Intrafamilial variability of myoclonic dystonia in a large French family carrying a novel SGCE variant.

Myoclonus-dystonia syndrome (MDS) is an autosomal dominant movement disorder most caused by pathogenic variants in SGCE, an imprinted gene subject to maternal silencing. While numerous pathogenic variants have been reported, the extent and determinants of intrafamilial variability remain incompletely understood. We investigated a large French family in which ten individuals across three generations presented with myoclonic jerks, dystonia, or combined phenotypes. Trio whole-exome sequencing performed in the proband, her affected brother, and her affected father identified a heterozygous SGCE v

CTNNB1-related disorders: clinical and radiological contributions from a French cohort.

CTNNB1 monoallelic pathogenic variants account for up to 4% of genetically determined cerebral palsy cases, yet their phenotypic spectrum remains poorly defined. We retrospectively analyzed 25 individuals with pathogenic CTNNB1 variants using medical records and a questionnaire. Data included genetic variants, perinatal history, developmental milestones, behavioral characteristics, head growth, feeding, sleep difficulties, neurological and ophthalmological assessments. Brain MRIs were reviewed by expert neuroradiologists. Twenty-two distinct heterozygous variants were identified. Microcephaly

Translation of expanded CGG repeats in LRP12 associated oculopharyngodistal myopathy.

Oculopharyngodistal myopathy (OPDM) is characterized by ptosis, ophthalmoparesis, dysphagia, and distal weakness. Myopathological features include rimmed vacuoles and intranuclear inclusions. OPDM is associated with a pathogenic CGG repeat expansions in the 5'UTR of LRP12, NOTCH2NLC, GIPC1, RILPL1 and ABCD3. Translation of the repeat in the glycine reading frame has been demonstrated for expansions in FMR1, NOTCH2NLC and GIPC1. To assess for a similar phenomenon with LRP12, we expressed normal or expanded CGG repeats in the context of the 5'UTR of LRP12, upstream of a green fluorescent protein

The Brain-Age Gap in Pediatric Dystonia: Neuroanatomical Deviations Inform Deep Brain Stimulation Outcomes.

BACKGROUND: Dystonia in children is a heterogeneous condition with variable response to deep brain stimulation (DBS). Brain-age gap, a machine learning-derived metric of structural deviation from norm, may capture signatures that differentiate underlying biotypes and predict outcomes. METHODS: A brain age model was trained on several thousand normative developmental trajectories (n = 2623). Brain-age gap (the difference between predicted and chronological age) was computed from pre-DBS T1-weighted magnetic resonance imaging in 37 children with dystonia and compared to matched healthy controls

Compulsion Profile Differences Indicate Distinct Functional Mechanisms in Autistic and Non-Autistic University Students.

Autistic individuals often exhibit high rates of obsessive-compulsive symptoms (OCS), yet traditional cognitive-behavioral therapy (CBT), especially exposure and response prevention (ERP), tends to be less effective for them. This may be due to differences in the function of compulsive behaviors: while OCD-related compulsions are typically ego-dystonic and aimed at reducing anxiety, autistic compulsions may be ego-syntonic, serving regulatory or sensory modulation purposes. This study investigated whether compulsions in autism are more aligned with regulation and sensory modulation than with a

Effects of chemogenetic inhibition of serotonergic raphe-striatal neurons on L-DOPA-induced behaviors and neurochemistry in a rat model of Parkinson's disease.

Parkinson’s disease (PD) arises from the loss of nigrostriatal dopamine (DA) neurons in the substantia nigra pars compacta (SNc) precipitating motor impairments such as tremor, rigidity, and akinesia. DA replacement therapy via levodopa (L-DOPA), though effective, often results in the development of levodopa-induced-dyskinesia (LID), characterized by excessive hyperkinetic choreic and dystonic movements. While a myriad of mechanisms are implicated in LID, maladaptive neuroplasticity within raphe-striatal serotonin (5-HT) neurons appear to influence LID induction and severity. Although prior wo

The Movement Disorder Spectrum of ATP1A3-Related Disorders: Cross-Sectional Analysis and Video Archive of 88 Patients.

BACKGROUND: ATP1A3-related disorders are characterized by genetic heterogeneity and phenotypic pleiotropy, posing significant challenges for classification. Although canonical phenotypes have traditionally guided decision-making, increasing evidence highlights their limitations in capturing the clinical complexity. OBJECTIVE: The aims of this study were to characterize movement disorders, paroxysmal features, and genotype-phenotype relationships; to build a curated video archive; and to assess alignment with canonical phenotypes. METHODS: This is an observational study of 88 individuals with

Putamen Atrophy as a Predictive Factor of Efficacy of GPi-DBS in Dystonia-Dyskinesia Syndrome Secondary to Perinatal Anoxic Encephalopathy.

BACKGROUND: Perinatal hypoxic-ischemic encephalopathy (HIE) is a severe condition resulting from impaired oxygen delivery to the developing brain, often leading to both motor deficits and dystonia-dyskinetic syndromes (DDS). In selected cases, deep brain stimulation of the globus pallidus internus (GPi-DBS) may provide a therapeutic option. However, predicting outcomes remains challenging because of clinical heterogeneity and variable responses. OBJECTIVES: This retrospective study aims to identify preoperative imaging predictors of GPi-DBS efficacy in patients with DDS secondary to HIE, focu

Tyrosine Hydroxylase Deficiency Impairs TH Axonal Transport, Brain Function, and Neuronal Plasticity.

Tyrosine hydroxylase deficiency (THD) is a rare genetic disorder caused by biallelic pathogenic variants in the Th gene, leading to a deficiency in the rate-limiting enzyme for the synthesis of dopamine (DA) and other catecholamine neurotransmitters. THD is associated with dystonia and infantile parkinsonism with a broad and complex spectrum and variable response to l-Dopa therapy. TH1-p.R202H is a frequent THD variant that affects TH stability and activity. The Th knock-in (Th-ki) mice with the equivalent mutation (Th-p.R203H) present reduced TH and DA levels, biological and molecular alterat

Multi-Omics Characterization of a KIF1C Structural Variant in a Patient with a Complex Movement Disorder Partially Responsive to Deep Brain Stimulation.

Biallelic KIF1C variants are associated with a spectrum of neurological phenotypes, including spasticity, cerebellar ataxia, chorea, and dystonia. Most reported patients involve single-nucleotide variants, whereas copy number variants (CNVs) have been described only once in the PubMed-listed literature. The therapeutic potential of deep-brain stimulation (DBS) in KIF1C-related disease has not yet been evaluated. We report a patient presenting with spasticity, ataxia, dystonia, and head tremor, in whom prior exome sequencing was inconclusive. A multi-omics approach was applied, including optica

Is comorbid Obsessive-compulsive Personality Disorder responsible for a longer duration of untreated illness in Obsessive-Compulsive Disorder?

BACKGROUND AND AIMS: Obsessive-Compulsive Personality Disorder (OCPD) is marked by a pervasive preoccupation with orderliness, perfectionism, and control, often overlapping with Obsessive-Compulsive Disorder (OCD) in terms of rigid and repetitive behaviors. While OCD behaviors are ego-dystonic and confined to specific areas of life, OCPD behaviors are ego-syntonic and affect overall functioning. Although the relationship between OCD and OCPD has been object of investigation, the impact of OCPD comorbidity on OCD remains underexplored. This study examines potential clinical differences between

Neurodevelopmental Disorder with Dystonia and Chorea Linked to De Novo Variants in the Splicing Regulator SRRM4.

BACKGROUND: SRRM4 is an exclusively neural-expressed splicing-factor gene not yet associated with a monogenic condition. OBJECTIVE: We sought to delineate movement disorders caused by SRRM4 variants. De novo splice-donor-site variants at position +2 of intron 5 of SRRM4 (c.464+2T>C, c.464+2T>A) occurred in three unrelated patients with dystonia and chorea. We present detailed phenotypic information on these individuals and characterize the effect of the splice-site alteration. METHODS: Exome and genome sequencing were used to identify SRRM4 variants. To assess the consequence of a mutant +2

Genetic analysis reveals phenotypic variability in three Colombian families with dopa-responsive dystonia: novel genotype-phenotype correlations.

BACKGROUND: Dopa-responsive Dystonia (DRD) due to GTP cyclohydrolase 1 (GTPCH1) deficiency is a neurogenetic disorder caused by pathogenic GCH1 variants. Tetrahydrobiopterin (BH4) deficiency impairs dopamine synthesis in the basal ganglia, leading to childhood-onset dystonia with excellent response to levodopa. CASE PRESENTATION: We report eight patients from three unrelated families with GCH1 (NM_000161.3) variants: c.142 C > T; p.(Gln48*), c.241T > C; p.(Ser81Pro), and c.607G > A; p.(Gly203Arg). Two individuals homozygous for c.142 C > T showed phenotypic discordance. Mean age at onset was 7

Childhood-Onset Myoclonus-Dystonia Due to KCTD17 Mutation: A Case Report and Review of Diagnostic Challenges.

BACKGROUND: Myoclonus-dystonia (M-D) is a rare hyperkinetic movement disorder most commonly associated with SGCE mutations, while KCTD17 represents a less frequent but distinct genetic cause. CASE REPORT: A 23-year-old man with childhood-onset Tourette-like symptoms developed progressive dystonia, upper-limb-predominant dystonia, upper-limb-predominant myoclonus, and laryngeal involvement. Genetic testing identified a pathogenic KCTD17 mutation in the patient and his affected mother. DISCUSSION: This case illustrates that Tourette-like manifestations may occur as an early presentation of KCT

Autoimmune Limbic Encephalitis With Leucine-Rich Glioma-Inactivated 1 (LGI1) Antibodies Presenting as Rapidly Progressive Cognitive Decline and Psychiatric Symptoms: A Case Report.

Autoimmune limbic encephalitis (LE) with leucine-rich glioma-inactivated 1 (LGI1) antibodies is rare but treatable, often presenting as rapidly progressive cognitive decline, behavioural changes, and hyponatraemia. These features can resemble psychiatric or neurodegenerative disorders, delaying diagnosis. We report a case of a 63-year-old man with confusion, memory loss, dream enactment, hallucinations, severe hyponatraemia, and specific higher cortical features, including apraxia, anomia, and executive dysfunction. Physiological Correlates of the Spectral Aggregate of the High-Passed Fundamental Frequency in Adductor Laryngeal Dystonia.

OBJECTIVE: The spectral aggregate of the high-passed fundamental frequency (SAHfo) is an automated acoustic measure associated with the primary acoustic discontinuities present in the speech of individuals with adductor laryngeal dystonia (AdLD). Acoustic discontinuities (e.g., phonation breaks, frequency shifts, creak) are also associated with supraglottic compression and visual laryngeal obstruction postures that occur during laryngeal spasms (i.e., phasic or tonic laryngeal dystonic activity). The purpose of this study was to determine whether SAHfo is similarly reflective of these physiolo
