Psychological and motor outcomes of botulinum toxin in hemifacial spasm and blepharospasm.

PURPOSE: Hemifacial spasm (HFS) and blepharospasm (BSP) impair motor function and psychosocial health. This study assessed anxiety, depression, and self-esteem changes following botulinum toxin A (BoNT-A) treatment. METHODS: In this prospective study, 158 adult patients (HFS: n = 112, BSP: n = 46) receiving ongoing BoNT-A treatment were recruited. Motor severity was assessed using the Hemifacial Spasm Grading Scale (HSGS) for HFS and the Modified Jankovic Rating Scale (MJRS) for BSP. Psychological status was measured using the Beck Anxiety Inventory (BAI), Beck Depression Inventory (BDI), and

A plastic surgery approach to Meige syndrome: Botulinum toxin for rebalancing facial dystonia and aesthetics - A case report.

Meige syndrome is a rare segmental dystonia characterized by blepharospasm and oromandibular dystonia, often leading to significant functional and aesthetic impairment. This report details a 59-year-old female patient with a 16-year history whose symptoms were refractory to prior drug therapies. An individualized botulinum toxin type A (BTX-A) injection protocol was devised based on detailed facial muscle mapping and plastic surgery principles of muscle balance. Following treatment, the patient showed marked improvement in facial spasms within 1 week, with resolution of nocturnal headaches and

Shared and Specific Patterns of Brain Functional Network Abnormalities in Patients With Idiopathic Dystonia and Across Subtypes.

AIMS: Accumulating neuroimaging evidence showed that idiopathic dystonia is a large-scale network disorder, but whether the various clinical sub-types shared imaging abnormalities remains controversial. We aimed to determine whether various forms of idiopathic dystonia have common topological changes or are distinct entities. METHODS: Resting-state functional magnetic resonance imaging and clinical data were obtained from 215 patients with various forms of dystonia and 160 healthy controls (HCs). Whole-brain functional networks were constructed, and the topological parameters were calculated

[Tremor].

Tremor is one of the most common movement disorders encountered in pediatric clinical practice and encompasses a broad spectrum of clinical presentations, etiologies, and outcomes. It is defined as an involuntary, rhythmic, oscillatory movement, and its evaluation in childhood is challenging due to phenotypic variability, limited standardization of complementary studies, and reliance on clinical observation. These recommendations from the Ibero-American Academy of Pediatric Neurology (AINP) aim to provide a practical and updated approach to the classification, diagnostic evaluation, treatment,

CACNA1C -Related Channelopathy Presenting With Adult-Onset Combined Dystonia-Parkinsonism: A Novel Neurological Presentation.

Pathogenic variants in CACNA1C have been associated with cardiac arrhythmias and neurodevelopmental disorders, occasionally accompanied by movement disorders. Here we report the case of a 66-year-old female patient with combined dystonia-parkinsonism carrying a pathogenic CACNA1C variant. Neurological examination revealed painful tremulous cervical dystonia, blepharospasm, divergent strabismus, hypomimia, and mild dystonic posturing of left hand, action tremor, and bradykinesia of the upper limbs. Botulinum toxin type A injections provided only partial relief of cervical dystonia, so the patie

Reviving Hope: Overcoming Deep Brain Stimulation Failure With Partial Cranial Nerve Radiofrequency Ablation in Primary Meige Syndrome.

INTRODUCTION: Deep brain stimulation (DBS) is a standard treatment for refractory primary Meige syndrome (PMS), yet 30%-50% of patients experience suboptimal outcomes. Management strategies for these "DBS failures" remain undefined. This study evaluates the efficacy of partial cranial nerve radiofrequency ablation (RFA) as a salvage therapy for this challenging population. METHODS: We report a retrospective series of three PMS patients classified as "DBS non-responders" based on clinical refractory status or ineligibility for lead revision. Patients underwent imaging-guided partial RFA of the

Is Current Evidence Sufficient to Establish the Efficacy of Botulinum Toxin A in Treating Persistent Dry Eye Disease? A Systematic Review and Meta-Analysis of Interventional Studies With a Critical Review Using GRADE Tool.

BACKGROUND AND AIMS: Standard management of dry eye disease (DED) relies on artificial tears, anti-inflammatory therapy, and punctal occlusion in selected cases. However, some patients continue to report persistent symptoms. The present article reviewed the efficacy of Botulinum Toxin A (BTX-A) for DED that remained symptomatic after these interventions, excluding patients with blepharospasm or hemifacial spasm. METHODS: We conducted a comprehensive search of seven databases (PubMed, Embase, Web of Science, Scopus, CENTRAL, Science Direct, and ProQuest), supplemented by grey literature search

Neurophysiological Basis of Tremor Disorders: Parkinson's Disease, Essential Tremor, Tremor in Dystonia, and Holmes Tremor.

Tremor disorders encompass heterogeneous syndromes with distinct clinical, electrophysiological, and pathophysiological characteristics. Parkinson's disease tremor typically presents as a unilateral 4 to 6 Hz rest tremor, with re-emergent postural components or pure postural in some, driven by abnormal pallido-thalamo-cortical (PTC) oscillations and modulated by cerebellar inputs. Essential tremor is characterized by bilateral 4 to 8 Hz kinetic >postural tremor with regular, rhythmic EMG bursts, reflecting dysfunction within the cerebello-thalamo-cortical (CTC) pathway. Tremor in dystonia is f

Diagnosis and pathogenesis of dystonia: clinical heterogeneity, shared mechanisms, and neurodevelopmental origins.

The diagnosis of dystonia is often delayed because of its many different clinical manifestations and causes. Updates to the definition and classification of dystonia can assist clinicians in improving its recognition and timely diagnosis. Dystonia is characterised by sustained or intermittent abnormal movements, postures, or both. Patterned phenomenology, alleviating manoeuvres, worsening by voluntary movement, and overflow to adjacent muscles are all clinical clues of the condition. Additional non-motor manifestations are part of the clinical spectrum. In parallel to the broad heterogeneity o

Shared and distinct dynamic brain activity and functional connectivity variability in patients with isolated craniocervical dystonia.

Isolated craniocervical dystonia is a hyperkinetic movement disorder with motor and non-motor features, yet the temporal dynamics of its brain activity remain underexplored. Using resting-state functional MRI from 201 isolated craniocervical dystonia patients (102 blepharospasm, 43 blepharospasm-oromandibular dystonia and 56 cervical dystonia) and 160 healthy controls, we applied a sliding-window approach to evaluate dynamic functional metrics and dynamic functional connectivity patterns. Isolated craniocervical dystonia patients showed widespread abnormalities in dynamic regional activity inv

The classificatory ambiguity of task-specific tremor.

Task-specific tremor is a clinically heterogeneous phenotype that remains difficult to accommodate within current tremor classification. Although defined by its occurrence during a specific motor task, the boundaries with position-specific and isometric tremors remain uncertain. Additional classificatory complexity arises when a task-specific tremor phenotype is accompanied by neurological signs that remain subtle, questionable, or uncertain, such as possible dystonic posturing, rest tremor, mild bradykinesia, or response to alleviating maneuvers, and do not clearly support classification as a

Effects of botulinum toxin on tear film and corneal epithelial remodeling in patients with essential blepharospasm.

PURPOSE: Benign essential blepharospasm (BEB) is a focal dystonia characterized by involuntary eyelid contractions that can disrupt tear film stability and corneal morphology. Botulinum toxin type A (BTX-A) injection is the standard treatment, yet its impact on ocular surface health and corneal epithelial structure remains unclear. This study aimed to assess the effects of periocular BTX-A on tear film parameters and corneal epithelial remodeling using anterior segment optical coherence tomography (AS-OCT). METHODS: This retrospective, single-center study included 35 eyes of 35 BEB patients t

A scoping review about the efficacy of clonazepam for the treatment of benign essential blepharospasm.

BACKGROUND: Benign Essential Blepharospasm (BEB) is a focal dystonia that leads to an increased rate of eyelid closure due to involuntary activation of the orbicularis oculi muscles. In severe cases, daily activities can significantly be affected, with reduced quality of life. Injections of OnabotulinumtoxinA are often used as valid therapeutical option. However, this treatment may not be tolerated by all patients, considering its invasive nature and the need for access to specialized centers. Despite oral agents, such as benzodiazepines (particularly clonazepam), are often used as a first-lin

Comparative efficacy and safety of botulinum neurotoxin type A in blepharospasm and hemifacial spasm: a systematic review and Bayesian network meta-analysis.

While botulinum neurotoxin type A (BoNT-A) has demonstrated efficacy in the treatment of blepharospasm and hemifacial spasm, comparative efficacy and safety of different BoNT-A formulations remain uncertain. We conducted a systematic review and network meta-analysis with random-effects Bayesian models of randomized controlled trials comparing BoNT-A preparations with each other or with placebo. Separate networks were constructed for duration of clinical effect, analyzed as mean differences (MD), and ptosis, analyzed as odds ratios (ORs). The network included 13 randomized controlled trials wit
