Delayed parkinsonism associated with hypotension in a child undergoing open-heart surgery.

An eight-year-old boy developed acute parkinsonism four days after open-heart surgery for repair of a ventriculo-septal defect. During the procedure he experienced a hypotensive episode which required administration of positive-inotropic agents. Complementing the clinical signs of parkinsonism, CT scan showed symmetrical hypodensities in the basal ganglia, and decreased regional cerebral blood flow was demonstrated using 99mTc HMPAO SPECT. These findings were suggestive of a hypoxic-ischaemic insult to the basal ganglia. The child was treated with levodopa/carbidopa and subsequently completely

[Translation and application of the Simpson and Angus Scale of Extrapyramidal Symptoms].

Extrapyramidal side effects of neuroleptics are important in clinical practice. Study of extra-pyramidal side effects is also of importance for researchers who test new antipsychotic agents or study tardive dyskinesia. A french translation of the Simpson-Angus Rating Scale of extra-pyramidal side effects thus appeared useful. This scale contains 10 items: Gait, Arm dropping, Shoulder shaking. Elbow rigidity, Wrist rigidity, Leg pendulousness, Head dropping, Glabella Tap, Tremor, Salivation. Each item is rated between 0 and 4. A total score is obtained by adding the items and dividing by 10. Sc

A study of the early signs of drug induced parkinsonism.

Postural tremor was recorded from psychiatric patients who had been treated with phenothiazine or butyrophenone neuroleptic drugs. None of the patients had previously been diagnosed as having extrapyramidal dysfunction. A significant number of these patients had abnormally low tremor frequencies. Low frequency tremors are often associated with Parkinson's disease (PD) so that some of these patients may have early signs of drug induced Parkinsonism (DIP). The results indicate that DIP is not necessarily characterised by a bilateral slow frequency tremor, but may initially be similar to idiopath

Risperidone versus perphenazine in the treatment of chronic schizophrenic patients with acute exacerbations.

Risperidone (RIS), a new neuroleptic with 5-HT2- and dopamine D2 receptor-blocking properties, was compared with perphenazine (PER) in a double-blind, multicentre, parallel-group study in 107 chronic schizophrenics with acute exacerbation. RIS 5-15 mg or PER 16-48 mg daily was given for 8 weeks. Psychopathology was assessed with the Positive and Negative Syndrome Scale (PANSS) and Clinical Global Impression. Seventy-eight patients completed the trial; there was an equal number of dropouts on both drugs. The mean daily dose at endpoint was 8.5 mg RIS and 28 mg PER. The reduction in total PANSS

Gait disorders of multi-infarct dementia. CT and clinical correlation.

Twenty-five patients with various types of gait disorders of multi-infarct dementia (MID) were reported. The types of gait disorders consisted of lower body parkinsonism (LBP) plus ataxia (6 patients), LBP plus apraxia (5 patients), and a combination of LBP plus ataxia and apraxia (14 patients). Hypertension occurred in 23 (92%) of the 25 patients. Nevertheless, individual stroke risk factors and the locations of infarcts were not significantly different between the subgroups. Ventriculomegaly and "leuko-araiosis" as demonstrated by computed tomography occurred in more than 80% of patients in

Central autonomic disorders.

Central autonomic dysfunctions can be due to primary (degenerative) or secondary disorders. Autonomic failure (AF) may be a major manifestation of multiple system atrophy (MSA) and idiopathic Parkinson's disease (IPD). In both MSA and IPD, AF is almost invariably associated with neuronal loss in the intermediolateral cell columns. Dysautonomia in MSA is early, severe, and progressive, including marked orthostatic hypotension and urinary incontinence and is complicated by respiratory disturbances, such as laryngeal stridor and sleep apnea. MSA/AF can be differentiated from primary (or pure) aut

L-dopa resistant parkinsonism in an adult woman with a cyst in the posterior fossa.

We report a case of a woman with L-Dopa resistant asymmetrical parkinsonism with a posterior fossa cyst compressing the lower brainstem on MR. She did not show improvement in any of her symptoms after cysto-cardiac derivation. It was not possible to delineate if this was a case of a new malformative syndrome or the coincidence of two different disorders.

[Efficacy of the proteic redistribution diet (PRD) in the antiparkinsonian effect of L-dopa].

The efficacy of PRD was evaluated in 21 patients with Parkinson's disease who presented motor fluctuations or a bad quality "on" time despite treatment with L-dopa or dopaminergic agonists. Eighteen of the 21 patients completed three months of treatment and only 14 (66.6%) were able to complete 12 months. The four patients who stopped treatment in this period did so because of subjective complaints despite obtaining good clinical response to PRD. Only three patients suspended the dietetic regime during first month of treatment. The 14 who completed the year of treatment showed a decrease in th

[Extrapyramidal syndrome induced by chemical substances].

Extrapyramidal signs are frequently observed in toxic diseases due to environmental and industrial chemical substances. The predominant manifestations are Parkinsonism and less frequently tremor. Parkinsonism has been described among the toxic diseases due to carbon monoxide, hydrogen sulfide, and hydrogen cyanide. In these diseases, Parkinsonism used to appear in the recovery stage from coma in the severe cases. This was also true in methyl alcohol poisoning. The extrapyramidal signs are usually symmetrical. Akinesia and rigidity are main features. Tremor is absent or mild. Levodopa and the o

Adverse effects of antipsychotic drugs.

Since the introduction of chlorpromazine in the 1950s, neuroleptic medications have been the mainstay of treatment of schizophrenia and other psychotic disorders. These medications do not always lead to complete remission of symptoms but they have allowed many patients to lead more productive and satisfying lives away from the restrictions of chronic hospitalisation. However, neuroleptics are associated with a number of adverse effects that can compromise their effectiveness. Extrapyramidal adverse effects include acute dystonic reactions, neuroleptic-induced Parkinsonism and akathisia. They c

Studies on the neuromuscular blocking action of MPTP in the mouse phrenic nerve-diaphragm.

The peripheral neurotoxicity of 1-methyl-4-phenyl-1,2,3,6- tetrahydropyridine (MPTP) in the mouse phrenic nerve-diaphragm preparation was studied. The inhibition of nerve-evoked twitches was dependent on the concentrations of MPTP, ranging from 1 to 200 microM, which only slightly increased the amplitude of the twitch evoked by direct stimulation of the muscle. Pretreatment with pargyline did not prevent this inhibitory action of MPTP. The inhibition of the twitch by MPTP was reversible and could be partially reversed by anticholinesterase agents such as neostigmine and physostigmine, as well

Reversible parkinsonism induced by hypercalcemia and primary hyperparathyroidism.

We describe a patient who had the recent onset of both primary hyperparathyroidism and parkinsonism. Surgical removal of a parathyroid adenoma was followed by spontaneous resolution of the parkinsonism. Hypoparathyroidism and other disorders of calcium metabolism are recognized causes of secondary parkinsonism, while hyperparathyroidism is not. We review the relevant literature, which supports the hypothesis that hypercalcemia may have induced cytotoxic changes in the basal ganglia of this patient.

Pathological overlap in cases of parkinsonism associated with neurofibrillary tangles. A study of recent cases of postencephalitic parkinsonism and comparison with progressive supranuclear palsy and Guamanian parkinsonism-dementia complex.

In recent years a number of patients suffering from long-standing postencephalitic parkinsonism have donated their brains to the United Kingdom Parkinson's Disease Society Brain Bank, in London. In view of the paucity of detailed neuropathological reports of the disease since the 1940s, we have carried out a clinicopathological study of eight recent cases. A spectrum of pathological change was seen, with highly variable involvement of cortical, subcortical and brainstem structures. There was no correlation between severity of disease and severity of pathology. The anatomical distribution of le

Anti-tau-positive glial fibrillary tangles in the brain of postencephalitic parkinsonism of Economo type.

Tau-immunoreactive astrocytes have been reported in the brain of progressive supranuclear palsy (PSP) and are referred to as glial fibrillary tangles (GFTs). We found a number of GFTs in the heavily degenerated brain region in four cases of postencephalitic parkinsonism of Economo (PPE) with a clinical history of over a half-century. GFTs had the appearance of tufts of spider-like radiating fibers or small thorn-like feature by Gallyas-Braak method and also by anti-tau immunostaining.

Juvenile parkinsonism caused by chronic meningoencephalitis: a clinicopathological study.

After a six-year history of poor concentration, intellectual and growth retardation, and a one-year history of absences, an 11-year-old girl developed parkinsonism and thereafter had a progressive, predominantly extrapyramidal disorder for the next nine years, until her death at age 20. Other clinical features included pyramidal signs, peripheral neuropathy and sideroblastic anemia. Despite a clinical course suggestive of a degenerative condition, the principal findings on examination of the brain were chronic meningitis, brainstem encephalitis and mild diffuse cerebral gliosis. Extensive inve

Circulating antibody to Nocardia in the serum of patients with Parkinson's disease.

Nocardia is a genus of aerobic Gram-positive bacteria which forms filamentous cells that fragmented into rod-shaped or coccoid elements or L-forms. According to the literature N. asteroides causes encephalitis with parkinsonian features, and it may be an unknown etiologic agent causing encephalitis with a parkinsonian syndrome. Because there are no reliable immunologic tests routinely used for diagnosing nocardial infection, less severe CNS infections with nocardiae may not be recognized or may be attributed to unknown or incorrect etiologic agents. Recently, it was shown that sublethal doses

Asymmetrical FDG-PET and MRI findings of striatonigral system in multiple system atrophy with hemiparkinsonism.

The asymmetry of the local cerebral metabolic rate for glucose (lCMRglc) and magnetic resonance (MR) imaging was studied in five patients with multiple system atrophy (MSA) presenting hemiparkinsonism. lCMRglc was measured with 18F-2-fluoro-2-deoxy-D-glucose and positron emission tomography (PET) in these patients and five normal control subjects. MR images were obtained in five patients and 11 control subjects. In all patients, T2-weighted MR images showed hypointense areas in the posterior lateral putamen. They were bigger or lower in intensity on the contralateral side than on the ipsilater

Induction of D2 dopamine receptor mRNA synthesis in a 6-hydroxydopamine parkinsonian rat model.

By injecting 6-hydroxydopamine into the pars compacta of the substantia nigra, we produced a hemiparkinsonian rat model in which there is almost complete destruction of the dopaminergic nigrostriatal pathway but sparing of the dopaminergic mesolimbic system. The lesion has been characterized by several criteria: a rotational behavior in response to apomorphine, a complete loss of tyrosine hydroxylase immunoreactivity in the lesioned substantia nigra, a near total depletion of dopamine and metabolites in the striatum ipsilateral to the lesion, and a supersensitivity of the dopamine D2 receptors

Differential vulnerability of primate caudate-putamen and striosome-matrix dopamine systems to the neurotoxic effects of 1-methyl-4-phenyl-1,2,3,6-tetrahydropyridine.

The meperidine analogue derivative 1-methyl-4-phenyl-1,2,3,6-tetrahydropyridine (MPTP) induces nigrostriatal fiber damage and severe parkinsonism in humans and animals. MPTP-induced parkinsonism has been proposed as a model of Parkinson disease, but doubts have been raised about whether the patterns of nigrostriatal fiber loss in the two conditions are similar. We report here observations on [3H]mazindol monoamine (principally dopamine) uptake-site binding in the striatum of monkeys (Saimiri sciureus) exposed to low doses of MPTP. We show that this treatment can produce a pattern of nigrostria
