associated with faster progression. The mechanisms underlying this metabolic phenotype remain unclear. Inflammation has been proposed as a contributing factor. We examined whether plasma cytokines, as markers of systemic inflammation, are associated with energy expenditure in plwALS.
METHODS: Plasma samples from 77 plwALS and 90 non-neurodegenerative

Spinal muscular atrophy (SMA) is caused by mutations in the Survival Motor Neuron 1 (SMN1) gene, leading to reduced SMN protein levels and widespread disruption in RNA metabolism. Despite of disease-modifying therapies, which remarkably improve patient outcomes, the long-term effects remain unknown. Dysregulated small non-coding RNAs, including microRNAs, have been reported in SMA, but their contribution to the disease remains unclear. We previously showed that flunarizine could improve the phenotype of an SMA mouse model; however its mode of action is incomplete. Here, we showed that flunariz

BACKGROUND: Administering intrathecal nusinersen to patients with spinal muscular atrophy (SMA) is commonly done under fluoroscopic guidance, and the use of the transforaminal intrathecal route has also been reported to be successful without major complications. However, this case report presents a patient who developed radicular neuritis following a transforaminal administration of nusinersen.
CASE REPORT: A 28-year-old patient with SMA type 2 and severe scoliosis experienced persistent back and leg pain after a transforaminal intrathecal injection. Lumbar magnetic resonance imaging revealed

Long-read sequencing, paralog-aware variant calling, and telomere-to-telomere (T2T) human genome assemblies now enable the resolution of copy-, haplotype-, and nucleotide-level complexities in segmentally duplicated loci, which were previously inaccessible with short-read sequencing. In this review, we highlight how current technologies and analysis methods reveal extensive diversity in copy number (CN), structure, and gene conversion within the spinal muscular atrophy-associated survival motor neuron (SMN) locus. We summarize how understanding population-level structural variation could be tr

The discovery of the SMN1 gene on chromosome 5q in 1995, and later, identification of SMN2 as a modifier gene was the breakthrough in the history of spinal muscular atrophy (SMA). It was the discovery of this gene that led to the discovery of 3 disease-modifying drugs that were approved for use by the US Food and Drug Administration and the European Medicines Agency. This brings to one's mind the fact that the earliest description of this disease happened a century ago. The persistent efforts of a few scientists have rewritten the destiny of children with SMA. With the discovery of the new dru

UNLABELLED: Spinal muscular atrophy (SMA) is an uncommon crippling hereditary condition that primarily affects the motor neurons in the spinal cord, resulting in skeletal muscle weakness and atrophy. Skeletal muscle is an important regulator of glucose, amino acid, and lipid homeostasis and is a protein reservoir of the body. Atrophy of the skeletal muscle can affect locomotion, general metabolism, eating, and respiration. Maintaining skeletal muscle mass and strength throughout life is essential for longevity and good health. Currently, orofacial muscle electromyographic evaluation and digita
