Therapeutic advances in spinal muscular atrophy: a review of clinical, safety, and economic considerations.

Spinal muscular atrophy (SMA) is a genetic neuromuscular disorder caused by insufficient levels of survival motor neuron (SMN) protein due to mutations in the SMN1 gene, leading to progressive degeneration of lower motor neurons in the spinal cord. In this comprehensive narrative review, we extensively summarise the clinical efficacy and real-world effectiveness of approved disease-modifying therapies, including nusinersen, onasemnogene abeparvovec, and risdiplam, underscoring significant improvements in survival, motor milestones, and quality of life, particularly with early treatment initiat

Clinical Practice of Single-Molecule Real-Time Sequencing in the Diagnosis of Spinal Muscular Atrophy.

OBJECTIVES: To investigate the distribution proportions of the SMN1 gene in spinal muscular atrophy (SMA) using single-molecule real-time sequencing (SMRT), thereby providing supplementary evidence for SMA genotyping. METHODS: A retrospective analysis was conducted on 771 subjects who underwent both real-time quantitative PCR and SMRT sequencing tests, followed by statistical analysis. RESULTS: Among the 771 enrolled individuals, the male-to-female ratio was 2:3, with a median age of 31 years. The detection results obtained from real-time quantitative PCR and SMRT sequencing were consistent.

When the Electromyography (EMG) and the Patient Disagree: Unraveling a Motor Neuron Disease Mimic With Small-Fiber Dysfunction.

Amyotrophic lateral sclerosis (ALS) is a progressive neurodegenerative disorder characterized by upper and lower motor neuron dysfunction. Electromyography (EMG) is central to its evaluation, but neurogenic abnormalities and fasciculations are not specific to ALS and must be interpreted within the clinical phenotype and disease trajectory. This study reports a 52-year-old man with an approximately eight-year history of intermittent muscle twitching, bilateral upper-limb tremor, muscular tightness and warmth, and asymmetric cutaneous hypersensitivity. An initial electrodiagnostic evaluation rai

Muscle biopsy to end a diagnostic odyssey in a genetically complex case of spinal muscular atrophy type 3: the importance of "old school" in the next-generation sequencing era.

Spinal muscular atrophy (SMA) is an autosomal recessively inherited disease leading to severe and progressive muscle weakness due to loss of motor neurons in the spinal cord and the brainstem. Motor neuron degeneration is caused by the absence of a functional SMN1 gene copy, a gene located on 5q13.2, encoding the SMN ("survival of motor neuron") protein. Initially classified into five groups with varying severity (SMA type 0-4), SMA is now regarded as a progressive neurodegenerative disease with a continuum of clinical severity, which genetic and epigenetic modifiers can modulate. The highly h

Progressive anterior opercular syndrome (Foix-Chavany-Marie syndrome) in neurodegenerative disease: a case series.

Anterior opercular syndrome (Foix-Chavany-Marie syndrome) is characterized by severe dysarthria, impaired voluntary orofacial and pharyngolingual movements, and preserved automatic or emotional movements (automatic-voluntary dissociation). Although classically associated with stroke, its progressive neurodegenerative form is rare and poorly understood. We report two patients to characterize its clinical and neuroanatomical features. The cases were identified retrospectively through review of clinical and neuroimaging records from behavioral neurology centers in the United States and Japan. Bot

Current concepts review: Low-vs high-tone neuromuscular spinal deformities: An evolving distinction with clinical implications.

BACKGROUND: Neuromuscular scoliosis (NMS) was historically classified into two groups as neuropathic and myopathic, which offers limited benefit beyond describing the underlying pathology. More recently, NMS is being classified according to changes in muscle tone, which more accurately characterizes presenting features, and also helps emphasize the critical differences in management. CLINICAL PRESENTATION: High-tone NMS, with cerebral palsy (CP) as a typical example, is characterized by spasticity, rigidity, and often reduced flexibility. Low-tone NMS, as in spinal muscular atrophy (SMA) and

LRSAM1, an E3 ubiquitin ligase, modulates TDP-43 proteostasis in amyotrophic lateral sclerosis.

Amyotrophic lateral sclerosis is a fatal neurodegenerative disease characterized by the cytoplasmic mislocalization, aberrant phosphorylation and pathological aggregation of TDP-43, a nuclear RNA-binding protein essential for RNA metabolism. Despite its central involvement in ALS pathogenesis, the molecular mechanisms that govern TDP-43 proteostasis remain incompletely understood. Here, we investigated the role of LRSAM1, an E3 ubiquitin ligase, in regulating TDP-43 localization and degradation. Postmortem spinal cord tissues from five patients with sporadic amyotrophic lateral sclerosis and f

Tissue-specific models of spinal muscular atrophy identify muscle functions required for Drosophila neuromuscular junction architecture.

Spinal Muscular Atrophy (SMA) is a neuromuscular disorder associated with motor neuron degeneration, yet the role of the well-conserved Survival of Motor Neuron (SMN) protein in muscle remains insufficiently understood. Using the Drosophila neuromuscular junction (NMJ) as a model, we combined tissue-specific Smn depletion with transcriptomic profiling and prior genetic modifier screens to identify SMN-dependent genes that contribute to synaptic architecture. Functional screening of candidate genes significantly and selectively effected in muscle identified genes whose knockdown phenocopied NMJ

Ubiquitin-Dependent Protein Quality Control in Polyglutamine Diseases.

Polyglutamine diseases comprise a family of nine age-dependent disorders caused by CAG triplet-repeat expansions that produce misfolded proteins with elongated glutamine tracts. Although these diseases share proteotoxic stress and engagement of protein quality control pathways as responses to their presence and disruptive activities, they differ in protein context, normal function, localization, interactomes and selective vulnerability. Here, we review how expanded polyglutamine proteins engage ubiquitin-dependent protein quality control across Huntington's disease, dentatorubral-pallidoluysia

Development of Antisense Oligonucleotide Gapmers for the Treatment of Dyslipidemia and Lipodystrophy.

Although technological advances in molecular genetics over the last few decades have greatly expedited the identification of mutations in many genetic diseases, the translation of the genetic mechanisms into a clinical setting has been quite challenging, with a minimum number of effective treatments available. The advancements in antisense therapy have revolutionized the field of neuromuscular disorders as well as lipid-mediated diseases. With the approval of splice-switching antisense oligonucleotide (AO)-based therapies for the treatment of spinal muscular atrophy (SMA) and Duchenne muscular

Potential Roles of Neuroimaging in Motor Neuron Disease.

PURPOSE OF REVIEW: The review is intended to summarize and discuss the increasing roles and perspectives of neuroimaging in the clinical and scientific work-up of amyotrophic lateral sclerosis (ALS) and other motor neuron diseases (MND). RECENT FINDINGS: The applications of magnetic resonance imaging (MRI) and positron emission tomography (PET) identify characteristic disease-specific patterns especially for ALS including its clinical subtypes. Guided by neuroanatomical pattern stratification, these MND-associated alterations could be detected by dedicated analysis parameters from MRI, includ

Consensus for the Diagnosis and Treatment of Patients With Spinal Muscular Atrophy (SMA) in Latin America.

BACKGROUND: 5q spinal muscular atrophy (5q SMA) is an autosomal recessive neuromuscular disorder caused by mutations in the survival motor neuron 1 gene (SMN1), leading to deficiency of SMN protein and a variable clinical spectrum influenced by survival motor neuron 2 gene (SMN2) copy number. The advent of disease-modifying therapies (DMTs) has transformed its clinical course, requiring timely diagnosis, multidisciplinary care, and longitudinal response assessment. In Latin America (LATAM), significant gaps and heterogeneity persist in access to genetic diagnosis, newborn screening, DMTs, and

Biphasic SMN condensates safeguard stress-induced cytoplasmic mislocalization of splicing RNPs.

Biogenesis of UsnRNPs occurs in distinct steps in the nucleus and the cytoplasm. Sequential cytoplasmic actions of CLNS1A in the PRMT5 complex and of the SMN complex assemble the Sm core structure consisting of RNA and proteins. Nuclear SMN, condensed in Cajal Bodies, promotes late maturation steps. Whether cytoplasmic SMN undergoes condensation, and how this contributes to UsnRNP biogenesis or homeostasis, is poorly defined. Here, we show that molecular crowding stress induces rapid, reversible condensation of cytoplasmic SMN into droplets and filamentous assemblies, S-bodies, that sequester

Fibro-Adipogenic Progenitor Ablation Triggers Muscle Atrophy Through Cell Death-Induced Inflammation.

BACKGROUND: Skeletal muscle mass maintenance involves coordination of myofibers with mononuclear cell populations, including satellite cells, resident macrophages and fibro-adipogenic progenitors (FAPs). FAPs, identified by surface expression of PDGFRα, are important contributors to muscle homeostasis, as genetic ablation of FAPs induces rapid muscle atrophy. However, mechanisms underlying this response remain poorly understood. METHODS: We utilized tamoxifen-inducible FAP specific diptheria toxin (DTA) mice (PdgfraCre-ERT2/+; Rosa26^DTA/+) to evaluate consequences of FAP deletion, measuring

Broader autism phenotype, the default mode network, and brain entropy: A network-level hypothesis.

Despite well-documented disparities by sexual and gender minority (SGM) status in suicide attempt and mortality rates, few studies have investigated the lived experiences that contribute to SGM people's disproportionate risk of suicide. Having a history of at least one near-fatal (or medically serious) suicide attempt serves as a proxy for suicide mortality, but no known study has involved SGM people who have made such an attempt. Ideation-to-action theories of suicide posit that individuals acquire the capability for suicide through repeated exposure to painful and provocative events - namely
