Immaturity of the neuromuscular junction in spinal muscular atrophy mouse models.

Spinal muscular atrophy (SMA) is caused by deficiency of the survival motor neuron (SMN) protein and is classically defined by degeneration of lower motor neurons. Extensive evidence from mouse models and human tissue demonstrates that dysfunction at the neuromuscular junction (NMJ) emerges early and precedes overt denervation. Here, we review structural, molecular, and functional studies showing that SMA NMJs fail to complete key postnatal maturation programmes that normally scale presynaptic release capacity to muscle growth and increasing functional demand. SMA motor terminals retain multip

Spinal cord changes in Neurodegenerative and Neuroinflammatory Diseases.

The spinal cord plays a critical role in the nervous system. Structurally, morphological changes in the spinal cord directly reflect nerve fiber tract reorganization and may indicate underlying pathological alterations in gray and white matter. Structural remodeling of the spinal cord underlies its functional impairments. This presents a range of clinical manifestations, such as partial or complete loss of voluntary motor control, altered reflexes, abnormal muscle tone, muscular atrophy, and sensory deficits. Spinal cord imaging provides researchers with a direct approach to explore morphologi

Analysis of the knowledge, attitude, and practice status of the primary caregivers of children with spinal muscular atrophy regarding family transitional care-a cross-sectional study.

BACKGROUND: The advent of disease-modifying therapies (DMTs) has revolutionized the management of spinal muscular atrophy (SMA), shifting the paradigm towards chronic disease management. Effective family transitional care is now paramount to maximizing the long-term benefits of these advanced treatments. However, the knowledge, attitude, and practice (KAP) of primary caregivers-the key agents of this care-remain poorly understood, creating a significant barrier to translating therapeutic advances into optimal patient outcomes. This study aimed to assess the KAP status and its influencing facto

Integrative network pharmacology delineates dual GPCR and non-GPCR mechanisms of blended and individual Taikong Blue lavender and Pingyin rose essential oils in neurodegenerative and psychiatric disorders.

Neurodegenerative and psychiatric disorders share overlapping molecular mechanisms, including neuroinflammation, oxidative stress, and neurotransmitter dysregulation. Essential oils from Lavandula angustifolia (TLEO) and Rosa rugosa (PREO) contain neuroactive compounds with therapeutic potential, but their mechanisms remain poorly defined. This study aimed to elucidate the shared and distinct molecular targets and pathways of TLEO and PREO using a multi-scale computational strategy. Compounds identified by GC-MS were evaluated through ADMET profiling, target prediction, and disease-target inte

Longitudinal cohort study of muscle function and metabolic biomarkers in disease-modifying treatment of spinal muscular atrophy.

Disease-modifying treatments (DMT) for spinal muscular atrophy (SMA) developed in the past decade have improved the prognosis of patients with this severe condition. However, treatment effects vary, highlighting the need for sensitive predictive biomarkers to identify individuals more likely to respond to DMT. A previous study detected changes in mitochondrial oxidative capacity in SMA arm muscles, using 31-phosphorus magnetic resonance spectroscopy (31P MRS). Here, we investigated whether 31P MRS can distinguish responders from non-responders to DMT. In this longitudinal observational cohort

Demographic, clinical and genetic characteristics of patients with amyotrophic lateral sclerosis from two specialised centres in Austria.

BACKGROUND: Amyotrophic lateral sclerosis (ALS) is a neurodegenerative disorder characterised by progressive muscle weakness and ultimately death from respiratory failure. Heterogeneity in disease trajectories and outcomes among patients with ALS (pwALS) is influenced by healthcare access, rehabilitation, and palliative care, but real-world data on demographic and clinical characteristics remain scarce in many countries, including Austria. OBJECTIVES: To characterise the demographic, clinical, and genetic landscape of pwALS in Austria. METHODS: In this retrospective cohort study, we included

Heterogeneous phenotype and cardiovascular comorbidities in Swedish patients with spinobulbar muscular atrophy.

BACKGROUND: Spinobulbar muscular atrophy (SBMA) is an X-linked neuromuscular disorder characterized by adult-onset progressive muscle atrophy, flaccid paresis, and bulbar palsy. In addition, increasing evidence indicates that SBMA is a multisystem disorder with prominent non-motor symptoms, such as sensory neuropathy, androgen insensitivity, and glucose intolerance. This study aimed to further characterize the clinical manifestations and biomarker profile in a large Swedish SBMA cohort. METHODS: 49 genetically confirmed SBMA patients were identified from a motor neuron disease database at Ume

Liver Steatosis in Induced Hepatocytes From Carriers of Spinal Muscular Atrophy.

INTRODUCTION/AIMS: Although classically characterized as a motor neuron disease, spinal muscular atrophy (SMA) is increasingly recognized as a multisystem disorder. We previously showed hepatocyte-intrinsic steatosis in SMA, raising the question of whether SMA carriers, who are typically asymptomatic, may also exhibit subclinical hepatic abnormalities. METHODS: We generated induced hepatocyte-like cells (iHeps) from induced pluripotent stem cells (iPSCs) derived from an SMA Type 2 proband, his isogenic wild-type (Iso-WT) line, and both carrier parents, comprised of three carrier lines from th

The spinal and bulbar muscular atrophy-health index: a disease-specific outcome measure.

Objective: Basic science research in spinal and bulbar muscular atrophy (SBMA) has facilitated the development of disease modifying therapeutics. However, there remains a need for disease-specific patient-reported outcome (PROs) measures to monitor changes in disease status. The spinal and bulbar muscular atrophy-health index (SBMA-HI) was designed to address this gap. Methods: Initial interviews with 21 individuals with SBMA and a subsequent international cross-sectional study with 232 participants were conducted to identify the most prevalent and impactful symptoms in SBMA. The most relevant

Non-Cell-Autonomous Mechanisms and Systemic Interactions in Spinal Muscular Atrophy.

Spinal muscular atrophy (SMA) is an inherited neurodegenerative disorder caused by a deficiency of the survival motor neuron (SMN) protein. Traditionally, it has been classified as a motor neuron disease. Over the past decade, however, numerous nonmotor neuronal and nonneural pathologies reported in both patients with SMA and mouse models have led to its redefinition as a systemic disorder. Although SMN protein expression outside the central nervous system is well established, it remains controversial whether its functional loss in nonneuronal cells/tissues merely represents a comorbidity or a

Therapeutic oligonucleotide (nusinersen) metabolism in cerebrospinal fluid samples of patients with spinal muscular atrophy based on liquid chromatography coupled with mass spectrometry data.

BACKGROUND: Spinal muscular atrophy (SMA) is a severe genetic neuromuscular disorder caused by a deficiency of the survival motor neuron protein. The introduction of antisense oligonucleotide therapy has markedly improved prognosis, particularly following approval of Nusinersen (Spinraza), the first SMA drug. Although modified with 2'-O-methoxyethyl and phosphorothioate groups, nusinersen is metabolized. Comprehensive characterization of these metabolites in cerebrospinal fluid is limited due to analytical challenges associated with antisense oligonucleotides. This study aimed to develop a fir

Safety and Efficacy of Magnetically Controlled Growing Rods for Early Onset Scoliosis in the Era of Disease-Modifying Therapies: Mid-term Results in a Homogeneous Cohort of Spinal Muscular Atrophy Type 1 Patients.

BACKGROUND: The advent of disease-modifying therapies (DMTs) has fundamentally altered the clinical course of spinal muscular atrophy type 1 (SMA-I), resulting in prolonged survival and a higher incidence of early onset scoliosis (EOS). Although growth-friendly surgery is a keystone of management, specific evidence regarding magnetically controlled growing rods (MCGR) in the SMA-I population remains sparse. We evaluate the safety and efficacy of MCGR in a specific, homogeneous cohort of SMA-I patients. METHODS: A retrospective investigation was performed on 8 consecutive children with genetic

Free flap reconstruction of a cast-related pressure ulcer in a pediatric patient with spinal muscular atrophy.

Spinal muscular atrophy (SMA) is a hereditary neuromuscular disorder characterized by progressive lower motor neuron loss, resulting in severe muscle atrophy, fatty degeneration, and impaired tissue perfusion. These changes increase susceptibility to pressure-related soft tissue injuries and delay wound healing. We report a rare case of free flap reconstruction in a pediatric patient with SMA type 2 who developed a cast-related pressure ulcer. A 9-year-old girl underwent bilateral hamstring release and distal femoral extension osteotomy followed by long leg circular casting. After cast removal
