Relationships between alcohol use and dementia: protocol for an observational study in the UK Clinical Practice Research Datalink.

INTRODUCTION: Alcohol consumption is an increasingly recognised modifiable risk factor for dementia, yet whether it has differential impacts on dementia subtypes and its role in disease progression remains unclear. This study aims to: (1) quantify the association between alcohol intake and incidence of dementia subtypes and (2) examine whether individuals who drink heavily and develop dementia referred to hereafter as 'alcohol-related'-have poorer post-diagnosis outcomes compared with other dementia cases. Clarifying these relationships will determine whether alcohol selectively increases risk

Post-rehabilitation self-management support on physical activity and nutrition, including mHealth, improves physical capacity, physical activity, and health related quality of life in people with Parkinson's- results from a randomised controlled trial.

BACKGROUND: Maintaining long-term engagement in physical activity (PA) and following nutrition recommendations after rehabilitation is challenging for people with Parkinson’s. Sustained behavioural change requires more than initial education; person-centred, self-management support may be key to maintain health-promoting routines. However, structured follow-up is often lacking. Digital health interventions, including mobile health (mHealth), offer scalable solutions to provide ongoing support beyond rehabilitation. This study aimed to examine the effect of an individualised, mHealth support pr

How Are Synaptic Vesicle Exocytosis and Endocytosis Affected in Parkinson Disease?

Synaptic vesicle exocytosis and endocytosis are 2 fundamental processes that involve the coordinated and timely activity of many proteins and protein-lipid interactions. Alpha-synuclein (α-Syn) regulates several steps of these processes. Impaired exocytosis and endocytosis are important consequences of Parkinson disease (PD), reflecting both loss of function of normal α-Syn and toxic gain of function of abnormal α-Syn aggregates. Studies in familial forms of PD indicate that abnormal functions of other critical proteins such as leucine repeat rich kinase 2 also impair normal synaptic vesicular

Drug-induced mania and parkinsonism in substance use disorder patient: A case report.

This case report investigates mania and parkinsonism induced by olanzapine and disulfiram in a patient with substance use disorder. DSM-5 criteria were used for the diagnosis of drug-induced parkinsonism, and movement disorder severity was assessed with the Simpson-Angus Scale. After initiating olanzapine and disulfiram therapy, the patient developed extrapyramidal symptoms and manic features. Naranjo scale indicated a probable drug reaction (score = 7), while Hartwig Severity Assessment (Level 4) suggested that hospitalization was required and these reactions were nonpreventable. The case und

Hydrogen sulfide protects dopaminergic neurons via NLRP3 inflammasome inhibition in an MPTP mouse model of Parkinson's disease.

BACKGROUND: Hydrogen sulfide (H2S) has emerged as a potential neuroprotective agent in neurodegenerative diseases, including Parkinson's disease (PD). However, its role in regulating neuroinflammation through the NLRP3 inflammasome remains unclear. METHODS: We established a PD mouse model using MPTP (30 mg/kg, i.p. for 5 days) and administered NaHS (an H2S donor, 5.6 mg/kg, i.p.) daily during the same period. Behavioral tests, immunohistochemistry, and Western blotting were used to assess motor function, dopaminergic neuron survival, NLRP3 inflammasome activation, and inflammatory cytokine le

Structural defects in amyloid-β fibrils drive secondary nucleation.

Formation of new amyloid fibrils and oligomers from monomeric protein on the surfaces of existing fibrils is an important driver of many disorders such as Alzheimer's and Parkinson's diseases. The structural basis of this secondary nucleation process, however, is poorly understood. Here, we ask whether secondary nucleation sites are found predominantly at rare growth defects: irregularities in the fibril core structure incorporated during their original assembly. We first demonstrate using the specific inhibitor of secondary nucleation, Brichos, that secondary nucleation sites on Alzheimer's d

Protocol of ASPro-PD: a phase 3 trial of ambroxol to slow progression in genetically stratified Parkinson's disease.

INTRODUCTION: Genetic studies have identified the GBA1 gene as a significant genetic risk factor for Parkinson's disease (PD), with 10-15% of PD patients carrying GBA1 variants. GBA1 variants affect the glucocerebrosidase (GCase) enzyme, often leading to reduced GCase activity and associated altered lysosomal function, implicated in PD pathogenesis. Ambroxol, a small molecule widely used for respiratory diseases, has emerged as a potential therapeutic agent for PD, acting by increasing GCase activity. A phase 2 trial demonstrated ambroxol's safety and efficacy in penetrating cerebrospinal flui

RNAseq Analysis of the Cerebellum Reveals Significant Gene Dysregulation That May Explain Chronic Disease Progression in Mild Traumatic Brain Injury.

Mild traumatic brain injury (mTBI) is a major public health concern worldwide and contributes to chronic, persistent neurodegenerative diseases; however, the mechanisms are not fully understood. Most studies have examined cerebral cortex, hippocampus, and thalamus brain tissue, but the impact on the cerebellum following repetitive mTBI (rmTBI) leading to secondary injury cascades and long-term pathophysiology is largely unexplored. This study investigated changes in gene expression in cerebellum tissue from an established murine model of rmTBI. The cerebellum tissue from 15 male C57BL/6J mice

Montelukast Modulates MPTP-induced Ferroptosis and Neuroinflammation Linked To the GPX4/ACSL4/5-LOX Pathway.

Ferroptosis, an iron-dependent form of regulated cell death, has been increasingly linked to neurodegeneration in Parkinson's disease (PD). The lipid-peroxidizing enzyme 5-lipoxygenase (5-LOX) contributes to ferroptotic stress, while montelukast, a leukotriene receptor antagonist widely used for asthma, indirectly interferes with this pathway. Here, we investigated whether montelukast protects against dopaminergic injury in a mouse model of PD induced by 1-methyl-4-phenyl-1,2,3,6-tetrahydropyridine (MPTP). Mice were evaluated for behavioral deficits and underwent histological and biochemical a

Unveiling the biochemical potential of Acacia jacquemontii as a therapeutic agent in parkinson's disease: A multi-model in Vitro, In Vivo, and In Silico Study.

Parkinson's disease (PD) is a progressive neurodegenerative disorder marked by oxidative stress, inflammation, and the degeneration of dopaminergic neurons. Current treatments focus more on symptom management rather than disease prevention. Acacia jacquemontii, rich in antioxidants, may offer a novel therapeutic approach for PD. This study aims to investigate the phytochemical composition, antioxidant capacity, anti-Parkinsonian efficacy, and in-silico validation of Acacia jacquemontii methanol extract (AJME) using liquid chromatography-mass spectrometry (LC-MS). Secondary metabolites were ide

[Medical evaluation of Parkinson's disease as an occupational disease].

Since 2023, a new occupational disease (OD), "Parkinson's syndrome caused by pesticides", can be recognized by the statutory accident insurance system in Germany. For a patient to be recognized as suffering from this disease, an expert medical assessment must confirm the presence of a "primary Parkinson's syndrome", and Parkinsonism must not be secondary to another underlying condition. In addition, there must be evidence of occupational exposure to pesticides, specifically fungicides, insecticides, or herbicides, on at least 100 working days within one functional group (insecticides, herbicid

Does Metabolic and Bariatric Surgery Reduce the Risk of Developing Parkinson's Disease: A Propensity Score Matching Analysis Using Data from the National Inpatient Sample.

INTRODUCTION: Although metabolically unhealthy obesity is an established independent risk factor for Parkinson’s disease, large-scale epidemiological evidence is lacking on whether metabolic and bariatric surgery (MBS), an obesity treatment, reduces Parkinson’s disease risk in people with obesity. This study aimed to evaluate the impact of MBS on Parkinson’s disease prevalence in people with obesity. METHODS: This multicenter, cross-sectional study utilized data from the 2018–2019 National Inpatient Sample (NIS) database. Hospitalized adult patients with obesity were stratified into MBS and no

The 201 Trial: a placebo-controlled randomized phase 2 study of safety and tolerance of the c-Abl kinase inhibitor risvodetinib in untreated Parkinson's disease.

The nonreceptor Abelson (Abl) tyrosine kinases have been implicated as key drivers of initiation and progression in Parkinson's disease (PD). Risvodetinib, a potent, brain-penetrant, selective inhibitor of the nonreceptor Abl kinases c-Abl1 and c-Abl2/Arg (collectively, c-Abl), was evaluated in a randomized, double-blind, placebo-controlled phase 2a trial ('the 201 Trial') using once-daily 50 mg, 100 mg, 200 mg or placebo in 137 participants with early, untreated PD. The primary end points in safety and tolerability were met, with 95% of enrolled participants completing the 12-week dosing regi

Association of Statin use with Parkinson's Disease Progression in a Prospective Cohort Study and Multi-omics Analyses.

BACKGROUND: While more than 30% of patients with Parkinson’s disease (PD) are prescribed statins, the impact of statin use on the progression of PD remains incompletely elucidated. We aimed to comprehensively investigate the impact of statin use on PD progression. We analyzed longitudinal data from the Parkinson’s Progression Markers Initiative (PPMI) to examine associations between statin use and clinical manifestations and cerebrospinal fluid (CSF) biomarkers. Mendelian randomization and Bayesian colocalization analyses were employed to assess genetic relationships between HMGCR inhibition (

Targeting the Microbiota-Gut-Brain Axis: Bacillus coagulans Protects Against Rotenone-Induced Parkinson's Disease in Rats.

Gut microbiota and its derived metabolites affect brain physiology through several pathways. Dysfunction of gut-microbiota is involved in the pathogenesis of Parkinson's disease (PD). Introduced the pleiotropic effect of probiotics (PBT) in the function of the central nervous system, can delay the disease progression through microbiota-gut-brain axis (MGBA). PD is characterized by aggregated alpha-synuclein (α-syn), oxidative stress and neuroinflammation leading to depletion of dopaminergic neurons in the midbrain region. Our study designed to assess the neuroprotective effect of PBT Bacillus

Longitudinal Evaluation of Polyneuropathy in Atypical Parkinsonian Syndromes.

Background: In Parkinson's disease (PD), a higher prevalence of polyneuropathy (PNP) is increasingly recognized, although the causal association is still under debate. In contrast, PNP in atypical parkinsonian syndromes (APS) has been insufficiently addressed, despite preliminary evidence suggesting elevated prevalence. Methods: Nerve conduction studies were performed on 13 patients with multiple system atrophy (MSA) and 9 patients with progressive supranuclear palsy (PSP) at baseline. Botulinum Toxin Injections for Drooling Improve Dysphagia in Patients with Parkinson's Disease.

Drooling and dysphagia are frequent and disabling complications in Parkinson's disease (PD) and often coexist, with drooling mainly resulting from impaired saliva clearance due to reduced oral motor control and potentially worsening swallowing function. This study aimed to evaluate whether botulinum toxin type A (BoNT/A) injections into the major salivary glands, beyond controlling drooling, could also improve swallowing performance using clinical and neurophysiological measures. Twenty PD patients with severe drooling and dysphagia underwent bilateral ultrasound-guided BoNT/A injections into
