Levodopa/carbidopa/entacapone-induced acute Pisa syndrome in a Parkinson's disease patient.

Pisa syndrome (PS) is a dystonic lateroflexion of the trunk with a postural disturbance resembling the leaning tower of Pisa. Initially reported as a side effect related to antipsychotic therapy, this original dystonic posture is also manifested in neurodegenerative disorders such as Alzheimer's disease and multiple system atrophy, or in rare idiopathic cases. Recent observations have described the onset of PS with subchronic course in patients affected by Parkinson's disease (PD). Here, we report on the acute development of PS in a parkinsonian patient during treatment with entacapone/levodop

Spinocerebellar ataxia type 12 was not found in Korean Parkinsonian patients.

BACKGROUND: Parkinsonism (PD) is occasionally seen in several types of spinocerebellar ataxia (SCA). Mutations in SCA gene have been reported in the patients of parkinsonism without ataxia. METHODS: We examined spinocerebellar ataxia type 12 mutation in 877 PD and 199 multiple system atrophy (MSA) patients. RESULTS AND CONCLUSIONS: No patients showed abnormal SCA12 expansion. It suggests that PD and MSA are not associated with SCA12 and it is not necessary to screen SCA12 in PD and MSA patients.

[The nursing experience of a patient with parkinsonian type of multiple system atrophy].

This article describes the nursing experience of a patient with multiple system atrophy-parkinsonian type (MSA-P). The writer collected subjective and objective information using observation, interviews, and nursing care during the time period May 22 to June 3, 2006. A holistic nursing assessment and nursing problems were established as body image disturbance, risk for aspiration, constipation and family support, self-care deficit and risk for fall. Through individual nursing intervention and family support, the patient accommodated himself to the physical activities and mental situation. We h

Cell type-specific neuronal loss in the putamen of patients with multiple system atrophy.

Using antibodies to calcineurin (CaN) and choline acetyltransferase (ChAT), we performed topographical and cellular immunohistochemical analysis on the posterior putamen of autopsied patients with multiple system atrophy with predominant parkinsonism (MSA-P). We document that in these patients, medium spiny neurons positive for CaN were severely depleted in the dorsolateral portion of the posterior putamen where ChAT-positive neurons are normally distributed. Our findings indicate that in patients with MSA-P, striatal neurons manifest a cell type-specific vulnerability to neurodegeneration.

Early development of autonomic dysfunction may predict poor prognosis in patients with multiple system atrophy.

BACKGROUND: Multiple system atrophy (MSA) is diverse in clinical phenotype, disease progression, and prognosis. Sudden death is a leading cause of death in patients with MSA. OBJECTIVE: To determine what clinical factors affect the progression and survival prognosis of those with MSA. DESIGN: A retrospective review of the medical records of 49 consecutive Japanese patients with pathologically confirmed MSA (29 men and 20 women; mean +/- SD age at onset, 59.8 +/- 6.5 years). Cox proportional hazards models were used to compare the risks of being in a wheelchair-bound state, being in a bedridd

[Olfaction and neurological diseases: a review of the literature].

Olfactory disorders are often misjudged and rarely rated in the clinical setting. They are nevertheless described in a wide range of neurological disorders, and their evaluation can be useful for diagnosis. Usually irreversible olfactory dysfunction is a well-known complication after head trauma. Severe changes in olfactory tests are observed in Parkinson's disease. Dysfunction is present at disease onset and evidenced with all behavioral tests. Regarding other parkinsonian syndromes, olfactory performances are severely impaired in Lewy body disease, less pronounced in multiple system atrophy

[Positron emission tomography with FDG and newly developed tracers for the assessment of brain metabolism and synaptic function in neurological disorders].

Positron emission tomography has enabled us to measure various fundamental parameters of human brain physiology and chemistry, such as cerebral blood flow, metabolism and synaptic functions. Blood flow and oxygen metabolism is important for the understanding of cerebro-vascular disease. Glucose metabolism is tightly coupled with brain function and FDG-PET is useful for the determination of epileptic foci and for the evaluation of tumor malignancy. Imaging analysis of functional neuroanatomy of these parameters is very promising for the early diagnosis of dementia, such as Alzheimer's disease.

Levodopa responsiveness in disorders with parkinsonism: a review of the literature.

A literature review was conducted to investigate whether or not levodopa (LD) responsiveness (LR) is a useful criterion in the diagnosis of parkinsonian disorders. Although LR does appear to differ among the parkinsonian disorders, there is considerable confusion in the literature. While most patients with Parkinson's disease (PD) have a sustained benefit from LD, a small minority of patients with documented PD do not respond. The literature suggests that the LR rate is higher for multiple system atrophy (MSA), progressive supranuclear palsy (PSP), and corticobasal degeneration (CBD) than base

Compartmental loss of striatal medium spiny neurons in multiple system atrophy of parkinsonian type.

Topographical or compartmental involvement of the putamen and caudate nucleus has not been fully elucidated in multiple system atrophy predominantly presenting with Parkinsonism (MSA-P). We carried out immunohistochemical studies using antibodies to calbindin (CALB) and calcineurin (CaN) as neurochemical markers for striatal medium spiny neurons. We found that in the caudal and dorsolateral putamen, the area most affected in MSA-P, the medium spiny neurons positive for CALB were severely depleted, while CaN-positive neurons were relatively spared in a mosaic pattern. In the dorsal caudate nucl

Olfaction in neurodegenerative disorder.

There has been gradual increase of interest in olfactory dysfunction since it was realised that anosmia was a common feature of idiopathic Parkinson's disease (IPD) and Alzheimer-type dementia. It is an intriguing observation that a premonitory sign of a disorder hitherto regarded as one of movement or cognition may be that of disturbed sense of smell. In this review of aging, IPD, parkinsonian syndromes, tremor, Alzheimer's disease (AD), motor neuron disease (MND), Huntington's chorea (HC) and inherited ataxia, the following observations are made: (1) olfactory senescence starts at about the

Iron metabolism in Parkinsonian syndromes.

Growing evidence suggests an involvement of iron in the pathophysiology of neurodegenerative diseases. Several of the diseases are associated with parkinsonian syndromes, induced by degeneration of basal ganglia regions that contain the highest amount of iron within the brain. The group of neurodegenerative disorders associated with parkinsonian syndromes with increased brain iron content can be devided into two groups: (1) parkinsonian syndromes associated with brain iron accumulation, including Parkinson's disease, diffuse Lewy body disease, parkinsonian type of multiple system atrophy, prog

Dystonia in Parkinson's disease.

Dystonia can occasionally be found in idiopathic Parkinson's disease. It is very uncommon in untreated patients and is more frequently seen as a complication of its treatment. In this review, the various types of dystonia occurring in PD, the differential diagnosis with other parkinsonian syndromes associated with dystonia and treatments available are revised. Dystonia unrelated to treatment can be typical (blepharospasm, torticollis), atypical (parkinsonian writer's cramp, camptocormia, anismus), or occurring in earlyonset Parkinson disease (the so-called kinesigenic foot dystonia, considered

Atypical parkinsonism combining alpha-synuclein inclusions and polyglucosan body disease.

Adult polyglucosan body disease (APGBD) is a rare disorder affecting the central and peripheral nervous systems and in which parkinsonism is unusual. A 71-year-old man presented levodopa-unresponsive parkinsonism with urinary incontinence and recurrent syncopes of 6 years standing masquerading as atypical parkinsonism of the multiple system atrophy (MSA-P) type. Brain histopathology demonstrated massive accumulation of polyglucosan bodies particularly in the putamen. In addition, there were dense alpha-synuclein-positive cytoplasmic oligodendroglial inclusions in the pons and in the middle cer

Myoclonic tremulous movements in multiple system atrophy are a form of cortical myoclonus.

We prospectively studied the clinical and electrophysiological features of myoclonic tremulous movements in patients with multiple system atrophy (MSA). Among 42 consecutive patients, 12 MSA-p (parkinsonian type) and 3 MSA-c (cerebellar type) patients showed small-amplitude myoclonic movements. These movements occurred in the distal part of the arms and fingers, particularly in posture or during voluntary movements. We conducted detailed electrophysiological studies in 11 patients. Electromyographic recordings showed irregular bursts of brief duration often synchronously involving the antagoni

Sensorimotor integration in patients with parkinsonian type multisystem atrophy.

Sensorimotor integration is an essential feature of the central nervous system that contributes to the accurate performance of motor tasks. Some patients with multiple system atrophy with parkinsonian features (MSAp) exhibit clinical signs compatible with an abnormal central nervous system excitability to somatosensory inputs, such as action myoclonus or enhanced cutaneo-muscular reflexes. To investigate further the site where such dysfunction in sensorimotor integration takes place, we examined the inhibitory effects of a cutaneous afferent volley at two different levels of the motor system i

Parkinsonism and nigrostriatal dysfunction are associated with spinocerebellar ataxia type 6 (SCA6).

SCA6 is a slowly progressive, late-onset cerebellar ataxia due to a trinucleotide expansion in the CACNA1A gene. We describe two unrelated cases that presented with Parkinsonism and cerebellar ataxia. One case was L-dopa-responsive with a pattern of (18)F-dopa uptake similar to Parkinson's disease, and the second case was not L-dopa-responsive and had an atypical pattern of nigrostriatal dysfunction. We suggest that SCA6, in common with SCA2 and SCA3, may be associated with Parkinsonism attributable to nigral loss and dopaminergic dysfunction. Moreover, isolated cases may be confused with mult
