18 F-FDG PET/MRI in Multiple System Atrophy-Parkinsonian Type (MSA-P).

A 64-year-old woman presented with a progressive movement disorder for 4 years, characterized by freezing of gait and left lower extremity discoordination. DaTScan performed 1 year after the presentation was abnormal and consistent with Parkinsonian syndrome. The patient went on to suffer from orthostatic symptomatology resulting in loss of consciousness. She also suffered from urinary incontinence and nocturia. The patient then underwent 18 F-FDG brain PET/MRI, which demonstrated markedly decreased tracer uptake in the bilateral putamina, with accompanying putaminal atrophy, marginal T2-hyper

Association between adiponectin and lipids in Parkinson's disease.

OBJECTIVES: There is increasing evidence linking Parkinson's disease (PD) to lipids, such as the presence of lipids in the core of Lewy bodies in PD brains or high-molecular-weight adiponectin (APN) in phosphorylated α-synuclein-positive Lewy bodies. This study aimed to verify whether APN levels are associated with neurodegenerative diseases. The association between APN and body weight was also investigated. MATERIALS AND METHODS: The following parameters were measured using venous blood sampling: HDL-C, LDL-C, glucose, and lipids, including APN. RESULTS: PD patients receiving dopaminergic t

Regional homogeneity differences in resting-state fMRI between Parkinson's disease and multiple system atrophy-parkinsonian type.

Both Parkinson's disease (PD) and the Multiple system atrophy-parkinsonian type (MSA-P) belong to the category of α-synucleinopathies, characterized by overlapping clinical manifestations and a high risk of misdiagnosis. The aim of this study is to provide objective imaging biomarkers for differentiating PD from MSA-P. We enrolled 25 patients with PD, 15 patients with MSA-P, and 20 healthy controls (HC). Resting-state functional magnetic resonance imaging (rs-fMRI) was employed to analyze regional homogeneity (ReHo) differences between PD and MSA-P groups. The discriminative capacity of ReHo v

Distinct AQP4 Alterations in Movement Disorders with Primary Synucleinopathy.

BACKGROUND: Aquaporin-4 (AQP4) is involved in clearing amyloidogenic proteins, but it remains unexplored how it is comparatively altered in neuron- and oligodendrocyte-predominant synucleinopathies. OBJECTIVE: The aim was to assess AQP4 protein localization and abundance in Parkinson's disease (PD) and multiple system atrophy (MSA). METHODS: The motor cortex and subcortical white matter of PD (n = 29), MSA (n = 19), and controls (n = 17) were immunohistochemically analyzed. RESULTS: In normal aging, neuritic plaques caused an increase in AQP4 abundance without altering polarization. Arterio

Comparison of Two α-Synuclein Seed Amplification Assays for Discrimination of Parkinson Disease and Atypical Parkinsonism.

BACKGROUND: Seed amplification assays (SAAs) for misfolded α-synuclein (syn) have shown inconsistent results in multiple system atrophy (MSA). OBJECTIVE: The objective of this study was to compare a novel syn SAA (synSAA) that distinguishes between Lewy body disease (LBD) and MSA syn-seeds (Amprion-SAA) with an LBD-specific synSAA (IRCCS Istituto delle Scienze Neurologiche di Bologna [ISNB]-SAA). METHODS: We applied both assays to cerebrospinal fluid samples from 114 patients with MSA, 49 patients with Parkinson disease (PD), 40 patients with progressive supranuclear palsy (PSP), and 46 cont

Prevalence of NOTCH2NLC and FMR1 Repeat Expansions in Atypical Parkinsonism Compared to Asymptomatic Elderly Individuals.

BACKGROUND: Repeat expansions in NOTCH2NLC and FMR1 share clinical features, including parkinsonism and ataxia, resembling atypical parkinsonian syndromes. We analyzed these expansions in atypical parkinsonism patients without corticomedullary junction hyperintensity on diffusion-weighted imaging, comparing them to asymptomatic elderly individuals. METHODS: We analyzed two cohorts: (1) 252 patients with atypical parkinsonism, including 165 with multiple system atrophy (MSA), 58 with progressive supranuclear palsy (PSP), and 29 with corticobasal syndrome, analyzed by repeat-primed polymerase c

Homer-3 antibody in autoimmune cerebellar syndromes: a potentially treatable mimic of MSA-C - A review.

Autoimmune cerebellar ataxia (ACA) associated with anti-Homer-3 antibodies is a rare but increasingly recognized immune-mediated neurological condition. It represents a potentially treatable cause of sporadic cerebellar syndrome and may clinically mimic primarily multiple system atrophy of the cerebellar type (MSA-C), and less frequently, other atypical parkinsonian disorders. Because of the significant clinical overlap with neurodegenerative diseases, particularly MSA-C, Homer-3-associated ACA may be underdiagnosed or misdiagnosed, delaying effective treatment. This narrative review synthesiz

[18F]THK5351 uptake in multiple system atrophy compared with other parkinsonian disorders.

BACKGROUND: The pathological processes in the early stages of multiple system atrophy (MSA) are still incompletely understood. Moreover, early-stage MSA is difficult to diagnose. OBJECTIVES: We investigated the monoamine oxidase-B positron emission tomography findings with an aim to characterize degeneration in the early stages of MSA. METHODS: Positron emission tomography using [18F]THK5351 was performed on three patients with relatively early stages of MSA: two with cerebellar-type MSA and one with parkinsonian-type MSA. The findings were compared with nine patients with other parkinsonian

Imaging of Proteinopathies in the Brains of Parkinsonian Disorders.

Neurodegenerative diseases such as Alzheimer's disease (AD), frontotemporal lobar degeneration (FTLD), and α-synucleinopathies-including Parkinson's disease (PD), dementia with Lewy bodies (DLB), and multiple system atrophy (MSA)-are characterized by the accumulation of misfolded protein aggregates. Advances in positron emission tomography (PET) imaging have enabled in vivo visualization of these pathologies, particularly tau and α-synuclein fibrils, facilitating early diagnosis and differential classification. Tau PET tracers such as 18F-florzolotau have demonstrated robust imaging of both AD

Rapidly Progressive Multiple System Atrophy-Cerebellar Type in a 52-Year-Old Woman: Diagnostic Challenges in a Resource-Limited Setting.

Multiple system atrophy (MSA) is a progressive adult-onset neurodegenerative disorder that manifests as a variable combination of cerebellar, parkinsonian, and autonomic dysfunctions. We report a case of a 52-year-old woman who presented with rapidly progressive gait ataxia, dysarthria, orthostatic hypotension, and urinary urgency. MRI of the brain demonstrated pontocerebellar atrophy with the characteristic "hot cross bun" sign, supporting a diagnosis of Multiple System Atrophy-cerebellar type (MSA-C). Despite receiving levodopa and supportive therapy, the symptoms progressed over 8 months, h

Vascular parkinsonism: an update.

Vascular parkinsonism (VP), resulting from cerebrovascular disease, is a rare disorder with a characteristic motor and non-motor clinical profile distinct from sporadic/idiopathic Parkinson disease (PD) and other parkinsonian disorders. It accounts for 3-6% of all parkinsonian syndromes and may overlap with other parkinsonisms. Clinical features of VP are heterogenous and include bilateral rigidity with lower body predominance, bradykinesia, postural instability, shuffling gait, falls, corticospinal symptoms and cognitive impairment, tremor being rare or absent. Predictive clinical factors for the progression from idiopathic late-onset cerebellar ataxia to multiple system atrophy cerebellar type.

OBJECTIVES: Patients initially diagnosed with idiopathic late-onset cerebellar ataxia (ILOCA) may develop multiple system atrophy cerebellar type (MSA-C). However, data on conversion time, associated clinical factors, and the role of the neurofilament light chain (NFL) in this process remain limited. This study aims to investigate the conversion from ILOCA to MSA-C and examine the associated factors and the predictive value of NFL. METHODS: This retrospective study included patients with ILOCA at the initial visit, recording the conversion to MSA-C and its duration. The median time to convers

Progress and challenges in sporadic late-onset cerebellar ataxias.

Sporadic late-onset cerebellar ataxia (SLOCA) is a syndrome defined by subacute or chronic and progressive ataxia occurring after the age of 40 years in individuals without a family history of ataxia. The 2022 publication of revised consensus diagnostic criteria for multiple system atrophy and the emergence of promising biomarkers provides a thorough diagnostic framework that now enables the diagnosis of numerous acquired causes of SLOCA, including autoimmune disorders and neurodegenerative diseases. The ongoing development and increased availability of DNA sequencing technology have uncovered

Cerebellar Volumetry in Ataxias: Relation to Ataxia Severity and Duration.

Cerebellar atrophy is the neuropathological hallmark of most ataxias. Hence, quantifying the volume of the cerebellar grey and white matter is of great interest. In this study, we aim to identify volume differences in the cerebellum between spinocerebellar ataxia type 1 (SCA1), SCA3 and SCA6 as well as multiple system atrophy of cerebellar type (MSA-C). Our cross-sectional data set comprised mutation carriers of SCA1 (N=12), SCA3 (N=62), SCA6 (N=14), as well as MSA-C patients (N=16). Cerebellar volumes were obtained from T1-weighted magnetic resonance images. To compare the different atrophy p
