Pilot randomised controlled trial of a novel form of exercise on parkinsonism symptoms.

OBJECTIVE: Exercise-assistance strategies are useful in allowing mobility-impaired patients to access the benefits of exercise. This trial is the first of the Reviver device, which facilitates exercise via a novel strength and balance training mechanism. The objective was to examine the effect of a 12-week Reviver intervention on symptoms of Parkinsonism, and to pilot the randomised controlled trial design, including randomisation and acceptance of the exercise intervention. METHODS: This was a pilot, parallel-arm randomised controlled trial with assessor blinding. Participants (n=30: 22 with

Cerebral Perfusion and Motor Deficits in Drug-Induced Parkinsonism: A Dual-Phase 18F-FP-CIT PET Study.

BACKGROUND AND OBJECTIVES: Drug-induced parkinsonism (DIP) is the second most common cause of parkinsonism after Parkinson disease (PD). It is crucial to diagnose DIP in patients presenting with parkinsonian symptoms because discontinuation of the offending drug can improve parkinsonism. However, the detailed pathophysiology of parkinsonism related to striatal dopamine receptor blockade and disease-specific cerebral changes remains unclear. METHODS: In this cross-sectional study, we recruited patients with DIP who underwent the Unified Parkinson's Disease Rating Scale (UPDRS) assessment, brai

Impaired theory of mind in Parkinson's disease: A systematic review of etiological debates.

Impairment of theory of mind (ToM)-the ability to attribute mental states to others-has been identified as a non-motor feature of Parkinson's disease (PD), potentially undermining social functioning and quality of life. A central controversy concerns whether ToM deficits arise directly from PD neuropathology or are secondary to general cognitive impairment, with growing evidence pointing to the latter. This systematic review examined 14 studies employing designs that allow stronger causal inference-including regression, between-group, and experimental paradigms-to assess the dependence or inde

Striatal dopamine synthesis capacity in Parkinson's disease: Effects of age, sex, and body mass index in a large [18F]fluorodopa PET cohort.

BACKGROUND: Positron emission tomography (PET) using radioligand [18F]fluorodopa detects reduced striatal dopamine synthesis capacity in Parkinson's disease (PD) patients. Demographic factors such as sex and BMI are also associated with dopamine synthesis capacity. The combined contribution of demographic and clinical effects however remains elusive. MATERIAL, AIMS, AND METHODS: For this retrospective register-based study, we used baseline [18F]fluorodopa PET data acquired at the Turku PET Centre between the years 1988-2016 with three scanners (Ecat 931, GE Advance, HRRT). The data involved 3

Impact of dual-tasking and balance confidence on turns and transitions: a cross-sectional study in Parkinson's disease.

BACKGROUND: Mobility, cognitive processing, and balance confidence impairments can negatively affect functional mobility in people with Parkinson's disease (PD). This study aimed to examine the effects of a cognitive dual-task on functional mobility during Timed Up and Go (TUG) sub-phases involving transitions and turns. A secondary aim was to explore whether balance confidence was associated with dual-task interference (DTI) on TUG total duration and sub-phases. METHODS: A cross-sectional design was employed. Participants completed TUG and TUG-COG (serial three subtractions) and inertial sen

Alkaloids and flavonoids of Solanaceae: Mechanisms of action in neurodegenerative diseases.

Neurodegenerative diseases (e.g., Alzheimer's disease [AD], Parkinson's disease [PD], Huntington's disease [HD]) pose a serious threat to human health. Current therapeutic approaches have limitations such as significant side effects and limited efficacy, making natural plant active ingredients an emerging research focus in this field. Solanaceae plants are widely distributed worldwide, rich in secondary metabolites such as alkaloids and flavonoids, and have long been used in traditional medicine. This review systematically summarizes the structural characteristics of Solanaceae-derived alkaloi

Lactobacillus acidophilus exerts neuroprotective effects in MPTP mice: potential links to the gut microbiota and GLP-1.

Previous studies have shown that Lactobacillus acidophilus (LA) improves gut microbiota, which is believed to play a role in Parkinson's disease pathogenesis by modulating neuroinflammation. Therefore, we investigated the role of LA in mice with PD induced by 1-methyl-4-phenyl-1,2,3,6-tetrahydropyridine (MPTP). To explore microbial-based interventions for Parkinson's disease (PD), we examined whether Lactobacillus acidophilus (LA) protects against neurodegeneration via the gut-brain axis. In MPTP-induced PD mice, LA treatment improved motor function, increased TH+ neurons, and suppressed neuro

Sex-related differences in the short and long-term outcome of internal pallidus stimulation for dystonia.

OBJECTIVES: The importance of sex as a determinant of Globus pallidus internus deep brain stimulation (GPi-DBS) outcome in adults with dystonia remains uncertain. We investigated whether sex is a determinant of the efficacy and safety of GPi-DBS in dystonia. METHODS: In this double-center, retrospective cohort study, we followed patients with idiopathic, inherited, or secondary dystonia for at least 12 months after surgery. Dystonia was assessed using the Burke-Fahn-Marsden Dystonia Rating Scale (BFMDRS). Adverse events were recorded. RESULTS: Fifty-six consecutive patients (22 M/34F) were stu

Heat shock protein 10 as a chaperone modulating α-synuclein amyloid fibril formation.

HSP10 is a well-known human co-chaperone that interacts with HSP60 to comprise the HSP60/10 chaperonin complex which upholds mitochondrial proteostasis. HSP10 also demonstrates independent roles in binding to misfolded proteins and interacts with several amyloidogenic client proteins. Using a variety of biophysical and biochemical methods, we studied the interactions of HSP10 with the amyloidogenic protein α-synuclein (α-syn) associated with Parkinson's disease. HSP10 efficiently inhibited fibril formation of wild type (WT) and disease-mutant A30P α-syn at sufficient concentrations of chaperon

Spatial propagation of movement-related basal ganglia activity predicts parkinsonian motor state.

Movement-related gamma activity (> 60 Hz) in cortico-basal ganglia networks reflects pro-kinetic synchronization dynamics. While in the cortex these temporal dynamics are known to unfold spatially across topographically distributed networks, it remains unclear whether a similar spatial propagation occurs within the basal ganglia, and how such spatial encoding may contribute to both physiological and disease-related mechanisms. The subthalamic nucleus (STN) is a key integrative hub for motor processing within the basal ganglia-cortical circuitry. At rest, STN activity is topographically distrib

[Autophagy and Neurological Diseases].

Autophagy is an essential degradation mechanism that maintains intracellular homeostasis. In recent years, an increasing number of cases with mutations in autophagy-related genes, such as ATG7, have been reported. These findings highlight the crucial role of autophagy in human neurodevelopment. However, the severity of clinical symptoms does not always correlate with the degree of autophagy impairment observed in patient-derived cells, and phenotypic manifestations can vary widely. These findings indicate that autophagy dysfunction alone does not fully explain disease mechanisms, even in neuro

Postbiotics and the gut-brain axis: A mechanistic review on modulating neuroinflammation and cognitive aging.

Aging triggers gut microbiota dysbiosis that disrupts the gut-brain axis (GBA), promoting neuroinflammation and neurodegeneration. Elderly exhibit reduced microbial diversity, depleted beneficial bacteria, and expanded pathobionts, elevating neurotoxic metabolites-lipopolysaccharides (LPS), trimethylamine-N-oxide, kynurenine derivatives, and secondary bile acids. These drive "inflammaging," blood-brain barrier breakdown, microglial activation, mitochondrial impairment, and proteinopathies in Alzheimer's and Parkinson's disease. Conversely, neuroprotective metabolites from commensals-short-chai

Deep brain stimulation for Parkinson's disease in Thailand: cost, utility and policy implications for universal health coverage.

BACKGROUND: Parkinson's disease (PD) is a chronic, progressive, disabling neurodegenerative disorder. Deep brain stimulation (DBS) surgery is a highly effective and beneficial therapy for advanced PD, but its substantial cost limits accessibility. This study aimed to evaluate the cost-utility and budget impact of rechargeable and non-rechargeable DBS compared with standard pharmacotherapy alone in patients with advanced PD. METHODS: We conducted a cost-utility analysis using a Markov model with a 6-month cycle length and a 20-year time horizon. Costs were valued in 2025 Thai Baht, and both co

Neuromodulation of Cortical Targets in Freezing of Gait.

Freezing of gait (FOG) is a disabling feature of Parkinson's Disease (PD) with unclear underlying pathophysiology. Evidence from multimodal neuroimaging studies suggests that complex interactions between cortical and subcortical areas may occur in FOG. While noninvasive neuromodulation techniques, such as transcranial magnetic stimulation (TMS), can effectively modulate large-scale networks involved in FOG, the development of noninvasive neuromodulation interventions is limited by an incomplete understanding of the interactions between underlying network disruptions and FOG behavior. Recent st

Peptidomimetics Inspired by α-Synuclein or Its Chaperone αB-Crystallin Differentially Modulate α-Synuclein Aggregation.

Aggregation of the α-Synuclein (αSyn) protein in neurons is responsible for synucleinopathies such as Parkinson's disease. In healthy cells, αSyn is primarily present as monomers. Under pathological conditions, oligomers and fibrils are formed, leading to neuronal toxicity and death. No treatment prevents fatal synucleinopathies. We designed small peptidomimetics based on the structure of αSyn aggregates and on its chaperone protein αB-Crystallin. Interestingly, a relationship between the impact of peptidomimetics on the αSyn aggregation process, their sequences, and secondary conformation has

Camaraderie for Care Partners: Addressing the Well-Being of Care Partners for Singing Group Participants With Parkinson's Disease Through a Phenomenological Qualitative Approach.

Care partners of individuals with Parkinson's disease experience challenges while managing new responsibilities like transportation, finances, and activities of daily living, which influence their own mental and physical health. While previous qualitative research has investigated the perspectives of people with Parkinson's disease who participate in singing groups, the perspective of their care partners is less frequently studied. The purpose of this manuscript is to understand the involvement and perspective of care partners of people with Parkinson's disease participating in a singing group

Effects of immersive virtual reality and adaptive cognitive training on cognition and function in parkinson's disease mild cognitive impairment: a randomized clinical trial.

Cognitive impairment is a common non-motor feature of Parkinson's disease with no established treatments. Non-pharmacological interventions may promote neural plasticity and enhance cognition. However, high-quality trials are needed to confirm their efficacy and underlying mechanisms. We conducted a 24-week, single-center, randomized, single-blind clinical trial to investigate the efficacy of Immersive Virtual Reality (IVR) and adaptive Computerized Cognitive Training (CCT) for mild cognitive impairment in Parkinson's disease (PD-MCI) in 51 individuals. Participants were randomized into an exe

Synthesis and Biological Evaluation of Rigid Benztropine Analogues with Potential DAT Inhibition Activity: A Prospect in Parkinson's Disease Treatment.

INTRODUCTION: The pharmacological treatment of Parkinson's, an incurable disease, consists of palliative drug combinations, generally including a Dopamine Transporter (DAT) inhibitor and an exogenous source of dopamine. Benztropine has been described as a commonly used DAT inhibitor. However, the ever-increasing secondary effects led to its substitution with other, more suitable drugs. It has been described that the tropine moiety of the molecule generates most of the interaction with DAT, while other relevant interactions correspond to the benzhydryl aromatic moiety with several degrees of fr

GLP-1RAs versus metformin and Parkinson's risk in type 2 diabetes.

BACKGROUND: Parkinson's disease (PD) is an increasingly prevalent neurodegenerative condition, particularly among individuals with type 2 diabetes mellitus (T2DM). While prior studies have suggested that GLP-1 receptor agonists (GLP-1RAs) and metformin may confer neuroprotective effects, most were limited by small sample sizes, short follow-up, uncontrolled designs, or lacked direct comparisons between therapies-making their findings inconclusive for clinical decision-making. METHODS: Using the TriNetX Global Collaborative Network, we identified 92 485 patients with T2DM initiating GLP-1RA th
