Dissociable associations of free-water fraction and DTI-ALPS index with disease manifestation and mutation carrier status in Parkinson's disease.

PURPOSE: Parkinson's disease (PD) is a biologically heterogeneous disorder. Diffusion MRI-derived free-water fraction (FWf) and DTI-ALPS index have been proposed as biomarkers of PD-related brain alterations; however, their relationships with clinical disease manifestation and genetic susceptibility remain unclear. This study investigated whether substantia nigra pars compacta (SNc) FWf and DTI-ALPS indices show different associations with disease status and mutation carrier status across the PD continuum. MATERIALS AND METHODS: Diffusion MRI data from the Parkinson's Progression Markers Init

STN-SNr co-stimulation in Parkinson's disease: a systematic review and meta-analysis of motor and freezing-of-gait outcomes.

BACKGROUND: Freezing of gait (FoG) and axial impairments often persist in Parkinson's disease despite optimized medication and subthalamic nucleus deep brain stimulation (STN-DBS). We systematically reviewed whether adding substantia nigra pars reticulata (SNr) stimulation improves motor and FoG outcomes. METHODS: Following PRISMA 2020 and PROSPERO registration (CRD420261343479), six databases were searched through 1 May 2026. Primary outcomes compared STN-SNr co-stimulation with STN-only stimulation; stimulation-off or baseline comparisons were secondary. Two reviewers independently screened

Locus coeruleus abnormalities on neuromelanin-sensitive MRI in Parkinson's disease: a systematic review and meta-analysis.

Neuromelanin-sensitive magnetic resonance imaging (NM-MRI) enables in vivo assessment of locus coeruleus (LC) integrity, but findings in Parkinson's disease (PD) vary across cohorts and methods. We searched PubMed/MEDLINE, Embase, Web of Science Core Collection and Scopus from inception to 5 September 2026 and pooled independent PD-healthy control (HC) comparisons using random-effects models. Sixty-five reports were included, of which 17 contributed to at least one main pooled outcome. The primary LC-signal analysis included 14 independent cohorts (1,078 participants with PD and 543 HCs) and s

Cross-species molecular stratification enables T cell receptor diagnosis of Parkinson's disease.

BACKGROUND: Developing peripheral blood-based diagnostic models for idiopathic Parkinson's disease (iPD), particularly those leveraging the T-cell receptor (TCR) repertoire, has long been considered infeasible because patient-derived TCRs appear to lack convergent sequence motifs. We reasoned that this apparent absence of shared TCR features likely reflects both insufficient sample sizes and unaccounted immune heterogeneity within the iPD population. METHODS: We reconstructed TCR repertoires from the two largest iPD cohorts currently available, anchoring them with three mechanistically distin

Visual feedback increases expiratory muscle training effort in Parkinson's disease: a randomised crossover trial.

BACKGROUND: Disorders of airway protection, including impaired cough and swallowing, are clinically important complications of Parkinson's disease. Expiratory muscle strength training (EMST) is an established non-pharmacological intervention targeting these deficits. The aim of this study was to evaluate the acute effect of visual feedback provided by the SpiroGym mHealth app on expiratory muscle activation during EMST in people with Parkinson's disease. METHODS: We conducted a single-center, randomised, two-period crossover trial in people with Parkinson's disease. Participants performed EMS

Evaluating Laryngeal Stability in Drug-Induced Parkinsonism Using Hilbert-Huang Transform and Chaos Dynamics: A Multi-Vowel Nonlinear Acoustic Framework for Differential Diagnosis.

Background/Objectives: Drug-induced parkinsonism (DIP) is a common extrapyramidal adverse effect of dopamine-receptor blocking agents, yet interpretable biomarkers of its acoustics remain limited. This study investigated whether nonlinear dynamics could provide physiologically interpretable markers of DIP and clarify how these abnormalities vary across vowels and extrapyramidal symptoms. Methods: Sustained phonations of five vowels (/a/, /i/, /u/, /e/, /o/) were analyzed in 82 patients with DIP and 30 healthy controls. Hilbert-Huang Transform (HHT) features were integrated along with Sample En

Gut Microbiome-Mediated Neuroprotective Effect of the Mediterranean Diet: A Narrative Review of Cognitive Aging, Alzheimer's Disease, and Parkinson's Disease.

Background: Alzheimer's disease (AD) and Parkinson's disease (PD) are major neurodegenerative disorders for which disease-modifying therapies are limited. Increasingly, evidence suggests that diet influences neurodegenerative risk through interactions among the gut microbiome, immune system, and brain. The Mediterranean diet (MD) is associated with improved cognitive health, but the mechanisms underlying its potential neuroprotective effects remain incompletely understood. Objective: In this narrative review, evidence on microbiome-mediated mechanisms linking MD adherence to neuroprotection wa

How do neurorehabilitation patients perceive their fundamental care needs? Findings from a qualitative study.

BACKGROUND: The Fundamentals of Care (FoC) framework provides a structured model to support the delivery of person-centered nursing care by integrating physical, psychosocial, and relational dimensions. Although the framework has been widely applied in acute care, little is known about its relevance in long-term neurorehabilitation settings, where patients often face complex, prolonged, and multidimensional care needs. OBJECTIVE: To explore how individuals with neurological disorders perceive their fundamental care needs during inpatient rehabilitation. DESIGN: A qualitative secondary analys

Conformational allocation of mitofusin 2 in neuronal mitochondrial dynamics: Mechanisms and therapeutic targeting.

Neurons must sustain functional mitochondria far from the cell body. Mitofusin 2 (MFN2), a dynamin-like large guanosine triphosphatase (GTPase), coordinates outer mitochondrial membrane remodeling with axonal transport, bioenergetic support, inter-organelle communication, and stress-responsive quality control. Studying these activities separately obscures how disruption of one protein produces diverse neuronal phenotypes. We propose a testable conformational-allocation framework in which molecular state distributions and transition kinetics bias MFN2 participation across cellular outputs. Inte

Effects of globus pallidus internus deep brain stimulation on freezing of gait in Parkinson's disease: a prospective short-term single-arm study.

BACKGROUND: Freezing of gait (FoG) is a debilitating symptom in Parkinson's disease (PD), often resistant to medication. Deep brain stimulation (DBS) has emerged as an effective treatment for advanced PD, while its effect on FoG remains inconsistent. Specifically, the effect of globus pallidus internus deep brain stimulation (GPi-DBS) against FoG is inconclusive, and relevant studies are scarce. OBJECTIVE: To describe the post-operative effect of GPi-DBS on FoG and identify pre-operative predictors of improvement. METHODS: Forty-six PD patients undergoing GPi-DBS were assessed pre-operativel

Fixed-Sequence OFF-to-ON Differences in Low-Beta State Dynamics in Parkinson's Disease: A Paired Source-Space Resting-State EEG Study.

Resting-state EEG studies in Parkinson's disease (PD) have largely emphasized static power or connectivity, whereas fixed-sequence OFF-to-ON session differences may also involve the temporal organization of network states. Paired source-space evidence under such fixed acquisition protocols remains limited. We analyzed 64-channel eyes-closed resting-state EEG in 39 patients with PD contributing paired OFF/ON recordings for the prespecified 90 s primary analysis; 24 also contributed paired 178 s recordings for duration robustness. After individual MRI-based source reconstruction, low-beta (13-20

Immunosenescence as a modifiable driver of neurodegenerative diseases: Pathways and multidimensional emerging strategies for Alzheimer's and Parkinson's diseases.

Immunosenescence represents a core upstream driver of Alzheimer's disease (AD) and Parkinson's disease (PD), exerting its pathological effects through four mechanistically interconnected cascades: chronic inflammatory imbalance mediated by the senescence-associated secretory phenotype (SASP), oxidative stress arising from immune-metabolic reprogramming, protein homeostasis dysfunction caused by autophagy-lysosomal failure, and disrupted neuro-immune crosstalk secondary to breakdown of central nervous system (CNS) immune privilege. Current clinical practice remains largely constrained to neurot

Interaction Between Exercise Intensity and Sedentary Behavior on VO 2 max Change in Patients with De Novo Parkinson Disease: The Study in Parkinson Disease of Exercise (SPARX) Phase 2 Randomized Clinical Trial.

BACKGROUND: Parkinson disease (PD) is associated with increased sedentary time and reduced cardiorespiratory fitness, making exercise an important component of disease management. Although exercise improves cardiorespiratory fitness, individuals with PD vary substantially in their response to exercise. Sedentary time may contribute to this variability, yet whether sedentary time modifies the response to intervention and varying exercise intensities within an intervention is unknown. We examined whether sedentary time interacted with 6 months of moderate-intensity or high- intensity aerobic exe

High frequency transcranial pulsed current stimulation improves quality of life in Parkinson's disease: A randomized controlled trial.

Parkinson's disease (PD) is a progressive multisystem disorder in which motor and non-motor symptoms impair quality-of-life. High-frequency transcranial pulsed current stimulation (tPCS) modulates network-level dysfunction and may serve as an adjunct therapy in PD. In a randomized controlled trial, 38 participants with Hoehn & Yahr stage I-III PD were randomized to active 400 Hz tPCS or sham for 4 weeks while receiving stable antiparkinsonian medication. Stimulation targeted bilateral dorsolateral prefrontal cortices and motor-cerebellar circuit. Primary outcome was the Parkinson's Disease Que

The Relationship of Peripheral Glycoprotein Nonmetastatic Melanoma Protein B and Parkinson's Disease: Insights From a Mendelian Randomization Study.

BACKGROUND: Genome-wide association studies (GWASs) have revealed that glycoprotein nonmetastatic melanoma Protein B (GPNMB) was associated with Parkinson's disease (PD). GPNMB has been found in plasma and cerebrospinal fluid, and its levels and the variation of single nucleotide polymorphisms (SNPs) have been linked to disease severity in PD patients. However, whether the altered expression of GPNMB is the cause or the result cannot be identified in the patient samples. METHODS: This study applied a Mendelian randomization analysis to explore the causal relationship between GPNMB and PD. The

Exercise modality and planned weekly exercise exposure for motor outcomes in Parkinson's disease: a systematic review, Bayesian network meta-analysis, and meta-regression.

BACKGROUND: Exercise modality and planned weekly structured-exercise exposure may provide complementary information for Parkinson's disease motor outcomes, but their outcome-specific effects and evidential confidence remain uncertain. METHODS: We synthesized 173 randomized trials using covariance-aware Bayesian random-effects network meta-analysis for motor symptoms, balance, walking speed, and Timed Up and Go (TUG). A key secondary analysis estimated exercise-class-adjusted associations per 250 metabolic equivalent task minutes/week, with pre-specified sensitivity analyses. RESULTS: The cat
