At-home wearables and machine learning capture motor impairment and progression in adult ataxias.

A significant barrier to developing disease-modifying therapies for spinocerebellar ataxias (SCAs) and multiple system atrophy of the cerebellar type (MSA-C) is the scarcity of tools to measure disease progression sensitively in clinical trials. Wearable sensors worn continuously during natural behaviour at home have the potential to produce ecologically valid and precise measures of motor function by leveraging frequent and numerous high-resolution samples of behaviour. Here we test whether movement building block characteristics (i.e. submovements), obtained from the wrist and ankle during n

Oligodendrocyte-specific overexpression of human alpha-synuclein results in elevated MBP levels and inflammatory responses in TgM83 mice, mimicking the pathological features of multiple system atrophy.

Multiple system atrophy (MSA) is a neurodegenerative disorder characterized by parkinsonism, cerebellar dysfunction, and autonomic failure. Key pathological features of MSA include the formation of glial cytoplasmic inclusions (GCIs) in oligodendrocytes (OLs), myelin loss, and neuroinflammation. Although both inflammation and myelination are known to be critical in MSA, the roles of myelin proteins and their relationship with inflammation have often been overlooked. In this study, we injected AAV-Olig001 vectors carrying either human SNCA (AAV-hSNCA) or eGFP (AAV-eGFP) into the striatum of TgM

Multiple System Atrophy (Cerebellar Type) With Overlapping Progressive Muscular Atrophy Features and Genetic Erb-B2 Receptor Tyrosine Kinase 4 (ERBB4) Amyotrophic Lateral Sclerosis Variant: A Case Report.

Multiple system atrophy (MSA) is a progressive disease with Parkinsonism, dysautonomia, and cerebellar symptoms wherein patients can present with a broad range of confusing and overlapping findings attributable to various neuroanatomical substrates. Although possible, weakness is an unusual primary complaint, warranting further work-up for another neurodegenerative disease. The involvement of the more central structures, such as the locus coeruleus, pontine micturition center, and the cerebellum, can explain the wide range of symptoms. While Onuf's nucleus contributes to the urinary symptoms,

Correlation Networks To Uncover Changes in Protein Relationships in Spinocerebellar Ataxia Type 2 and Cerebellar Multiple System Atrophy.

Rare neurological diseases (RNDs) are complex diseases characterized by significant heterogeneity in genetic, molecular, and pathological characteristics, which make them poorly understood and still challenging to diagnose and treat. Therefore, identifying methods that help improve diagnosis and discrimination efficiency is crucial. In this context, patients presenting overlapping phenotypes and different etiologies may offer the opportunity to study disease-specific pathways and differential characteristics in fluid biomarkers. In this study, a label-free proteomic analysis was set up to capt

Biallelic TSEN2 variants causing pontocerebellar hypoplasia type 2.

Pontocerebellar hypoplasia type 2 (PCH2) is an autosomal recessive neurodegenerative disorder caused by biallelic pathogenic variants in tRNA splicing endonuclease (TSEN) subunit genes. Variants in TSEN54 are most common, with very few cases of TSEN2-related PCH2B reported to date. Here, we report a 7-year-old girl with typical PCH2 features, including progressive microcephaly, epilepsy, developmental delay, cerebellar atrophy, and dystonia. Exome sequencing revealed compound heterozygous TSEN2 variants, a known missense variant NM_025265.4:c.926A>G p.(Tyr309Cys) and a novel nonsense variant c

Facilitated α-synuclein oligomer sharing among glial cells by a centrally acting connexin inhibitor attenuates a rapidly progressive multiple system atrophy-cerebellar type model by reducing the neuronal α-synuclein burden.

Glial connexins (Cxs) that make up astrocyte/oligodendrocyte gap junctions are extensively altered in multiple system atrophy-cerebellar type (MSA-C). Here, we investigated how Cx alterations affect the propagation of α-synuclein (α-syn) oligomers and phosphorylated (p)-α-syn aggregates in MSA-C using a centrally acting pan-Cx blocker, INI-0602. Our Plp1-tTA::tetO-SNCA*A53T transgenic (Tg) mice express mutant human A53T α-syn in oligodendrocytes after dietary doxycycline withdrawal at 8 weeks of age; they typically develop progressive ataxia around 22 weeks and die by 30 weeks. These Tg mice w

Acetyl-DL-leucine (Tanganil™) in three patients with advanced multiple system atrophy.

BACKGROUND: Multiple system atrophy (MSA) is a rare, rapidly progressive alpha-synucleinopathy characterized by autonomic dysfunction, parkinsonism and marked cerebellar ataxia, with very limited treatment options. N-acetyl-DL-leucine (ADLL) (Tanganil™), a racemic derivative of leucine is usually well tolerated and has shown promise in improving cerebellar symptoms and reducing REM sleep behavior disorder (RBD), a common non-motor dream-sleep disorder feature of alpha-synucleinopathy. Given this dual potential, we treated three patients with advanced-stage MSA under compassionate use rules. C

A unique microglia subset associated with aggressive α-synucleinopathy uncovered in a rapidly progressive multiple system atrophy cerebellar type model.

Multiple system atrophy (MSA) is a progressive and fatal α-synucleinopathy characterized by α-synuclein-positive (α-syn+) glial cytoplasmic inclusions in oligodendrocytes. The cerebellar variant (MSA-C) primarily affects olivopontocerebellar fibers, resulting in extensive demyelination and glial activation. To model this pathology, we developed a Tet-Off-based MSA-C mouse model with oligodendrocyte-specific overexpression of human A53T α-syn. Upon doxycycline withdrawal at 8 weeks of age, mice developed progressive cerebellar ataxia by 26 weeks and succumbed by 30 weeks. These mice exhibited s

Clinical and neuroimaging correlates of disease related gait patterns in patients with multiple system atrophy cerebellar type.

Multiple system atrophy-cerebellar type (MSA-C) is a rapidly progressive neurodegenerative disorder, yet objective digital biomarkers for disease severity remain scarce. This cross-sectional study aimed to identify disease-relevant gait patterns using a 2D video-based gait analysis algorithm and examine their clinical and neuroimaging correlates. Gait features were extracted from videos of patients with MSA-C using Gaitome, and an MSA-C gait pattern score was derived. This score significantly distinguished MSA-C from healthy controls (area under the curve = 0.98) and showed significant correla

Interpretable machine learning for differentiating SCA3 and MSA-C using gait and postural features from wearable sensors.

BACKGROUND: Spinocerebellar ataxia type 3 (SCA3) and the cerebellar subtype of multiple system atrophy (MSA-C) both manifest with cerebellar ataxia and postural instability in the early stages, leading to considerable clinical overlap and frequent misdiagnosis or delayed intervention. Current diagnostic tools offer limited support when genetic confirmation is unavailable or neuroimaging findings are inconclusive. Clinical rating scales, while commonly used, rely heavily on subjective judgment and lack sensitivity to dynamic monitoring. Therefore, this study aims to develop an interpretable mac

Neurocognition, cerebellar functions and psychiatric features in spinocerebellar ataxia type 34: a case series.

OBJECTIVE: This study primarily aimed to comprehensively characterize the neurological, neuroradiological and neurocognitive profiles, as well psychiatric features of individuals with Spinocerebellar Ataxia Type 34 (SCA34) associated with pathogenic variants in the ELOVL4 gene. Secondarily, we investigated the relationship between neurocognitive functions and cerebellar morphology in individuals with SCA34 by correlating structural changes to cognitive performance. Given involvement of the cerebellum in SCA34, our findings will contribute to a broader understanding of the role of the cerebellu

Tri-modal assessment reveals early visual pathway degeneration in patients with MSA-C.

BACKGROUND AND OBJECTIVES: Post-mortem evidence suggests neurodegeneration in the visual pathway in multiple system atrophy-cerebellar type (MSA-C), yet robust in vivo evidence remains scarce. This study aimed to characterize these visual pathway changes in MSA-C patients by integrating optical coherence tomography (OCT), visual evoked potential (VEP), and magnetic resonance imaging (MRI). METHODS: This cross-sectional study prospectively recruited 156 participants, including 53 healthy controls and 103 early-stage MSA-C patients (mean disease duration: approx. 2 years). The Value of Quantitative Susceptibility Mapping and Morphometry in the Differential Diagnosis of Parkinsonism.

BACKGROUND AND PURPOSE: Differentiating Parkinson disease (PD) from atypical parkinsonism syndrome (APS), including multiple system atrophy (MSA) and progressive supranuclear palsy (PSP), is challenging, and there is no reference standard. Integrating quantitative susceptibility mapping (QSM) and morphometry can help differentiate PD from APS and improve the internal diagnosis of APS. MATERIALS AND METHODS: In this retrospective study, we enrolled 55 patients with PD, 17 with MSA-parkinsonian type (MSA-P), 15 with MSA-cerebellar type (MSA-C), and 14 with PSP. Thirty-three age-matched healthy

Pathomechanisms of neuropsychiatric disturbances in atypical parkinsonian disorders: a current view.

Multiple system atrophy (MSA), corticobasal degeneration (CBD) and progressive supranuclear palsy (PSP) are the most common atypical parkinsonisms. These adult-onset and lethal neurodegenerative disorders of unknown etiology are clinically characterized by varying combinations of autonomic, levodopa-poorly responsive parkinsonsm, motor, non-motor, cerebellar syndromes, behavioral, cognitive and other neuropsychiatric disorders. Although their pathological hallmarks are different-MSA α-synucleinopathy, CBD and PSP 4-repeat (4R) tauopathies-their neuropsychiatric disturbances include anxiety, de

Motor-Cognitive Dual-Task Cost and Associated Micro Lesions of Cerebellum and Brainstem in Multiple System Atrophy (Parkinsonian Type).

In multiple system atrophy with parkinsonian type (MSA-P), the dual-task cost and the underlying neurological mechanisms remain under-researched. We included 20 early-stage MSA-P patients and 10 matched healthy controls (HC). Using a video-based gait analysis machine, we explored gait characteristics under three conditions: single-task gait (STG), dual-task gait with backward counting (DTG-BC), and dual-task gait with spontaneous animal naming (DTG-SAN). Neuroimaging scans were collected to analyze the gray matter and white matter structures related to the dual-task cost in MSA-P. Our neuroima
