[Precocious Parkinson's disease associated with "eye-of-the-tiger" type pallidal lesions].

We report the case of a 56-years-old woman patient, born to unrelated parents, who since 26-years-old gradually developed bradykinesia, rigidity, tremor of both hands, and speech and gait difficulties. Her past history was unremarkable. There was no family history of neurologic disease. She was admitted to our Hospital at age 39 and at that time she presented a full parkinsonian syndrome. The following tests were normal or negative: routine blood studies, serum copper, ceruloplasmin and cerebrospinal fluid examination. There was not Kayser-Fleicher ring, and fundoscopic examination revealed no

Myopathic involvement in two cases of Hallervorden-Spatz disease.

Muscle biopsy was performed in two patients with Hallervorden-Spatz disease and increased serum creatine kinase levels. Morphological analysis showed myopathic signs such as subsarcolemmal accumulation of myeloid structures, dense bodies and debris, endomysial macrophage activation, focal necrosis and fiber splitting. We emphasize the finding of muscle involvement in Hallervorden-Spatz disease, like in other forms of neuroacanthocytosis.

In vivo diagnosis of Hallervorden-Spatz disease.

The authors present the MRI findings of two children with insidious walking difficulties, signs of corticospinal tract involvement, and signs and symptoms of extrapyramidal dysfunction such as rigidity and generalized dystonia, the latter with predominance of oromandibular involvement. In one child, MRI revealed prominent hypo-intensity in the globus pallidus and in the substantia nigra on T2-weighted spin echo images, consistent with iron deposition and thus with previous post-mortem findings of Hallervorden-Spatz disease. In the other case, the hypo-intensity was restricted to the globus pal

[A 75-year-old man with parkinsonism and sudden death].

We report a 75-year-old man with parkinsonism who died suddenly. The patient was well until 64 years of the age when he had an onset of tremor in his left hand. He was treated with a medicine in another hospital, and his tremor subsided. Five years after the onset, he started to note difficulty in fine finger movements and gait disturbance. He tended to lean backward with frequent falls. He was treated with bromocriptine, trihexyphenydil, and L-dops without apparent improvement. He visited our out patient clinic on November 11, 1993 when he was 75 years of the age. Neurologic examination at th

The nosology of Hallervorden-spatz disease.

The development of magnetic resonance imaging has increased the number of clinical and pathological reports of Hallervorden-Spatz disease and Hallervorden-Spatz syndrome. The case-to-case variability is considerable. However, if gene loci and basic pathogenetic mechanisms are to be appreciated, it is imperative that like cases be compared and studied. The designation Hallervorden-Spatz disease should be reserved for the pediatric neurodegenerative disorder, recognizing that it occurs either as a familial or a sporadic disorder. The diagnosis of Hallervorden-Spatz syndrome is non-specific and e

Magnetic resonance imaging in progressive supranuclear palsy and other parkinsonian disorders.

High field intensity MRI may demonstrate signal abnormalities consistent with deposits of iron or other paramagnetic substances in several extrapyramidal disorders. Hallervorden-Spatz disease was the only disorder widely known to have iron deposits in the pallidum, that are now easily demonstrated in vivo by MRI. However, lower field intensity MRI may also demonstrate characteristic findings. In progressive supranuclear palsy, definite atrophy of the midbrain and of the region around the third ventricle is seen in slightly more than half of the cases. Minimal signal abnormalities are sometimes

Early clinical and imaging (high-field MRI) diagnosis of Hallervorden-Spatz disease.

In a 10-year-old girl, with progressive motor disturbance since the age of seven years and dominant pyramidal signs, MRI revealed marked overall low signal from the globus pallidus on each side, with central zones of high signal. There was also strikingly low signal in the zona reticularis of the substantia nigra, indicative of Hallervorden-Spatz disease (HSD).

Progressive ataxia in Swedish children: a re-evaluation study.

The results of follow-up of 76 children with progressive ataxia first reported in 1988 are described with special reference to type of potential underlying metabolic diseases. In 70% of cases the clinical follow-up did not lead to reconsideration of diagnoses. Six of 23 biochemically and morphologically re-examined children got a new and definite diagnosis: 1 myoclonic encephalopathy with ragged red fibres, 2 carbohydrate-deficient glycoprotein syndrome, 1 neuroborreliosis, 1 Hallervorden-Spatz disease and 1 leucodystrophy. Different clinical groups are discussed.

Occipital contrecoup in a case of Hallervorden-Spatz disease.

This case report describes the unusual complications of a simple fall in a person with a profound neurological disorder, Hallervorden-Spatz disease. The fall produced a laceration of the forehead, skull fractures and severe occipital contrecoup contusions. Such contrecoup contusions are rare and may be a feature of this case because of the abnormal limb rigidity displayed in Hallervorden-Spatz disease. The mechanisms of injury are discussed.

Non-Alzheimer neurofibrillary tangles show beta-amyloid-like immunoreactivity.

As well as being a neuropathological hallmark of Alzheimer's disease (AD), neurofibrillary tangles (NFT) are present in the brain in a number of other neurodegenerative conditions. beta-Amyloid (beta/A4) plaque formation is a central event in AD, although recent reports indicate that beta/A4, and its precursor protein (beta APP), are also associated with AD NFT. Using an antibody to a beta APP epitope overlapping the beta/A4 region, we demonstrate that immunoreactivity is found in extracellular NFT in Niemann-Pick disease, Hallervorden-Spatz disease, progressive supranuclear palsy and subacute

Hallervorden-Spatz disease: MR and pathologic findings.

PURPOSE: To compare the MR findings of eight cases with clinical diagnosis of Hallervorden-Spatz disease (HSD) with the pathologic findings of two other cases of HSD. MATERIALS AND METHODS: The eight imaged cases were studied with 0.5-T (seven cases) and/or 1.5-T (five cases) units. Six patients also had CT scans. The two other cases with proven HSD had detailed histologic evaluation. RESULTS: The 1.5-T findings showed abnormalities confined to the pallidum, which presented a diffuse low signal intensity in T2-weighted images, and an anteromedial area of high signal intensity (eye-of-the-tig

[Contribution of nuclear magnetic imaging in the diagnosis of Hallervorden-Spatz syndrome].

BACKGROUND: The manifestations of the Hallervorden-Spatz disease (HSD) usually appear during childhood or in adults. They are not specific and magnetic resonance imaging (MRI) has improved its diagnosis. CASE REPORT: A girl developed progressive motor difficulties, dystony, rigidity, spasticity and mental deterioration, beginning at the age of 18 months. Examination at the age of 13 years showed pigmentary degeneration of the retina and acanthocytosis. The CT scan showed symmetrical calcifications in both globus pallidus. The MRI showed heterogeneity of the globus pallidus, decreased signal i

[Hallervorden-Spatz syndrome: report of a case].

It is reported the case of a male patient with 19 years old presenting progressive behavior disturbance and dystonic posture beginning on right, with approximately five years of evolution. Extrapyramidal and pyramidal syndrome was evidenced at examination. This finding suggests Hallervorden-Spatz disease which was confirmed by resonance imaging (MRI). The purpose of reporting this case is to call attention for the importance of MRI in the study of degenerative diseases of the nervous system.

Hallervorden-Spatz syndrome and MRI: the "tiger's eye". One case.

The MRI exploration of a woman suspected, on clinical grounds, of having Hallervorden-Spatz disease (or rather syndrome) revealed, on T2-weighted sequences, the "tiger's eye" or "target" image of the pallidum described by previous authors: i.e. a high-intensity signal in the centre of a distinct low-intensity signal; it also showed an abnormal low-intensity signal of the substantia nigra. These changes are related to the iron deposits and neuro-axonal lesions which characterize the disease. The MRI semeiology of Hallervorden-Spatz disease has been analyzed in the literature. The images we obta

Clinical features of neuroleptic malignant syndrome in basal ganglia disease. Spontaneous presentation in a patient with Hallervorden-Spatz disease in the absence of neuroleptic drugs.

Hallervorden-Spatz disease is a rare autosomal recessive disorder in which dopaminergic deficiency in the substantia nigra and its nigrostriatal projection has been identified. It is characterised by a slow but progressive course culminating in death. This case report describes a 13-year-old male, with a clinical diagnosis of Hallervorden-Spatz disease, who developed recurrent episodes of an acute illness, the features of which closely resembled those of the neuroleptic malignant syndrome. However, in this patient there had been no exposure to neuroleptic medication. The clinical events in thi

Hallervorden-Spatz disease with bilateral involvement of globus pallidus and substantia nigra: MR demonstration.

Hallervorden-Spatz disease presents with relentless mental and physical deterioration. Pathologically there is abnormal iron deposition in the globi pallidi, substantia nigra, and red nuclei. A spectrum of abnormal basal ganglia signal intensities have been described on MRI. A pediatric case with low signal intensity in the globi pallidi and substantia nigra is presented.

[Hallervorden-Spatz disease].

Hallervorden-Spatz disease is an autosomal recessive disorder which usually begins in childhood, progresses irreversibly and leads to death in early adulthood. The main manifestations are extrapyramidal signs, such as, dystonia and rigidity, pyramidal signs and mental deterioration. Pathological changes are found mainly in globus pallidus. They are characterized by (1) pigmentation with iron, and (2) numerous spheroids which represent abnormal axonal swelling. The cause of the disease is not clear but excessive iron storage may contribute to pathogenesis. The patients are treated symptomatical

Follow-up magnetic resonance imaging in Hallervorden-Spatz disease.

Bilateral high signal emitting areas in the globus pallidus surrounded by low signal emitting areas have been described as a typical MRI finding in Hallervorden-Spatz disease (HSD). We made a diagnosis of HSD in an 11-year-old girl with progressive dystonia of 4 years duration who showed these typical MRI abnormalities. An initial MRI at the age of 9 was normal. Pathological confirmation of these typical MRI findings has not yet been described, but earlier reports as well as our case suggest that MRI may be helpful in making a clinical diagnosis of HSD. This case further shows that MRI may be
