Differentiating early Parkinson's disease from multiple system atrophy on multimodal PET/MRI using atlas-guided segmentation and vision transformer classification.

INTRODUCTION: Early differentiation between Parkinson's disease (PD) and multiple system atrophy-parkinsonian type (MSA-P) remains clinically challenging because of overlapping motor symptoms and subtle imaging abnormalities. METHODS: We developed a fully automated multimodal PET/MRI framework for early PD-MSA-P differentiation that integrates atlas-guided subject-specific label generation, nnU-Net segmentation of the caudate nuclei and putamina, multimodal image fusion, and Vision Transformer classification with multi-slice voting (ViT+MSV). A total of 155 early-stage patients, including 121

Increased CCR2-immunopositive myeloid cells in the putamen and cerebellar white matter with blood-brain barrier disruption in multiple system atrophy.

Multiple system atrophy (MSA) is a progressive neurodegenerative disease characterized by parkinsonism, cerebellar ataxia, and autonomic dysfunction. Recently, a close relationship of neuroinflammation to MSA pathogenesis was identified. Studies in mouse models of MSA have indicated that peripheral-type myeloid cells and endogenous microglia promote neuroinflammation in MSA. However, their distinct roles in MSA pathogenesis remain unknown. We performed immunohistochemical analyses of brain tissues obtained at autopsy from patients with MSA and age-matched controls to examine the putamen and ce

A case report of spinocerebellar ataxia with TRPC3 gene mutation and review of literature.

Spinocerebellar ataxia type 41 (SCA41) is a rare autosomal dominant cerebellar ataxia caused by mutations in the transient receptor potential canonical 3 (TRPC3) gene. We report a case of a patient with SCA41 whose clinical manifestations were initially suspected to be multiple system atrophy cerebellar-type (MSA-C). Whole-exome sequencing (WES) revealed a c.1955A>G (p.K652R) mutation in the TRPC3 gene of this patient. After treatment with transcranial magnetic stimulation and rehabilitation training, the patient reported a slight improvement in unsteady gait compared with before. This is the

Therapeutic suppression of Tubb4a rescues H-ABC leukodystrophy.

Hypomyelination and atrophy of basal ganglia and cerebellum (H-ABC) is a rare leukodystrophy associated with causal variants in β-tubulin 4A (TUBB4A). The recurring variant p.Asp249Asn (D249N) presents in infancy with dystonia, communication deficits, and loss of ambulation during the first decade of life. In this study, we characterized a genetic murine series (Tubb4aKO/KO, Tubb4aD249N/+, Tubb4aD249N/KO, and Tubb4aD249N/D249N) to demonstrate that disease severity correlates with the expression of mutant Tubb4a and relative preservation of wild-type tubulin. Long-term efficacy and disease-specific responsiveness to protirelin in patients with spinocerebellar degeneration: A retrospective study.

INTRODUCTION: The thyrotropin-releasing hormone (TRH) analog protirelin has short-term benefits for cerebellar ataxia (CA), but its long-term efficacy and the subtype-specific responsiveness of spinocerebellar degeneration (SCD) patients remain unclear. METHODS: We retrospectively reviewed the records of 92 SCD patients (273 courses) treated with protirelin between July 2011 and October 2025, including patients with idiopathic CA (IDCA), multiple system atrophy with predominant CA (MSA-C) or parkinsonism (MSA-P), and genetic subtypes including spinocerebellar ataxia (SCA) type 6, SCA31 and de

Diagnostic Clues and Pitfalls in Pontocerebellar Hypoplasia Type 2A.

BACKGROUND: Pontocerebellar hypoplasia type 2A (PCH2A) is a rare autosomal recessive neurodegenerative disease caused by a specific pathogenic variant in the TSEN54 gene (p.A307S). Affected children show early but initially unspecific symptoms, diagnosed primarily through postnatal magnetic resonance imaging (MRI), with confirmation by genetic testing. This study examines the diagnostic process and key considerations for accurate diagnosis. METHODS: We retrospectively collected data from 65 children (33 girls, 32 boys) with genetically confirmed PCH2A as part of a Natural History Study. Data

Safety and Efficacy of Mesenchymal Stem Cell Therapy in Multiple System Atrophy: Systematic Review.

OBJECTIVE: To systematically evaluate the efficacy and safety of mesenchymal stem cell (MSC) therapy for patients with Multiple System Atrophy (MSA) by synthesising available clinical trial evidence and clarifying signals of disease modification. BACKGROUND: MSA is a rapidly progressive and fatal neurodegenerative disorder for which no disease-modifying therapies exist. MSC therapy has emerged as a potential treatment, with mechanisms centered on neuroprotection and clinical benefit through anti-inflammatory and trophic effects rather than direct cell replacement. METHODS: We systematically

Functional Characterization of a Novel Splice-Altering Intronic Variant in AMPD2 Associated with Pontocerebellar Hypoplasia Type 9.

BACKGROUND: A two-year-old female patient was referred to our clinic due to speech and gait disturbances, strabismus, vacant staring, truncal hypotonia, and spasticity in the extremities. The patient had a history of a complicated delivery resulting in perinatal asphyxia. Electrocardiogram (ECG), echocardiogram (ECHO) and abdominal ultrasound findings reported no abnormalities. Previously performed spinal muscular atrophy (SMA) test, chromosomal microarray and karyotype analyses yielded normal results. On clinical examination, facial dysmorphic features included: prominently low-set ears, stra

Functional recovery strategies in progressive supranuclear palsy with cerebellar predominance.

This report details a case of a patient in his 60s who exhibited ataxic gait, frequent falls, limb incoordination, tremors and rigidity, leading to a diagnosis of progressive supranuclear palsy with cerebellar predominance (PSP-C). Clinical assessment indicated slurred speech, increased muscle tone, deep tendon reflexes that were mildly reduced asymmetrically on the left side and reduced strength, while sensations remained intact. PSP-C is an uncommon variant of PSP often misidentified as multiple system atrophy-cerebellar type (MSA-C) because of shared characteristics. The patient participate

Glymphatic dysfunction in cerebellar neurodegeneration: mechanistic insights and emerging clinical implications.

BACKGROUND AND PURPOSE: The glymphatic system is a glial-dependent perivascular clearance pathway that maintains central nervous system homeostasis through cerebrospinal fluid and interstitial fluid exchange. Emerging evidence suggests that glymphatic dysfunction contributes to the pathophysiology of neurodegenerative disorders involving cerebellar networks. This review summarizes current evidence linking glymphatic dysfunction to cerebellar neurodegeneration, with a particular focus on spinocerebellar ataxia type 3, multiple system atrophy, progressive supranuclear palsy, and essential tremor

Transcranial alternating current stimulation for treatment of multiple system atrophy cerebellar type: A randomized controlled trial.

BACKGROUND: No disease-modifying treatments exist for the cerebellar subtype of multiple system atrophy (MSA-C). Because transcranial alternating current stimulation (tACS) has demonstrated safety and efficacy in spinocerebellar ataxia type 3, a cerebellar disorder, we tested whether tACS improves clinical outcomes in MSA-C. METHODS: We conducted a randomized, double-blind, placebo-controlled trial of tACS in MSA-C. Participants received daily 40-min sessions of 70 Hz, 2 mA tACS or sham stimulation, 5 days/week for 2 weeks. The primary outcome was the change from baseline to 2 weeks in the Un

Pearls & Oy-sters: Reversible Leukoencephalopathy and Parkinsonism Due to CNS Involvement in Cryoglobulinemia.

We present a case of reversible leukoencephalopathy and parkinsonism associated with type III mixed cryoglobulinemic vasculitis in a patient who initially presented with systemic vasculitic symptoms such as rash and arthralgias. Neither hepatitis C nor monoclonal gammopathy-typical triggers of cryoglobulinemia-was identified. The neurologic syndrome evolved with tremor, rigidity, significant cognitive decline, and extensive white matter abnormalities on MRI, mimicking primary neurodegenerative conditions such as multiple system atrophy. Diagnostic workup ruled out infectious, metabolic, autoim

Clinically advanced NLRP3 inhibitor modulates microglial transcriptome and alleviates α-synuclein-induced progression of parkinsonism.

BACKGROUND: Parkinson’s disease (PD), the second most common neurodegenerative disorder after Alzheimer’s disease, and the rare disorder multiple system atrophy (MSA), are both characterized by intracellular accumulation of α-synuclein fibrils and early, sustained microglial reactivity in parallel to the neurodegeneration. Activation of the NLRP3 inflammasome in disease-associated reactive microglia is increasingly recognized as a key pathogenic driver and a promising therapeutic target in synucleinopathies. Dapansutrile (OLT1177®) is a selective, orally bioavailable NLRP3 inhibitor with a fav

Cerebrospinal fluid HOMER1 and NPTX2 as candidate signatures in early-stage multiple system atrophy-parkinsonism.

Different morphological and pathological features of abnormal α-synuclein in synucleinopathies-multiple system atrophy (MSA) and Lewy body diseases, including Parkinson's disease (PD) and dementia with Lewy bodies (DLB)-may underlie disease-specific synaptic dysfunction. To compare synaptic profiles associated with abnormal α-synuclein, we examined temporal lobe tissues from patients with MSA (N = 4), DLB (N = 5), and controls (N = 9). We further analysed cerebrospinal fluid (CSF) samples from patients with early-stage MSA-parkinsonism (MSA-P: N = 8) and PD (N = 8) to determine whether synapti

Comparative transcriptomics reveal molecular convergence and divergence in parkinsonian disorders.

Neurodegenerative parkinsonisms are phenotypically diverse disorders including Parkinson's disease, dementia with Lewy bodies, multiple system atrophy, progressive supranuclear palsy, and corticobasal degeneration. These currently incurable diseases are characterized by overlapping but also distinct clinicopathological and molecular features, and largely unknown pathogenesis. Transcriptomic studies have offered insights, but are limited by small sample sizes, technical variability, and limited reproducibility. Here, we present a large comparative bulk transcriptomic analysis of parkinsonian sy
