Skin intraneural phosphorylated α-synuclein is a highly specific biomarker for early Parkinson's disease.

The early diagnosis of Parkinson's disease (PD) represents a challenge, and novel accurate biomarkers are therefore needed urgently. Detection of phosphorylated α-synuclein (p-α-syn) in skin nerve fibres has shown promise as such a marker. However, its accuracy for the identification of PD among patients with early signs of parkinsonism has not been explored thoroughly. In this blinded, multicentre, prospective and follow-up study, 151 patients diagnosed with early-stage parkinsonism (<18 months duration) were enrolled at three tertiary movement disorder centres. Clinical scales were performed

Neurovascular Integration Failure and Freezing of Gait: Rethinking Parkinsonism Through a Vascular Lens.

A subset of patients with parkinsonian syndromes, particularly older adults, present with early gait dysfunction, postural instability, cognitive-motor dissonance, and limited dopaminergic responsiveness. These features, often labeled as atypical Parkinson's disease (PD), may instead represent a distinct neurovascular endophenotype, vascular parkinsonism (VP), driven by cerebral small-vessel disease (SVD). We introduce the Neurovascular-Locomotor Integration Failure (NLIF) hypothesis, which interprets VP symptoms-especially freezing of gait (FOG)-as manifestations of vascularly mediated networ

Discriminating vascular parkinsonism from early-stage postural instability gait difficulty subtype dominant Parkinson's disease: a dual-task gait analysis using wearable sensors.

BACKGROUND: Vascular parkinsonism (VP) and the postural instability and gait difficulty (PIGD) subtype of Parkinson's disease (PD) exhibit similar gait characteristics. However, most research emphasizes lower-limb gait parameters, often neglecting the role of cognitive function in gait regulation. Therefore, this study investigates differences in cognitive-motor interactions between VP and PIGD to identifying specific gait biomarkers and develop a diagnostic model. METHODS: We recruited 37 PIGD patients and 37 VP patients between year 2022 to 2024 and used wearable devices to record gait para

Current Neuroimaging Modalities to Distinguish Parkinson's Disease from its Mimics: Imaging Features and Implications for Clinical Practice.

Parkinson's disease (PD) is a chronic progressive neurodegenerative disorder characterized by tremors, rigidity, and bradykinesia. This is primarily attributed to loss of nigrostriatal dopaminergic neurons to varying degrees. Many conditions that present similar classic motor symptoms of PD, known as atypical parkinsonian syndromes (APS), have also been identified. These encompass multiple system atrophy (MSA), progressive supranuclear palsy (PSP), dementia with Lewy bodies (DLB), and corticobasal degeneration (CBD). On the other hand, causes of non-neurodegenerative parkinsonism include vascu

A cross-sectional study of autonomic dysfunction in patients with Parkinson's disease and vascular parkinsonism.

This study aimed to evaluate autonomic function differences between Parkinson's disease (PD) and vascular parkinsonism (VP) for improved differential diagnosis. A total of 120 patients (60 PD, 60 VP) underwent comprehensive autonomic assessments, including skin scratch test, oculocardiac reflex, postural heart rate changes, heart rate variability (HRV), and 24-hour urinary vanillylmandelic acid (VMA). Disease severity was measured using MDS-UPDRS and Hoehn-Yahr staging in PD and NIHSS in VP. Results demonstrated significantly more severe autonomic impairment in PD across all measures (all P <

Acupuncture for Mild Cognitive Impairment in Vascular Parkinsonism: A Prospective, Randomized, Controlled Trial Protocol.

PURPOSE: Vascular parkinsonism (VP) is a form of secondary parkinsonism caused by cerebrovascular disease and is typically associated with progressive cognitive decline, which greatly disrupts patients' daily lives and well-being. Vascular parkinsonism with mild cognitive impairment (VP-MCI) represents an early and potentially reversible stage of cognitive dysfunction, and an important window for early intervention. However, effective therapies for VP-MCI remain limited. Acupuncture is a non-pharmacological intervention that has shown potential benefits for motor and cognitive functions in vas

Efficacy of Clotiazepam for Respiratory Dyskinesia in a Patient With Parkinsonism: A Case Report.

This report describes a rare case of respiratory dyskinesia in a patient with vascular parkinsonism that was successfully controlled with clotiazepam. An 89-year-old man with a history of vascular parkinsonism presented with dyspnea and irregular breathing associated with marked orofacial and truncal dyskinesia. These symptoms became prominent following the upward titration of a ropinirole patch to 24 mg/day. Although mirtazapine was subsequently added to address comorbid depressive symptoms, it failed to alleviate his respiratory distress. In contrast, the administration of clotiazepam (10 mg

Lower body parkinsonism: rethinking MRI planimetry.

BACKGROUND: Magnetic resonance imaging (MRI) planimetric biomarkers such as the midbrain to pons (M/P) ratio, magnetic resonance parkinsonism index (MRPI), and MRPI 2.0 have demonstrated high diagnostic accuracy discriminating Progressive Supranuclear Palsy (PSP) patients from other atypical parkinsonisms and Parkinson's Disease (PD). However, these indexes have not been studied in the specific context of lower body parkinsonism (LBP), where the differential diagnosis between PSP, vascular parkinsonism (VP) and normal pressure hydrocephalus (NPH) is of great clinical relevance. OBJECTIVES: Ou

Secular trends of incidence and prevalence of Parkinsonism subtypes: a cohort study in the United Kingdom.

To evaluate secular trends of incidence and prevalence of Parkinson's Disease (PD), Vascular Parkinsonism (VP), and Drug-induced Parkinsonism from 2007 to 2021 in the UK. We used primary care data, Clinical Practice Research Datalink GOLD, from the UK. Individuals were included if they were registered from January 2007 to December 2021 with at least one year of prior observation. Age-standardized and crude incidence and prevalence were calculated annually; age-standardized rates were stratified by sex, and crude rates by age and sex. From 2007 to 2019, the age-standardized incidence of PD decr

A Therapeutic Paradox: Intractable Hiccups in Acute Stroke and Vascular Parkinsonism Following Initiation of Carbidopa/Levodopa.

Intractable hiccups are an uncommon but clinically significant condition that can lead to sleep disturbance, fatigue, impaired oral intake, and reduced quality of life. The hiccup reflex arc involves complex peripheral and central pathways, with dopaminergic signaling believed to play a modulatory role. In neurologically complex patients, distinguishing medication-related hiccups from structural central nervous system etiologies remains challenging. We present a 94-year-old male with a history of vascular Parkinsonism and multiple comorbidities who was admitted for acute ischemic stroke involv

Exploring cerebral small vessel disease signatures in familial Parkinson's disease.

Familial Parkinson's disease (PD) and vascular parkinsonism (VP) present overlapping features and may co-exist. To investigate whether PD and VP may share a potential pathogenic link and to what extent white matter hyperintensities may influence PD phenotype, we used the modified Scheltens scale and presented a descriptive and exploratory analysis of the classic neuroradiological features of cerebral small vessel disease (cSVD) in the axial T2-FLAIR MRI sequences in a cohort of 104 familial PD and PD prodromal patients and 48 age-matched controls from the PPMI publicly available database. Disproportionate bulbar dysfunction following small corona radiata infarction in vascular parkinsonism: A case report.

RATIONALE: Subcortical lacunar infarctions commonly occur in cerebral small vessel disease and contribute to progressive functional decline in older adults. Infarction of the corona radiata may disrupt converging corticobulbar pathways. In patients with vascular parkinsonism, diminished subcortical motor reserve may render bulbar function particularly vulnerable, resulting in neurological deficits disproportionate to lesion size. PATIENT CONCERNS: A 75-year-old woman with vascular parkinsonism presented with acute dysarthria, severe dysphagia requiring nasogastric tube feeding, and marked gai

Clinical utility of DaTscan in clinically uncertain parkinsonian syndromes: a systematic review and meta‑analysis.

BACKGROUND: Clinically uncertain parkinsonian syndromes (CUPS) pose diagnostic challenges, particularly in distinguishing degenerative from non‑degenerative etiologies. DaTscan (123I-ioflupane SPECT) is frequently used to clarify ambiguity, but the magnitude of its diagnostic impact across different initial clinical suspicions remains unclear. METHODS: A systematic review of MEDLINE, EMBASE, PsycINFO, and the Cochrane Library was conducted from January 2002 to February 2026, with no language restrictions. Studies reporting a diagnostic change after DaTscan were included. Two reviewers indepen

Bile duct injuries (BDI) tend to be more complex in laparoscopic than in open cholecystectomy procedures, and frequently involve young adults with benign pathologies. The ultimate consequence may be a liver transplantation (LT), making this situation one of the most rare transplant indications. Fatal post-transplant outcome is extreme infrequently reported. Aim of this study is to report on our single-case experience and to review the literature concerning lethal outcome after LT for major BDI following cholecystectomy. The Role of the Claustrum in Parkinson's Disease and Vascular Parkinsonism: A Matter of Network?

BACKGROUND: The mechanisms underlying extrapyramidal disorders and their anatomical substrate have been extensively investigated. Recently, the role of the claustrum in Parkinson's disease and other neurodegenerative conditions has been better detailed. The main aim of this review was to summarize the supporting evidence for the role of the claustrum in degenerative and vascular parkinsonism. METHODS: The anatomy, biology, vascular supply, and connections of the claustrum in humans were identified and described, providing the substrate for the vascular involvement of the claustrum in large- a

Cerebrospinal fluid and blood neurofilament light chain in Parkinson's disease and atypical parkinsonian syndromes: a systematic review and Bayesian network meta-analysis.

BACKGROUND AND OBJECTIVE: The value of neurofilament light chain (NfL) levels as a biomarker for the diagnosis and differential diagnosis in patients with Parkinson's disease (PD) and atypical parkinsonian syndromes (APS) remains controversial. Furthermore, few studies have directly compared NfL levels among specific APS categories. This study aimed to compare cerebrospinal fluid (CSF) and blood NfL levels among PD, APS, other PD-related disorders, and controls, as well as rank NfL levels across these groups. METHODS: PubMed, Embase, Web of Science, and the Cochrane Library were searched from

Neurophysiology of Atypical Parkinsonian Syndromes: A Study Group Position Paper.

Atypical parkinsonian syndromes (APs) are characterized by parkinsonian features combined with additional motor and non-motor signs and symptoms. Neurophysiological studies have contributed to clarifying differences and similarities between APs and idiopathic Parkinson's disease (PD) and to unravel specific pathophysiological features of APs. A comprehensive and updated evaluation of the potential clinical utility of the available neurophysiological tools in APs is, however, currently needed. The Neurophysiology Study Group of the International Parkinson and Movement Disorder Society reviewed
