Local Translation in Glial Cells of the Brain.

Local protein synthesis is a conserved mechanism that allows cells with intricate architectures to perform compartment-specific functions. By translating messenger RNAs (mRNAs) at distinct subcellular locations, cells can respond swiftly and precisely to localized stimuli. This strategy is crucial in neurons, whose long processes extend far from the cell body. Disruptions in neuronal local translation have been implicated in neurological disorders, including fragile X syndrome, amyotrophic lateral sclerosis, and spinal muscular atrophy. While much of the spotlight has been on neurons, glial ce

Sexual health in neuromuscular diseases: Neglected challenges revealed by a scoping review.

BACKGROUND: Neuromuscular diseases (NMDs) are heterogeneous disorders causing progressive motor decline, multisystem complications, and reduced quality of life. Sexual health, a key component of well-being, remains largely overlooked. This scoping review mapped and synthesized evidence on sexual dysfunction (SD) in adults with NMDs, focusing on prevalence, underlying mechanisms, and research gaps. METHODS: A systematic search of PubMed, Cochrane Library, and Google Scholar (February-March 2025) identified studies reporting original data on SD in adults with genetically or clinically confirmed

The Dynamics of Neurofilament Light Chain in Spinal Muscular Atrophy.

OBJECTIVE: Newborn screening (NBS) for spinal muscular atrophy (SMA) facilitates early diagnosis and treatment for affected individuals. However, fluid biomarkers that provide early insights into disease activity and outcomes in a neonatal cohort and those unable to access (due to reimbursement criteria) or deferring immediate treatment are lacking. This study evaluated neurofilament light chain (NfL) levels to provide insights into disease activity and outcomes in newborns and children with SMA. METHODS: This study correlated pretreatment NfL levels in the serum and cerebrospinal fluid (CSF)

Effects of apomorphine, spinal rTMS, and BoNT on camptocormia: an exploratory wearable sensor-based analysis in a patient with MSA.

Multiple System Atrophy (MSA) is a progressive neurodegenerative disorder characterized by autonomic dysfunction, parkinsonism, and cerebellar signs. Among its motor manifestations, camptocormia-defined as an abnormal forward flexion of the trunk-often emerges early in the disease course and significantly compromises patients' quality of life. Aside from physical therapy, the treatment of camptocormia remains particularly challenging, especially in MSA, where surgical approaches such as deep brain stimulation and spinal cord stimulation are generally contraindicated. This underscores the urgen

Quantitative Whole-Body Muscle MRI in Adults With Spinal Muscular Atrophy-A Sensitive Tool for Long-Time Evaluation of Disease Progression.

BACKGROUND: Spinal muscular atrophy (SMA) is a neuromuscular disorder characterized by progressive muscle weakness due to SMN protein deficiency. While effective therapies exist, their impact on slowly progressive adult SMA patients remains unclear. Reliable biomarkers for monitoring disease progression and treatment response are urgently needed. This pilot study evaluated the utility of longitudinal quantitative muscle MRI (qMRI) to monitor disease progression in adult SMA patients treated with nusinersen over an extended period. METHODS: Nine adult patients with genetically confirmed 5q-SMA

[Comparison of MRI of Paraspinal Muscles in Patients after Open and Minimally Invasive Percutaneous Stabilisation of Thoracic and Lumbar Spine Fractures].

PURPOSE OF THE STUDY: Magnetic resonance imaging (MRI) was used to evaluate paraspinal muscle damage after classic open surgery (OPEN) and minimally invasive spine surgery (MISS) after injury. In a randomised prospective study, we evaluated our hypothesis that MISS would result in a lower extent of atrophy and fibrosis as detected by MRI. MATERIAL AND METHODS: MRI was performed after the injury and not earlier than 3 months after the removal of implants. 16 OPEN and 16 MISS patients underwent the final follow-up. Both atrophy and muscle fibrosis were assessed based on the progression of the c

Upper and Middle Thoracic Esophageal Dilation in patients with Spinal and Bulbar Muscular Atrophy.

Objective Spinal and bulbar muscular atrophy (SBMA) causes oral and pharyngeal dysphagia; however, esophageal involvement remains unclear. This study aimed to clarify esophageal involvement in SBMA by using chest computed tomography (CT). Methods Patients with SBMA who visited our hospital between 2014 and 2025 and underwent chest CT were included. Male stroke patients served as controls. The maximum thoracic esophageal diameter was measured on chest CT in patients with SBMA and stroke. A videofluoroscopic swallowing study (VFSS) was performed in the SBMA group. Results Fifty-seven patients (1

Protein-stabilizing and neurotransmission-potentiating activities of a synaptic chaperone modify spinal muscular atrophy in model mice.

Spinal muscular atrophy (SMA) is an oft-fatal infantile-onset neuromuscular disease caused by low SMN protein. Administration of SMN-inducing agents to SMA newborns prevents early mortality, but therapeutic outcomes vary considerably, and disease mechanisms remain poorly understood. Genetic modifiers can provide clues to disease mechanisms and serve as targets for novel treatments. Here, we describe how one such modifier suppresses SMA in model mice. We show that the modifier, an Hspa8 G470R synaptic chaperone variant we previously identified, functions beyond an already defined role as an SMN

Fibro-adipogenic progenitor cells from murine SMA muscles are intrinsically adipogenic.

Spinal muscular atrophy (SMA) is a neurodegenerative disorder caused by mutations in the SMN1 gene. Although classically viewed as a neurogenic disease, SMA patients exhibit poor skeletal muscle regeneration and increased fatty-fibrotic infiltration. Fibro-adipogenic progenitors (FAPs) are mesenchymal precursor cells that contribute to muscle remodeling and underlie fat and fibrosis formation. Because FAPs transiently express Smn1 during regeneration, FAPs were examined in muscles from adult C/C SMA and control mice to determine if reduced Smn activity altered their properties. We performed a

Epidemiology of Spinal Muscular Atrophy in Iceland: A Population-Based Study.

INTRODUCTION: Spinal muscular atrophy (SMA) is a hereditary neuromuscular disorder characterized by the progressive degeneration of motor neurons and the subsequent loss of muscle function. With new therapeutic options for SMA, it is important to understand the epidemiology of the disease. As of January 2024, SMA has been included in newborn screening in Iceland. OBJECTIVE: The aim of the study was to estimate the point prevalence and incidence rate of SMA in Iceland. METHODS: This retrospective, population-based study collected data from the institutional records of the University Hospital

Trends in the Timeliness of Spinal Muscular Atrophy Detection in US Infants, 2016-2023.

Screening for spinal muscular atrophy (SMA) was adopted by all US state newborn screening programs between 2018 and 2024; by the end of 2022, 48 states were screening for SMA. We assessed trends in health insurance records of SMA diagnoses to quantify improvements in the timeliness of SMA identification following the adoption of screening. We used nationally representative Medicaid claims data for approximately half of US births covered by public insurance and a convenience sample of employer-sponsored health plans. We analyzed records for birth cohorts with at least 1 full year of follow-up (

Clinical Experience of Timing Treatment in Newborns with Spinal Muscular Atrophy: A Call for Standardized Screening Practices in Italy.

Spinal muscular atrophy (SMA) is a rare neuromuscular disorder causing progressive muscle weakness. Severe SMA forms are typically observed up to six months postnatally. Disease-modifying therapies provide significant benefits, making newborn screening (NBS) essential for timely diagnosis and treatment initiation. The NBS programme evaluated infants born between April 2023 and October 2024 in the Campania region, Italy. DNA was amplified to detect homozygous deletion of the SMN1 gene by RT-PCR and SMN2 copy number using multiplex ligation-dependent probe amplification. Following treatment, mot

Restoring early postnatal synaptic dysregulation rescues motor neuron degeneration in a mouse model of Spinal and Bulbar Muscular Atrophy.

Spinal and bulbar muscular atrophy (SBMA) is an adult-onset neurodegenerative disorder caused by expansion of a polyglutamine tract in the androgen receptor (AR). Here, we show that polyglutamine-expanded AR accumulates in the nucleus of motor neurons and induces aberrant upregulation of glutamatergic synaptic genes through dysfunction of the master transcriptional repressor REST during early postnatal development in a mouse model of SBMA (AR-97Q mice). Reducing mutant AR or restoring REST function using antisense oligonucleotides during the neonatal period attenuated the upregulation of gluta
