Leukoaraiosis does not impact motor outcomes in Parkinson's patients post deep brain stimulation.

OBJECTIVES: We sought to assess the impact of leukoaraiosis (LA) on motor outcomes in Parkinson disease (PD) patients undergoing DBS. We hypothesized that LA is associated with less improvement in motor function in PD patients post-DBS. METHODS: We reviewed data of adult patients with PD treated with DBS in a single center between 2012 and 2021. Demographics, risk factors, medications, Hoehn and Yahr scale and Unified Parkinson's Disease Rating Scale (UPDRS) Motor Score before and after DBS and severity of LA were collected. Simple linear regression (SLR) was used to determine variables of in

Digital Inclusion in Parkinson's Disease: A Case-Control Study.

BACKGROUND: People with Parkinson's disease (pwPD) are a candidate population to regularly use telemedicine services. However, their implementation into clinical practice can be limited by the so-called "digital exclusion" of the target population. OBJECTIVE: We aimed to (1) explore the level of digital inclusion in a cohort of pwPD as compared to sex- and age-matched healthy controls (HC), and (2) to determine the possible influence of both socio-demographic factors and disease features on the level of digital exclusion. METHODS: Four hundred and ninety pwPD and 140 HC completed the Digital

A Bottom-Up Analysis of the Cost of Illness of Parkinson's Disease in the Netherlands from a Societal Perspective.

BACKGROUND: Parkinson's disease (PD) places a high burden on patients, their families, healthcare systems, and society as a whole, which is rising due to the increasing prevalence of PD. There has been wide variation in the items included in previous cost of illness (COI) studies for PD, possibly underestimating the costs associated with this disease. OBJECTIVES: This study aims to undertake a detailed estimate of the COI of PD in the Netherlands from a societal perspective. METHODS: Between November 2022 and May 2023, we conducted a cross-sectional COI study with a bottom-up, prevalence-bas

Temporal progression of pathological features in an α-synuclein overexpression model of Parkinson's disease.

Parkinson's disease (PD) is a common neurodegenerative disorder, affecting 1-5% of individuals over 60, with a higher incidence in men. It is clinically characterized by progressive motor impairments, including rigidity, bradykinesia, tremors, and gait disturbances. The neuropathological hallmark of PD is the aggregation of α-synuclein (α-syn) into Lewy bodies (LB) and neurites (LN). Although α-syn plays essential physiological roles, its misfolding and accumulation drive neurodegeneration. In this study, we investigated the temporal progression and anatomical distribution of α-syn pathology u

A Fully Integrated Wearable Sweat Sensing Patch for Online Analysis of Multiple Parkinson's Disease-Related Biomarkers.

Parkinson's disease (PD) is marked by a prolonged asymptomatic "window period" (several years). Early prediction and diagnosis during this window are crucial, as timely interventions can slow disease progression. In this study, a fully integrated wearable sweat-sensing patch capable of real-time detection of three key PD biomarkers: L-Dopa, ascorbic acid, and glucose is developed. The system includes a biomimetic microfluidic module for sedentary sweat collection, an advanced electrochemical sensing platform for biomarker analysis, on-site signal processing circuitry for data management, and c

Therapeutic potential of flavonoids against α-synuclein aggregation in Parkinson's Disease: Integrative in silico and in vitro analysis.

Amyloidogenic protein aggregation is the key factor in neurodegenerative diseases. Due to the cost-effectiveness and low side effects, attention to herbal medicines has been recently increased. Flavonoids are a group of plant compounds with high potential for reducing the accumulation of amyloidogenic proteins. This research aims to identify the most effective flavonoids for inhibiting the fibrillation of α-synuclein (α-syn). For this purpose, 98 flavonoids from different databases were selected for analysis. The pharmacokinetic properties of these flavonoids were evaluated using OSIRIS and Sw

Rethinking Parkinson's: The role of proteostasis networks and autophagy in disease progression.

Protein dyshomeostasis is identified as the hallmark of many age-related NDDs including Parkinson's disease (PD). PD is a progressive neurodegenerative disorder (NDD) characterized by the accumulation of misfolded proteins, particularly α-synuclein (α-syn) leading to formation of Lewy bodies and cause degeneration of dopaminergic neurons in substantia nigra pars compacta (SNpc). Disruption of the cell's normal protein balance, which occurs when cells experience stress, plays a key role in causing the formation of harmful protein clumps. Functional proteostasis relies on coordinated mechanisms

Nicotine Attenuates Pathogenesis of Parkinson's Disease via α7-nAChR-Mediated Lipid Metabolic Reprogramming and Anti-inflammatory Signaling.

Nicotine-an abundant tobacco alkaloid-has been epidemiologically linked to a lower incidence of Parkinson's disease (PD). In a 6‑hydroxydopamine (6‑OHDA) rat model, we found that chronic nicotine (1.5 mg kg-1 day-1) was associated with better motor performance and a smaller drop in striatal brain‑derived neurotrophic factor (BDNF) (p < 0.05). Untargeted lipidomics indicated partial normalization of 6‑OHDA‑induced changes in sphingolipids, phospholipids, and neutral lipids, alongside reduced lipid‑peroxidation markers. In SH‑SY5Y cells, nicotine attenuated 6‑OHDA‑related cytotoxicity, mitochond

Research progress of chronic fatigue in neurodegenerative diseases.

The incidence of Neurodegenerative Diseases is on the rise, paralleling the societal trend of an aging population and the extension of the average life expectancy. NDs frequently manifest varying levels of chronic fatigue, a phenomenon whose underlying mechanisms remain obscure. This condition significantly impairs the quality of life for patients and presents a formidable challenge to our nation’s healthcare system. This paper delves into the potential mechanisms, assessments, and therapeutic approaches for Parkinson’s Disease and Multiple System Atrophy in the context of chronic fatigue. The

Deep-Learning-Based Partial Volume Correction in 99mTc-TRODAT-1 SPECT for Parkinson's Disease: A Preliminary Study on Clinical Translation.

99mTc-TRODAT-1 SPECT is effective for the early detection of Parkinson's disease (PD). However, SPECT images suffer from severe partial volume effect, which impairs tissue boundary clarity and subsequent quantification accuracy. This work proposes an anatomical prior- and segmentation-free deep learning (DL)-based partial volume correction (PVC) method using an attention-based conditional generative adversarial network (Att-cGAN) for 99mTc-TRODAT-1 SPECT. A population of 454 digital brain phantoms modelling anatomical and 99mTc-TRODAT activity variations in different PD categories are used to

[Subtypes of Parkinson's disease].

Parkinson's disease is characterized by a large inter-individual variability of symptom constellations and courses. In routine clinical practice, standard treatment strategies are often used, but outcomes differ considerably from patient to patient. The definition of different Parkinson's subtypes may help to better predict the individual course of the disease and to find the most suitable treatment options for different patient groups.

ANKK1, ANKRD50, GRK5, PACSIN1 and VPS8 are novel candidate genes associated with late onset Parkinson's disease: Definition of a novel predictive protocol based on polygenic model of inheritance.

Parkinson's disease (PD) represents one of the most frequent neurodegenerative disorders for which genetic diagnosis is still challenging due to the high genetic heterogeneity associated with the disease and to the difficulty in interpreting test results. We have recently reported the identification of rare new gene variants in PD patients that support polygenic contribution to the disease. Here we report the identification of novel candidate PD genes and an exploratory protocol for predictive analysis of PD risk. The study includes the whole exome data of 22 PD families, 300 unrelated familia

Subthalamic beta power and gait in Parkinson's disease during unsupervised remote monitoring.

BACKGROUND: Brain-sense enabled deep brain stimulation (DBS) in Parkinson's disease (PD) allows understanding the neurophysiological role of basal ganglia in gait dysfunction. Subthalamic nucleus (STN) local field potentials (LFPs) indicate pathological PD gait changes, but previous evidence is based on data obtained in controlled settings. This study aims to examine the relationship between STN oscillations and sensor-derived gait parameters in PD, in an unsupervised, real-life environment. METHODS: Patients with PD and STN-DBS (Medtronic Percept™-PC) were enrolled. Collected data included m

Investigating the Blood Microbiome in Parkinson's Disease, Schizophrenia, and Posttraumatic Stress Disorder.

INTRODUCTION: Recent studies have challenged the idea of sterile blood, suggesting the presence of a blood microbiome. The detection of microbial nucleic acids in blood is thought to reflect the status of distant microbial niches, including the gut. However, the origins and viability of these microbes remain debated. In this study, blood microbiome signatures in Parkinson's disease (PD), schizophrenia (SCZ), and posttraumatic stress disorder (PTSD) were evaluated by extracting RNA-sequencing (RNA-seq) reads that did not map to the human genome. Furthermore, we investigated the correlation betw

Differentially Private Multimodal Laplacian Dropout (DP-MLD) for EEG representative learning.

Recently, multimodal electroencephalogram (EEG) learning has shown great promise in disease detection. At the same time, ensuring privacy in clinical studies has become increasingly crucial due to legal and ethical concerns. One widely adopted scheme for privacy protection is differential privacy (DP) because of its clear interpretation and ease of implementation. Although numerous methods have been proposed under DP, it has not been extensively studied for multimodal EEG data due to the complexities of models and signal data considered there. In this paper, we propose a novel Differentially P
