Paraneoplastic Leucine Zipper 4 IgG Associated Motor-Predominant Polyradiculoneuropathy.

BACKGROUND AND AIMS: Leucine Zipper 4 (LUZP4)-immunoglobulin G (IgG) is a novel antibody implicated in germ cell tumor-related paraneoplastic neurologic syndrome. We report a case of painful, progressive tetraparesis associated with testicular seminoma and LUZP4-IgG seropositivity. CASE REPORT: A 51-year-old male presented with a 5-week history of lower limb predominant progressive weakness. Nerve conduction studies (NCS) and electromyography (EMG) revealed a subacute axonal polyradiculoneuropathy. MRI lumbar spine showed thickening of the cauda equina and enhancement of the lower thoracic sp

Successful assisted pregnancy outcome in a patient with POEMS syndrome.

POEMS (Polyneuropathy, Organomegaly, Endocrinopathy, Monoclonal plasma cell disorder and Skin changes) syndrome is a rare paraneoplastic syndrome caused by a clone of aberrant plasma cells, mostly prevalent in middle-aged men. This is very rare, especially in women of reproductive age group. We are presenting a rare case of POEMS syndrome in pregnancy, who suffered from a full-blown picture of the multisystem involvement of hypothyroidism, hyperprolactinaemia, Empty Sella syndrome leading to panhypopituitarism and benign intracranial hypertension, peripheral neuropathy with trigeminal neuralgi

How Well do We Evaluate Small Fiber Neuropathy?: A Survey of American Academy of Neurology Members.

BACKGROUND: Clinical evaluation of distal symmetric polyneuropathy (DSP), which can include small fiber neuropathy (SFN), differs among neurologists, neuromuscular specialists, and internists. The American Academy of Neurology (AAN) 2009 Practice Parameter guides evaluation of DSP, but there are no guidelines or AAN practice parameters for the evaluation of SFN. OBJECTIVE: Determine how neurologists evaluate and test for SFN in their clinical practice and compare responses between neuromuscular (NM) and non-neuromuscular specialists (non-NM). DESIGN/METHODS: Eight hundred randomly selected A

Clinically probable paraneoplastic vasculitic peripheral neuropathy in primary central nervous system lymphoma: a case report.

Primary central nervous system lymphoma (PCNSL) is a malignant tumor known to cause direct mass effects and various paraneoplastic syndromes. However, paraneoplastic vasculitic peripheral neuropathy is rarely associated with PCNSL. In this case report, we describe a patient with PCNSL who presented with probable paraneoplastic vasculitic peripheral neuropathy. The patient initially experienced recurrent pain and weakness in bilateral lower limbs for two years, followed by a sudden decline in consciousness over two days. A biopsy of the right sural nerve revealed probable vasculitic peripheral

Anti-CV2/CRMP5 autoantibodies as drivers of sensory neuron excitability and pain in rats.

Paraneoplastic neurological syndromes arise from autoimmune reactions against nervous system antigens due to a maladaptive immune response to a peripheral cancer. Patients with small cell lung carcinoma or malignant thymoma can develop an autoimmune response against the CV2/collapsin response mediator protein 5 (CRMP5) antigen, with approximately 80% of these patients experiencing painful neuropathies. Here we investigate the mechanisms underlying anti-CV2/CRMP5 autoantibodies (CV2/CRMP5-Abs)-related pain and find that patient-derived CV2/CRMP5-Abs bind to their target on rat dorsal root gangl

The Neuropathies of Vasculitis.

Vasculitic neuropathies are a group of disorders characterized by inflammation and destruction of vessel walls, resulting in nerve ischemic injury that leads to acute-to-subacute sensory and motor deficits. This can arise from systemic inflammation or be confined to the peripheral nervous system. Causes include primary systemic vasculitis, secondary vasculitis associated with connective tissue disorders, viral infections, paraneoplastic and drug-induced reactions, and nonsystemic vasculitic neuropathy. Early recognition and prompt treatment are essential to mitigate the morbidity and mortality

Gastrointestinal Dysautonomia After Immune Checkpoint Inhibitor Therapy: A Case Series and Systematic Review.

BACKGROUND: Immune checkpoint inhibitors (ICIs) have transformed cancer treatment but are associated with gastrointestinal (GI) immune-related adverse events (IrAEs). GI dysautonomia is a rare IrAE due to enteric nervous system dysfunction with/without generalized autonomic failure. Here, we present a case series of GI dysautonomia following ICI therapy and conduct a systematic review of the literature. METHODS: We present three patients with ICI-induced GI dysautonomia referred to our tertiary ICI-neurotoxicity service. A systematic review was conducted in OVID, Cochrane, and Scopus until De

POEMS syndrome: diagnosis, treatments, and outcomes.

POEMS (polyneuropathy, organomegaly, endocrinopathy, M protein, skin changes) syndrome is a complex paraneoplastic syndrome related to a lambda-restricted low-tumor burden neoplasm. The diagnosis must be considered in the context of what might otherwise be believed to be a "monoclonal gammopathy of undetermined significance," with unusual features including, but not limited to, peripheral neuropathy, thrombocytosis, and extracellular volume overload. Once a diagnosis of POEMS syndrome is made, besides very specific supportive care, the treatments resemble those of multiple myeloma or solitary

Morphological and functional phenotyping of skeletal muscle and bone in the zQ175 knock-in mouse model of Huntington's disease.

Background: Huntington's disease (HD) is a progressive neurodegenerative disorder primarily affecting the central nervous system (CNS). However, emerging evidence suggests that peripheral tissues, including skeletal muscle and bone, also undergo pathological changes contributing to disease burden. Objective: To characterize musculoskeletal impairments in the zQ175 knock-in (KI) mouse model of HD, through integrated behavioral, biomechanical, and imaging analyses. Methods: Motor function was assessed using grip strength, rotarod, and open field testing. Ex vivo contractility of the extensor dig

Metabolic Implications of PCOS.

BACKGROUND: In children, necrotizing pneumonia (NP) is an uncommon, severe complication of pneumonia. It is characterized by destruction of the underlying lung parenchyma resulting in multiple small, thin-walled cavities and is often accompanied by empyema and bronchopleural fistulae. REVIEW: NP in children was first reported in children in 1994, and since then there has been a gradual increase in cases, which is partially explained by greater physician awareness and use of contrast computed tomography (CT) scans, and by temporal changes in circulating respiratory pathogens and antibiotic pre

From Brain to Muscle: The Role of Muscle Tissue in Neurodegenerative Disorders.

Neurodegenerative diseases (NDs), like amyotrophic lateral sclerosis (ALS), Alzheimer's disease (AD), and Parkinson's disease (PD), primarily affect the central nervous system, leading to progressive neuronal loss and motor and cognitive dysfunction. However, recent studies have revealed that muscle tissue also plays a significant role in these diseases. ALS is characterized by severe muscle wasting as a result of motor neuron degeneration, as well as alterations in gene expression, protein aggregation, and oxidative stress. Muscle atrophy and mitochondrial dysfunction are also observed in AD,

Peripheral nervous system manifestations of MOG antibody associated disease.

Recent studies have reported the involvement of peripheral nervous system (PNS) in association with MOG-IgG, including isolated neuropathies. In this retrospective study we characterized the PNS involvement in MOG antibody associated disease (MOGAD). Six out of 215 MOGAD patients had PNS involvement (all polyradiculopathy) that occurred concurrently with a CNS demyelinating episode. We also demonstrated MOG expression in healthy human controls' proximal nerve root. Nine patients with true-positive MOG-IgG1 had PNS involvement temporally unrelated to a CNS demyelinating event. All these patient
