The environment in childhood and risk of motor neuron disease.

To investigate a possible link between subclinical infection with poliovirus in childhood and increased risk of motor neuron disease in adult life, environmental determinants of infection in early life were compared in 98 cases of motor neuron disease and 335 age and sex matched controls. A weak but consistent relation was found between motor neuron disease and factors in the childhood environment known to increase likelihood of enteric infection. Relative risks associated with spending the first 10 years of life in a house without domestic amenities such as a bathroom, running hot water or fl

Motor neuron disease and polio in Scotland.

An analysis of mortality and morbidity rates for motor neuron disease (MND) in Scotland has confirmed earlier observations that the disease is more common in men and older age groups. The geographical distribution is non-uniform and related to discharge rates for all neurological diseases. Discharge and mortality rates are increasing but there has been no decline in populations who would have been vaccinated against polio.

Amyotrophic lateral sclerosis and life-sustaining therapy: patients' desires for information, participation in decision making, and life-sustaining therapy.

To identify the wishes of patients with amyotrophic lateral sclerosis (ALS) for information, participation in decision making, and life-sustaining therapy and to determine whether these wishes are stable over time, we conducted a prospective survey (baseline and 6-month follow-up interviews) of 38 consecutive patients with an established diagnosis of ALS at the University of Chicago Motor Neuron Disease Clinic. Demographic data, clinical stage of ALS, illness experience, wishes for information, and desires for participating in decisions about life-sustaining therapy were elicited. Patients rea

Immunosuppressive treatment in multifocal motor neuropathy.

We report the results of immunosuppressive treatments of 13 patients with multifocal motor neuropathy and elevated titers of serum antibodies to the GM1 ganglioside. All patients failed to respond to oral prednisone. There was no clinical response in 4 patients treated with plasma exchange. Nine patients received cyclophosphamide, with clinical improvement and fall in antibody titers in 8. In 3 patients, cyclophosphamide was discontinued with ensuing clinical relapse and rise in the titers of serum anti-GM1 antibodies. These patients provide further evidence for the efficacy of cyclophosphamid

Invited review: motor neuropathies, motor neuron disorders, and antiglycolipid antibodies.

High titers of IgM anti-GM1 antibodies are commonly found in the serum of patients with some lower motor neuron disorders and peripheral neuropathies. Enzyme-linked immunosorbent assays (ELISA) are useful for the detection and quantitation of anti-GM1 antibodies. Testing for serum anti-GM1 activity is indicated in the diagnostic evaluation of lower motor neuron syndromes. The presence of high titers of anti-GM1 antibodies mandates careful electrophysiologic testing for the motor conduction block that is found in multifocal motor neuropathy, a treatable disorder. Quantitation of anti-GM1 antibo

[Macro electromyography in motor neuron disease].

Macroelectromyography (macroEMG) is a technique that provides information about the size of a motor unit which the conventional EMG is not able to. We used macroEMG to study 11 cases of motor neuron disease (MND), presenting with upper extremities symptoms (no lower extremities symptoms). Muscles we selected to study were the biceps brachii and the tibialis anterior. Amplitude, area and fiber density (FD) were analysed. The controlled groups consisted of 11 cases of myasthenia gravis (MG) and 11 normal subjects. Our study demonstrated that in the MND group, amplitude, area and FD of both muscl

Identification of glycoconjugates which are targets for anti-Gal(beta 1-3)GalNAc autoantibodies in spinal motor neurons.

Human IgM anti-Gal(beta 1-3)GalNAc antibodies which bind to GM1 and GD1b, are implicated in the pathogenesis of predominantly motor neuropathy or motor neuron disease. By immunofluorescence microscopy, the human antibodies immunostain the surface of motor neurons from bovine spinal cord. The motor neurons are also immunostained by cholera toxin (CT), which is specific for GM1. Glycolipid analysis using thin-layer chromatography (TLC) and immunostaining reveals that the relative concentration of GM1 and GD1b in motor neurons is greatly reduced in comparison to whole spinal cord, and that other

Transcranial stimulation of motor cortex in upper motor neurone syndrome: its relation to the motor deficit.

The purpose of this investigation was to clarify the functional significance of the fastest cortico-motoneuronal connections in chronic upper motor neurone syndromes. Using magneto-electrical stimulation of motor cortex the intactness of cortico-motoneuronal connections was assessed in 51 patients presenting with variable degrees of impairment. There was a gross correlation between clinical impairment of the patient and the degree of pathology of cortico-motoneuronal efferents. Covariation of clinical data with transcranial stimulation was better than covariation with the size of lesion on CT

Electrical and magnetic transcranial stimulation in patients with corticospinal damage due to stroke or motor neurone disease.

Twenty patients with hemiplegia and 13 patients with motor neurone disease were studied with electrical and magnetic transcranial stimulation. Motor evoked potentials were recorded from the biceps, thenar and tibialis anterior muscles. In both groups of patients magnetic stimulation with a Novametrix stimulator revealed fewer abnormalities than electrical stimulation with a Digitimer D180 stimulator. In patients with hemiplegia, motor evoked potentials after electrical stimulation were absent in 70% of muscles, delayed in 22% and normal in 8%; after magnetic stimulation, they were absent in 53

Dysarthria of motor neuron disease: longitudinal measures of segmental durations.

Motor neuron disease encompasses a group of terminal, demyelinating diseases affecting upper- and lower-motor neurons and producing muscular weakness resulting in a flaccid, spastic, or spastic-flaccid dysarthria of speech. The present study presents measurements of the temporal-acoustic characteristics of dysarthria in three subjects with Motor Neuron Disease over a two-year recording period. Changes seen over the course of the disease varied by type of motor neuron disease, though all types demonstrated some degree of neutralization of the prevocalic VOT, target vowel duration, and postvocal

Subacute, reversible motor neuron disease.

Four patients with a clinical syndrome closely resembling amyotrophic lateral sclerosis recovered completely, without treatment, 5 to 12 months after onset. Electrodiagnostic tests revealed acute and chronic denervation, with normal motor and sensory nerve conduction studies. The CSF was normal, and tests for paraproteinemia, heavy metal intoxication, and systemic illness were negative. Although such cases are rare, the possibility of spontaneous recovery should always be considered when counseling patients with suspected ALS.

Association between glycoconjugate antibodies and Campylobacter infection in patients with Guillain-Barré syndrome.

In a retrospective study, we have analysed sera from a well-characterised Guillain-Barré syndrome (GBS) patient group for antibodies that react with gangliosides. Of 95 GBS patients and 85 control patients analysed, we found that 14 (15%) of GBS patients but only one control patient had antibodies that react with the gangliosides GM1 and/or GD1b but not GM2, GD1a and GT1b using a sensitive enzyme-linked immunosorbent assay (ELISA). This pattern of reactivity suggests binding to the carbohydrate structure Gal(beta 1-3)GalNAc which is shared between some glycolipids and glycoproteins. Similar an

Clinical correlations of anti-GM1 antibodies in amyotrophic lateral sclerosis and neuropathies.

Clinical correlations of antiganglioside GM1 antibodies are important because high titers of these antibodies may have therapeutic significance. To further evaluate this significance, we reviewed our experience with 78 patients who had the following diagnoses: amyotrophic lateral sclerosis (ALS), ALS syndromes in patients with gammopathy or thyroid abnormalities, cervical spondylosis simulating ALS, motor neuropathies, and chronic inflammatory demyelinating polyneuropathies (CIDP). Antiganglioside antibody titers were measured "blind" by ELISA assay at the neuromuscular clinical laboratory, Jo

Comparison of electrodiagnostic criteria for primary demyelination in chronic polyneuropathy.

Three sets of electrodiagnostic criteria for establishing primary demyelination in chronic polyneuropathy are evaluated. Sensitivity is assessed in 70 patients with clinically established chronic inflammatory demyelinating polyradiculoneuropathy (CIDP). The criteria use different abnormal values, one adjusts for the effects of axonal loss, while another relies only on conduction velocity. However, even when consideration is given to sufficient number of nerves tested, there is no significant difference (P = 0.37) in diagnostic sensitivity among them, with 48% to 64% of CIDP patients fulfilling

Motor neuron disease and dementia reported among 13 members of a single family.

All 49 members of four generations of a family were identified. In the first three generations eight members were afflicted with dementia, whereas in the fourth generation only one was demented but three of four were afflicted with motor neuron disease and they also had slight cognitive deficiencies. The pattern of heredity is compatible with dominant autosomal inheritance. Neuropsychological testing revealed affection mostly of the frontal lobes. A pedigree and six case reports are presented.

Lower motor neuron degeneration and familial predisposition to colonic neoplasia in two adult siblings.

A previously unreported association between a familial predisposition to colonic neoplasia and familial adult onset lower motor neuron (LMN) degeneration is reported. Two brothers presented at the ages of 53 and 44 years with multiple colonic adenomata and invasive colonic carcinoma respectively. Subsequently both developed a virtually identical pattern of motor neuron disease of progressive muscular atrophy type. At presentation both had LMN weakness affecting predominantly the upper limb and neck muscles. The disease progressed rapidly to involve the lower limb and bulbar musculature and bot

Sensitivity to curare in patients with upper and lower motor neurone dysfunction.

Sensitivity to the action of nondepolarising relaxants was compared in muscles of upper and lower limbs in four syringomyelic patients undergoing elective neurosurgical procedures. It was observed that muscles with signs of lower motor neurone dysfunction are supersensitive to the action of nondepolarising relaxants. Terminal sprouting of motor axons and the occurrence of newly formed neuromuscular junctions may be responsible for a low synaptic efficacy and may explain the high sensitivity to factors that reduce the safety margin of neuromuscular transmission.

Antibodies to the ganglioside GD1b in a patient with motor neuron disease and thyroid adenoma.

Patients with motor neuron disease with thyroid disorders have been described, although the relationship between the two conditions is unclear. We treated a patient with amyotrophic lateral sclerosis who also had a follicular adenoma of the thyroid gland. Because thyroid gland plasma membranes contain high concentrations of complex gangliosides, such as GD1b, and some patients with motor neuron disease have IgM antibodies to GD1b, we decided to assay serum from this patient for the presence of antiganglioside antibodies. IgM antibodies to GD1b were detectable at serum dilutions of 1:500 and 1:

Molecular basis of an adult form of beta-hexosaminidase B deficiency with motor neuron disease.

A patient (KL) with progressive motor neuron disease associated with partial Hex A (alpha beta) and no Hex B (beta beta) activity, synthesized beta-chains which only associated with alpha-chains. To identify the molecular basis of this inability of beta-chains to self associate, RNA from cultured fibroblasts was reverse transcribed, the cDNA encoding the beta-chain amplified by polymerase chain reaction, subcloned, and sequenced to reveal two types of single missense mutation. The first mutation, (Type I) 619A----G, was paternally inherited and converted a 207IIe----Val in a highly conserved r
