Detection of Treponema pallidum DNA for diagnosis, resistance identification, and treatment outcome prediction in early syphilis among men who have sex with men.

OBJECTIVES: We investigated the use of Treponema pallidum DNA (TP-DNA) for diagnosis, resistance identification, and treatment outcome prediction in early syphilis among men who have sex with men (MSM). METHODS: MSM seeking care for sexually transmitted infections were prospectively enrolled from September 2021 to August 2024. Oral rinse, rectal swab, and urethral swab samples were tested for TP-DNA. Resistance-associated mutations (RAMs) to macrolides and tetracyclines were identified. Treatment responses were compared between syphilis cases with detected TP-DNA and those without. RESULTS:

Syphilitic Cholangiopathy Mimicking Primary Sclerosing Cholangitis.

INTRODUCTION: Syphilis is a sexually transmitted disease with variable symptoms, often imitating various other disorders. Syphilis progresses through primary, secondary, latent, and tertiary stages, each with distinct clinical manifestations. A sudden rise in serum hepatic enzyme levels and imaging findings that mimic sclerosing cholangitis, both associated with a positive response to targeted antibiotic treatment, may indicate a diagnosis of acute syphilitic hepatitis. CASE PRESENTATION: We report a case of early syphilis in the secondary stage, manifesting as sclerosing-cholangitis-like cha

Kaposi Sarcoma Masquerading as Methicillin-Resistant Staphylococcus aureus Soft Tissue Infection of the Foot in an Immunocompromised Transgender Female.

Kaposi sarcoma (KS) is an acquired immunodeficiency syndrome-defining malignancy caused by human herpesvirus-8 (HHV-8), predominantly affecting immunocompromised individuals, particularly those with untreated or advanced human immunodeficiency virus (HIV). Here, we present the case of a 28-year-old homeless transgender female with untreated HIV (CD4 count = 175 cells/μL, HIV RNA = 221,000 copies/mL), latent syphilis, and methamphetamine use disorder, who presented with a five-month history of progressive left foot pain, ulceration, and swelling. Initial examination revealed extensive bilateral

Sexually transmitted infections in MSM with tenofovir/emtricitabine-resistant or cabotegravir-resistant HIV.

OBJECTIVE: Sexually transmitted infections (STIs) are associated with an increased risk of HIV transmission, raising concerns in case of virus resistant (R) to the drugs currently approved for preexposure prophylaxis. We explored the incidence of STIs in MSM with HIV (MSMWH) and resistance to tenofovir/emtricitabine (TXF/FTC) and/or cabotegravir (CAB). DESIGN: Retrospective, cohort study on MSMWH on antiretroviral treatment (ART) with at least one genotyping resistance test (GRT) including integrase. METHODS: The following STIs were included in the analysis: Neisseria gonorrhoeae , Chlamydia

IFN-γ gene polymorphisms +874 T/A and +2109 A/G are associated with the serofast state after early syphilis treatment: a prospective observational study.

BACKGROUND: In approximately 20% of patients with early syphilis, the classical serological response pattern is absent following treatment. They experience a serofast state, which manifests as less than a 4-fold decline in non-treponemal titres, without any clinical signs of treatment failure or reinfection. The effectiveness of the immune defense against T. pallidum, as well as its potential failure and the occurrence of the serofast state, depends on the Th1 cellular response, including cytokines such as IFN-γ. The aim of this prospective observational study was to investigate the impact of

Exploring syphilis activity for personalized treatment strategies in latent syphilis: a 2-year cohort study.

Background Relying simply on the stage of latent syphilis may lead to excessive treatment strategies. Utilizing nontreponemal immunoglobulin M (IgM) antibodies to assess syphilis activity and tailor treatment strategies may offer enhanced therapeutic advantages. Objectives To investigate whether nontreponemal IgM antibodies can serve as a serological marker for assessing the activity of latent syphilis and inform personalized treatment strategies. Methods We evaluated nontreponemal IgM antibodies in 412 latent syphilis patients and conducted a 2-year follow up to analyze their rate of seroconv

Severe Rectal Syphilis in the Setting of Profound HIV Immunosuppression: A Case Report Highlighting ERG/CD38 Immunophenotyping and a Review of the Literature.

Background and Aim: Syphilis, caused by Treponema pallidum, classically presents with genital or anal chancres; rectal involvement is rare and frequently misdiagnosed as inflammatory bowel disease or malignancy. We describe an unusually severe case of syphilitic proctitis in the setting of advanced HIV-related immunosuppression (CD4 39 cells/µL), in which targeted immunophenotyping (ERG and CD38) was a valuable adjunctive tool in the differential diagnosis. Case Presentation: A 46-year-old man with a recent history of erosive gastritis and esophageal candidiasis presented after six months of u

Subcutaneous Infusion of Benzathine Penicillin G Is Acceptable and Preferred Over Intramuscular Injections for Syphilis in Western Australian Sexual Health Clinic Attendees: A Mixed Methods Sub-study.

BACKGROUND: Controlling the syphilis epidemic in Australia is a public health priority. Regular intramuscular (IM) injections of benzathine penicillin G (BPG) are the current standard of care for late latent syphilis in Australia; however, repeated IM BPG injections are painful, and treatment completion rates are low. Early-phase clinical trials have demonstrated the tolerability and safety of high-dose subcutaneous infusions of BPG (SCIP), where the total treatment dose can be delivered at a single visit. Here we describe the experiences and preferences of attendees of Western Australian sexu

Evaluation of a novel comprehensive sexual health e-testing pathway to inform state-wide roll out in new South wales, Australia.

AIM: To evaluate the acceptability of MyCheck for asymptomatic sexually transmissible infections (STIs) testing. BACKGROUND: Regular testing for STIs in at-risk populations is recommended by Australian guidelines as many STIs are asymptomatic. Consequently, MyCheck was developed to enhance access by combining a telehealth consultation with direct referral to pathology, avoiding the need for in-person clinic visits. MyCheck was piloted at Sydney Sexual Health Centre (SSHC)-the largest publicly funded urban sexual health clinic in New South Wales-between June 2021 and February 2022. METHODS: M

An outbreak-driven emergency department screening program for syphilis in North Queensland's Aboriginal and Torres Strait Islander population.

BACKGROUND: There is an ongoing syphilis outbreak in regional and rural areas of Australia, with Aboriginal and Torres Strait Islander peoples disproportionately represented in notification data. Innovative screening approaches are needed in healthcare settings frequented by at-risk communities. The emergency department (ED) was hypothesised to be an ideal location for screening Aboriginal and Torres Strait Islander peoples, who traditionally underutilise healthcare services. METHODS: A quality improvement initiative introduced an opt-out syphilis screening program at the Townsville Universit

Necrotizing Pneumonia Complicated by Bartonella-Associated Culture-Negative Endocarditis in an Immunocompetent Adult.

Bartonella-related culture-negative endocarditis (CNE) is an infrequent and difficult diagnosis, due to the organism's unique growth characteristics and frequent negative cultures. We describe a case of a 42-year-old previously healthy male with necrotizing pneumonia complicated by empyema and bronchopleural fistula. Despite signs of systemic inflammation and infection, multiple blood cultures remained negative. Our diagnosis was made with serologic testing, which revealed elevated Bartonella antibody levels, and echocardiographic findings of endocarditis. Bartonella species are zoonotic, time

Mathematical modeling and analysis of a sex-structured syphilis model with control measures.

Syphilis remains a significant global public health challenge, with over seven million new adult infections each year despite the availability of effective treatment. Its continued persistence, particularly among high-risk and underserved populations, highlights the need for robust analytical models to understand transmission dynamics and inform targeted control strategies. This study presents a novel sex-structured compartmental model that captures the biological and behavioral complexity of syphilis transmission, incorporating multiple infection stages (primary, secondary, and latent), gende
