Case Report: Decoding genetic risks of vascular parkinsonism: a case series.

BACKGROUND: Vascular parkinsonism (VaP) is a subtype of parkinsonism which needs better characterization of its risks and determinants. OBJECTIVE: The aim of this report is to present an understanding of genetic risks of vascular parkinsonism. METHODS: Five participants diagnosed with VaP were recruited and Whole Exome Sequencing (WES) was performed to analyze deleterious variants in relevant genes associated with vascular and parkinsonian diseases. RESULTS: We identified several candidate risk variants for VaP in our patients, particularly in LRRK2, PLA2G6, TGM6, BSN, UBR4, CD36 and NOTCH3

NOTCH3 p.R544C variant in vascular Parkinsonism: Clinical and imaging correlates in a Taiwanese cohort.

BACKGROUND: Vascular parkinsonism (VP) is a heterogeneous syndrome caused by cerebrovascular pathology. While generally distinguishable from Parkinson's disease (PD), overlap may occur in some cases. The genetic basis of VP remains unclear. The role of the NOTCH3 p.R544C variant-common in East Asian cerebral autosomal dominant arteriopathy with subcortical infarct and leukoencephalopathy (CADASIL) cohorts-in VP has not been systematically evaluated. METHODS: We conducted a prospective study of 253 patients with parkinsonism (idiopathic PD, VP, or atypical parkinsonism), who underwent clinical

Clinical MRI Biomarkers to Differentiate Parkinson Disease from Its Mimics.

The clinical discrimination of Parkinson disease (PD) from its mimics can be challenging, especially in the early stages. These mimics include atypical parkinsonian syndromes (APSs), essential tremor, drug-induced parkinsonism, and vascular parkinsonism. Moreover, differential diagnosis of PD is not exclusive; overlapping and coexisting neurodegenerative diseases are common. Most patients with suspected PD undergo standard clinical MRI (including T1- and T2-weighted sequences) as the first imaging examination, with the primary objective to exclude other diseases. Standard MRI can help detect v

Utility of Locus Coeruleus Signal Intensity on High-resolution T1-weighted MR Imaging with Magnetization Transfer for Differentiating Parkinson's Disease from Atypical Parkinsonism.

PURPOSE: This study aimed to evaluate the diagnostic utility of locus coeruleus (LC) signal intensity on high-resolution T1-weighted imaging with magnetization transfer (T1WI with MT) for distinguishing Parkinson's disease (PD) from a broad range of atypical parkinsonism (AP) subtypes, including early-stage cases. METHODS: We retrospectively analyzed T1WI with MT data from 214 participants, including patients with PD (n = 125), corticobasal syndrome (CBS, n = 12), multiple system atrophy (MSA, n = 16), progressive supranuclear palsy (PSP, n = 19), essential tremor (n = 17), vascular parkinson

The role of presynaptic dopaminergic imaging in acquired neurological conditions affecting basal ganglia: a systematic review.

BACKGROUND: Dopaminergic imaging has become a pivotal tool in the diagnosis of Parkinson's disease (PD) and related disorders. Its ability to assess presynaptic dopamine transporter function provides crucial insights for distinguishing PD from other acquired neurological disorders. Recent advances have also demonstrated its utility in evaluating diseases beyond PD, including non-degenerative conditions associated with parkinsonism. OBJECTIVES: This review aims to explore the diagnostic and therapeutic value of dopaminergic imaging across a range of acquired disorders, including normal pressur

Vascular parkinsonism: an update.

Vascular parkinsonism (VP), resulting from cerebrovascular disease, is a rare disorder with a characteristic motor and non-motor clinical profile distinct from sporadic/idiopathic Parkinson disease (PD) and other parkinsonian disorders. It accounts for 3-6% of all parkinsonian syndromes and may overlap with other parkinsonisms. Clinical features of VP are heterogenous and include bilateral rigidity with lower body predominance, bradykinesia, postural instability, shuffling gait, falls, corticospinal symptoms and cognitive impairment, tremor being rare or absent. An international working group

Moyamoya Disease and the Risk of Parkinson's Disease.

OBJECTIVES: Moyamoya disease (MMD) is a rare cerebrovascular disorder characterized by the progressive narrowing of arteries at the base of the brain, forming abnormal collateral vascular networks. While vascular parkinsonism is noted in MMD, its link to Parkinson's disease (PD) has not been explored. We aimed to determine whether the risk of PD is increased in patients with MMD and to identify the potential role of the RNF213 gene. METHODS: We report two cases of PD with a history of MMD associated with the Arg4810Lys variant of RNF213. Using the Korean National Health Insurance Service data

Structural Network Efficiency Predicts Conversion to Incident Parkinsonism in Patients With Cerebral Small Vessel Disease.

BACKGROUND: To investigate whether structural network disconnectivity is associated with parkinsonian signs and their progression, as well as with an increased risk of incident parkinsonism. METHODS: In a prospective cohort (Radboud University Nijmegen Diffusion Tensor and Magnetic Resonance Cohort study) consisting of 293 participants with small vessel disease (SVD), we assessed parkinsonian signs and incident parkinsonism over an 8-year follow-up. In addition, we reconstructed the white matter network followed by graph-theoretical analyses to compute the network metrics. Conventional magnet

New insight on the possible role of statins in Vascular Parkinsonism: A need for presumptive therapy.

Vascular Parkinsonism (VP) is clinical term represents a progressive ischemic changes and subcortical lacunar infarct leading to Parkinsonism mainly in the lower limbs so called lower body Parkinsonism. The VP neuropathology is differed from that of PD neuropathology which rarely associated with basal ganglion lesions. Dopamine transporters are normal in VP but are highly reduced in PD, and dopaminergic agonists had no effective role on VP. The neuropathological mechanisms of VP are related to vascular injury which induces the interruption of the neural connection between basal ganglion and ce

Association of Parkinson's disease to Parkinson's plus syndromes, Lewy body dementia, and Alzheimer's dementia.

BACKGROUND: Parkinson's disease (PD) is a condition that affects movement and is usually seen in those over the age of 50. It is caused by the death of dopaminergic neurons, particularly in the substantia nigra. PD has shifted from being perceived as an uncommon condition to a significant neurological illness, mostly due to the increasing number of elderly individuals and the impact of environmental factors. Parkinson's plus syndromes, such as progressive supranuclear palsy (PSP), multiple system atrophy (MSA), corticobasal degeneration (CBD), and vascular Parkinsonism (VaP), provide difficult

I123-FP-CIT (DaTSCAN) SPECT beyond the Most Common Causes of Parkinsonism: A Systematic Review.

BACKGROUND: As the diagnosis of Parkinson's disease (PD) is fundamentally clinical, the usefulness of ioflupane (123I) single-photon emission computed tomography (SPECT) or DaTSCAN as a diagnostic tool has been a matter of debate for years. The performance of DaTSCAN is generally recommended in the follow-up of patients with a clinically uncertain diagnosis, especially in those with a suspected essential tremor, drug-induced parkinsonism, or vascular parkinsonism. However, there is a dearth of DaTSCAN findings regarding neurodegenerative parkinsonisms besides PD and atypical parkinsonisms. To

Expanding the Neurological Phenotype of Anderson-Fabry Disease: Proof of Concept for an Extrapyramidal Neurodegenerative Pattern and Comparison with Monogenic Vascular Parkinsonism.

Anderson-Fabry disease (AFD) is a genetic sphingolipidosis involving virtually the entire body. Among its manifestation, the involvement of the central and peripheral nervous system is frequent. In recent decades, it has become evident that, besides cerebrovascular damage, a pure neuronal phenotype of AFD exists in the central nervous system, which is supported by clinical, pathological, and neuroimaging data. This neurodegenerative phenotype is often clinically characterized by an extrapyramidal component similar to the one seen in prodromal Parkinson's disease (PD). We analyzed the biologica

A General Neurologist's Practical Diagnostic Algorithm for Atypical Parkinsonian Disorders: A Consensus Statement.

PURPOSE OF REVIEW: The most common four neurodegenerative atypical parkinsonian disorders (APDs) are progressive supranuclear palsy (PSP), multiple system atrophy (MSA), corticobasal syndrome (CBS), and dementia with Lewy bodies (DLB). Their formal diagnostic criteria often require subspecialty experience to implement as designed and all require excluding competing diagnoses without clearly specifying how to do that. Validated diagnostic criteria are not available at all for many of the other common APDs, including normal pressure hydrocephalus (NPH), vascular parkinsonism (VP), or drug-induce

Vascular parkinsonism showing dopamine transporter scan findings mimicking those of Parkinson's disease.

Dopamine transporter (DAT) scan is important in the diagnosis of Parkinson's disease (PD). We herein report a patient with vascular parkinsonism (VP) who showed dopamine transporter (DAT) scan findings which mimicked those typically seen in patients with Parkinson's disease (PD). DAT scan findings in patients with VP are characterized by a lower striatal asymmetry index than in patients with PD and decreased uptake in the area consistent with old cerebral infarction or hemorrhage. However, we should be aware that, as in our patient, VP patients with asymmetric basal ganglia lesions, may show i

Acute Ischemic Stroke Presenting as Hemiparkinsonism: A Case Report.

Parkinsonism, commonly associated with Parkinson disease (PD), can also arise from other neurodegenerative disorders or non-neurodegenerative causes such as vascular parkinsonism (VP). This case highlights the acute onset of VP following a stroke, contributing to the understanding of VP's varied presentations and the challenges in its diagnosis. A 54-year-old male with a history of cerebrovascular disease presented with behavioral changes and reduced activity, initially raising the suspicion of a central nervous system infection. Clinical findings included agitation, decreased social interacti

Spatial discrimination in patients with MSA, PSP, DIP, and VP with pain.

Pain is common in Parkinson's disease and frequently observed in other diseases involving parkinsonism. Abnormal scaling function in PD has been reportedly associated with pain, but the role of this function in pain in other parkinsonism-related diseases remains unknown. We screened 127 patients with multiple system atrophy (MSA, n = 24), progressive supranuclear palsy (PSP, n = 15), drug-induced parkinsonism (DIP, n = 56), or vascular parkinsonism (VP, n = 32). After screening, 79 patients with parkinsonism (23 MSA, 10 PSP, 28 DIP, and 18 VP patients) were included in the study. We divided th
