Comparison of In Vitro Metrics With Real-World Risk of Drug-Induced Parkinsonism Due to Antipsychotic Drugs: Retrospective Cohort Study.

BACKGROUND: Drug-induced parkinsonism (DIP) predominantly occurs due to antipsychotic drugs (APDs) blocking dopamine D2 receptors (D2Rs). However, in vitro assays often fail to fully reflect real-world variability in clinical outcomes. OBJECTIVE: This study aimed to evaluate whether in vitro pharmacological metrics correspond to real-world risk of DIP associated with APD use. METHODS: For 8 commonly used APDs, key in vitro parameters-including inhibition constants (Ki) of D2Rs and the serotonin 2A receptor, reversal rate (Kr) of D2Rs, and blood-brain barrier (BBB) penetration rate-were compi

Spinal cord stimulation therapy for gait impairment in Parkinson's disease: a double-blinded, randomised feasibility trial with an open extension.

A trial of spinal cord stimulation (SCS) was performed in people with gait-impaired Parkinson's (ClinicalTrials.gov: NCT05110053 ). Fourteen patients underwent gait assessments and [18F]-FDG and [18F]-FEOBV PET at baseline, six, and twelve months after SCS. Twelve participants were randomised to six-month MicroBurst or sham, followed by six-month extension with stimulation. The primary outcomes were feasibility and safety, as captured by the trial process measures and nature and frequency of adverse events, respectively, and the Postural Instability and Gait Disorder (PIGD) score as a clinical

Protein structure and coupled solvent dynamics in α-synuclein fibrils under controlled confinement.

The formation of amyloid fibrils by the intrinsically disordered protein α-synuclein in human brain neurons and extracellular tissue is a hallmark of Parkinson disease. Although the stable protein β-structure in α-synuclein fibrils has been determined at high resolution, the status of the dynamically disordered domains and their responses to in vivo confinement and small-molecule interactions remain elusive. We use electron paramagnetic resonance spectroscopy of the spin probe, TEMPOL, colocalized with α-synuclein fibrils, and temperature-controlled, ice boundary confinement to characterize th

Efficacy and safety of repetitive transcranial magnetic stimulation on motor function, depression, and cognitive dysfunction in Parkinson's disease: A systematic review and meta-analysis of randomized controlled trials.

BACKGROUND: Parkinson's disease (PD) is a multifactorial neurodegenerative disease with a high prevalence worldwide, leading to motor and non-motor symptoms. Moreover, PD presents a progressive aggravation alongside time, the middle and late patients in PD requires the use of a variety of anti-Parkinson's drugs with obvious side effects, which bring serious impact on the quality of life of patients. In recent years, repetitive transcranial magnetic stimulation (rTMS), as a kind of non-invasive neuromodulation therapy, has drawn increasing interest from neurologists, and has been effectively ut

Opisthotonus: Revisiting a classic movement disorder.

Opisthotonus is characterized by backward arching of the trunk, associated with retrocollis, caused by involuntary contractions of extensor paraspinal muscles. Classical etiologies include tetanus, strychnine intoxication, dystonia (particularly tardive dystonia and neurodegeneration with brain iron accumulation), seizures, and functional movement disorders (MDs). Inborn errors of metabolism, epileptic encephalopathies and other gene mutations should be considered in newborns and infants presenting with opisthotonus. We surveyed 1216 consecutive patients presenting for evaluation in a tertiary

BACKGROUND: Freezing of gait (FOG) is a common symptom of Parkinson's disease (PD) that impairs gait and quality of life. Current pharmacological and surgical treatment strategies cannot eliminate FOG. The supplementary motor area (SMA) is an important part of the motor network that contributing to generate anticipatory postural adjustments (APA) and becomes impaired in FOG. Repetitive transcranial magnetic stimulation (rTMS), a non-invasive brain stimulation technique, can regulate the SMA and its connections with other brain regions. It may serve as a valuable adjunctive therapeutic approach

Cycloastragenol Derivatives Improve Tyrosine Metabolism, Regulate TLR4/NF-κB/TERT Signaling Pathways, and Inhibit MPTP Induced Neuroinflammation and PD Symptoms.

BACKGROUND: Parkinson's disease (PD) is a neurodegenerative disease closely related to neuroinflammation and with obvious age characteristics. Existing therapeutic drugs have problems such as insufficient efficacy and side effects. Cycloastragenol (CAG) is a known natural telomerase activator, and previous studies have found that it has a good improvement effect on PD. As a lead compound, there is significant room for improvement in pharmacological activity. Therefore, we further explore safe and efficient small molecules for PD drug exploration through structural optimization. METHODS: We in
