Is spinal angiography useful in the differential diagnosis of myelopathies of unknown origin? Analysis of a series of 93 patients.

BACKGROUND: Myelopathies of unknown origin (MUO) constitute a challenge for the clinician, due to the absence of image findings that explain the symptoms of the patients. The role of spinal angiography (SA) in MUO has not been studied before in the medical literature. METHODS: Retrospective study of 93 SA performed in a neurointerventional reference center in a ten-year period (January 2013-December 2022). Suspected, angiopraphic and final diagnosis for each patient were studied and compared. RESULTS: SA constituted an 0.8% of the total neurointerventional procedures performed (93 out of 109

SMN deficiency-induced alternative splicing dysregulation in cardiac defects.

AIMS: Spinal muscular atrophy (SMA) caused by reduced survival motor neuron (SMN) protein, is increasingly recognized as a multisystem disorder with cardiac involvement. We aimed to determine how SMN deficiency disrupts postnatal cardiac development and to explore the potential molecular mechanism linking SMN loss to cardiomyocyte dysfunction. MATERIALS AND METHODS: A severe Taiwanese SMA mice model was used to evaluate postnatal cardiac growth and function. Cardiac phenotypes were characterized by histological analysis, transmission electron microscopy, and echocardiography. Transcriptomic p

Performance Evaluation of a PCR/Nanopore Assay for Carrier Screening for Cystic Fibrosis, Spinal Muscular Atrophy, and Fragile X Syndrome.

Cystic fibrosis, spinal muscular atrophy, and fragile X syndrome are among the most common inherited genetic disorders, making carrier screening essential for identifying at-risk couples. Traditional screening often involves multiple workflows and may miss rare variants. Comprehensive sequencing offers broader variant detection across diverse populations. This study validated a PCR/Nanopore-based assay for comprehensive assessment of CFTR, SMN1/2, and FMR1. Samples included anonymized DNA from: whole blood (archival clinical samples; n = 53), cell lines (n = 19), and residual College of Americ

Could an outcome-based agreement be operationalized using real-world data from the Canadian Neuromuscular Disease Registry? Perspectives from an expert-led assessment in spinal muscular atrophy.

BACKGROUND: Outcome-based agreements (OBAs) may facilitate earlier patient access to promising therapies, particularly when evidence is limited. The authors of this paper investigated how to operationalize an OBA using real-world data (RWD) from the Canadian Neuromuscular Disease Registry (CNDR). OBJECTIVE: The first objective of this research was to determine which spinal muscular atrophy (SMA) health outcomes in the CNDR are suitable for an OBA. The second objective was to evaluate the current process of data collection in the CNDR and explore how to operationalize data processes to support

Machine learning-based identification and ranking of risk factors for lumbar paraspinal muscle atrophy.

BACKGROUND: Recent studies highlight the crucial role of the paraspinal musculature (PM), particularly the multifidus (MF), in spinal health and patient outcomes. However, factors associated with PM atrophy and their relative importance remain unclear. To address this gap, we analyzed factors linked to PM atrophy in patients undergoing lumbar fusion using machine learning, aiming to clarify the multifactorial mechanisms underlying this condition. METHODS: Fatty infiltration (FI) of the lumbar MF was measured as a proxy for muscular atrophy in patients undergoing lumbar spinal fusion. Two machi

Utility of Far-Field Potentials as a Biomarker of Neurodegeneration in Spinal Muscular Atrophy.

INTRODUCTION/AIMS: Far field potentials (FFP) have been proposed as a reliable neurophysiological prognostic biomarker in amyotrophic lateral sclerosis (ALS). This study evaluated the utility of ulnar nerve FFP as a robust research biomarker of lower motor neuron degeneration in spinal muscular atrophy (SMA). METHODS: Peripheral neurophysiological assessments were performed in 13 participants with SMA, 19 with amyotrophic lateral sclerosis (ALS), and 19 healthy controls. The ulnar nerve was stimulated at the wrist, and motor responses were recorded over the abductor digiti minimi (ADM) muscle

Spatial transcriptomics identifies dysregulated programs across neural and non-neural tissues in spinal muscular atrophy.

Spinal muscular atrophy (SMA) is caused by insufficient levels of the survival motor neuron (SMN) protein and clinically manifests as profound weakness due to motor neuron degeneration. While recent evidence suggests it is a multisystem disorder, the pathological programs in neural and peripheral tissues are poorly understood. We applied spatial transcriptomics to cross-sections from the entire lumbar region of pre-symptomatic SMA and control mice to define early, tissue-wide transcriptional consequences of SMN deficiency. This approach enabled spatially preserved analysis of spinal cord, dors

Astrocyte targeted SMN1 gene therapy and forskolin application improves astrocyte filopodia actin defects and motor neuron synaptic dysfunction in human SMA disease pathology.

BACKGROUND: Peri-synaptic astrocyte processes (PAPs) play a fundamental role in synapse formation and function. Central afferent synapse loss and astrocyte dysfunction greatly impede sensory-motor circuitry in spinal muscular atrophy (SMA) disease progression, however mechanisms underpinning tripartite synapse dysfunction remains to be fully elucidated. The aims of this study were to further define PAP and motor neuron synaptic defects in human SMA disease pathology and implement a therapeutic intervention strategy to improve motor neuron function. METHODS: We derived astrocyte monocultures a

Longitudinal Analysis of Superoxide Dismutase 1 Seeding Activity in Amyotrophic Lateral Sclerosis Cerebrospinal Fluid.

Twenty percent of familial amyotrophic lateral sclerosis (fALS) cases are linked to mutations in the Superoxide Dismutase 1 ( SOD1) gene and accumulation of misfolded SOD1 aggregates. SOD1 misfolding from the broader ALS population without SOD1 mutations is less clear. Here, we report SOD1 seeding activity in antemortem cerebrospinal fluid (CSF) from ALS participants with and without SOD1 mutations during ALS progression. Antemortem CSF from controls, SOD1- ALS, and sporadic ALS (sALS) patients was subjected to SOD1 seed amplification real-time quaking induced conversion (RT-QuIC) assays. SOD1

SMN1 mutation spectrum and functional analysis of novel SMN1 variants in a Chinese spinal muscular atrophy cohort.

Spinal muscular atrophy is the most common fatal neurogenetic disorder in infancy and early childhood, caused by insufficient expression of SMN protein due to mutations in the survival motor neuron 1 (SMN1) gene. In this study, we describe the SMN1 mutation spectrum in our SMA cohort and the clinical characteristics of 30 patients with SMN1 compound heterozygous mutations. We find the c.22_23insA hotspot mutation has a founder effect, and identify 6 novel SMN1 variants and verify the pathogenicity of these variants at molecular levels. This study proposes that SMA newborn screening should inte

Navigating new motor function trajectories: Consensus recommendations for assessment in the era of newborn screening and early treatment in SMA.

BACKGROUND: Approved disease modifying treatments (DMTs) and the implementation of newborn screening for spinal muscular atrophy (SMA) have allowed individuals treated early to gain new motor abilities and achieve more typical motor milestones. In this changing landscape, current motor function outcome measures (MF-OMs) used in SMA clinical practice may no longer be sufficiently sensitive enough to optimally quantify new disease trajectories and evolving functional progression. OBJECTIVE: To develop a consensus-derived recommendation for MF-OMs to be used in assessing infants and children liv

Locked nucleic acid (LNA) based PCR approach for the diagnosis and screening of spinal muscular atrophy.

Background and objectives Spinal muscular atrophy (SMA) is an autosomal recessive disorder most often caused by homozygous deletion of exon 7 in the SMN1 gene. Current guidelines recommend initial testing of SMN1 exon 7, with additional analyses if no deletions are found. Commercial kits are typically probe-based; however, LNA (locked nucleic acid)-modified primers provide high sensitivity and specificity at lower cost. We aimed to develop an LNA-based assay for the diagnosis and screening of SMA. Methods Peripheral blood samples (2 mL) were collected from 31 patients diagnosed with SMA, 37 co

Clinicodemographic and Genetic Profile of Children with Spinal Muscular Atrophy in Kerala, India: A Single-Center Experience.

BACKGROUND AND OBJECTIVES: Spinal muscular atrophy (SMA) is the most common neurodegenerative disorder in children, with an incidence of 1 in 6000-10000 live births. It is characterized by progressive lower motor neuron weakness. Studies on the natural history and long-term follow-up of an Indian cohort are sparse. We aim to study the clinical course, systemic complications, and treatment outcomes of children with genetically confirmed SMA during a 2-year follow-up. METHODS: A prospective observational study was conducted at Aster MIMS Hospital Calicut, Kerala, including patients with genetic

Acceptability of newborn screening for spinal muscular atrophy: views of the UK public, screened families, health professionals and the SMA community.

With the advent of novel gene therapies, rare genetic diseases once lacking treatments are now being considered for newborn screening programmes (NBS). Wilson and Jungner criteria (drawn on worldwide to guide screening programme evaluation) necessitate effective interventions for a recommendation of screening. Spinal muscular atrophy (SMA) is an example of a condition for which the case for NBS has rapidly gathered pace in recent years. With the introduction of disease-modifying therapies (that are most efficacious when delivered pre-symptomatically), many countries are now piloting or impleme

Survival motor neuron protein-independent amelioration of spinal muscular atrophy by pharmacological inhibition of c-Jun-NH2 terminal kinase.

Spinal muscular atrophy (SMA) is an autosomal recessive neurodegenerative disorder caused by mutation of the survival motor neuron 1 (SMN1) gene. SMA is characterized by degeneration of the spinal cord motor neurons caused by chronic low levels of survival motor neuron (SMN) protein. Prevention or slowing of neurodegeneration has been shown to ameliorate SMA disease severity. Significant progress has been made to develop SMN-dependent treatments that increase SMN levels. However, there is an unmet need to develop alternative therapeutic methods that are SMN-independent. The c-Jun-NH2 terminal
