A study of environmental factors in relation to fatal infantile spinal muscular atrophy (SMA type I). An analysis of birth order and parental age effects, social class, seasonal influence, and clustering in time and place.

Major studies of the childhood spinal muscular atrophies have suggested that environmentally-produced phenocopies might occur, and that environmental factors might be important in some clinical features of these diseases. A formal analysis of some possible intra-uterine and post-natal influences has been undertaken in 78 index cases (72 families) of acute infantile SMA (acute Werdnig-Hoffmann disease; SMA Type I). There is no evidence to suggest that social class, parental age, birth order, or season of birth influences this disease or that clustering in time and place occurs.

A genetic study of subacute and chronic spinal muscular atrophy in childhood. A nosological analysis of 124 index patients.

A genetic study of the subacute spinal muscular atrophies (SMA) of late infancy and early childhood has been undertaken. All such patients with chronic disease (with ages at onset up to 14 years, and excluding SMA Type I) known to 2 large Neurological Centres were reassessed clinically and genetically. There were 124 index patients (67 females and 57 males) and 17 secondary cases, which formed two consecutive unselected series. To investigate the genetic composition of this group, 4 nosological approaches were used; cluster analysis of clinical features of the disease, Haldane's sib-sib analys

Genetic studies of acute infantile spinal muscular atrophy (SMA type I). An analysis of sex ratios, segregation ratios, and sex influence.

An analysis of segregation and sex ratios, and of sex influence, was undertaken in a series of 78 index patients with acute infantile spinal muscular atrophy (SMA type I). The sex ratio of index patients was 2.0, and the excess of males was shown to occur principally among sporadic cases. The sex ratio of familial cases did not differ significantly from 1.0. The implications of this are discussed. No sex influence on age at onset, or on life expectancy, was present. The segregation ratio (Weinberg Proband method) was 0.29 for all index cases, and 0.26 for all cases excluding those referred spe

Anterior horn cell disease associated with pontocerebellar hypoplasia in infants.

Three sibs presented with an identical clinical picture of severe mental retardation, cortical blindness, and extensive peripheral paralysis of lower motor neurone type, and died before one year of age. In the one necropsied case, spinal cord lesions, indistinguishable form those of Werding-Hoffman disease, were associated with extreme hypoplasia and atrophy of the cerebellum, and with atrophy of the ventral part of the pons. No prominent abnormalities were found in the nerves sampled despite gross reduction of motor and sensory conduction velocities in two infants. It is proposed that this fa

High incidence of spinal muscular atrophy type I (Werdnig - Hoffmann disease) in the Karaite community in Israel.

Spinal muscular atrophy (S.M.A.) type I (Infantile werdnig - Hoffmann Disease) was found in 4:1,600 (1:400) infants of the Egyptian Karaite community. This group constitutes a representative sample of the Egyptian Karaite community in Israel, which numbers at present somewhat more than 7,000. The community existed as a religious and reproductive isolate for over 10 centuries. The very high gene frequency, 0.05 for the autosomal recessive gene of S.M.A. type I, is probably the result of genetic drift.

Peripheral motor and sensory neuropathy of early childhood, simulating Werdnig-Hoffmann disease.

A five-year-old girl developed neurogenic muscular atrophy during infancy. On the basis of clinical findings and a muscle biopsy study a diagnosis of infantile spinal muscular atrophy, benign type II, was entertained. As the disease progressed, involvement of sensory pathways was detected by electrophysiological studies. Examination of a sural nerve specimen suggested a chronic polyneuropathy rather than spinal muscular atrophy. These findings were confirmed at autopsy by demonstrating severe loss of myelinated axons in ventral and dorsal spinal roots, peripheral nerves and dorsal columns of t

Distinct subtypes of type I fibers of human skeletal muscle.

We have identified two distinct and consistent type I fiber subtypes in histochemically normal biopsies (vastus lateralis or biceps brachialis) from 12 healthy persons, both sexes, ages 15 to 42. The type I fibers were uniformly light with the regular ATPase (9.4) and uniformly dark with the acid-preincubated reverse ATPase reactions. However, the subtype IA fibers were stained darker than subtype IB fibers with the nicotinamide adenine nucleotide dehydrogenase-tetrazolium reductase, Oil red O, reduced nicotinamide adenine dinucleotide phosphate-tetrazolium reductase, nonspecific esterase, suc

Electromyographic studies in parents of children with spinal muscular atrophy.

Electromyographic studies have been carried out on the quadriceps and deltoid muscles of a number of healthy parents of children with infantile spinal muscular atrophy (types I and II) and the results compared with those obtained in healthy controls of the same sex. The results indicated an increase in mean amplitude of action potentials in quadriceps and deltoid muscles in both fathers and mothers (statistically significant in the quadriceps in the latter) of children with type I spinal muscular atrophy (Werdnig-Hoffmann disease). Increase in the mean amplitude in the quadriceps and deltoid m

Chronic generalized spinal muscular atrophy of infancy and childhood. Arrested Werdnig-Hoffman disease.

Recent studies have shown that the acute fatal form of infantile spinal muscular atrophy (acute Werdnig-Hoffmann disease or spinal muscular atrophy Type I) is a distinct genetic and clinical entity. This has prompted clinical re-examination of the disease known as `arrested Werdnig-Hoffmann disease' which hitherto was thought to be a spectrum variant of the acute fatal form. A total of 18 such patients with the chronic generalized form of spinal muscular atrophy has been known to The Hospital for Sick Children over the past 10 years. Patients with this characteristic clinical syndrome comprise

Possible neurogenic factor in muscular dystrophy: its similarity to denervation atrophy.

Muscle biopsy specimens from 179 cases of muscular dystrophies and from 140 cases of anterior horn cell disorders (from a total of 1,348 biopsied patients) were examined histologically. There were 72 cases of Duchenne type muscular dystrophy (DMD), five of Becker type MD, four girls with myopathy resembling DMD, 40 with limb-girdle, 10 with facioscapulohumeral, seven with late onset, 13 with congenital, and 28 with unclassifiable muscular dystrophies. Groups of small atrophied muscle fibres were encountered in 42 (23%) of the cases in this group, most frequently in patients with limb-girdle, f
