Parkinsonism and neurofibrillary tangle pathology in pigmented nuclei.

Four parkinsonian patients had neurofibrillary tangles, neuronal loss, and gliosis restricted to the substantia nigra and locus ceruleus. No Lewy body inclusions or other neuropathological changes accounting for parkinsonism were found in any of these patients. Their clinical features were characterized by an early age of onset, absence of dementia, absence of other neurological abnormalities, good response to drug therapy, and a long, slowly progressive course of illness. None of the patients had any history of encephalitis. These patients either represent a forme fruste of postencephalitic p

[Hitler's neurologic disease--differential diagnosis of Parkinson syndrome].

This paper deals with the differential diagnosis of Hitler's Parkinsonian syndrome based upon our recently published results of an analysis of Hitler's motor functions in German news-reels, upon archived materials with notes and comments of Hitler's doctors, various memories, and our own interrogations of eye-witnesses of Hitler's nearest entourage. According to these sources, Hitler's Parkinsonian syndrome developing since mid-1941 cannot be attributed to any of the symptomatic forms of Parkinsonism. In light of family history, age of manifestation and clinical features the assumption of idio

[Correlation of clinico-physiologic indices and the serotonin content of the blood of patients with parkinsonism].

On withdrawal of L-DOPA-containing drugs the blood serotonin content decreased in parkinsonism patients irrespective of form and etiology of the disease. L-DOPA-containing drugs caused the blood serotonin levels to rise. Substantial correlations of the blood serotonin levels and clinico-physiologic indices were found mostly in postencephalitic parkinsonism.

The neurotoxin MPTP does not reproduce in the rhesus monkey the interregional pattern of striatal dopamine loss typical of human idiopathic Parkinson's disease.

Using high-pressure liquid chromatography with electrochemical detection, we measured dopamine (DA) and homovanillic acid (HVA) in caudate nucleus, putamen and substantia nigra in 4 untreated rhesus monkeys and 4 monkeys with permanent parkinsonism produced by repeated injections of 1-methyl-4-phenyl-1,2,3,6-tetrahydropyridine (MPTP; total dose: 2.1-6.45 mg/kg, i.m.). MPTP consistently produced a severe striatal and nigral loss of DA and HVA and an increase in the ratio 'HVA/DA'. In this respect, MPTP mimicked the changes found in human Parkinson's disease (PD). However, MPTP lowered the DA in

Ubiquitin is a component of neurofibrillary tangles in a variety of neurodegenerative diseases.

Ubiquitin has been shown to be a component of neurofibrillary tangles in Alzheimer's disease. We now show immunocytochemically that it is also a component of neurofibrillary tangles in several other neurodegenerative diseases of diverse aetiology, including Down's syndrome, dementia pugilistica and postencephalitic parkinsonism, and in normal ageing. Ubiquitin immunoreactivity is not, however, generally found in the neurofibrillary tangles of progressive supranuclear palsy. These findings show that while associated ubiquitin is not a feature unique to the tangles of Alzheimer's disease, it is

Trigeminal trophic syndrome.

Trigeminal trophic syndrome is an unusual condition also known as trigeminal neurotrophic ulceration or trigeminal neuropathy with nasal ulceration. The diagnosis is suggested when ulceration of the face, especially of the ala nasi, occurs in a dermatome of the trigeminal nerve that has been rendered anesthetic by a surgical or other process involving the trigeminal nerve or its central sensory connections. A history of paresthesias and self-induced trauma to the area further support the diagnosis. Neurological deficits causing trigeminal trophic syndrome may result from surgical trigeminal ab

Anatomy and pathology of the basal ganglia.

Neurotransmitters of the basal ganglia are of three types: I, amino acids; II, amines; and III, peptides. The amino acids generally act ionotropically while the amines and peptides generally act metabotropically. There are many examples of neurotransmitter coexistence in basal ganglia neurons. Diseases of the basal ganglia are characterized by selective neuronal degeneration. Lesions of the caudate, putamen, subthalamus and substantia nigra pars compacta occur, respectively, in chorea, dystonia, hemiballismus and parkinsonism. The differing signs and symptoms of these diseases constitute stron

Sulfated glycosaminoglycans in Alzheimer's disease.

Using the sulfated alcian blue and alcian blue-MgCl2 techniques for demonstrating sulfated glycosaminoglycans (GAGs), we have shown sulfated GAGs to be associated with the amyloidotic lesions of Alzheimer's disease, the neuritic plaques, the neurofibrillary tangle, and the congophilic angiopathy. To determine how specific these findings are to Alzheimer's disease, other neurologic disorders with neurofibrillary tangles and filamentous inclusions were examined. These included progressive supranuclear palsy, Pick's disease, subacute sclerosing panencephalitis, and postencephalitic parkinsonism.

Isolation and partial characterization of neurofibrillary tangles and amyloid plaque core in Alzheimer's disease: immunohistological studies.

Fractions enriched in neurofibrillary tangles (NFT) and amyloid fibrils were isolated from the cerebral cortex of three cases of senile dementia of the Alzheimer type. Distilled water suspensions of these fractions were excluded from all pore size gels and resisted digestion with various proteolytic enzymes. Formic acid/chloroform treatment of each fraction resulted in the appearance of 4,000-6,000, 15,000-17,000 and 24,000 molecular weight proteins, with concomitant diminution in the amount of excluded material at the top of each gel. The 4,000-6,000 dalton band was best seen in fractions con

Xenon contrast CT-CBF measurements in parkinsonism and normal aging.

Local cerebral blood flow (LCBF) and local tissue:blood partition, coefficient (L lambda) values were measured during CT scanning while patients with different types of Parkinson's syndrome (N = 14) inhaled a contrast mixture of 35-37 per cent stable xenon gas in oxygen. Single-compartment analysis fitted to infinity was used to calculate L lambda and LCBF values. Results were compared with results from normal age-matched volunteers (N = 24). Mean hemispheric (p less than 0.05) and subcortical (p less than 0.05) gray matter LCBF values were reduced in idiopathic Parkinson's disease (N = 11), c

Infection of the basal ganglia by a murine coronavirus.

The coronavirus, mouse hepatitis virus strain A59 (MHV-A59), causes mild encephalitis and chronic demyelination. Immunohistochemical techniques showed that MHV-A59-infected C57BL/6 mice contained dense deposits of viral antigen in the subthalamic nucleus and substantia nigra, with fewer signs of infection in other regions of the brain. The animals showed extra- and intracellular vacuolation, neuronal loss, and gliosis in the subthalamic-nigral region. Such localization is unprecedented among known viral encephalitides of humans and other species. This infection by a member of a viral class cap

Epidemiology of parkinsonism: incidence, classification, and mortality.

An epidemiological study of parkinsonism over a 13-year period (1967 through 1979) is presented, updating previous reports on incidence and trend in the population of Rochester, Minnesota. The overall average annual incidence of parkinsonism per 100,000 population was 20.5, adjusted to the 1970 total United States population, which is virtually unchanged from previous observations. Incidences calculated for each calendar year (1967 through 1979) revealed no remarkable change following the 1976 swine flu vaccination program. There was no sex difference and the peak incidence occurred between ag

[Late results of stereotaxic surgery in parkinsonism].

The authors analyzed the long-term results of uni- and bilateral stereotaxic operations in 70 patients (38 males and 32 females) with idiopathic and postencephalitic parkinsonism aged 32 to 68 years at the time of examination. It was established that the effect of the first operation with regard to tremor and muscular rigidity did not depend on the side of surgical intervention. Noticeable difference was observed between the results of the first and the second operation in relation to their efficacy and sequel incidence. In cases of bilateral intervention, the second operation on the left hemi

The viral hypothesis in parkinsonism.

The most crucial unanswered question in Parkinson's disease is its fundamental cause. Since Carlsson's original suggestion that dopamine may be a transmitter in the central nervous system involved in the control of motor function and that it may be involved in the Parkinsonian syndrome (Carlsson, 1959), and the now-classic paper by Ehringer and Hornykiewicz (1960) which definitively showed the significant reduction of dopamine concentration in the neostriatum of cases of idiopathic Parkinson and postencephalitic parkinsonism, the vast amount of work on the subject has focused on the biochemica

Loss of neurons in the nucleus basalis of Meynert in Alzheimer's disease, paralysis agitans and Korsakoff's Disease.

The nucleus basalis of Meynert, the major source of cholinergic innervation of the cerebral cortex, was morphometrically investigated in 58 cases of neuropsychiatric disorders and compared to 14 controls. The results demonstrate a loss of neurons in the nucleus basalis of Meynert in Alzheimer's disease (70%), paralysis agitans (77%), and Korsakoff's disease (47%) but no marked reduction of neurons in postencephalitic parkinsonism, Huntington's disease, chronic alcoholism without dementia, schizophrenia and infantile brain damage. Oculogyric crises and parkinsonism. A case of recent onset.

Oculogyric crisis in association with postencephalitic parkinsonism has been reported only following encephalitis lethargica (Economo's disease). Generally, it has been assumed that this phenomenon would fade away with the demise of these postencephalitic patients. Therefore, we report a case of recent onset of persistent oculogyric crisis with parkinsonism in a 35-year-old man following an apparent attack of encephalitis in 1972.

Immunocytochemical comparison of neurofibrillary tangles in senile dementia of Alzheimer type, progressive supranuclear palsy, and postencephalitic parkinsonism.

Antiserum raised against a two-cycle-purified human brain microtubule fraction that binds specifically to tangles of senile dementia of the Alzheimer type was tested on the subthalamus, basis pontis, and midbrain sections of two patients with progressive supranuclear palsy. Tangles composed of 15 nm straight fibers in these areas showed positive immunolabeling with the antiserum. The labeling tangles appeared as a fibrous meshwork in neuronal cell bodies and occasionally in short segments of axons. Corresponding sections of brains obtained from patients with idiopathic parkinsonism, striatonig

Seborrheic dermatitis in neuroleptic-induced parkinsonism.

An increased prevalence of seborrheic dermatitis has previously been noted in idiopathic Parkinson's disease and in postencephalitic parkinsonism. Our study of 42 hospitalized patients with drug-induced parkinsonism and 47 hospitalized psychiatric patients without that disorder showed a statistically significant higher prevalence of clinically diagnosed seborrheic dermatitis in the group with drug-induced parkinsonism (59.5% v 15%). To our knowledge, this is the first report of an increased prevalence of seborrheic dermatitis with drug-induced parkinsonism.
