A novel UPLC-ECD method for simultaneous quantification of levodopa, benserazide, and their major metabolites in human plasma and cerebrospinal fluid.

Levodopa (LD) combined with peripheral dopa decarboxylase inhibitors (DDCIs) remains the cornerstone therapy for Parkinson's disease (PD). Benserazide (BZ), a widely used DDCI, is rapidly converted in vivo to 2,3,4-trihydroxybenzylhydrazine (THBH), an active metabolite with comparable inhibitory activity. Although BZ and THBH are generally considered to exhibit negligible penetration across the blood-brain barrier, evaluation of their potential central exposure requires highly sensitive quantification in cerebrospinal fluid (CSF). However, no validated method is currently available for simulta

Restless legs syndrome and other neurological disorders: A systematic-narrative hybrid literature review with exploratory meta-analysis.

Restless legs syndrome (RLS) is a sleep-related sensorimotor disorder affecting approximately 3% of adults worldwide. This review aims to synthesize current evidence on associations between RLS and major neurological conditions, provide an exploratory quantitative synthesis summarizing association patterns and heterogeneity, and identify priority mechanistic and clinical research gaps. Databases were searched from inception until August 2026. In our exploratory synthesis of 119 unique studies, RLS was significantly associated with Parkinson's disease (case-control OR 3.22, 95% CI 2.35-4.41; co

A transcriptomic resilience gap between the substantia nigra and cerebellum is associated with regional vulnerability in Parkinson's disease.

The mechanisms underlying the selective vulnerability of the substantia nigra (SN) in Parkinson's disease (PD) are incompletely understood. We computed the tissue-specificity index τ for 19,355 protein-coding genes across 68 GTEx v11 tissues, defining a gene as tissue-specific when τ ≥ 0.8, median TPM > 1, and the tissue of interest was the absolute maximum. Under this definition, the SN harboured only 12 SN-specific genes whereas the cerebellum (CB) contained 367 CB-specific genes. The direct pairwise contrast was not statistically significant (exact p = 0.090). The primary evidence is a nega

Discovery of benzoxazole derivatives as novel α-synuclein aggregate ligands with potential for PET tracer development.

Parkinson's disease (PD) is the second most prevalent neurodegenerative disorder worldwide, and the abnormal aggregation of α-synuclein (α-Syn) constitutes its core pathological hallmark. Positron emission tomography (PET) imaging of cerebral α-Syn aggregates holds great promise for the early diagnosis of PD. However, the imaging quality critically depends on the ligands with high binding affinity and selectivity for α-Syn aggregates. In this work, we developed a new type of benzoxazole-based α-Syn aggregate ligands via an SPR-based screening. Among them, candidates D15 (KD = 0.12 μM) and D16

Altered interlimb coupling during unimanual and bimanual force control in aging and Parkinson's disease.

Bimanual coordination depends on the ability to flexibly regulate interlimb coupling while maintaining independent control of each limb. Aging and Parkinson's disease (PD) may disrupt this balance by altering sensorimotor regulation and shared neural input between limbs. This study examined force control, coordination dynamics, and physiological coupling during unimanual and bimanual isometric force tasks in healthy young adults (HYA), healthy older adults (HOA), and individuals with Parkinson's disease (PD). Participants completed 30 s unimanual and bimanual force tasks with target forces set

Morning lighting intervention reduces fatigue and improves sleep in Parkinson disease.

Fatigue and sleep disturbances are common in Parkinson Disease (PD) and significantly reduce quality of life, yet effective pharmacological treatments are limited. Light therapy offers a promising non-pharmacologic approach, but most lighting interventions have focused on improving sleep. Using a novel and easily administered morning lighting intervention, we previously demonstrated improvement in sleep duration. We have now extended the prior study through the addition of 23 individuals to investigate the intervention's effect on fatigue and sleep. In this single-arm intervention study, parti

Tristetraprolin attenuates microglial neuroinflammation via post-transcriptional destabilization of TRAF6 mRNA in Parkinson's disease.

BACKGROUND: Microglia-driven neuroinflammation is a central pathological hallmark of Parkinson's disease. Tristetraprolin (TTP), an RNA-binding protein, suppresses inflammation through facilitating the degradation of target mRNAs. However, its specific role in microglial activation during Parkinson's pathogenesis remains poorly characterized. METHODS: Transcriptomic analysis was performed to identify the differentially expressed genes in A53T α-synuclein transgenic mice. TTP expression and function were assessed in vitro and in vivo. Mechanistic studies employed RNA immunoprecipitation, dual-

Depression in the prodromal phase of Parkinson's disease: Clinical manifestations, biomarker evidence, and predictive implications.

Depression is one of the earliest and most frequent non-motor symptoms in the prodromal phase of Parkinson's disease (PD), often preceding motor signs by years to decades. This review synthesizes epidemiological, neuroimaging, biomarker, and genetic evidence to clarify whether depression is a risk factor, a direct prodromal manifestation, or both. Meta-analyses consistently show a two-fold increased risk of subsequent PD in individuals with depressive symptoms or major depressive disorder (MDD), with the highest risk occurring within 1-2 years of diagnosis but persisting for up to 25 years. La

Research Progress of Low-Intensity Pulsed Ultrasound in Parkinson's Disease.

Parkinson's disease (PD) is a progressive neurodegenerative disorder where current pharmacological and surgical interventions, including Deep Brain Stimulation, often fail to reverse neuronal degeneration or halt disease progression. Low-intensity pulsed ultrasound (LIPUS) has emerged as a novel, non-invasive neuromodulation technology with significant therapeutic potential. This review synthesizes current research on the key acoustic parameters, biological mechanisms, and clinical applications of LIPUS in PD. Unlike High-Intensity Focused Ultrasound (HIFU), which relies on thermal ablation to

Coexistence of anhedonia and sensation-seeking with context-dependent gut dysbiosis in a prodromal transgenic mouse model of Parkinson's disease.

Accumulating evidence indicates that gut microbiota plays a role in the progression of Parkinson's disease (PD). Microbiota-based treatments for prodromal PD, which is accompanied by a wide range of neuropsychiatric symptoms, are being considered a potential early intervention but are still under-investigated. This study examined the association between symptoms of affective and motivational impairments and gut dysbiosis in a transgenic mouse model (line G2-3) that overexpresses A53T mutant α-synuclein, a key protein involved in the disease pathology. By introducing biologically meaningful var

Family and community care for older persons with Parkinson's disease in Kenya: a qualitative, ethnographic study.

BACKGROUND: Informal family care remains central to care for older adults in Kenya, yet evidence hints towards an 'erosion' of large, multi-generational family care networks and a conditionality on reciprocity across many African countries. The increasing incidence and prevalence of chronic conditions associated with later life have made care needs more complex and resource intensive. Parkinson's is one such example of a progressive condition characterised by a host of motor and non-motor symptoms, which results in increasing disability over time and, therefore, care requirements and resources

Cross-species molecular stratification enables T cell receptor diagnosis of Parkinson's disease.

BACKGROUND: Developing peripheral blood-based diagnostic models for idiopathic Parkinson's disease (iPD), particularly those leveraging the T-cell receptor (TCR) repertoire, has long been considered infeasible because patient-derived TCRs appear to lack convergent sequence motifs. We reasoned that this apparent absence of shared TCR features likely reflects both insufficient sample sizes and unaccounted immune heterogeneity within the iPD population. METHODS: We reconstructed TCR repertoires from the two largest iPD cohorts currently available, anchoring them with three mechanistically distin

Initiating levodopa pump therapy: a qualitative study of patient and caregiver perspectives.

BACKGROUND: Parkinson's disease (PD) is a progressive neurodegenerative disorder affecting motor and non-motor functions. When oral medication no longer provides adequate control, device-aided therapies (DAT), including levodopa infusion therapy, may be introduced. Although DAT can improve symptom management, little is known about how people with PD (PwPD) and family caregivers (FC) experience the initiation process. This study explored PwPD and their FC perceptions during decision-making, therapy initiation, and subsequent adaptation to levodopa pump therapy. METHODS: This qualitative study

Hearing Loss and Parkinson's Disease: Insights From Human and Animal Studies.

BACKGROUND: Hearing loss (HL) has been increasingly implicated in Parkinson's disease (PD), but the epidemiological association between them and the underlying biological mechanisms remain unclear. This study aimed to investigate the association between HL and PD and to explore potential biological links between auditory deprivation and PD-related pathology. METHODS: Data from the National Health and Nutrition Examination Survey (NHANES) and the China Health and Retirement Longitudinal Study (CHARLS) were analyzed using multivariable logistic regression, with sensitivity analyses performed in

[Emerging Therapeutic Strategies for Central Nervous System Regeneration].

Reperfusion therapy has markedly improved outcomes after acute ischemic stroke; however, many patients remain ineligible for treatment or sustain irreversible brain injury despite successful recanalization. Regenerative medicine therefore represents an important strategy for restoring damaged neural tissue and promoting functional recovery. Stem cell therapies may exert therapeutic effects through multiple mechanisms, including cell replacement, secretion of neurotrophic factors, modulation of inflammation and immune responses, angiogenesis, and intercellular transfer of mitochondria and other

Case Report: Lithium-induced bradycardia triggered by cecal volvulus: a case of obstruction-associated drug accumulation.

BACKGROUND: Lithium toxicity can rarely manifest as cardiac arrhythmias, often initially presenting as bradycardia. Bowel obstruction is a rare cause of acute lithium toxicity. We present a case of lithium-induced symptomatic bradycardia induced by cecal volvulus. CASE REPORT: An 84-year-old female with a history of bipolar 1 disorder on chronic lithium therapy was admitted for symptomatic bradycardia and abdominal pain. She also presented with myoclonus, polydipsia, and encephalopathy. Laboratory evaluation revealed lithium toxicity in the setting of renal impairment, and abdominal imaging f

Marked intrafamilial phenotypic heterogeneity associated with the CSF1R c.2539G>A pathogenic variant in a Japanese family.

Pathogenic variants in the colony-stimulating factor 1 receptor (CSF1R) gene cause CSF1R-related disorder (CSF1R-RD), which encompasses a broad phenotypic spectrum including a classical adult-onset leukoencephalopathy phenotype. Within this spectrum, considerable phenotypic heterogeneity has been increasingly recognized. The pathogenic missense variant c.2539G>A has been reported in a small number of patients with CSF1R-associated leukoencephalopathy; however, its associated clinical spectrum remains incompletely characterized. To characterize the clinical spectrum associated with the CSF1R c.
