Attenuated melanopsin-mediated activity in focal dystonia.

BACKGROUND AND OBJECTIVES: Focal dystonia (FD) is an extrapyramidal movement disorder, but various disease aspects challenge the view of a condition only involving neuronal motor systems. Here, we were interested in potential changes of retinal functions, accessible by chromatic pupillometry. METHODS: Quantitative chromatic pupillometry was performed in 26 persons with FD and 20 age-/sex-matched healthy controls. Upon dark adaptation, standardized red (632 nm) and blue (462 nm) light pulses (100 cd/m2) were delivered to the pupils. Constrictions were recorded one and 6 s after the stimuli. Th

Luteolin use in Integrated Stress Response: insight from a case of EIF2AK2-related dystonia.

This study presents the first reported case of a 3-year-old child with EIF2AK2-related dystonia treated with adjunctive luteolin supplementation. EIF2AK2-related dystonia, characterized by exacerbations during infections, is associated with disruptions in the integrated stress response (ISR). The ISR, a cellular signaling pathway activated in response to stress, culminates in the phosphorylation of eIF2α, which modulates protein synthesis and can induce cell death. Pathogenic variants in EIF2AK2 disrupt this pathway, contributing to the development of dystonia. Luteolin, a flavonoid possessing

DYT-AOPEP: A case series from India expanding the clinical and genetic spectrum.

BACKGROUND: DYT-AOPEP (Aminopeptidase O) or DYT31 is a rare, newly discovered genetic cause of autosomal recessive monogenic adult-onset isolated dystonia. We describe three Indian patients with likely pathogenic variants in the AOPEP gene, expanding the clinicogenetic spectrum DYT-AOPEP. CASES: First proband presented with adult-onset generalised isolated dystonia, which was medically refractory, but responded to GPi-DBS. He harboured a homozygous splice site variant (NM_001193329.3:c.1916+1G > C). The second proband had adult-onset isolated generalised dystonia without any family history, w

Clinical and genetic spectrum of L-dopa responsive dystonia: insights from seven Egyptian cases.

BACKGROUND: Dopa-responsive dystonia (DRD) encompasses a group of treatable movement disorders with significant clinical and genetic heterogeneity. Despite being treatable, DRD is often misdiagnosed, leading to delays in therapy. METHODS: This retrospective study aims to describe the clinical and genetic features of seven Egyptian patients from four unrelated families with confirmed DRD. Medical records were reviewed to document clinical data, neuroimaging, and genetic testing. RESULTS: We identified six patients with Tyrosine hydroxylase-deficient DRD and one with GTP cyclohydrolase 1 defic

Structural brain analysis in focal upper limb dystonia.

Prior structural magnetic resonance imaging (MRI) studies that investigated structural alterations in focal dystonia have reported inconsistent findings, potentially due to methodological limitations or sample heterogeneity. This work investigates gray and white matter changes in patients with upper limb dystonia using T1-weighted images and diffusion tensor imaging (DTI). Imaging data collected with MRI at 3T from 28 right-handed individuals with right upper limb dystonia and 29 healthy controls were analyzed. T1-weighted images were analyzed using FreeSurfer to examine cortical thickness, vo

Intrafamilial variability of myoclonic dystonia in a large French family carrying a novel SGCE variant.

Myoclonus-dystonia syndrome (MDS) is an autosomal dominant movement disorder most caused by pathogenic variants in SGCE, an imprinted gene subject to maternal silencing. While numerous pathogenic variants have been reported, the extent and determinants of intrafamilial variability remain incompletely understood. We investigated a large French family in which ten individuals across three generations presented with myoclonic jerks, dystonia, or combined phenotypes. Trio whole-exome sequencing performed in the proband, her affected brother, and her affected father identified a heterozygous SGCE v

Exome Sequencing Resolving a Complex Pediatric Neurodevelopmental Disorder After Inconclusive Prenatal Testing: A Case Report.

BACKGROUND: Prenatal detection of fetal structural anomalies often prompts chromosomal analysis; however, chromosomal microarray analysis (CMA) has limited diagnostic yield for monogenic disorders. Whole-exome sequencing (WES) has emerged as a powerful tool for identifying single-gene etiologies, particularly in cases with complex neurodevelopmental phenotypes. CASE PRESENTATION: We report a female infant presenting with prenatally detected ventriculomegaly and inconclusive chromosomal testing. Prenatal investigations, including karyotyping and genome-wide chromosomal sequencing, identified s

Symptom-Oriented, Connectome-Informed Deep Brain Stimulation for Asymmetric Dystonic Tremor: Unilateral Ventral Intermediate Nucleus (VIM) DBS Targeting a Tremor-Dominant Network.

Background: Deep brain stimulation (DBS) has traditionally followed diagnosis-driven, nucleus-centered targeting paradigms. Increasing evidence supports a circuit-based framework in which clinical outcomes depend on modulation of symptom-relevant networks rather than diagnostic labels alone. This approach is particularly relevant in mixed movement disorder phenotypes such as dystonic tremor, where the most disabling symptom may not align with the conventional surgical target. Methods: We report a clinically illustrative single case treated using a symptom-oriented, connectome-informed DBS stra

Bilateral pallidal deep brain stimulation versus bilateral pallidotomy in the management of post anoxic generalized dystonia.

BACKGROUND: Post-anoxic acquired generalized dystonia is a rare and severely disabling condition in which medical therapy is often ineffective. Pallidal deep brain stimulation (DBS) is widely used for generalized dystonia; however, its efficacy in acquired dystonia is less well established, and access may be limited in resource-constrained settings. Bilateral posteroventral pallidotomy (PVP) represents a potential alternative, but comparative data between the two approaches in post-anoxic dystonia are lacking. OBJECTIVE: To compare clinical outcomes and safety profiles of bilateral globus pal

CTNNB1-related disorders: clinical and radiological contributions from a French cohort.

CTNNB1 monoallelic pathogenic variants account for up to 4% of genetically determined cerebral palsy cases, yet their phenotypic spectrum remains poorly defined. We retrospectively analyzed 25 individuals with pathogenic CTNNB1 variants using medical records and a questionnaire. Data included genetic variants, perinatal history, developmental milestones, behavioral characteristics, head growth, feeding, sleep difficulties, neurological and ophthalmological assessments. Brain MRIs were reviewed by expert neuroradiologists. Twenty-two distinct heterozygous variants were identified. Microcephaly

Translation of expanded CGG repeats in LRP12 associated oculopharyngodistal myopathy.

Oculopharyngodistal myopathy (OPDM) is characterized by ptosis, ophthalmoparesis, dysphagia, and distal weakness. Myopathological features include rimmed vacuoles and intranuclear inclusions. OPDM is associated with a pathogenic CGG repeat expansions in the 5'UTR of LRP12, NOTCH2NLC, GIPC1, RILPL1 and ABCD3. Translation of the repeat in the glycine reading frame has been demonstrated for expansions in FMR1, NOTCH2NLC and GIPC1. To assess for a similar phenomenon with LRP12, we expressed normal or expanded CGG repeats in the context of the 5'UTR of LRP12, upstream of a green fluorescent protein

The Brain-Age Gap in Pediatric Dystonia: Neuroanatomical Deviations Inform Deep Brain Stimulation Outcomes.

BACKGROUND: Dystonia in children is a heterogeneous condition with variable response to deep brain stimulation (DBS). Brain-age gap, a machine learning-derived metric of structural deviation from norm, may capture signatures that differentiate underlying biotypes and predict outcomes. METHODS: A brain age model was trained on several thousand normative developmental trajectories (n = 2623). Brain-age gap (the difference between predicted and chronological age) was computed from pre-DBS T1-weighted magnetic resonance imaging in 37 children with dystonia and compared to matched healthy controls

Compulsion Profile Differences Indicate Distinct Functional Mechanisms in Autistic and Non-Autistic University Students.

Autistic individuals often exhibit high rates of obsessive-compulsive symptoms (OCS), yet traditional cognitive-behavioral therapy (CBT), especially exposure and response prevention (ERP), tends to be less effective for them. This may be due to differences in the function of compulsive behaviors: while OCD-related compulsions are typically ego-dystonic and aimed at reducing anxiety, autistic compulsions may be ego-syntonic, serving regulatory or sensory modulation purposes. This study investigated whether compulsions in autism are more aligned with regulation and sensory modulation than with a

Effects of chemogenetic inhibition of serotonergic raphe-striatal neurons on L-DOPA-induced behaviors and neurochemistry in a rat model of Parkinson's disease.

Parkinson’s disease (PD) arises from the loss of nigrostriatal dopamine (DA) neurons in the substantia nigra pars compacta (SNc) precipitating motor impairments such as tremor, rigidity, and akinesia. DA replacement therapy via levodopa (L-DOPA), though effective, often results in the development of levodopa-induced-dyskinesia (LID), characterized by excessive hyperkinetic choreic and dystonic movements. While a myriad of mechanisms are implicated in LID, maladaptive neuroplasticity within raphe-striatal serotonin (5-HT) neurons appear to influence LID induction and severity. Although prior wo
